1.Comparative study on daily operation of inguinal hernia,traditional hernia repair and Liechtenstein hernia repair

Objective Summarize anterior open tension-free inguinal hemiorrhaphy, especially application experience and understanding in the 952 cases of day surgery, and clarify the advantages. Method During December 2004 - June 2007, we treated 952 Patients of inguinal hernia and femoral hernia,using local anesthesia, tension-free, in the form of day surgery. And the resulte were compare to traditional hemiorrhaphy, tension-free hemiorrhaphy of hospital in all aspects. Results Patients in this group were all cured.Intraoperative and postoperative pain was mild.All patients should be used only a small amount of postoperative oral analgesics, and no urinary retention. They got out of bed immediately after surgery, and they recorered fast ,with less complications. They could be discharged home in surgical day (2 h later). Af ter 18 months follow-up, only 2 cases recurrence. Conclusions (1) Compared with the traditional hemiorrhaphy and ambulatory tension-free hemiorrhaphy, tension-free during the day has lots of advantages, such as easier method, wider surgical indications, faster postoperative recovery, shorter hospital stay, less complications and lower recurrence rate;(2)Tension-free hernion'haphy day surgery is safe,feasible,and has obvi-ous advantages.
2.Nosocomial Infection Problems Found in Terminal Patients Records Inspection

OBJECTIVE To check the defects of nosocomial infections reports,recording of the patient′s history,and antibiotics usage,so as to improve hospital management and reduce nosocomial infection.METHODS Patient′s records from Jan 1,2008 to Dec 31,2008 were reviewed to find out the defects and feedback to the physicians.RESULTS Some problems of 5995 cases were checked out and corrected,some suggestions were made.CONCLUSIONS Checking the patient′s records may supplement the defects of prospective monitor,direct the antibiotics therapy,and monitor occurrence of drug resistance.
3.Analysis of the causes of 92 medical disputes over prosthodontic treatments

Objective To find out the occurrence patterns of medical disputes over prosthodontic treatments through surveys and analyses so as to provide basis for the scientific prevention of such occurrences.Methods A statistical analysis was made of the causes of 92 medical disputes over prosthodontic treatments that occurred in the past three years,the data were processed using self-made tables,and the occurrence rates of the disputes were calculated according to their causes.Results The disputes over prosthodontic treatments in the past three years accounted for 22% of the total number of medical disputes over prosthodontic treatments.The cause leading to the most disputes involved medical quality,followed successively by service management,service manners,personal factors on the part of the patients,and charges.Conclusion There are certain patterns for the occurrence of medical disputes over prosthodontic treatments,which can be avoided to some extent if appropriate preventive measures are taken.
4.Effect of hypoxia combined with exercise training on mitochondrial con-tent in skeletal muscle of rats

[ABSTRACT]AIM:Toinvestigatetheeffectofhypoxiacombinedwithexercisetrainingonmitochondrialcon-tent, and the role of mitochondrial biogenesis and mitophagy in this process .METHODS:Male Sprague-Dawley rats were randomly divided into 4 groups:normoxia control (NC) group, normoxia+training (NT) group, hypoxia+control (HC) group, and hypoxia+training (HT) group.The hypoxic animals were housed in normobaric hypoxic tent (11.3 % oxy-gen) for consecutive 4 weeks.The exercise training animals were exercised on a motor-driven rodent treadmill (5°) at a speed of 15 m/min, 60 min/d, 5 d/week for 4 weeks.Mitochondrial membrane potential was determined using JC-1 fluo-rescent probe .ATP synthesis capacity was determined using a bioluminescence technique .The protein expression of cyto-chrome C oxidase IV (COXIV), voltage-dependent anion channel-1 (VDAC-1), peroxisome proliferator-activated receptor gamma cofactor 1 alpha (PGC-1α), mitochondrial transcription factor A (Tfam),Bcl-2/adenovirus E1B 19 kD-interacting protein 3 (Bnip3) and beclin-1 in the muscles was detected by Western blotting .RESULTS:Compared with NC group, hypoxia attenuated mitochondrial membrane potential , ATP synthesis capacity , and the expression of COXIV , VDAC-1, PGC-1αand Tfam.Furthermore, hypoxia increased the expression of Bnip 3 and beclin-1.Compared with HC group , the exercise training elevated mitochondrial membrane potential , ATP synthesis capacity , and the expression of COXIV , VDAC-1, PGC-1α, Tfam, Bnip3 and beclin-1.CONCLUSION:A combination of reduced mitochondrial biogenesis and increased mitophagy seems to be responsible for the decrease in mitochondrial content after hypoxia .Exercise training with hypoxia elevates mitochondrial content and function in hypoxia , which may be mediated by appropriate increase and co-reg-ulation of mitochondrial biogenesis and mitophagy .
5.Treatment of teenage varicocele by transumbilical mini pure single-port laparoscopic Palomo's surgery: a report of 78 cases

