1.Application of targeted surveillance of hospital infection in the Department of Neurosurgery

Objective To explore the effects of the application of targeted surveillance in the Deparment of Neurosurgery. Methods Someone was specially assignedd to work out both process and outcome goals by means of perspective investigation, clinical groups were formed and information feedback, evaluation of effects and correction of deviation were regularly conducted. Results The process goals were obtained: from July 1996 to June 1997, the rate of hospital infection in the Department of Neurosurgery droppoed from 26.07% to 21.32% (x2 = 4.3465,P <0.05). The outcome goals were also obtained: from July 1997 to June 1998, the rate of infection dropped from 21. 32% to 13.40% (x2 = 16.1193,P<0.01). Conclusion Targeted surveillance is a really effective form of surveillance and information feedback and evaluation of effects are indispensable. Application of targeted surveillance in the Department of Neurosurgery has won initial success.
2.Effect of Aitongxiao Granules Containing Serum on Proliferation and Apoptosis of SMMC-7721 and Bel-7404 Cells in Vitro

Objective To observe the anti-cancer effect in vitro of Aitongxiao Granules containing serum on SMMC-7721 and Bel-7404 liver cancer cells, thus revealing the anti-cancer mechanism of the treatment in adjusting proliferation and apoptosis. Methods Male Sprague-Dawley rats were administered by gastrogavage Aitongxiao decoction of 86.4, 43.2, 21.6 g/kg twice a day for 1 week. Using serum pharmacologic method, the proliferation of SMMC-7721 and Bel-7404 liver cancer cells were determined by MTT chromatometry after co-cultured with medicated serum containing different concentration of Aitongxiao decoction. The apoptosis of SMMC-7721 and Bel-7404 liver cancer cells were detected by flow cytometry. Results The research in vitro showed that Aitongxiao Granules containing serum low, medium and high dose groups had varying degrees of inhibition on hepatoma cells, and this inhibition showed a concentration dependence within a certain range. Aitongxiao Granules containing serum could induce apoptosis of human hepatoma SMMC-7721 cells and Bel-7404 cells. Conclusion Aitongxiao Granules showed effect of inhibiting proliferation and inducing apoptosis on SMMC-7721 and Bel-7404 cells in vitro.
3.Clinical significance of combining second trimester serum and ultrasound screening for Down syndrome

objectives To explore the clinical significance of the triple screening method and fetal sonographic markers in Down syndrome screening. Methods The retrospective study included maternal blood serum triple-marker screening per-formed in normal singleton pregnancies at 14+1 to 19+6 weeks and ultrasonography screening for fetal chromosomal trisomy at 18+1 to 23+6 weeks from 2010 to 2013. Results In 24 368 pregancies, Karyotype analysis reveal 35 Down syndromes, the incidence is 0.14%(35/24 368). 25/35 were pregnancies with an expected date of delivery lower than 35 years old and 10/35 were pregnan-cies with an expected date of delivery higher than 35 years old. In 1 215 pregnancies, second-trimester serum screening showed high risk (cutoff>1:380), 14 were confirmed as Down syndromes ( sensitivity 40%;specificity 95.06%). In 1 142 pregnant women showed ultrasonography abnormalities, of which 11 were found to have Down syndrome (sensitivity 60%;specificity 91.09%). In 112 pregnancies, second-trimester serum screening and ultrasonography showed high risk, 4 were conformed as Down syn-drome (sensitivity 11.43%;specificity 99.56). Conclusions The most effective method of screening for Down syndrome is by maternal serum biochemistry. Ultrasonography is important in screening fetal trisomy 21. combined screening methods can improve the detection rate of trisomy 21.
4.Common problems and countermeasures on medical equipment bidding in public hospitals

Objective To analyze the problems frequently found in medical equipment bidding at public hospitals and come up with countermeasures for building a scientific and standardized bidding procedure.Methods 168 cases of medical equipment bidding of the hospital in the recent five years served as objects of study,for a full-process longitudinal study.It covered the preparation of technical parameters,compilation of bidding documentation,bid opening/evaluation process,all the way to final acceptance and after-sales service,and complaints responses.Results In the 168 cases,imported equipments accounted for 86.90%;only 16.07% of the purchases involved consumables,yet their importance deserved high attention;59.52% of the equipments involved after-sales maintenance costs,calling for regular maintenance to ensure smooth operation of the equipments.Conclusions Quality of domestic equipments should be enhanced,for optimization of the ratio between domestic and imported equipments;equipment purchase,after-sales service and consumables purchase should be taken into account in general;scientific evaluation levels and standards should be established to enhance supervision,for the purpose of scientific and rational purchase of medical equipments.
5.NEURONS OF THE HEAD OF CAUDATE NUCLEUS INHIBITING UNIT DISCHARGE OF THE CINGULATE CORTEX. AN HRP MICROIONTOPHORETIC STUDY

The cingulate cortex, especially its anterior portion has been found to be connected with the head of the caudate nucleus in our previous study. In order to ensure accurate placement of adequate amounts of HRP in the cingulate cortex, a microiontophoretic delivery technique coupled with single cell recording was employed. The use of electrophysiological criteria as aids in placing the injection is described in detail. It was found that the influence of the caudate neuron was inhibitory, and the cingulate neurons responded with monosynaptic IPSPs to caudate neuron stimulation. Following HRP injections into the cingulate cortex, the retrogradely labeled cells were found in the head of the caudate nucleus. It is obvious that some neurons of the head of caudate nucleus which have inhibitory effect on unit discharge of the cingulate cortex project to the anterior portion of the cingular cortex.
6.Serum resistin and neuron-specific enolase in patients with cerbral infarction or multi-infarct dementia

