1.Study on Medication Rules of Spleen-stomach Deficiency Syndromes inLin Zheng Zhi Nan Yi An Based on Data Mining

Objective To discuss the relation and differences among spleen-deficiency syndromes, stomach-deficiency syndromes and spleen-stomach deficiency syndromes.Methods Terms about spleen-stomach deficiency syndromes in the book ofLin Zheng Zhi Nan Yi Anwere screened. Syndrome names about “spleen deficiency”, “stomach deficiency”, “stomach-yang deficiency”, “tu deficiency”, “spleen-stomach deficiency”, “middle-jiao deficiency”, “yang-ming deficiency”, and “tai-yin deficiency” should be included in this study. And then medication and compatibility of medicines of spleen-deficiency syndromes, stomach-deficiency syndromes and spleen-stomach deficiency syndromes were determined according to the main efficacy of the medication and compatibility of medicines. Relevant data were standardized and then database was built. Apriori algorithm in SPSS Clementine12.0 was used to conduct data mining.Results Totally 421 relevant cases were collected in this study, involving 525 prescriptions (92 spleen-deficiency syndromes, 276 stomach-deficiency syndromes and 157 spleen-stomach deficiency syndromes). 11 medication association rules about spleen-deficiency syndromes, 38 about stomach-deficiency syndromes, and 28 about spleen-stomach deficiency syndromes were found based on data mining. ConclusionMost prescriptions for spleen-stomach deficiency syndromes are “warm dryness, cool and moister, sweet and warm to tonify the midde jiao”, which embody Ye’s medication features that emphasizes treating spleen and stomach separately, the differences of lifting spleen and dropping stomach, and in nourishing yin and tonifying yang.
2.Balo′s concentric sclerosis: a correlative study of MRI,~1H-MR spectroscopy and biopsy

Objective To describe the feat ures of Balo′s concentric sclerosis o n MRI and ~1?H-MRS. Methods Three patients of Balo′s concen tric sclerosis were rep orted on the basis of brain MRI and stereotactic brain biopsy. Proton MR spectro scopy was carried out on 1 patient. Results Multiple ring-li ke lesions of lam inated demyelination alternating with spared white matter were demonstrated in a ll 3 patients, and the lesions were best seen in post-contrast images. ~1 ?H-MR S showed a decreased NAA peak, an increased choline peak, and the presence of la rge lipid peak. Conclusion Balo′s concentric sclerosis has cha racteristic findi ngs on MRI and ~1?H-MRS. MRI is very useful in the diagnosis of Balo′s c oncentric sclerosis.
3.The effect of necrostatin-1 on expression of liver monocyte chemotactic protein-1 in septic rats

Objective To investigate the effect of necrostatin-1 (Nec-1) on the expression of liver monocyte chemotactic protein-1 (MCP-1) in septic rats and its mechanism. Methods Forty-eight male Sprague-Dawley (SD) rats were randomly divided into sham group, model group, and Nec-1 group by randomized digital number method, with 16 rats in each group. The model of sepsis was reproduced by cecal ligation and puncture (CLP). Rats in sham group received anesthesia, and flipping the cecum followed by closure of the abdomen without ligation of the cecum. Rats in Nec-1 group were given 1 mg/kg Nec-1 [25 mg Nec-1 solution dissolved in 2.5 mL of dimethyl sulfoxide (DMSO)] through caudal vein 30 minutes before operation, while the rats in model group were given 0.1 mL/kg of DMSO only. Blood from abdominal aorta and liver tissue in each group were collected at 0 hour and 8 hours after operation. Serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were determined with automatic biochemistry analyzer. The pathological changes in liver were observed under light microscope using hematoxylin-eosin (HE) staining. The serum levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) were determined by enzyme linked immunosorbent assay (ELISA). The MCP-1 mRNA expression in the liver was determined by reverse transcription-polymerase chain reaction (RT-PCR). Results There was no significant differences in the levels of serum ALT, AST, TNF-α, IL-6 and expressions of liver MCP-1 mRNA at 0 hour among three groups, and the liver cellular structure was normal. At 8 hours, compared with sham group, the expressions of serum ALT, AST, TNF-α, IL-6 and liver MCP-1 mRNA were significantly increased in model group and Nec-1 group [ALT (U/L): 172.35±21.88, 129.67±18.20 vs. 60.04±11.74, AST (U/L): 511.03±34.92, 363.51±25.25 vs. 254.83±31.04, TNF-α(ng/L): 603.96±24.18, 483.87±26.60 vs. 265.74±15.14, IL-6 (ng/L): 975.62±65.37, 712.09±45.47 vs. 310.42±13.88, MCP-1 mRNA (2-ΔΔCt): 7.09±0.18, 5.51±0.45 vs. 0.99±0.06, all P < 0.05]. Levels of the above parameters in Nec-1 group at 8 hours were significantly decreased compared with those of model group (all P < 0.05). Under light microscopy, it was noted that the structure of hepatic lobules was destroyed, with exacerbation of immunocyte infiltration at 8 hours in model group. At 8 hours, it was found that Nec-1 alleviated the pathological damage in Nec-1 group. Conclusion Nec-1 can protect the liver of rats with sepsis, lower the expression of serum TNF-α and serum IL-6 and liver MCP-1 mRNA, and obviously reduce the damage of inflammation.
4.Clinical and prognosis analysis in 78 patients with advanced renal cell carcinoma treated with sunitinib: a report from single center

