1.The effect of low-dose aprotinin on the expression of neutrophil CD_(11b)/CD_(18) in open heart surgery

Objective To investigate the inhibition of low-dose aprotinin on systematic inflammatory response during cardiopulmonary bypass. Methods Twenty-eight ASA Ⅱ-Ⅲ patients(male 13, female 15) undergoing valve replacement were studied. The age ranged from 27 to 55 years and body weight from 37 to 70 kg. CPB time ranged from 66 to 218 mm and aortic cross clamping time from 30 to 140 min . The patients were divided into two groups: control group(n=14) and aprotinin group(n = 14) Premedication included intramuscular phenobarbital sodium 0.1 g and scopolamine 0.3 mg. Anesthesia was induced with intravenous midazolam 0. lmg/kg, fentanyl 20-30?g/kg and vecuronium 0. 1-0. 12mg/kg, and maintained with intermittent bolus injection of fentanyl and vecuronium supplemented with isoflurane inhalation. In aprotinin group aprotinin 1?106KIU was infused after induction of anesthesia until thoracotomy, after thoracotomy aprotinin was infused at a rate of 2.5? 106 KIU/h and aprotinin 2. 5 ?106KIU was added to the prime fluid as suggested by Levy. Blood samples were obtained before anesthesia, before CPB and 1h and 24h after CPB for determination of CD11b and CD18 expression on the surface of neutrophil by immunofluoresence flow cytometry. Results The expression of CD11b or CD18 and CPB time were positively correlated(?= 0.644, 0.538, P0.05). 1 h after CPB CD11b/ CD18 expression increased significantly in the control group and was significantly higher than that in the aprotinin group, but at 24h after CPB only CD11b expression in control group was significantly higher than that in aprotinin group. In aprotinin group there was no significant difference in CD11b/CD18 expression between the four intervals.Conclusions CPB-induced systematic inflammatory response may be positively correlated with CPB time. Low dose aprotinin can inhibit the increase in CD11b/CD18 expression after CPB.
2.Construction of hTERT-siRNA expression vector and its inhibitory effects on the growth of MCF-7 cells and the activity of telomerase

Objective To construct the RNAi expression vector of hTERT gene,and to investigate the inhibitory effects of the vector on the growth of mammary adenocarcinoma cell MCF-7 and the activity of telomerase.So as to present a new approach to the gene therapy for breast cancer which aim at inhibiting the telomerase activity.Methods An interference target sequence TGTTCAGCGTGCTCA ACTA which aimed at hTERT gene was designed and the siRNA expression plasmid pGenesil-hTERT was constructed by the means of recombination.Meanwhile a negative control recombinant-pGenesil-HK that did not aim at any gene was constructed by the same method.The recombinants expression vectors pGenesil-hTERT and pGenesil-HK were identified by electrophoresis after digestion and DNA sequencing.The two recombinants were transfected into the MCF-7 cells by lipofectamineTM 2000.The telomerase activity was tested by telomerase repeat sequence amplification and polyacrylamide gel electrophoresis.Flow cytometry was used to detect the cell apoptosis.Results The results of analysis and the sequencing identification confirmed that the target sequence was inserted into the predicted site precisely and the recombining plasmids were successfully constructed.The PAGE results showed that the mark traps of telomerase were obviously decreased and the telomerase activity of pGenesil-hTERT was obviously inhibited.The apoptotic rate of the pGenesil-hTERT group was significantly higher than that of the pGenesil-HK group(P

Objective To study the value of unenhanced and dynamic contrast-enhanced MR imaging in follow-up of liver carcinomas after gamma knife therapy.Methods 57 patients examined by MR imaging after gamma knife therapy were analyzed retrospectively,and compared with outcome of AFP.Results Lesions treated with gamma knife therapy on precontrast imaging showed 4 kinds of appearances:(1) Slightly low signal intensity on T1WI and slightly high signal intensity on T2WI.(2)Slightly low signal intensity on both T1WI and T2WI.(3)Slightly high signal intensity on T1WI,isointensity and slightly high signal heterogeneous intensity on T2WI.(4)Slightly high signal intensity on both T1WI and T2WI.The necrotic lesions showed sustained ring-shaped or honeycomb enhancement or patching enhancement in hepatic tissue around the lesions with lesions unenhancing.Recurrent lesions or new lesions were enhanced in arterial phase,followed by rapid wash-out in the delay phase.The rate of completely necrosis was 93.5% in small liver carcinoma group(
4.Effects of equol on colon cancer cell proliferation

