1.Refined management in key descipline construction in hospitals

To promote the development of key discipline construction in hospitals,we described the integration of refined management into the construction of key disciplines and different aspects including personnel training,featured specialty,research,and hardware were discussed.We concluded that refined management in essential to the development of hospital,and a mechanism needs to be established to ensure the long-term effect.
2.Expression of Ezrin protein and c-met in human esophageal squamous cell carcinoma and their clinical significance

Objective To investigate the expression of Ezrin protein and c-met in esophageal squamous cell carcinoma, their significance in the progress of esophageal squamous cell carcinoma development and metastasis. Methods The expression of Ezrin protein and c-met in 50 patients with esophageal squamous cell carcinoma was studied using immunohistochemical (SP) method. The correlation of each score according to the intensity and percentage of labeled cells or intercellular substance with relevant clinical data was statistically analyzed. Results In normal esophageal squamous epithelium, the positive expression of Ezrin protein was 34 %, the positive expression of c-met was 8.0 %. In esophageal squamous cell carcinoma, the positive expression of Ezrin protein was 76.0 %, the positive expression of c-met was 58.0 %. The expression of Ezrin protein had close relation to depth of invasion and lymph node metastasis(P < 0.05). The positive rate of c-met protein expression in lymph node metastasis was obviously higher than those without lymph node metasis (P <0.05). The expressinn of Ezrin protein in c-met positive eases was obviously higher than those of c-met negative cases(P <0.05). Conclusion The test of Ezrin protein and c-met may have significant prognostic value for assessing the degree of malignancy and potential lymph node metastasis probability of esophageal squamous cell carcinoma. Ezrin protein may be a target to evaluate the tumor prognosis.
3.Effect of sub-chronic exposure to deltamethrin on spatial learning and memory in mice

Objective To study the effect of sub-chronic exposure to dehamethrin(DM) on the behavior of mice in learning and memory.Methods 60 Female SPF Kunming mice were randomly divided into 4 groups and given DM by gavaging for 60 days.Morris water maze (MWM) was used to evaluate spatial memory in mice.Results After exposure to DM,the escape latency of the solvent control group and the treatment groups were (12.20±6.5)s,(14.99±5.4) s,(15.64±8.3)s,(22.71±6.2)s on the fifth day.The escape latency of the high-dose group was higher than those of the low-dose group (P=0.0041) and the solvent control group (P=0.019) in the navigation test.The number of crossing position of the platform in the high-dose group ((2.93± 1.53)times) and the middle-dose group ((3.40± 1.12) times) were lower than that in the solvent control group ((5.87 ± 1.55) times) and the low-dose group ((4.90± 1.41)times)(P<0.05).Conclusion Sub-chronic exposure to DM can damage the spatial learning and memory of mice.
4.Effect of sub-chronic exposure to deltamethrin on neural behavior and expression of NMDA receptor in hippocampus of mice

ObjectiveTo study the effect of sub-chronic exposure to deltamethrin(DM) on neural behavior and expression of NMDA receptor in hippocampus of mice.Methods 60 Female SPF Kunming mice were divided into 4 groups and given DM 60 days by gavage.Hot-plate,rotarod,grip strength,hing limb landing foot splay were used to examine the sensory and motor change of mice.Autonomic activity test was used for detecting the functional status of the central nervous system in mice.Passive avoidance test for detection of the behavior changes of learning and memory,and RQ-PCR was employed to measure the expression of NMDA receptor in hippocampus of mice.ResultsThe behavior of sensory and motor of mice sub-chronic exposure to deltamethrin did not have changes significantly(P > 0.05 ).In the test of autonomic activity test,the average of autonomic activity times were (93 ± 18) times,(107 ± 13) times,(105 ±22) times.Compared with the control group,the average of autonomic activity times in middle-and high-dose groups were increased significantly (P < 0.05 ).The latent periods in poisoning groups were (175.4 ±38.4) s,(146.4 ±51.2)s,(132.3 ±45.0) s,and the error times were (0.7 ±0.3)times,( 1.4 ± 0.5 ) times,( 1.8 ± 0.9) times.Compared with the control group,latent periods of high-dose group were decreased and the error times of middle-and high-dose groups were increased significantly (P < 0.05 ).Compared with the control group,the relative expression levels of NR1 and NR2A mRNA in hippocampus of middle and high-dose groups were increased significantly (P< 0.05 ),and the relative expression level of NR2B mRNA in highdose group was decreased significantly(P < 0.05).ConclusionSub-chronic exposure to DM could increase the excitability of mice,damage the function of learning and memory,and influence the expression of NMDA receptor in hippocampus of mice.
5.Ultrasonic controlling of degradation of polymer materials