Serum concentrations of resistin and neuron-specific enolage(NSE)were determined in 72 patients with cerebral infarction(CI),60 patients with multi-infarct dementia(MID),and 60 healthy controls by means of ELISA.Our data showed that serum resistin and NSE in the CI acute phase group were significantly higher than those in the normal control group(P<0.01).No significant difference was found in serum resistin and NSE between the CI recovery phase group and normal controls group.Moreover,no significant difference was found in serum resistin between the MID group and CI recovery phase and normal control groups(P>0.05);but NSE Was relatively lower(P<0.05).This study suggest that serum resistin and NSE might be of great significance to patient's condition and prognosis.
7.Implant stability during osseointegration using osteotome technique

BACKGROUND:Implant stability is the basic requirement of osseointegration and also one of important parameters to judge whether the implant is implanted successfully. Generaly, the implant stability is closed related to bone quality (bone hardness and bone density) in the implant zone, implant shape, diameter and length. OBJECTIVE:To continuously monitor the changing trend of implant stability during early healing period due to the utilization of osteotome technique by resonance frequency analysis. METHODS:Twenty patients with class Ⅳ defects in the posterior maxila who underwent implant restoration (4.8 mm×12 mm) from 2010 to 2011 at the Department of Stomatology, the 521 Hospital of China North Industries Group Corporation were recruited. Resonance frequency analysis was used to measure the implant stability at implant insertion, 1, 2, 3, 4, 6, 8 and 12 weeks postoperatively. RESULTS AND CONCLUSION:Al the implants achieved osseintegration uneventfuly within 12 weeks. At implant instalation, the mean implant stability quotient value was 69.66±4.75. An increase trend in implant stability quotient values was visible within 1 week, and the implant stability quotient value reached the peaked at 1 week, and then decreased to the lowest point at 2 weeks, which were significantly different from that at implant instalation (P < 0.05). In the secondary stability phase, the increasing slope of implant stability quotient values reached a plateau by the 8th week. The resonance frequency analysis can estimate the quantitative change of implant stability after applying the osteotome technique, and the osteotome technique can promote the implant initial stability.
8.Clinical significance of visfatin and VE-cadherin and their association with kawasaki disease

Objective To explore the levels and clinical significance of visfatin and VE-cadherin in the serum of children with Kawasaki diseases (KD).Methods Serum levels of visfatin and VE-cadherin were measured in 90 children with KD and 30 healthy children (control group) by ELISA,the outcome was estimated in combination with clinical symptoms.Results The serum levels of visfatin [ (32.35 ± 4.82)μg/L ] and VE-cadherin[ (5.03 ± 1.06)mg/L] in children with KD were obviously higher than those in control group[ (16.83 ±4.36) μg/L,(2.86 ± 1.02) mg/L] (t = 15.23,12.47,P <0.01).The serum levels of visfatin[ (36.61 ±5.22) μg/L] and VE-cadherin [ (6.14 ± 1.23) mg/L] in children with KD and CAL were obviously higher than children of KD without CAL[ (28.16 ± 4.67)g/L,(4.02 ± 1.01)mg/L] (t =8.38,7.16,P <0.01).In children with KD,the serum levels of visfatin and VE-cadherin were significantly correlated (r = 0.65,P < 0.05).Conclusions Serum levels of visfatin and VE-cadherin increased in the acute phase of KD,and it was significantly increased in those KD children with CAL.Visfatin and VE-cadherin may be used as an important serological indicator of KD with CAL.
9.Development and animal tests of a miniaturized electrical chest compression device.

This paper introduces the development and animal tests of a miniaturized electrical chest compression device. Based on pulse width modulation technology produced by micro control unit, the device can control the frequency and depth of the compression accurately, as well as perform real-time adjustment. Therefore, it can perform continuous and stable chest compression for long time, which may increase the successful rate of cardiopulmonary resuscitation (CPR). Besides, the device can also produce different types of compression waveforms, including trapezoidal and triangular waveforms. Then, the performance and efficacy of the device was assessed with a rat model of asphyxial cardiac arrest (CA).

10.Combined Pharmacokinetics-pharmacodynamics Study of Rabeprazole in Inhibition of Gastric Acid Secretion

Objective To investigate the pharmacokinetics ( PK ) and pharmacodynamics ( PD ) processes of rabeprazole in inhibiting gastric acid secretion with the combined PK-PD model. Methods A total of 10 healthy volunteers were given a intravenous infusion of 20 mg rabeprazole over a 30-min period. The concentration of rabeprazole in the plasma at different time points was detected by HPLC,and the PK parameters were calculated by DAS 2. 0 software. At the same time the intragastric pH was monitored over 24 hours to fit the PD parameters with indirect inhibition model. Results The main PK parameters,t1/2,Cmax,and AUC were(60. 5±17. 3)min,(1 299. 1±201. 0)ng·mL-1,and(106. 4±26. 0)mg·min·L-1, respectively.The corresponding PD parameters,Kin,Ke,IC50 and Imax were(8.200±3.362)h-1,(1.080±0.378)h-1,(0.286± 0. 129)mg·L-1 and(6. 93± 2. 15)pH,respectively. Conclusion The PK of rabeprazole in healthy volunteers conforms to one compartment model,and the PD fits the indirect response inhibition model. The equation can effectively establish the relationship between the blood drug concentration and the effect.
