Objective To investigate an effective method for detecting lymphatic micrometastasis in gastric carcinoma.Methods RT-PCR technique was applied to examine MMP-7 mRNA in 94 lymph nodes in 24 cases of gastric carcinoma.Results Routine pathological method detected lymphatic metastasis in 54 lymph nodes,while MMP-7 mRNA RT-PCR showed positive in 78 lymph nodes. When re-examination for the 28 negative lymph nodes in the initial pathological examination and positive in RT-PCR, 8 lymph nodes with metastasis were found by routine pathological method.2 of 5 patients whose lymph nodes were negitive in pathological examination,but positive in MMP-7mRNA were found liver metastasis 16 and 22 months after radical gastrectomy. Conclusions MMP-7 mRNA RT-PCR is a sensitive method for detecting lymphatic micrometastasis for patients with gastric carcinoma. It is a great help for eluvating the postoperative prognosis and supplemental treatmemt.

BACKGROUND: Maintenance and activation of cascade reaction influence T cell proliferation or transformation into nonreactive state even apoptosis. B7 binding to CD28 effectively activates T cells in combination with T cell receptor pathway, and enhances T cell proliferative activity. OBJECTIVE: To investigate the costimulated activation of peripheral blood mononuclear cells (PBMC) with CD28 and CpG containing oligodeoxynucleotides (CpGODN) MoAb combined with CD80, and its killing effect on human gastric cancerous cell line MKN45 in vitro.METHODS: PBMCs were isolated by Ficoll density gradient centrifugation method, and cocultured with interleukin-2, CD28 and CpGODN MoAb for 1-5 days. MKN45 cells were divided into 4 culture conditions: CD28/CpGODN, CD80 plus CD28/CpG ODN, CD80 alone, and blank control.The killing efficiency was measured by MTT method.The ultramicrostructure of cells was observed by electron microscope. Apoptosis was verified by a flow cytometery. RESULTS AND CONCLUSION: CD80 alone did not display killing effect on MKN45 cells. By MTT method, combination of costimulated activation of PBMC with CD28/CpGODN and CD80 showed enhanced killing effect compared with single therapy (P < 0.05), and the ratio of effector cell and target cell at 15: 1 resulted in half killing efficiency. Electron microscope and flow cytometery verified necrotic or apoptotic cells after 24 hours exposure to costimulated activation. Compared with blank control group, CD80 alone elevated the apoptosis rate of MKN45 cells (P < 0.01). Results from the present study show that CD80 can elevate the killing effect of costimulated activation of PBMC with CD28/CpGODN on MKN45 cells in vitro.

Objective To find a new method to prepare the acellular matrix of the penile cavernous body. Methods Intact specimens of the penile cavernous body were collected from healty adult rabbits,treated with Triton-X100 and NH3?H2O,and then stained with HE.Light microscopy was employed to observe the acellular cells.Results Acellular matrix of the penile cavernous body was obtained after a 25-day treatment with Triton-X100 and NH3?H2O.The matrix was intact.HE staining showed that the matrix was composed of regularly arranged elastic fibers with wide intra-spaces without any cells inside.Conclusion Acellular matrix of the penile cavernous body can be prepared by using Triton-X100 and NH3?H2O.

Objective To evaluate the imaging technique of magnetic resonance tomographic angiography (MRTA) in the investigation of etiology of hemifacial spasm (HFS). Methods Multiplane MRTA imaging of facial nerve of both sides was performed in all 121 HFS patients. Among them, 23 HFS patients received MVD subsequently. Results Among the 121 HFS patients, 119 patients had one side hemifacial spasm and 2 patients had double side hemifacial spasm,among them, neurovascular compression was found in the root entry zone (REZ) of facial nerve in 116 cases by MRTA,and the sensitivity was 94 3%;among the 119 cases which had no HFS,no neurovascular compression was found in the REZ of facial nerve in 112 cases by MRTA,and the specificity was 94 1%. Among 23 patients who received one side MVD, 21 patients had results that agreed with the multiplanar MRTA views. Conclusion Multiplanar MRTA was considered to be a very useful imaging technique for the study of etiology of HFS, and it also seemed to be a very important preoperative examination for MVD.