Objective:To investigate the effect of equol on the proliferation of colom cancer cells and to explore the mechanisms.Methods: Colon cancer cells (DLD1,HCT15,COLO205,LOVO,SW480) were incubated, the cell proliferation was identified by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide cell proliferation assay.Reverse transcription PCR and Western blot were used to measure the mRNA and the protein expression of estrogen receptor and nuclear factor (erythroid-derived 2)-like 2 (Nrf2)in the colon cancer cells, respectively.Moreover, 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide cell proliferation assay was used to investigate the effect of estrogen receptor(ER) inhibitor,ERα agonist, and estrogen receptor ERβagonist on the cell proliferation.Results: ERα was faintly expressed in the DLD-1 and HCT-15 cells.However, ERβ expression in DLD1, HCT15, COLO205, LOVO, and SW480 colon cancer cells.Different concentrations of equol (0, 0.5, 1, 5, 10 μmol/L) significantly inhibited the growth of HCT-15 cell with the expression of ERα and ERβ.More-over, different concentrations of equol (0, 0.5, 1, 5, 10 μmol/L) significantly inhibited the growth of LOVO, and SW480 cells with the ERβ expression in a dose-dependent manner as demonstrated with a 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide cell proliferation assay.mRNA expressions of ERα and ERβ in HCT-15 were stimulated significantly.Western blotting proved that the protein expressions of ERα and ERβ increased with the increasing of equol dose.Moreover we found significant difference of Nrf2 protein expression in HCT-15 cell stimulated by different concentrationss of equol.After the similation of estrogen receptor inhibitor, ERα agonist, or ERβ agonist, we found that only dif-ferent concentrations of ERβ agonist(0, 1, 10, 100, 1 000, 10 000 nmol/L) significantly inhibited the growth of HCT-15, LOVO, and SW480 in adose-dependent manner.Estrogen receptor inhibitor and ERα agonistdid not present significant effect on the cell proliferation of HCT-15, LOVO, and SW480.Conclusion: Equol inhibited the colon cancer cell proliferation by its estrogenic activities and antioxidant activities.
5.Integrative Development of Field Battle Portable X-ray Machine

Objective To carry out integrative development to available field battle portable X -ray machine,solving functional defect in quondam tripod and inconvenience in transport. Methods We used anthrop -machine engineering,electricity engineering and machinery engineering to undertake integrative design on whole machine. Results The whole machine can be quickly extended and folded in 2 minutes. Centric position of the sucker is 170 cm over the ground,and 45 cm away from column stand when the whole machine working. The machine can work in multi-angle and omnibearing. The whole machine can deposit in an appropriative crate with efficiency of anti-shock and seepage water when folding. Conclusion Integrative design of field battle portable X-ray machine increases projection range,boosts projection effect,strengthens medical service safeguard ability of radio-staff consumedly.
6.Effect of pretreatment with butylphthalide injection on prognosis of intravenous thrombolysis in ultra-early acute cerebral infarction

