1.Forensic Analysis of 9 Cases with Enterovirus 71 Infection

Objective To investigate the forensic characteristics of death cases of EV71 through retrospective analysis of 9 death forensic cases of Enterovirus 71 (EV71) . Methods Nine cases of EV71 death cases were collected,and the population characteristics, histopathological change,toxicology,pathology and causes of death of these cases were discussed. Results All 9 patients had fever, cough, emesis, and were treated as influenza or hand-foot and mouth disease (HFMD) in local hospitals, the condition was still deteriorating and finally died. The 9 autopsy cases were diagnosed as brain stem encephalitis and death in forensic diagnose. Conclusion The HFMD disease caused by EV71 infection progresses rapidly often leads to medical disputes, the patient's family and hospital's opinions on the causes of death in patients are often different,identifying the cause of death is conducive to resolve the disputes.
2.Clinical applications of free fibula flap in repair of large composite tissue defect in upper limb

Objective:To evaluate the clinical effect of fibula flap in the repair of large segment of composite tissue defect in upper limb.Methods:From April, 2015 to June, 2019, 7 patients with large composite tissue defects in upper limbs were treated. All of them with various skin, vessel, nerve, tendon and other tissue defects. Repairing was well planned before surgery according to the type, location and size of defect. While in repairing of the bone and skin defect, fibula flap was taken from the shank and to repair the defects of nerve, tendon and vessels in upper limb. Regular followed-ups were made after surgery.Results:The 7 fibula flaps all survived. Postoperative follow-up ranged between 8 to 36 (averaged of 15) months. All the reconstructed limbs were in satisfactory appearance and function recovery. All the patients were able to manage their daily activities and live independently. The shape and function of donor sites were good. According to the Enneking system, the outcomes were graded as excellent in 4 cases and good in 3, with the average score was 25.9 points.Conclusion:Free grafting of vascularised fibula flap is especially feasible to be used in the repair of large bone tissue defect of upper limb. It repairs the defects of skin, vessel, nerve and tendon with the flap from a single donor site.
3.Pulmonary Infection Status, Drug-resistance and Risk Factors of Extended-spectrum ?-Lactamases-producing Bacteria

OBJECTIVE To survey the patient with pulmonary infection induced by extended-spectrum ?-lactamases-producing bacteria and their Enterobacteriaceae clinical characteristics, drug-resistance and countermeasure. METHODS Isolation, cultivation, identification, drug-sensitivity tests and confirmation of ESBLs-producing bacteria were done for the bacteria of sputum specimens collected from our hospital from Feb 2001 to Sept 2004. Susceptibility testing was performed by disk diffusion(K-B)method. RESULTS Totally 541 strains of Enterobacteriaceae were cultivated altogether and ESBLs-producing bacteria were 135 strains. The ESBLs- producing strains were sensitive to imipenem, and the resistance rates to it were 0.00% . The resistance rates of ESBLs-producing strains to cefoperazone/sulbactam and piperacillin/tazobactam were 31.11% and 44.44%, respectively . 5.A Meta analysis on the effectiveness of multiple health education implementation forms on exercise behavior among college students

Objective:
To compare the effectiveness of three forms of health education including classroom teaching, network teaching and after class tutoring on exercise behavior among college students.
Methods:
A literature search was performed using web of science, PubMed, ScienceDirect, Scopus, Cochrane Library, and CNKI database on randomized controlled trials of health education intervention on exercise behavior among college students. Literature filtering, data extraction, and assessment of the risks of bias were conducted by two independent researchers using Stata 16.0 software based on traditional Meta analysis and network meta analysis.
Results:
A total of 23 literatures were included. The results showed that health education courses could effectively increase the weekly exercise time of college students( SMD =0.29), exercise frequency ( SMD =0.25) and number of students participating in exercise( OR =1.33). Subgroup analysis revealed that the increase in classroom teaching and network teaching associated with increased weekly exercise time ( SMD =0.45, 0.36), exercise frequency ( SMD =0.22, 0.37) and exercise number ( OR =1.36, 1.26)(all P <0.05); The effect of after class tutoring on the time and frequency of weekly exercise was not statistically significant( P >0.05). Indirect comparison showed that the effects of classroom teaching and network teaching on weekly exercise time, and exercise frequency were superior to that of extracurricular counselling( P <0.05); There were no significant differences in the weekly exercise time, frequency and number of participants between the two groups( P >0.05). Based on SUCRA method and the effect size, health education in the form of classroom teaching may have the greatest effect on the weekly exercise time, frequency and the number of students participating in the exercise( SUCRA =91.5, 94.1, 87.1).
Conclusion
Health education in the form of classroom teaching and network teaching can positively influence exercise behavior of college students.It is suggested that health education courses should be carried out in college courses so as to better promote college students to participate in physical exercise.
6.Meta analysis of the effects of high intensity interval training on metabolic syndrome in overweight college students

