2.Preventive effect on menstrual migraine treated with subgaleal acupoint injection with metoclopramide.

OBJECTIVETo observe the differences in clinical effect on menstrual migraine between subgaleal acupoint injection with metoclopramide and oral administration of medication.
METHODSSixty-four patients of menstrual migraine were randomized into a subgaleal acupoint injection group and a medication group, 32 cases in each one. In the subgaleal acupoint injection group, the acupoint injection started 10 days before menstruation. Eight acupoints on the head were selected and injected alternatively in two groups, once every 2 days, 4 treatments made one session and 3 sessions (3 menstrual cycles) were required. In the medication group, flunarizine was applied with oral administration, 2.5 mg each time, once every night. The duration of treatment was 3 months. The analgesic effect, frequency and time of pain attack were observed in 3 and 6 months after treatment in the two groups.
RESULTSAfter treatment, VAS scores were lower than those before treatment in the two groups (P<0.01, P<0.05), the attack frequency and pain time were all reduced as compared with those before treatment (all P<0.01). After treatment, VAS score, the frequency and time of pain attack in the subgaleal acupoint injection group were improved significantly as compared with the medication group (all P<0.01).
CONCLUSIONThe subgaleal acupoint injection with metoclopramide achieves the superior preventive effect in clinical treatment of menstrual migraine as compared with flunarizine.

4.Clinical application of arterial embolization for the treatment of uterine leiomyoma

Objective To explore the curative effects, side effects and complications of arterial embolization for the treatment of uterine leiomyoma.Methods Bilateral uterine arterial embolization with silk particles of 350~500 ?m was performed in 16 cases of uterine leiomyomas with uterine hemorrhage as the main complaint.Results 15 patients underwent technically successful embolization. No serious complications occurred. The patients were followed up for 3~12 months. Menses returned to regular cycle. In the patients with anemia, the hemoglobin concentration rose up to normal level. The sizes of uterine leiomyoma decreased by 52% after 6 months of the treatment.Conclusions Uterine arterial embolization is an effective and less invasive therapy with no serious complications for patients with uterine leiomyoma, especially for presentation of the uterus.

6.A Retrospective Comparative Study Between Transumbilical Single-incision and Traditional Three-port Laparoscopic ;Appendectomy

Objective To discuss the safety and feasibility of single-incision laparoscopic appendectomy ( SILA ) . Methods A retrospective analysis was made on clinical data of patients diagnosed as having appendicitis by clinical manifestations , ultrasound or computer tomography at our department from June 2009 to December 2014.There were 189 cases of transumbilical SILA ( transumbilical group ) and 123 cases of traditional three-port laparoscopic appendectomy ( three-port group ) .Patients with incomplete records, combination with other surgery , pregnancy appendicitis , acute appendicitis over 72 hours were excluded .Intraoperative and postoperative data of two groups were compared . Results No conversion to open surgery was required in both groups .The operation time, postoperative exhaust time , and postoperative hospital stay had no significant differences between the transumbilical group and the three-port group [(45.5 ±12.7) min vs.(46.3 ±17.8) min, t=-0.461, P=0.650; (1.4 ±0.5) d vs.(1.5 ±0.6) d, t=-1.588, P=0.112;(3.8 ±1.6) d vs.(4.1 ±1.9) d, t=-1.554, P=0.121].The cosmetic scores at 3 months after surgery was higher in the transumbilical group than that in the three-port group [(4.5 ±0.6) points vs.(4.2 ±0.5) points, t=4.585, P=0.000].No significant differences were identified in the incidence of complications and pathological types between the two groups . Conclusion SILA is technically feasible and safe with better cosmetic effects .
7.Modified method for umbilical reconstruction after single-incision laparoscopic surgery

Objective:To introduce a modified method for umbilical reconstruction after single-incision laparoscopic surgery.Methods:Between June 2012 and June 2015,173 patients underwent single-incision laparoscopic cholecystectomy at our institution.All the patients adopted our modified method for umbilical reconstruction.Results:All the surgeries were successful.The umbilical incision healed well with good cosmetic result.Conclusion:The modified method for umbilical reconstruction can improve cosmesis and decrease the possibility of incision hernia after single-incision laparoscopic surgery.More studies are needed to evaluate definitive clinical advantages of this technique.

; Objective; To observe the preventive and therapeutic effects of Jieduquyuziyin methods in treating corticosteroid osteoporosis in systemic lupus erythematosus (SLE) and investigate into its mechanism. Method; 147 cases of SLE were randomly divided into the integrated traditional Chinese and western medicine group (79 cases) and the western medicine group (68 cases). There 21 cases which were diagnosed primarily among all 147 cases. 30 healthy cases were taken as the normal control group. The western medicine group (68 cases) was treated mainly with prednisone only, while the integrated traditional Chinese and western medicine group (79 cases) with Jieduquyuyiyin methods on the basis of normal western medicine. Both groups were treated for 3 months as a course. 16 cases in the western medicine group and 25 cases in the integrated traditional Chinese and western medicine group were kept on being treated and observed for 2 years. The lever of serum Ca, P, ALP, BGP, IPTH, 25-(OH) D3 and urinary Ca, P and BMD were checked and observed. 10.Mastoidectomy in the treatment of secretory otitis media.

OBJECTIVE:
To investigate mastoidectomy efficacy in treating secretory otitis media.
METHOD:
Retrospective analysis of 22 cases (24 ears) with chronic secretory otitis media,20 ears were treated with intact canal wall mastoidectomy combined with facial recess opening,4 ears were treated with opened mastoid surgery,3 ears simultaneously accepted tube insertion. Ventilation tube was pulled out in 6 months. Hearing test was inspected before and after surgery.
RESULT:
None of the patients had hearing loss, 19 ears had varying degrees of hearing improvement. Seventeen ears were type A tympanometry curve, 7 ears were C-shaped curve. No recurrence of otitis media was observed after 6 - 36 months followed-up.
CONCLUSION
Mastoidectomy may improve eustachian tube function and decrease the risk of recurrence of secretory otitis media.