Objective To observe the effect of early using butylphthalide injection before and after thrombolytic therapy with intravenous recombinant tissue plasminogen activator (rt-PA) on the clinical prognosis of patients with ultra-early acute cerebral infarction (ACI). Methods A prospective study was conducted, 81 patients with ACI admitted to the Department of Neurology of Tangshan Worker's Hospital from September 2014 to March 2016 were enrolled, and they were divided into a control group (40 cases) and an observation group (41 cases) according to the random number table. Both groups were given routine treatments, such as drugs for lowering blood pressure and blood sugar, decreasing blood lipid to stabilize plaque, neuroprotection, activating blood circulation and removing blood stasis, etc. On the basis of conventional treatment, the control group was directly treated with rt-PA intravenous (IV) thrombolytic therapy according to the guidelines of thrombolytic therapy; in the observation group, the patients immediately underwent CT head examination after admission to decide whether the thrombolytic therapy was necessary, if the therapy was decided to be done, during doctors waiting for the laboratory results or transferring patients, IV drip of butylphthalide sodium chloride 100 mL. After IV drip thrombolytic therapy, if the disease condition was stabilized, the head CT was re-examined to exclude intracranial hemorrhage, if no such hemorrhage, IV drip of butylphthalide sodium chloride 100 mL was continuously given, twice daily for consecutive 14 days with the interval between the two times of IV drip being 7 hours daily. When patient's condition was changed, the re-examination of head CT could be done at any time; if the patient's condition was not changed, the head CT was routinely performed 24 hours after IV drip thrombolysis. After exclusion of intracranial hemorrhage, the patients in both groups were treated additionally by the platelet aggregation drug on the basis of their original treatment. The National Institutes of Health Stroke Scale (NIHSS) scores, Bartherl index (BI) scores were recorded before and after treatment, and the recovery situation of neurological function, hemorrhage conversion rate, mortality and adverse reactions were observed after thrombolysis. Results After treatment, the NIHSS scores were lower, and the BI index scores were higher than those before treatment in the two groups, and the change in the observation group after 14 days of treatment was more significant (NIHSS score: 3.87±3.46 vs. 7.37±4.18, BI score: 87.38±9.34 vs. 75.67±8.05, both P < 0.05); the total effective rate of the observation group was significantly higher than that of the control group [73.2% (30/41) vs. 55.0% (22/40), P < 0.05], the rate of bleeding conversion rate was lower than that of the control group [2.4% (1/41) vs. 7.5% (3/40), P < 0.05], the difference in fatality rate between the two groups was not statistically significant [2.4% (1/41) vs. 2.5% (1/40), P > 0.05]. Conclusion The clinical therapeutic effect of butylphthalide injection is relatively good for treatment of patients with ultra-early ACI.
7.A pilot study of different thrombolytic therapies for acute cerebral infarction due to occlusion of middle cerebral artery

ObjectiveTo evaluate the effect and safety of different thrombolytic therapies for acute cerebral infarction due to occlusion of middle cerebral artery(MCA).MethodsOne hundred and thirty-two cases of acute cerebral infarction in territory of MCA were randomly divided into 3 groups,all of which were treated with alteplase.Group A (48 cases) was treated by intra-venous therapy with alteplase,group B (43 cases) was treated by infusing alteplase at the site of the internal carotid artery,and group C(41 cases) was treated by infusing alteplase into the thrombus.The improvement of neurological function,complications and mortality rate were recorded and statistically compared,with analysis of variance for counting data of normal distribution,x2 test for quantitative data,and the mean difference was significant at the 0.05level.ResultsThe effective rates of group A,B and C at 2 h,24 h,2 w were 18.8% (9/48),39.6% ( 19/48),45.8% (22/48) ;39.5% (17/43),53.5% (23/43),58.1% (25/43) ;78.0% (32/41),85.4% (35/41 ),87.8% (36/41)respectively.The effective rate of group C was obviously better than group A( x2 =12.809,9.979,9.289,P < 0.01 ) and B (x2 =31.295,19.425,17.161,P < 0.01 ) with statistical significance.The effective rate of group B was better than group A at 2 h after thrombolytic therapy with statistical significance (x2 =4.801,P < 0.05 ).The effective rate of group A and B did not have significant difference at 24 h,2 w after therapy ( x2 =1.765,1.375,P > 0.05 ).The hemorrhage rates of group A,B and C were 14.6% (7/48),14.0% (6/43),7.3% (3/41 ),the mortality rates of group A,B and C were 6.2% (3/48),4.6% (2/43),2.4% (1/41),and there was no significant difference among the 3 groups ( x2 =1.328,0.786,P > 0.05 ).ConclusionIt is suggested that the thrombus-imbeded thrombolytic therapy is a better way in treating acute cerebral infraction due to occlusion of MCA for its rapid and better therapeutic effect.
8.Effects of shear stress and lysophosphatidylcholine on adhesion molecules expression of endothelial cells.