Objective To evaluate the clinical superiority of transumbilical mini pure single-port laparoscopic Palomo's surgery on the treatment of primary teenage varicocele.Methods All operations were successfully completed using mini pure single-port laparoscopic Palomo's surgery.Its clinical effects were evaluated by the operative time,blood loss volume,intra-or post-operative complications,and hospital stay.Six to eighteen months after surgery,follow-up was made in 78 patients.Results All the operations were successful.The operation time was 5-15 (mean 8) minutes,the intra-operative loss blood was 1-5 (mean 1.5) mL,and the post-operative hospital stay was 2-4 (mean 3) days.No intra-operative or post-operative complications happened.Sperm analysis of 95% patients came back to normal in 6-18 months.Conclusions The transumbilical mini pure single-port laparoscopic Palomo's technique has more obvious superiority and it is worthy of clinical application and generalized widely.
6.Effects of short-term high-intensity interval training on ventricular remodeling and mitochondrial content in rats after myocardial infarction

BACKGROUND:It is unclear whether short-term high-intensity interval training (HIT) can be used to protect against myocardium injury after acute myocardial infarction, as wel as the underlying mechanism.
OBJECTIVE:To observe the effects of short-term HIT on the ventricular remodeling and mitochondrial content after acute myocardial infarction, and the biological effect of mitochondrial biogenesis and autophagy in this process.
METHODS:Male Sprague-Dawley rats were modeled into acute myocardial infarction by ligating coronary artery. One week later, HIT was performed:each interval consisted of 4-minute high-intensity running (80%of maximal oxygen consumption) and 3-minute active recovery (40%of maximal oxygen consumption), for 4 consecutive weeks of 5 days each week, repeated 7 cycles.
RESULTS AND CONCLUSION:Four-week HIT after acute myocardial infarction could markedly enhance left ventricular pump function and mitochondrial content, improve mitochondrial membrane potential and ATP synthetic activity, inhibit mitochondrial reactive oxygen species generation, up-regulate PGC-1α/Tfam induced mitochondrial biogenesis and Bnip3/Beclin-1 induced autophagy. These results indicate that short-term HIT can improve normal mitochondrial content after acute myocardial infarction, which in turn ameliorates myocardial systolic property and energy metabolism. As a cardiac rehabilitation method, HIT exhibits fine timeliness.
7.Effect of different type of exercise on mitochondrial homeostasis in rats with myocardial infarction

Objective To investigated the different effect of moderate-intensity continuous training (MCT) and high-intensity interval exercise training (HIT) on ventricular remodeling and mitochondrial homeostasis after acute myocardial infarction (AMI).Methods The AMI rat model was achieved by ligating coronary artery.The AMI and sham operation rats were randomly (random number) divided into four groups:sham operation group (Sham),AMI control group (AMI),AMI MCT group (AMI + M),and AMI HIT group (AMI + H).Animals in the AMI + M and AMI + H groups underwent 4 weeks MCT and HIT respectively.Five weeks after AMI,hemodynamic changes,mitochondrial bioenergetics,and PINK1,Beclinl,Mfn2,Drp1,Tfam,COXⅣ,PGC-1α were detected.Results Comparing with AMI group,in AMI + M and AMI + H groups,Beclin1,PINK1,Mfn2 and PGC-1α expression elevated significantly (P <0.05 or P <0.01),whereas ROS generation and Drp1 expression showed dramatic decrease (P < 0.05 or P<0.01).In addition,in AMI + H group,±dp/dt max,mitochondrial membrane potential,ATP synthesis activity,Tfam and COXⅣ expression improved significantly (P < 0.05).Comparing with AMI + M group,in AMI + H group,± dp/dt max,PGC-1α,Tfam and COX Ⅳ expression improved significantly (P < 0.05).Conclusions HIT is superior to MCT for ameliorating ventricular remodeling and mitochondrial homeostasis after AMI.
8.Percutaneous kyphoplasty versus percutaneous vertebroplasty for osteoporotic vertebral compression fractures:a randomized comparison

