1.Construction of thesis-quality assurance system for postgraduates with specialized master degree of public health

This paper analyzed the main problems in the thesis of postgraduates with specialized master degree of public health.The problems existed were the poor quality of students,untargeted topics of the thesis and unsound evaluation system,etc.Specific measures were proposed for the construction of quality assurance system for postgraduates with specialized master degree of public health including four focus management,multiple evaluation system and post thesis evaluation.
2.Arthroplasty versus joint preservation for displaced 3-and 4-part proximal humeral fractures:a meta-analysis

BACKGROUND:Currently, the treatment of proximal humeral fractures mainly contains joint preservation (conservative treatment, open reduction and internal fixation) and arthroplasty, but how to choose the treatment is stil controversial. OBJECTIVE:To evaluate and compare the clinical outcomes of joint preservation versus arthroplasty in the treatment of displaced 3-or 4-part humeral fractures in randomized control ed trials using meta-analysis. METHODS:Medline (January 1966 to December 2013), PubMed (January 1980 to December 2013), Embase (January 1990 to December 2013), Science (January 1990 to December 2013), Springer (January 1990 to December 2013), China National Knowledge Infrastructure (1994 to 2013), and Wanfang database (1982 to 2013) were searched for randomized control ed trials addressing joint preservation and arthroplasty for 3-or 4-part proximal humeral fractures. Articles meeting the inclusion criteria were included. The related data were extracted and loaded onto Comprehensive Meta-Analysis Software for meta-analysis. RESULTS AND CONCLUSION:Seven articles with 320 patients (165 patients undergoing joint preservation and 155 patients receiving arthroplasty) were accepted in this mete-analysis and al of them were high-quality English researches by modified Jadad Scale. Meta-analysis results displayed that the random-effects mean Constant score across al types was 55.9 (95%CI:50.7-61.1;P<0.001). Constant score was higher in the joint preservation group than in the arthroplasty group (P<0.01). The study displayed significant heterogeneity (I2 0.001). In the meta-analysis, Constant scores were decreased with increasing rate of male to female and rate of tuberosity resorption. These results suggested that 3-or 4-part proximal humeral fractures demonstrate improved Constant scores when treated with joint-preserving options compared with arthroplasty. Moreover, age, fracture pattern, gender ratio and complication rate are significant predictors of the Constant score. Given the observed heterogeneity and variance in treatment techniques in the included studies, more randomized control ed trial studies are needed to definitively recommend joint-preserving techniques versus arthroplasty for treatment of 3-or 4-part proximal humeral fractures.=88%, Q statistic=107.6, Q=13;P<0.001). 3.Characteristics,diagnosis and treatment of interdigitating dendritic cell sarcoma

Interdigitating dendritic cell sarcoma (IDCS)is a rare malignant tumor of the dendritic cell, derived from the hematopoietic tissue.The major clinical manifestation of IDCS is superficial lymphadenopathy, and the enlarged lymph nodes may appear in some atypical ereas,such as the lung,kidney,bladder and the pleura,etc.With the development of the pathological diagnosis and the application of immunohistochemical staining and electron microscopes,the case detection rate is apparently improved.With the high degree of malignant,rapid progress and poor prognosis of the disease,currently,surgical therapy is still the main approach to the treatment of IDCS.
4.Indwelling transpulmonary artery thrombolytic therapy for acute pulmonary embolism

Objective To evaluate indwelling intrapulmonary artery catheter thromolysis for acute pulmonary embolism.Methods From June 2011 to June 2013 56 cases of acute pulmonary embolism were diagnosed by multi-row spiral CT and admitted at the Department of Vascular Surgery.The average age was (56 ± 11) years.Inferior vena cava filter was implanted and pulmonary angiography,catheter thrombectomy,indwelling pulmonary intraarterial thrombolytic therapy was performed.Postoperatively low molecular weight heparin and warfarin was given,dosage adjusted by prothrombin international normalized ratio at 2-3.Results Mean pulmonary arterial pressure (mPAP) decreased from (43 ± 7) mmHg to (22 ± 6) mmHg (P < 0.05),arterial partial pressure of oxygen (PO2) rose from (49 ± 8) mmHg to (83 ± 9) mmHg (P < 0.05).Clinical symptoms significantly relieved in 51 out of the 56 cases (91%).45 patients were followed up for an average of (15 ± 4) months,with one recurrence.There was no filter migration,vena cava thrombosis,chronic obstructive pulmonary disease and other complications.Conclusions Emergency pulmonary artery indwelling catheter thrombolysis is safe and effective therapy for acute pulmonary thromboembolism.
6.Biomechanical evaluation of dynamic hip screw with bone cement augmentation in normal bone

