Objective: To explore the clinical value of four-dimensional ultrasound imaging in obstetric examination. Methods:Four-dimensional ultrasound was detected by V730 on 60 pregnant women who used ultrasonography in prenatal screening. Results:There were 60 cases in which 20 cases with limb malformation, 15 cases with facial deformity, 12 cases with spinal malformation, 8 cases with abdominal malformation, and 5 cases with chest malformation. Conclusion:Four-dimensional ultrasound can better dynamic observation of fetal surface malformations location, which can provide more worthy information for clinical diagnosis and treatment.

3.Explore Syndrome Differentiation and Treatment of Systemic Lupus Erythematosus in Different Periods of Using Glucocorticoid

Systemic lupus erythematosus is a diffuse connective tissue disease, which is mediated by autoimmune mechanisms.It results in multiple autoanti-bodies in the serum and injury organs and systems.Modern medicine uses glucocorticoid as the main medicine in treating systemic lupus erythematosus.Glu-cocorticoid is pure yang product, not only wil it injure yin and qi for using long time, but also disturb syndrome type of traditional Chinese Medicine of SLE for using it in large number.In the starting phase of using glucocorticoid, if the patients present manifestation of toxic heat flourishing, we should clear heat and remove toxicity.If the patients present manifestation of yin deficiency and internal heat, we should nourish yin and clear heat.If the patients pre-sent manifestation of stagnated heat stagnation, we should remove toxic heat and blood stasis.In reduction stage of using glucocorticoid, patients present manifestation of qi and yin deficiency.Then we should benefit qi and nourish yin. In maintenance stage of using glucocorticoid, patients present manifesta-tion of yang deficiency of spleen and kidney.So we should warm the kidney and invigorate the spleen.During the whole course of treating, we should also care for blood stasis and toxin.The clinicians should also remove toxic heat and blood stasis depend on the changing of syndrome. Using GCS in treating SLE should depend on the syndrome differentiation and choose the right Traditional Chinese Medicine in order to reach the clinical effect.
4.Influence of Treating Principles of Detoxication - Stasis Removing - Yin - Nourishing on Hemorheology in Cases of Lupus Erythematosus

By combining clinical observation and animal experiments and using FASCO - 3010 hemorheological devices to examine the relevant indices, it was found that no obvious changes can be found in the group treated with hormone before and after the treatment, whereas in the group with traditional drugs added, the RBC deformation, and specific viscosity of plasma were all markedly lowered. Comparison of rheological changes in SD rat showed that traditional Chinese drugs can markedly improve the hemorheology in such patients, indicating that these TCM treating principles can improve the concentrated, viscous, coagulated, agglutinated pathological blood status in SLE patients.
5.Effects of Jiedu Quyu Ziyin Decoction on TLR4 Signal Pathway in Lung, Spleen and Peritoneal Macrophagocyte of MRL/lpr Lupus Mice Treated by Prednisone

[Objective] To observe the effect of Jiedu Quyu Ziyin decoction on TLR4 signaling pathway in macrophages of MRL/lpr lupus mice. [Method] MRL/lpr lupus mice were divided into four groups:model group, prednisone group, Jiedu Quyu Ziyin decoction group(hereinafter referred to as:Chinese medicine group) and prednisone plus Chinese medicine group (hereinafter referred to as:combination of Chinese and western medicine group). The mice were gavaged with saline, prednisone, Jiedu Quyu Ziyin decoction and prednisone added Jiedu Quyu Ziyin decoction for 4 weeks. Macrophages of lung, peritoneal and spleen were collected and the expression of related genes was detected by RT-PCR. [Result] TLR4 mRNA in lung macrophages, and TLR4 protein in splenic macrophages increased significantly(P<0.05) after the treatment of prednisone. The increased TLR4 protein in splenic macrophages was significantly decreased by combining with Jiedu Quyu Ziyin decoction(P<0.05). Prednisone can significantly reduce the TLR4 downstream molecules such as MyD88, IFN-α, iNOS mRNA expression in lung and peritoneal macrophages(P<0.05). The decreased MyD88 mRNA in lung macrophages was increased significantly by combining with Jiedu Quyu Ziyin decoction(P<0.05). [Conclusion] TLR4 signaling pathway is changed in macrophages after glucocorticoid administration in MRL/lpr lupus mice. Jiedu Quyu Ziyin decoction can reduce the abnormal glucocorticoid-induced TLR4 protein expression.
6.Discussion on Treatment of Fire and Heat Disease by LIU Wansu Based on ‘Depression of Yang Qi’ Theory