Objective To explore the value of 2 dimension and color Doppler flow imaging (2D CDFI) in detection of deep venous thrombosis of the lower limbs. Methods One hundred and one patients, 118 limbs clinically suspected as deep venous thrombosis were examined by CDFI and ascending phlebography. Results The sensitivity of ultrasonogram was 94 4% and accuracy was 98 4%. The highest incidence is 30 4% in the femoral vein. With the ratio 2 2∶1, the incidence of left lower limbs was much higher than the right. Because of different pathological changes in acute, subacute and chronic thrombosis, there are diverse character in 2D CDFI. Conclusions Deep venous thrombosis of the lower limbs can be diagnosed by 2D CDFI, which is a noninvasive, easy and accurate method.

Objective To observe the therapeutical effect of posterior vertebral column resection on chronic thoracolumbar tuberculosis with secondary paraplegia and to provide a safe and effective method for the treatment of chronic thoracolumbar tuberculosis with secondary paraplegia.MethodsFrom Aug.2007 to Mar.2010,12 cases with chronic thoracolumbar tuberculosis and secondary paraplegia were surgically treated by posterior vertebral column resection and Titanium net support for bone graft and internal fixation operation treatment.Cobb angle was measured,and conditions of internal fixation were observed before and after the operation by X-ray films.Neurological status were evaluated by Frankel grades.ResultsThe follow-up periods was 6- 18 months( on average 11 months).Operations eased all patients' back and chest pain.Frankel grade increased from C preoperatively to grade D or E postoperatively in 6 cases,from grade D to E in 4 cases and from grade B to C in 1 case.No obvious improvement of Frankel grade was observed in the other patient of grade B.The average Cobb angles were(76.0 ± 23.4) ° before surgery,( 15.5 ± 6.3 ) ° at one week after surgery and ( 16.0 ± 8.2) °at the last follow-up.The difference in the Cobb angle before and at one week after treatment was significant( t =3.41,P ＜ 0.01 ).No difference was found in the Cobb angle between at one week after treatment and at the last follow-up (t =1.58,P ＞ 0.05 ).All patients got bony fusion with Titanium net.No complications occurred with internal fixation.Conclusion Posterior vertebral column resection is a feasible method for the treatment of chronic thoracolumbar tuberculosis with secondary paraplegia.It achieves neurological decompression with high correction rate and minor injury,and no anterior surgery is needed.

AIM: To investigate the effects of simvastatin on myocardial fibrosis and connective tissue growth factor (CTGF) in renovascular hypertensive rats. METHODS: The experimental rats were randomly divided into three groups, such as sham operation group rats, hyperpiesia rats and hyperpiesia rats treated with simvastatin (5 mg㎏kg -1 ?d -1 ). Systolic blood pressure (SBP) was measured and collagen concentration (Coll) was also detected in terms of hydroxyproline concentration. VanGieson and immunohistochemistry staining combined with computed morphometry were used to evaluate the total collagen volume fraction (CVF) and CTGF expression in left ventricular tissue. Objective To optimize the process of extracting total saponins from Platycodon grandiflorum by orthogonal experimental design and central composite design-response surface method. Methods Independent variables were concentration of ethanol, reflux extraction time, reflux time and solvent fold. Dependent variables were extraction rate of total saponins in Platycodon grandiflorum and the yield of dry extract.Linear or nonlinear mathematic models were used to estimate the relationship between independent and dependent variables. Results The result of the orthogonal experimental design was 70%ethanol, 180 min for reflux, 6 fold of solvent and 2 times for extraction. The result of central composite design-response surface method was 70% ethanol, 140 min for reflux, 10 fold of solvent and 2 times for extraction. Conclusion The central composite design-response surface method is better than orthogonal experimental design in studying extraction process of Platycodon grandiflorum, which provides basis for feasibility of application of central composite design-response surface method in optimization of extraction process of Chinese traditional medicine.

Objective To assess the value and safety of SonoVue in ultrasonic diagnosis of abdominal diseases. Methods A total of 204 patients (149 male, 55 female) with abdominal examination of contrast-enhanced ultrasonography (CEUS) with SonoVue from August 2007 to February 2009 were reviewed. Results Definitive diagnosis were obtained 215 times in 204 patients (233 times) with abdominal CEUS. The diagnostic accuracy of CEUS was 92.27%, significant higher than that of conventional ultrasonography (62.23%). Anaphylactic shock happened 1 time (0.43%) in 1 patient. Conclusion CEUS with SonoVue can provide a high diagnostic accuracy with relatively safety of the possibility of serious adverse event.