BACKGROUND: Dynamic hip screw (DHS) is a standard internal fixation for intertrochanteric fracture, whereas the patient combined with osteoporosis, cut-out incidence of lag screw is common. The articles in China and abroad indicate bone cement augmentation of DHS to achieve firm fixation. As for normal bone, no reports is published that whether bone cement augmentation is effective.OBJECTIVE: To investigate the biomechanics of DHS with bone cement augmentation for the fixation of intertrochanteric fracture specimen that has a normal bone density.DESIGN, TIME AND SETTING: Bilateral contrast observation study of the same sample was performed in the Laboratory of Biomechanics, Hebei Orthopaedic Research Institute (Shijiazhuang, Hebei Province, China) between March and April In 2005.MATERIALS: Bilateral upper femora from the embalmed male cadaver were provided by Anatomy Department of Hebei Medical University (China). X-ray scan results proved the absence of tuberculosis, anatomical deformity and tumor.METHODS: Twenty-four matched pairs of the upper femora (48 sides) were used to make the specimens of the intertrochanteric fracture of type A2. The right femur specimens were fixed with DHS augmented by bone cement, as the augmentation group (The screw track of femoral neck was expended by curette, and the femoral head facing upwards were injected with 2mL low viscosity bone cement. Then lag screw was wrested to keep the position unchanged till the bone cement coagulated. Placing barrel, compressing through tighten tail screw, and cortical screw fixing side-plate were. followed). And the left femur specimen was fixed with DHS conventional fixation, as the control group. The bending and torsional tests were performed in the two groups.MAIN OUTCOME MEASURES: The maximum load and the maximum torque in the two groupsRESULTS: The maximum load and the maximum torque were (3852.1602±143.6031) N and (15.5+2.6) Nm in the augmentation group, and (3702.9667±133.8601) N and (14.7±3.4) Nm in the control group. There was no significant difference in the biomechanical effects between the two groups (P>0.05).CONCLUSION: The augmenting fixation with bone cement for intertrochanteric fracture specimen has no significant effect on the strength of DHS fixation or on the overall stability of the fractured bone in normal bone density.
7.Analysis the characteristic of early myocardial ischemia with functional tomoscintigraphy

Objective To observe the characteristic changes in cardiac function and to raise diagnostic accuracy and sensibility.Methods 39 silent myocardial ischemia and 58 angina and 46 age-matched healthy subjects and 62 obsolete myocardial infarction underwent myocardial tomography imaging and gated ventricular imaging.Covariance analysis was used to compare difference between SPECT imaging and parameters of function.The age was tested as covariate factor,then SNK-Q test and multiple comparison were carried on.Results The silent myocardial ischemia and angina showed reversible radioactive defect or fixed defect.Most of myocardial SMI showed single peak exempt TPFR.EF,PFR and PER showed remarkably statistical difference (P<0.01).The multiple comparison of PFR and PER was increased according by OMI,AN,SMI and negative control group (P<0.05).Conclusions The myocardial damage and PFR,PER in SMI is lower than that in AN group,which have no remarkably influence on the parameters of diastolic and retractile function in left ventricular.
8.Research of radionuclide brain blood perfusion imaging toward the clinical diagnosis of in tractable paroxysmal epilepsy