BACKGROUND:Degradable polymer materials initiate the degradation process immediately after implantation. How to regulate the degradation of these materials is rarely reported at present. OBJECTIVE:To study the effect of ultrasonic wave on control ing the degradation of polymer materials. METHODS:The sample is made ofε-caprolactone/L-lactide copolymer, and its core was coated with low density polyethylene on the surface with the fol owing four different methods. (1) The core surface was firstly covered with CaCl 2 powder, and then coated with polyethylene. (2) The core was firstly coated with polyethylene and coarsened for 3 hours. (3) The core surface was firstly covered with CaCl 2 powder, and then coated with polyethylene, and coarsened for 3 hours. (4) The core was directly coated with polyethylene. The four kinds of specimens obtained were embedded in pork for ultrasonic bombardment experiment in vitro. RESULTS AND CONCLUSION:In the specimens prepared with methods 1 and 4, the lyophobic layer could protect core materials before ultrasonic treatment, and no absorption peak was found at 631 nm. After ultrasonic treatment, the lyophobic layer was destroyed, toluidine blue dye was released, leading to change the color of immersion solution and increase the absorption peak at 631 nm. In the specimens prepared with methods 2 and 3,the lyophobic layer cannot exhibit the protection effects, the absorption peak was found at 631 nm. Under electron microscope, the appearance of the specimens in four groups was changed obviously. It is feasible to control the starting of the degradation by coating the degradable copolymer with LDPE and using ultrasonic as a trigger.

Objective To study the effect of sub-chronic exposure to deltamethrin(DM) on the behavior of mice in learning and memory and expression of GABAA receptor α1 and γ2 subunits in the hippocampus of mice.Methods 60 female SPF Kunming mice were randomly divided into solvent control group,low-dose group,middle-dose group and high-dose group, 15 mice in each group.The latter three groups were exposured to deltamethrin for 60 days by gavage.Open field test (OFT) was applied to evaluate locomotor activity and exploratory behavior in mice.RQ-PCR was employed to measure the expression of GABAA receptor α1 and γ2 subunits in hippocampus of mice.Results After exposure to DM,the moving distance of the central area in the middle-dose group ((555.1 ± 12.8) cm) and high-dose group ((516.4± 11.88) cm) was significantly higher than that in the solvent control group ((327.3± 117.8) cm, P<0.05).Numbers of standing in marginal area (F=4.117, P=0.010) and total movement distance (F=2.914, P=0.042) in the high-dose group ((27.9±9.9) times, (3211.3±379.8) cm) were also significantly higher than that in the solvent control group ((15.1 ±8.9)times, (3211.3±379.8)cm).The expression of GABAA receptor α1 subunit mRNA in the middle-dose group and high-dose group was significantly lower than that in the low-dose group and solvent control group(F=8.508, P=0.001) and the expression of GABAA receptor γ2 subunit mRNA in high-dose group was significantly lower than that in the other groups (F=6.738, P=0.002).Conclusion Sub-chronic exposure to DM can damage the function of locomotor activity and exploratory behavior,and inhibit the expression of GABAA receptor α1 and γ2 subunits in the hippocampus of mice.
7.The quantitative assay and clinical significance of JAK2V617F mutation in 131 patients with chronic myeloproliferative disorders

