Objective To approach the feasibility of the development and routinely carrying out of gynecological analgesia. Methods A total of 286 pregnant women, 58 were primiparae with full-term fetus and 228 were for mid-term delivery or induced abortion, were enrolled in present study. The combined lumbar-epidural analgesia (CLEA) and the patient-controlled epidural analgesia (PCEA) were given to the primiparae and the ones for mid-term delivery, and the intravenous anesthesia was given to those for induced abortion. Results Among the primiparae and the ones for mid-term delivery, VAS scores were rapidly reduced from 87.7?12.5 and 51.1?14.1 to 8.7?8.8 and 10.6?6.2, respectively, within 3 minutes by CLEA, the effective rates of analgesia were 94% and 100%, respectively. For all of them the Bromage evaluation was in 0 degree. After the induction of labor, no algesia memory existed in the women for induced abortion, the operative time was 15.28?2.57 minutes, the dosage of propoful was 162.9?39.8 mg (about 3mg/kg), and the dosage of fentanyl was 0.05 mg. The incidence of hypotension after intravenous anesthesia was 5.8%. Conclusions Intravenous anesthesia with propoful may remit the panic and fearful sense of pregnant women to induced abortion, and reduce the pain during operation. It is suggested to develop widely and carry out routinely the intravenous anesthesia as an effective and safe technique in gynecological analgesia.

BACKGROUND:Artificial teeth computer aided design and computer aided manufacture (CAD/CAM) is a high technology involving various subjects.Many CAD/CAM systems have designed.However,they have some disavantages and a high cost.Therefore,only the system with proprietary intellectual property can solve this problem.OBJECTIVE:To optimize the connector of the CAD/CAM fixed bridge.METHODS:Three-dimensional finite element models of the connector were established which was used to design different styles of connectors.Stress under different vertical loading conditions was compared.RESULTS AND CONCLUSION:The connectors with different appearances displayed varying stress.Stress on the connector with arc transition was less than connector without arc transition.Connector was supposed to design to style with arc transition.

Objective To investigate the combination of compound anisodine and physical therapy in the treatment of amblyopia,and its possible mechanism.Methods Totally 300 outpatients with amblyopia(3 to 14 years old) were randomly and equally divided into treatment group and control group.In the treatment group,subcutaneous injection of compound anisodine(2 ml,once per day) to the superficial temporal artery of eye was given for a course of 14 d and followed by another course after 5 days'interval.Cover treatment was carried out at the same time.The control group was only treated with physical therapy.Vision,central retinal artery peak systolic velocity(PSV) and diastolic resistance index(RI) were measured during the 3 months' follow-up.Results The 3 to 6-year-old efficiency was 75.0%,7 to 9-year-old efficiency was 69.6%,and 10 to 14-year-old efficiency was 61.1% ;The difference of efficiency between therapy group and control group was very significant(P0.05).Conclusion Compound anisodine plus physical therapy for amblyopia at different ages and varying degrees are effective and safe.The mechanism may be due to enhanced retinal blood supply.

Objective To examine the expression of CD8+CD28- T cells in the peripheral blood and explore their roles in the pathogenesis of AS. Methods The expression of CD8+CD28- T cells obtained from 50 AS patients and 21 healthy controls were assayed by flow cytometry. CRP was also measured. Results The percentage of peripheral blood CD8+CD28- T cells in patients with AS was significantly increased compared to normal individuals [(18±6)% vs (14±5)%, P=0.020], while the percentage of peripheral blood CD3+,CD8+ CD28+ T cells in patients with AS was significantly decreased[ (65±9)% vs (69±8)%, P=0.039]; [ (15±5)% vs(18±4)%, P=0.038]. No difference was found in CD8+ T cells between patients with AS and normal individuals(P＞0.05). Conclusion The percentage of peripheral blood CD8+CD28- T cells in patients with AS is significantly increased comparing to normal individuals. This suggests that CD8+CD28- T cells may play an important role in the pathogenesis of AS.