Extensive monocyte recruitment is an early phenomenon associated with the development of atherosclerotic lesion. Although the molecular mechanisms are not completely understood, monocyte recruitment into these early lesions may involve changes in endothelial adhesion for monocyte, in which adhesion molecules expressed by endothelial cell play an active role. In vivo, the function of endothelial cells is not only affected by the chemical factors, but also by the mechanical factors. The purpose of this article was to investigate the induction of adhesion molecules expression by synergistic effects of Lysophosphatidylcholine (Lyso-PC) and shear stress in cultured human umbilical vein endothelial cells (HUVEC). The surface expression of ICAM-1, VCAM-1 and E-selectin on HUVEC induced by Lyso-PC(30 micrograms/ml) and shear stress(2.23, 4.20 dyne/cm2) were analyzed using flow cytometry. The results showed that: Compared with what were simultaneously exposed to shear stress and Lyso-PC, activating the cells with Lyso-PC prior to shear stress, or pre-conditioning the cells exposed shear stress prior to Lyso-PC incubation, a significantly higher expression of ICAM-1 and VCAM-1(P < 0.05) was resulted. HUVEC were exposed to shear stress and Lyso-PC at the same time or treated with each agonist alone, E-selectin expression was not significantly different from the control group. However, a sequential action of the two stimuli significantly increased E-selectin expression(P < 0.05). We concluded that: a sequential action of the shear stress and Lyso-PC induced an even greater expression of ICAM-1 and VCAM-1, thus it could be understood that the flows-hear stress in combination with endothelial activated by chemical factors may increase the ability of endothelial cells to recruit leukocytes even under the mechanical environment unfavorable for cell adhesion.

9.From surgery to endoscopy: the evolution of the bariatric discipline

Obesity, with its increasing morbidity and prevalence, is now a worldwide public health problem. Obesity and its related comorbidities impose a heavy burden on societal health and the economy. The practice of bariatric surgery has evolved from its early surgical procedures, many of which are no longer routine operations. With clinical practice, research, and experience, bariatric surgery has gradually become an important last resort for the control of weight and obesity-related metabolic diseases in moderately and severely obese patients. However, there is still room for further improvements in bariatric surgical procedures, especially with regard to long-term issues and complications. Endoscopic weight loss technology has developed rapidly in recent years. The advantages of this technology include minimal invasiveness, an obvious weight loss effect, and few complications, thus filling the gap between medications and lifestyle adjustments and surgical treatment of obesity. Endoscopic weight loss technology may even replace surgical bariatric procedures. This review summarized the current status of bariatric metabolic surgery and newly developed bariatric endoscopic procedures.
10.Therapeutic effect of acupoint catgut embedding therapy on transient cer-ebral perfusion in patients with transient ischemic attacks

Objective To discuss the effect of acupoint catgut embedding therapy on transient cerebral perfusion in patients with transient ischemic attacks (TIA).Methods 67 patients with TIA were ran-domly divided into treatment group and control group.The patients of the control group took orally aspirin enteric-coated tablets 100 mg per night,in addition,those of treatment group underwent acupoint catgut embedding therapy with the embedding points of Fēngchí(GB 20),Dàzhuī(GV 14),Fēnglóng (ST 40).In order to understand the cerebral blood flow perfusion and curative effect,CT perfusion imaging was performed before and one month after treatment.Results After one month of treatment,the efficacy of the treatment group with effective rate of 91.18 % (remarkable effect 18 cases,effect 13 cases and in-efficacy 3 cases)was significantly better than that of the control group with effective rate of 63.64 % (re-markable effect 10 cases,effect 11 cases,and inefficacy 12 cases).And the improvement of cerebral is-chemia in CTPI imaging was better than that in the control group (P <0.01).Conclusion The positive therapeutic effect of acupoint catgut embedding therapy has been proved on TIA,which can significantly improve the patients with cerebral perfusion.