Objective To evaluate the clinical diagnosis value of 99mTc-single photon emission computed tomography(SPECT)brain blood perfusion imaging in intractable paroxysmal epilepsy and analyse the imaging character.Methods Two hundred and thirty-eight intractable paroxysmal epilepsy patients performed99mTc-SPECT,MRI,CT,and electroencephalogram(EEG).The results of examinations were ana lyzed and compared.Results The positive rate of epileptic focus in brain SPECT blood perfusion imaging in the generalized tonic-clonic seizure,absence seizure,partial seizure and complex partial seizure were 85.7%,95.7%,89.8%and 90.9%,the positive rate were much higher than those in other methods(P<0.01).The 23 patients with SPECT focus were carried on a-γ-knife treatment with follow-up for more than one year,and the efficient rate was 78.26%.Conclusions The detection sentivity of SPECT is distinctively higher than that in others.SPECT has an important clinical value to define target region of the stereotaxic radio surgery.
9.Umbilical cord blood mesenchymal stem cell transplantation for Parkinson’s disease:a feasibility study

BACKGROUND:Stem cel s can be induced to differentiate into dopaminergic neurons in vivo and in vitro, which provides a theoretical basis for stem cel transplantation in the treatment of Parkinson’s disease
OBJECTIVE:To explore the feasibility and mechanism of intracerebral transplantation of umbilical cord blood mesenchymal stem cel s for treatment of Parkinson’s disease rats.
METHODS:Intracerebral injection of 6-hydroxydopamine was used to make Parkinson’s disease models in SD rats. Twenty-two model rats were randomized into cel transplantation group (n=12) and control group (n=10) and respectively injected intracerebral y with umbilical cord blood mesenchymal stem cel suspension and PBS. At 1-8 weeks after cel transplantation, intra-abdominal injection of apomorphine was performed every week to observe the rotation behaviors of rats;at the 2nd and 8th weeks, rat’s striatum and substantia nigra were taken for immunohistochemistry staining.
RESULTS AND CONCLUSION:The rotation behaviors were gradual y decreased with time in the cel transplantation, but had no changes in the control group. At 3-8 weeks after transplantation, there were significant differences in the rotation behaviors between the two groups (P<0.05). At 2 weeks after transplantation, tyrosine hydroxylase-positive cel s were found within and around the striatum of the cel transplantation group;but there were no exogenous cel s in the control group. At 8 weeks after transplantation, there were stil active cel s and tyrosine hydroxylase-positive cel s in the striatum of cel transplantation group, and there was no tyrosine hydroxylase expression in the striatum of the control group. These findings suggest that transplanted umbilical cord blood mesenchymal stem cel s can survive in the brain that are positive for tyrosine hydroxylase, which can improve the behavior abnormalities of Parkinson’s disease rats.
10.A pilot study for decellularized human umbilical arteries as small-diameter vascular grafts

Objective Exogenous small-diameter vascular grafts have been developed with tissue-engineered small-di-ameter vascular grafts for the reconstructive surgery in the treatment of patients with coronary artery diseases in whom restenosis of the initially transplanted autografts occurred.This study was conducted to develop decellularized umbilical arteries (HUAs) and evaluate their physical and mechanical properties,as well as to assess the usefulness of decellularized HUAs in the coronary artery bypass grafting procedures.Methods After HUAs were harvested,their inner diameters were measured with a sliding caliper and the blasting pressure was measured with a pressure gauge.HUAs were firstly infused with a compound liquid consisted of 0.25% Trypsin and 0.01% Ethylene Diamine tetraacetic Acid(EDTA) and then with 1% Sodium Lauryal Sulfate (SDS).Fragments of the vessels were collected and observed under light microscope and electron microscope.The mechanical characteristics of HUAs were identified with an electronic experiment machine before and after decellularization.Fibroblasts and endothelial progenitor cells growing along the lining of decelluarized human umbilical arteries(dHUAs) could be observed.Results The average inner diameter of HUAs was ( 3.50 ± 0.55 ) mm.Two of thirty HUAs were broken at a pressure of 300 mm Hg.The process for the lining of HUAs to be decellularized with a peristaltic pump took 0.5 hour with 0.25% Trypsin and 0.01% EDTA and 3 hours with 1% SDS.Observations with light microscope and electron microscope demonstrated that all of the original lining cells were decellularized.The ultimate stress of the HUAs did not change significantly after decellularization ( P >0.05 ).Attached fibroblasts and endothelial progenitor cells could be seen along the decellular lining of the grafts and the ultimate stress of the HUAs did not change significantly after implantation of the two kinds of cells.Conclusion dHUAs,with good histocompatibility and properties mentioned above,may be used as potential vascular grafts in CABG.