Objective To discuss and evaluate the clinical efficacy and safety of sunitinib for patients with advanced renal cell carcinoma and further to analyze the associated prognostic factors.Methods A retrospective analysis was performed in 78 cases with advanced renal cell carcinoma, receiving sunitinib therapy from April 2009 to December 2014.Patients consisted of 53 males and 25 females, with median age of 54 years old, ranged from 25-85 years old.Therapeutic regimen was described as following: 52 cases receiving sunitinib 50.0 mg/d 4 weeks on and 2 weeks off (4/2 regimen), 26 cases receiving 50.0 mg/d 2 weeks on and 1 weeks off (2/1 regimen).The dosage and regimen were adjusted according to the severity of side effects.Efficacy evaluation and drug-related toxicity were based on RECIST version 1.1 and CTCAE version 3.0.Progression-free survival (PFS) and overall survival (OS) were evaluated using the KaplanMeier method.Univariate and multivariate Cox proportional hazards model were used to assess the risk factors of PFS and OS.Results Nineteen cases switched from 4/2 to 2/1 regimen.Attenuated dosage was allowed in 49 cases to ameliorate drug-related toxicities.The most common drug-related toxicities were handfoot syndrome (HFS) in 63 cases (80.8%), diarrhea in 59 cases (75.6%), fatigue in 59 cases (75.6%) and thrombocytopenia in 6 cases (71.8%).The most common grade Ⅲ-Ⅳ toxicities were HFS in 9 cases (11.5%), thrombocytopenia in 6 cases (7.7%) and hypertension in 5 cases (6.4%).In RECIST evaluation, complete response (CR) was not recorded.8 cases (10.3%) achieved partial response (PR) , 59 cases (75.6%) kept stable disease (SD) and 11 cases (14.1%) suffered progressive disease (PD).The objective response rate (ORR) was 10.3% and the disease control rate (DCR) was 85.9%.The median PFS was 11.0 months and median OS was 21.8 months.Multivariate Cox proportional hazards model showed two independent risk factors for PFS, including number of metastasis organs ≥ 2 and a high ECOG score.One independent risk factor for OS was number of metastasis organs ≥ 2.Conclusions Sunitinib shows encouraging efficacy and safety for patients with advanced renal cell carcinoma.Patients with multiple metastatic organs and poor performance status seems to be high risky of poor prognosis.
5.Conventional MRI and Perfusion Weighted Imaging of Central Neurocytoma

Objective To investigate the conventional MRI and perfusion weighted imaging (PWI) features of central neurocytoma(CNC), and to evaluate the potentiality of them in diagnosing accuracy. Methods Three patients with CNC proved pathologically were collected, 4 patients with ependymoma were as control group, with conventional MRI and perfusion weighted imaging were performed in all of them. MRI features were observed, and relative regional cerebral blood volume (rrCBV) were calculated.Results CNCs were isosignal on T_1WI, hyperintense on T_2WI, minimal enhancment. Ependymomas were hypointense or isosignal on T_1WI, slightly hyperintense or hyperintense on T_2WI and minimal enhancement. 6.The Value of MR Perfusion Weighted Imaging in Typing Meningioma Preoperation