Objective To investigate the frequency and mutational status of JAK2V617F mutation in Chinese patients with chronic myeloproliferative disorders(CMPD) and to study the relative quantification of mutated JAK2 mRNA and the clinical significance. Methods JAK2V617F mutation and the mutational status were screened with amplification-refractory mutation system polymerase chain reaction(ARMS-PCR), the relative quantification of mutated JAK2 mRNA was studied by using capillary electrophoresis. Results A higher prevalence of JAK2V617F in either the heterozygotc or homozyote status in essential thrombocythemia (ET) was observed in elderly patients with ET (P<0.05). The presence of JAK2V617F was found to be significantly correlated with the age at diagnosis (P<0.05); patients with age ≥ 60 years showed significantly higher JAK2 mutated RNA levels than those with age < 60 years (P<0.05); the presence of JAK2V617F in polycythemia vera (PV) and ET was found to be significantly associated with higher hemoglobin level and higher leukocyte count (P< 0.05). In addition, higher leukocyte count was observed in homozygous ET patients than in heterozygous ET patients (P<0.05). The frequency of JAK2V617F mutation and the prevalence of homozygote in PV patients were higher than those in ET patients (P<0.05). The differences of JAK2V617F mRNA levels among PV, ET and chronic idiopathic myelofibrosis (IMF) were not significant. Conclusions ARMS-PCR technique can be used to detect the frequency and mutational status of JAK2V617F mutation owing to its sensitivity and along with capillary electrophoresis, quantitative assay for mutated JAK2 mRNA, diagnosis of CMPD and judgement of prognosis become possible.
8.The modified Madigan′s prostatectomy in conjunction with the bladder-neckotomy for treatment of prostatic middle lobe hyperplasia

Purpose:To evaluate the clinical effect on applying the modifed Madigan′s prostatectomy to treat the prostatic middle lobe hyperplasia.Methods:39 patients with benign prostatic middle lobe hyperplasia were excised by the modified Madigan′s prostatectomy in conjunction with the bladder-neckotomy dealing with any intracystic lesions at the same time.After the operation,the patients are followed up 2~24 months.Results:All patients were operated successfully and recovered smoothly.After discharging from the hospital,the average IPSS score is 3.8,the living quality score is 0~2,the maximal uroflow rate is 14.5~23.5 ml/s,the residual urine is less than 20 ml,All the patients have been followed up,not any serious complications appear.Conclusions:This technique not only excises the prostatic middle lobe which protrudes into the bladder,but also deals with any intracystic lesions,and protects the urethral mucosa from injury.It′s a better way to deal with the benign prostatic middle lobe hyperplasia.
9.Role and mechanisms of hepatocyte nuclear factor 4α in bile acid induced gastric intestinal metaplasia