Objective To explore the effect of the teacher-student co-construction of skills training team in teaching of nursing skills for higher vocational nursing students. Methods About 129 students from grade 12 were enrolled in the control group, where the traditional teaching method was used. Another 136 nursing students from grade 13 were enrolled in the experiment group, where the teacher-student co-construction of skills training team was used. The two groups were compared in terms of test scores on nursing skills . Result The scores of the nursing skill in the experiment group were significantly higher than those of the control group (P < 0.05). Conclusion The implementation of teacher-student co-construction of nursing skills training team can improve the nursing skills of the nursing students, motivate students' initiative and creativity and enhance their sense of responsibility and other comprehensive quality.

ObjectiveTo find the proper way of assessing the image noise characteristic through the analysis of noise characteristic in the digital mammographic images.MethodsThe CDMAM phantom was firstly exposed with different filter materials and tube voltages in the range of 23 to 35 kVp at a fixed tube current 90 mAs,then exposed with different filter materials and tube current in the range of 20 to 200 mAs at a fixed tube voltage 29 kVp.The parameters such as background standard deviation,joint standard deviation,subtracted standard deviation and relative standard deviation were calculated and measured from both 60 pixels of signal disc and background beside of the disk for all the images.The IQFinvs were calculated for the achieved images.Non-parametric rank sum tests were used to compare the difference of the background standard deviation,joint standard deviation and subtracted standard deviation,t test was used to compare the IQFinvs between Mo and Rh filers in each group.ResultsWith the same exposure parameters in the test above,the maximum value was the background standard deviation,while the subtracted standard deviation was the minimum,and the value of joint standard deviation was in the middle. With other parameters unchanged,the values of three kind of standard deviation increased with the increasing of tube voltage or tube current,while the differences between them getting more significant.Under the condition of 90 mAs and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 10.91,10.34 and 9.87,the results were 11.24,10.57 and 9.96 while Rh filter material was used,significant difference was found ( x2 =26.0,P ＜ 0.01 ). Under the condition of 29 kVp and Mo filter material,background standard deviation,joint standard deviation,subtracted standard deviation were 12.30,11.61 and 11.05,the results were 12.29,11.58 and 10.87 while Rh filter material was used,significant difference was found( x2 =20.0,P ＜ 0.01 ).The IQFinvs were 149.14 ± 23.87 and 139.16 ± 23.14 under the condition of 90 mAs and Mo filter material ( t =9.129,P ＜ 0.01 ).The IQFinvs were 149.75 ±34.85 and 145.2 ±34.64 while the parameters were 29 kVp and Rh filter material(t =1.304,P ＞0.05 ).The value of relative standard deviation decreased with the increasing of tube voltage or tube current,and the image quality got better with the decreasing of relative standard deviation.ConclusionRelative standard deviation is suitable not only for the expression of the digital mammographic images,but also for the evaluation of the image quality.

AIM:To observe the expression of nuclear factor-kappa B ( NF-κB) , N-methyl-D-aspartic acid re-ceptor 2B (NR2B) and inducible nitric oxide synthase (iNOS) in the spinal cord in a rat model of chronic constriction in-jury (CCI) of the sciatic nerve.METHODS:Fifty-six adult male Sprague-Dawley rats weighing 180~220 g were random-ly divided into sham group (n=8) and CCI group (n=48).The mechanical withdrawal threshold (MWT) and paw with-drawal latency (PWL) of the hind paws were measured 1 d before CCI and 1 d, 4 d, 7 d, 14 d and 21 d after surgery.The L4~L6 segment of the spinal cord was taken for determining the expression of NF-κB, NR2B and iNOS by RT-PCR and Western blotting.RESULTS:At 1 d, 4 d, 7 d, 14 d and 21 d after surgery, the MWT and PWL in CCI group were obviously lower than those in sham group .The expression of NF-κB, NR2B and iNOS at mRNA and protein levels in-creased significantly.Positive correlations were found between the mRNA expression of NF-κB and iNOS (r=0.842, P<0.05), and between the mRNA expression of NR2B and iNOS (r=0.833, P<0.05).CONCLUSION:The generation and maintenance of hyperalgesia in sciatic nerve injury rats may attribute to the activation of NF -κB and NR2B and concom-itant increase in iNOS .