Objective To investigate the value of MR perfusion weighted imaging(PWI) features in typing meningioma preoperation.Methods MR perfusion weighted imaging was performed on 39 patients with pathologically proved meningioma.The relative regionalcerebral blood volume(rrCBV) and relative mean transit time(rMTT) were calculated.Results The rrCBV and rMTT of angioblastic meningiomas were 11.8 and 1.8,which were higher than that of the others subtype.The rrCBV of meningothelial meningiomas andtransitional meningiomas were 8.3,7.5 respectively,the rMTT of them were 1.5 and 1.3,there was statistically significant difference in rMTT between two groups,but no difference in rrCBV.The rrCBV of fibrous meningiomas was 4.0,which was lower than that of the others.Conclusion PWI is very useful in typing meninigioma preoperation.
7.Effect of rofecoxib on expressions of VEGF and inducible NOS in human gastric carcinoma tissues established in nude mice by orthotopic transplantation

Objective To evaluate the inhibitory effect of rofecoxib in human gastric carcinoma tissues established in nude mice by orthotopic transplantation through the expressions of vascular endothelial growth factor (VEGF) and inducible nitric oxide synthase (iNOS). Methods After the models of human gastric carcinoma in nude mice by orthotopic transplantation were established, rofecoxib was administered intragastrically. The expressions of VEGF and iNOS were evaluated in the local tumors by Envision immunohistochemical method. Objective To investigate the expression of MMP-2 and MMP-7 and their elinieopathologieal significance in eoloreetal carcinomas. Methods The expression of MMP-2 mRNA, MMP-7 mRNA and MMP-2, MMP-7 in 42 samples of eoloreetal carcinomas and its adjacent normal eoloreetal mueosa were examined using fluo-rescence quantitative RT-PCR (FQ-RT-PCR) and immunohistoehemistry. The relationship of their expression with some elinieopathologieal characteristics was analyzed. Results MMP-2 mRNA, MMP-7 mRNA and MMP-2, MMP-7 were significantly over-expressed in eoloreetal carcinomas compared with its adjacent normal eoloreetal mueosa( P < 0.05 ) , and they were positively associated with bowel wall invasion, the regional lymph node metastasis and Dukes stage. The expression of MMP-2 mRNA and MMP-7 mRNA was positively correlated with MMP-2 and MMP-7 in eoloreetal eareinornas. Conclusions MMP-2 and MMP-7 may play a key role in eoloreetal carcinogenesis,tumor in-vasion and metastasis. Examination of combined MMP-2 and MMP-7 expression may have an important significance to judge the malignant degree and biological behavior of human eoloreetal carcinoma and for evaluating the progno-sis.
9.Survival analysis of renal cell carcinoma patients with bone metastases treated with targeted therapy

Objective To investigate the prognostic related factors in patients with renal cell carcinoma(RCC) and bone metastases,treated by targeted therapy.Methods Forty-five patients with RCC and osseous metastases were treated by targeted therapy between June 2006 and April 2013.The mean age was 59 years (range 32-81 years) with 33 male cases and 12 female cases.Twenty-seven cases were diagnosed as RCC accompanied with bone metastases initially,and the median time between the diagnosis of RCC and that of osseous metastasis for the other 18 cases was 12.5 months.All the cases underwent target therapy with sorafenib in 38 cases and sunitinib in 7 cases.Data was retrospectively analyzed and the relationship between several clinical features and overall survival (OS) was examined univariately.The Cox proportional hazards model was then performed multivariately to identify the independent risk factors.According to the independent risk factors,RCC patients with osseous metastases were categorized into high risk group (≤ 1 favorable factors) and low risk group (> 1 favorable factors).The median OS in those groups was compared.Results The median OS from the diagnosis of bone metastasis was 19 months,and overall survival was 74.7% at 1 year,and 32.7% at 2 year.Clinical features correlated with longer survival in the multivariate analysis were the absence of osseous metastases when initially diagnosed as RCC (HR:2.401,95%CI:1.210-5.699),the resection of primary renal tumor (HR:2.635,95%CI:1.021-6.307) and the absence of extraosseous metastases (HR:2.323,95%CI:1.003-6.221).The median OS of high risk group in 23 patients was 16months.On the other hand,22 patients in the low risk group had a longer median OS with 22 months.There was a significant difference in median OS between the two groups (P<0.05).Conclusions The three prognostic factors including the absence of osseous metastases when initially diagnosed as RCC,the resection of primary renal tumor and the absence of extraosseous metastases could be favorable factors for RCC patients with bone metastasis treated with target therapy.