Objective To investigate the roles and mechanisms of hepatocyte nuclear factor 4α (HNF4α) in chenodeoxycholic acid (CDCA) induced gastric intestinal metaplasia (IM).Methods After the immortalized gastric mucosal epithelial cells GES-1 were stimulated with CDCA at different concentration,the changes of HNF4α,caudal-related homeobox 2 (CDX2) and trefoil factor family 3 (TFF3) expressions at mRNA and protein levels in GES-1 cells and gastric cancer cell lines (AGS,SGC7901 and BGC823) were detected by real time-polymerase chain reaction (RT-PCR) and Western blotting.After GES-1 were transfected with HNF4α short hairpin RNA (shRNA) or control shRNA,and followed by CDCA stimulation,the expressions of HNF4α,CDX2 and TFF3 at protein level were determined by Western blotting.HNF4α was overexpressed in GES-1 cells and SGC7901 cells,and HNF4α was silenced in BGC823 cell line and AGS by lentiviral vector system.The expressions of HNF4α,CDX2 and TFF3 at mRNA and protein levels were tested by RT-PCR and Western blotting.Luciferase reporter assay was used to analyze the regulation role of HNF4α on CDX2.T test was performed for statistical analysis.Results The expressions of HNF4α in GES-1,SGC7901,BGC823 and AGS cells at mRNA level were 1.00 ± 0.12,263.01±10.23,848.01±18.13 and 3 049.86±91.75,respectively.The mRNAlevels of HNF4α in AGS,BGC823 and SGC7901 cells were all higher than that of GES-1 cells,and the differences were statistically significant (t=33.23,46.72 and 25.62,all P<0.01).The expressions of HNF4α in GES-1,SGC7901,BGC823 and AGS at protein level were consistent with mRNA level.The expressions of CDX2 and TFF3 at protein level of HNF4α shRNA transfected group were lower than those of non-HNF4α shRNA transfected group.In GES-1 cells,the expressions of HNF4α,CDX2 and TFF3 of HNF4α overexpressed group at mRNA level were 16 281.839 ± 1 843.017,6.275 ± 0.137 and 17.310± 1.533,respectively;which were all higher than those of overexpressed control group (1.000 ± 0.048,1.000 ± 0.012 and 1.000±0.108,respectively),and the differences were statistically significant (t =8.83,38.29 and 10.61,all P<0.01).In AGS cells,the expressions of HNF4α,CDX2 and TFF3 of HNF4α silenced group at mRNA level were 0.021 ± 0.001,0.088 ± 0.007 and 0.074 ± 0.002,respectively,which were lower than those of silenced control group (1.000 ± 0.108,1.000 ± 0.131 and 1.000 ± 0.122),and the differences were statistically significant (t=9.09,6.93 and 7.57,all P<0.01).In GES-1 overexpressed cells and AGS silenced cells,the expressions of HNF4α,CDX2,TFF3 at protein level were consistent with mRNA level.In double reporter plasmid containing the CDX2 promoter CDX2 1 (-2 000~-1 bp) and CDX2-2 (-1 510~1 bp),after transfected with HNF4α shRNA,the activities were 0.387 ± 0.013 and 0.533 ± 0.040,respectively,which were lower than those of HNF4α shRNA transfected control group (0.605 ± 0.012 and 0.882 ± 0.019),and the differences were statistically significant (t =21.49 and 13.53,both P<0.01).Conclusion HNF4α may be involved in bile acid induced intestinal metaplasia by upregulating the expression of CDX2.
10.Ruptured Peripheral Cerebral Aneurysms Associated With Moyamoya Disease: A Systematic Review

Background:
and Purpose A ruptured peripheral cerebral aneurysm (PPCA) associated with moyamoya disease (MMD) is a rare but potentially life-threatening condition with controversial management strategies. We aim to summarize the clinical characteristics, treatment strategies, and prognostic factors of PPCAs in MMD.
Methods:
We reviewed studies published in PubMed between 1980 and 2023 and used logistic regression analysis to identify the risk factors for adverse outcomes.
Results:
Of 425 identified studies, 48 eligible studies involving 121 participants were included in the current study. The mean age at diagnosis was 40.8±15.1 years, with a peak age of onset between 41 and 50 years. Among the identified participants, 59.6% were female, and 55.9% presented with impaired consciousness. Aneurysms were present in the posterior (35.5%) or anterior (30.6%) choroidal arteries in 66.1% of the cases, and 71.1% of the patients presented with intraventricular hemorrhage (IVH) with or without intracerebral hematoma (ICH). The treatment strategies were embolization (28.9%), direct surgery (21.5%), revascularization (22.3%), and conservation (27.3%). Favorable outcomes were achieved in 86.8% of all cases, with 97.1% for embolization, 65.4% for direct surgery, 96.3% for revascularization, and 84.8% for conservative treatment. Aneurysm rebleeding occurred in 11 (26.8%) of 41 patients managed conservatively, leading to worse outcomes in 7 patients (63.6%). Impaired consciousness (odds ratio [OR], 8.61; 95% confidence interval [CI], 2.06–36.00) and aneurysm rebleeding (OR, 16.54; 95% CI, 3.08–88.90) independently predicted poor outcomes.
Conclusion
PPCA should be considered in patients with hemorrhagic MMD, particularly those with IVH with or without ICH. Endovascular and bypass treatments are recommended as first-line options, with direct open surgery as an alternative in urgent hematoma evacuation cases. Detailed preoperative planning and intraoperative technical assistance are necessary to reduce procedure-related complications. Conservative management should be selected with caution because of the high risk of rebleeding and poor outcomes. Impaired consciousness and aneurysm rebleeding appeared to be independent risk factors for adverse prognoses. We emphasize that treatment selection should be personalized, and the potential benefits should be weighed against the associated risks.