Objective To observe the histological change of different waves in treating SD rats of the long-pulse 1064nm Nd:YAG laser and the 560～1200 nm intense pulse light,in order to provide the theory bases of non-ablative rejuvenation.Methods Two waves were used on experimental mice.The dermic thickness and the expression of collagen typesⅠand Ⅲwere detected by HE stain and immunohistochemical methods. Semiquantitative analysis was used to determine the mean of absorbance.Results Thedermal thicknesses and the mean of absorbance of collagen typesⅠandⅢin two different waves were higher than those in common control groups(P＜0.05).The effect of Nd:YAG laser groups were higher than IPL groups(P＜0.05).The expression of collagen typeⅠwas higher than that of collagen type Ⅲ(P＜0.001).Conclusion After Nd:YAG laser or IPL irradiation,the dermal thickness and collagen typesⅠandⅢof SD rats are increased.The effects of Nd:YAG laser are better than those of 560～1 200 nm IPL.The expression of collagen type Ⅲ is obviously more than that of collagen typeⅠin the early,whereas the expression of collagen typeⅠis obviously more than that of collagen type Ⅲin the later.It proves that the mechanism of dermal remodeling of non-ablative skin rejuvenation is mainly correlation with raising range and time of collagen typeⅠ.

Objective To analyze the pathologic and biology features and prognosis of HER-2,ER and PR negative breast cancer(TNBC).Methods Clinic pathologic data of 329 breast cancer patients was retrospectively analyzed. The expression of HER-2, ER, PR and P53 was determined by immunohistochemistry. The patients were divided into three groups, including TN group [ HER-2 (-), ER (-), PR (-)], HER-2 group [HER-2(+),ER(-),PR(-)] and HR group [ER(+),PR(-) or (+),HER-2(-) or (+) ].The pathology and biology features and P53 masculine expression of the three groups were compared.The 5-year overall survival and disease free survival were analyzed by Kaplan-Meier method.Results Of the 329 patients, 20.97% (69/329) was TN group, 34.04% (112/329)was HER-2 group, and 44.98% (148/329) was HR group.The percentage of with lymph node metastasis in TN group 55.07% (38/69) was higher than that in HER-2 group 42.86% (48/112) and HR group 43.24% (64/148) (χ2 = 12.57, P ＜ 0.05).The rates of P53 positive, operation recurrence and metastasis in TN group were 44.93%, 27.54% and 20.29%, which were higher than that in HER-2 group (20.54% ,16.07%,16.96%) and HR group (18.24%, 12.84%, 10.81%) (χ2=12.23, 8.36, P ＜0.05).The ratio of tumor ≥5cm, Ⅲ stages, Ⅲ grade and soakage canula cancer among three groups had statistical difference (χ2 = 7.25,8.79,9.23,8.48, P ＜ 0.05).The 5-year overall survival in three groups were 75.36%,82.14% ,85.14% and disease free survival were 68.12% ,78.57% ,82.43% (χ2 =8.52, P ＜0.05).Conclusion The pathology and biology traits of TNBC were high rate of P53 (+) and recurrence and lymph node metastasis.The most important factor for poor prognosis of TNBC was the low rate of disease free survival for 5 years.

Objective To investigate the prevalence of fecal incontinence (FI) among adult women in Beijing area, and to analysis risk factors associated with FI. Methods A stratified multiple-cluster systemic method was used to recruit women ≥ 20 years old in 6 districts of Beijing. A self-administrated questionnaire was used to collect information about Fl and possible risk factors. Results There were three thousand and 58 women participated in the survey, the average age was (48 ± 16 ) years (range 20 -79 years). The prevalence of FI was 1.28% ( 39/3058 ), which was related to age. There were five factors entered the logistic regress model, which were included age ≥40 years old ( OR = 3.3, 95% CI: 1.7 -6. 8), urinary incontinence ( OR = 3.0, 95% CI: 1.5 - 6. 1 ), vaginal delivery ( OR = 2.4, 95% CI:1.2 -4. 9), household per capita income ≤2000 RMB per month ( OR = 3. 3, 95 % CI: 1.6 - 6. 5 ), and feeling fatigue ( OR = 3.0, 95% CI: 1.5 - 5.8). Conclusion Prevalence of FI is low among adult women in Beijing area, while risk factors associated with Fl are complicated and further studies are necessary to be conducted.