[Objective]To explore LIU Wansu's methods in treating fire and heat disease from depression of yang qi.[Methods]To analyse LIU Wansu's understanding of fire and heat disease and highlight its treatment from depression of yang qi through reading his works like Exploration to Pathogenesis in Plain Questions,Yellow Emperor's Plain Questions on Xuanminglun Formulae and so on.[Results]LIU Wansu had profound medical knowledge,rich clinical experience and initiative spirit.Under the guidance of nineteen items of pathogenesis from Huangdi's Canon of Medicine,he expanded fire and heat disease and put up six exogenous pathogenic factors can transfer to fire theory.To be specific,excessive wind,cold,dampness and dryness would block Xuan Fu and depress yang qi,causing fire and heat disease.He regardeddepression of yang qi as a key link in six exogenous pathogenic factors' transferring to fire and put up cooling combined with ventilating method to dissipate depressed yang qi,including relieving exterior syndrome with pungent-cool drugs,clearing depressed heat,relieving exterior and interior syndrome,eliminating dampness and relieving depression as well as combination of cold and warm drugs.[Conclusion]LIU Wansu treated fire and heat disease from depression of yang qi,providing new methods in treating fire and heat disease for later generations as well as guiding significance in current clinical work.
7.Correlation between TCM Syndrome Type and Gastro-intestinal Hormones Level in Patients with Irritable Bowel Syndrome

Objective To explore the effects of Jiedu Quyu Ziyin Recipe (JQZR) on the apoptosis and expressions of bcl-2 and bax mRNA of peripheral-blood lymphocyte in MRL/lpr mice. Methods 80 MRL/lpr mice were randomly divided into model group,TCM group, Western medicine group and TCM and Western medicine group,20 mice in each group, meanwhile,20 Kunming mice were selected as normal group, then intragastrically administered normal sodium, JQZR apozem, prednisone suspension and JQZR apozem and prednisone suspension, 0. 5ml every time,once daily for 12 weeks respectively. At the end of the 12th week, peripheral-blood lymphocytes of every mice purified by gradient centrifugation were cultivated for 48 hours ,then the apoptosis was detected by flow cytometry. Furthermore,the expressions of bcl-2 and bax mRNA of peripheral-blood lymphocyte were detected by RT-PCR. Results At Oh or 48h,the apoptosis ratios of PBLC in normal group, TCM group,Western medicine group and TCM and Western medicine group are higher than model group and the differences are significant(P <0. 05 ,P <0. 01 ) ,while,at Oh,which are not significant within Western medicine group, normal group and TCM and Western medicine group ( P > 0. 05) ,even if which are significant between TCM group and TCM and Western medicine group or between model group and TCM and Western medicine group( P < 0. 05 ). At 48h, the differences of apoptosis ratios of PBLC are significant between Western medicine group and TCM and Western medicine group( P < 0. 05 ), and between TCM group and TCM and Western medicine group or between model group and TCM and Western medicine group which are significant, too( P < 0. 01 ). The ratios of bcl-2/bax mRNA of normal group,TCM group and TCM and Western medicine group are significantly lower than which of model group or Western medicine group ( P < 0. 05, P < 0. 05, P <0. 01 ,respectively) ,but there is no significant difference between which of model group and Western medicine group.Conclusion JQZR has adjustable effects on the apoptosis and expressions of bcl-2 and bax mRNA of peripheralblood lymphocyte in MRL/lpr Mice.
9.The clinical analysis of two different surgical treatment on unilateral middle cerebral artery infarction

Objective To study the different effects on unilateral middle cerebral artery infarction with standard large trauma craniotomy and frontotemporal craniectomy and decompression treatment.Methods The clinical data of 56 patients with unilateral middle cerebral artery infarction were retrospectively analyzed.They were divided into the two groups according to the different operation methods,and compared the incidence of postoperative complications and GCS score after 1 week,GOS score after 1 month,and ADL score after 6 months and so on.Results The differences were significant between the two groups in incidence of postoperative complications [ incidence of rebleeding after the operation(A group 5 cases,B group 4 cases),showing of brain pools(A group 23 cases,B group 14 cases),lung infection(A group 7 cases,B group 13 cases),gastrointestinal bleeding(A group 8 cases,B group 17 cases),χ2 =0.579,4.703,8.606,7.081] and postoperative GCS score after a week[12 -15points(A group 5 cases,B group 2 cases),9-11points(A group 15 cases,B group 10 cases),5-8points(A group 6 cases,B group 8 cases),3-4points(A group 2 cases,B group 4 cases),death(A group 1 case,B group 3 cases),W value was 599.500,P=0.028] (all P<0.05).There was significant difference in postoperative GOS score and rate of brain tissue bulging after a month [ advantages ( A group 7 cases, B group 4 cases ) , good ( A group 15 cases, B group 8 cases),middle(A group 3 cases,B group 6 cases),poor(A group 2 cases,B group 4 cases),death(A group 1 case,B group 2 cases),encephalocele(A group 9 cases,B group 19 cases),W value was 492.000,P=0.007](all P<0.05).There was no difference in postoperative ADL score half of a year(P>0.05).Conclusion Standard large trauma craniotomy has features as decompression full,low early complication rate,and can improve the short-term efficacy of unilateral middle cerebral artery infarction in patients,and promote recovery.