BACKGROUND: Recent literatures have showed that percutaneous kyphoplasty can effectively avoid nerve damage, pulmonary embolism, and insufficient vertebral height and other security risks when bone cement is infused into affected vertebrae in percutaneous vertebroplasty. OBJECTIVE: To compare the effect of percutaneous kyphoplasty and percutaneous vertebroplasty in repair of osteoporotic vertebral compression fractures. METHODS: A total of 106 patients with senile osteoporotic vertebral compression fractures were randomly divided into trial group and control group (n=53 per group). Patients in the trial group were treated with percutaneous kyphoplasty, and those in the control group treated with percutaneous vertebroplasty. Al patients were fol owed up for 6 months after repair. The vertebral compression deformation, bone cement distribution, midline vertebral bone cement condition, vertebral height restoration, bone cement leakage, vertebral kyphosis, progressive spinal col apse, nerve damage, as wel as visual analog scale scores and Oswestry disability index scores in these two groups were compared. RESULTS AND CONCLUSION: Compared with the control group, there was less bone cement leakage and vertebral compression deformation in the trial group. Moreouer, in the trial group, bone cement distributed uniformly, vertebral height restoration was good and effective, pain was obviously relieved, and the probability of vertebral kyphosis, progressive spine col apse and nerve damage was significantly reduced (al P < 0.05). These results suggest that percutaneous kyphoplasty can effectively relieve the pain of patients with osteoporotic vertebral compression fractures, restore vertebral body height and reduce the incidence of complications, which effectively guarantees the postoperative restoration of motor function.
9.Distribution of puerarin contained in Zige freeze-dried powder injection in rat liver and kidney.

OBJECTIVETo investigate the distribution process of puerarin contained in Zige freeze-dried powder injection in rat liver and kidney and the safety of Zige freeze-dried powder injection.
METHODRats were divided into the Zige freeze-dried powder injection group and the puerarin freeze-dried powder injection control group randomly. The liver and kidney samples were collected at 5, 10, 20, 30, 45, 60 and 120 min after intravenous administration of puerarin (26.7 microg x g(-1)) through caudal vein and detected by HPLC.
RESULTThe concentration of puerarin in kidney reached the max value of 58.12 microg x g(-1) for the Zige freeze-dried powder injection group and 71.28 microg x g(-1) for the Puerarin freeze-dried powder injection group. The value of AUC(0-2h) was 26.24 microg x h x g(-1) for the Zige freeze-dried powder injection group and 35.24 microg x h x g(-1) for the puerarin freeze-dried powder injection group, MRT(0-2h) was 0. 39 h for the Zige freeze-dried powder injection group and 0. 42 h for the puerarin freeze-dried powder injection control group. Compared with the control group, the Zige freeze-dried powder injection group showed a significant decrease in Cmax and AUC(0-2h) (P < 0.05), with no notable difference in peak time tmax and MRT(0-2h). The two groups showed no obvious difference in tmax Cmax, AUC(0-2h) and MRT(0-2h) of puerarin in rat kidney.
CONCLUSIONCompared with Zige freeze-dried powder injection, Zige freeze-dried powder injection can reduce the distribution of puerarin in rat kidney, with no obvious change in the elimination of puerarin in rat kidney. It also showed no significant change in distribution and elimination in liver. This indicates that Zige freeze-dried powder injection is safer to kidney than puerarin freeze-dried powder injection.

10.Effect of extract of selenium-enriched Astragalus membranaceus on insulin resistance in streptozotocin-induced diabetic rats

To explore the effects of extract of selenium-enriched Astragalus membranaceus(ESAM)on insulin resistance(IR)in streptozotocin(STZ)diabetic rats. In this study, type 2 diabetes mellitus(T2DM)was established; ESAM and pioglitazone were used to intervene T2DM model rats; the general condition of rats in each group was observed, and body weight and fasting blood glucose(FBG)were recorded before modeling and after modeling. At the end of the fourth week, the blood and liver tissues of each group were collected. The total cholesterol(CHOL), triglyceride(TG), high-density lipoprotein(HDL), low-density lipoprotein(LDL)in blood were detected by biochemical detector; ELISA was used to detected content changes of blood fasting insulin(FINS), tumor necrosis factor-α(TNF-α), interleukin-6(IL-6), C-reactive protein(CRP), interleukin-1β(IL-1β); Western blotting was used for the detection of insulin receptor substrate protein 1(IRS1), phosphorylated IRS1(p-IRS1), nuclear factor κB inhibitor kinase β(IKKβ), phosphorylated IKKβ(p-IKKβ), c -Jun N-terminal kinase(JNK), phosphorylated JNK(p-JNK)protein changes. Results showed that compared with the blank group, the body weight of the model group was significantly decreased. FBG, CHOL, TG, LDL, TNF-α, IL-6, CRP, IL-1β in the blood and p-IRS1, p-IKKβ, p-JNK in the liver was increased significantly; after intervention, pioglitazone, ESAM can reverse the body weight of the model rats and the changes of the above cytokines and proteins. In conclusion, ESAM can reduce the expression of inflammatory cytokines in T2DM rats, inhibit the increase of free fatty acids(FFA), decrease the activation of IKKβ and JNK phosphorylation in surrounding tissues, activate insulin pathway and improve the IR effect of T2DM.