Objective:
In this study, a meta analysis was conducted to evaluate and compare the effects of different types of interval training, such as, repeated sprint training(RST), high intensity interval training(HIIT) and sprint interval training(SIT) on body index of overweight/obese college students and the effect of moderate intensity continuous training(MICT), so as to provide a reference for taking appropriate exercise measures.
Methods:
The data was searched and selected from the database of Web of Science, PubMed, Scopus, The Cochrane Library and CNKI, and from the articles about random research on the effects of HIIT, SIT, RST, and MICT on overweight/obese college students for the evaluation of bias risk. And Stata 16.0 software was used for Meta analysis and network analysis.
Results:
Totally 815 samples selected from 18 articles were included in the study. Meta analysis showed that HIIT( SMD=-0.26, 95%CI =-0.52--0.00, P <0.05) and SIT( SMD=-0.39, 95%CI =-0.72--0.07, P <0.05) could make greater effects on BMI than MICT. The differences between RST and MICT were of no statistical significance( SMD=0.28, 95%CI =0.32-0.87, P >0.05). According to the SUCRA method combined with effect size, the best effect on improving MetS related physiological indexes of overweight college students was found in SIT( SUCRA =79.3), followed by HIIT( SUCRA =78.2), RST( SUCRA =56.8), and MICT( SUCRA =35.7).
Conclusion
Compared with MICT, high intensity interval training can greatly improve BMI of the obese/overweight college students, and could achieve the optimal effect of reducing the fat by exercising through SIT.
7.The cranial MRI appearance of cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy in a family

ObjectiveTo recognize the cranial MRI appearance in cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy(CADASIL). MethodsFive patients with CADASIL from two generations in a family underwent routine MRI and MRA examinations.Three patients with CADASIL were confirmed by the Notch3 genetic testing and the vascular pathological Resultsand one was diagnosed on basis of MR and clinical manifestations. The imaging data from 4 patients with CADASIL were analyzed.ResultsFour cases achieved preliminary diagnosis of CADASIL and one was excluded by MRI.In 4 patients with CADASIL,bilateral symmetrical,confluent white matter lesions in the subcortical and Deriventricular regions were seen frequently in the temporal,frontal and parietal lobes,but the occipital lobes were less involved.These lesions appeared as long T1 and long T2 signal.O'Sul]ivan sign was shown in all cases and subcortical lacunar lesions was seen in 2 cases. In the centrum semiovale,well-defined,round or oval cystic infarcts(black holes)were demonstrated in 3 cases and multiple tiny round enlarged perivascular spaces(pepperpot appearance)in all cases.The corpus callosum was involved in all cases,and it was evidently atrophic in 2 cases.The anterior part of internal capsule and external capsule were were present in the basal ganglia and brainstem. Only one case revealed a small infarct in the right cerebellum. Four Datients shared mild or moderate atrophy of brainstem, cerebellum and cerebrum. No marked abnorillalitv of large vessels was seen in all cerebral MRA. ConclusionsThe cranial MRI appearance in CADASIL shows some characteristics.MRI may give some infotznation in the preliminary diagnosis or exclusion of CADASIL.
8.Risk factors for 302 hepatic malignancies cases with residual tumor after ultrasound-guided radiofrequency ablation treatment

Background and purpose: Radiofrequency ablation (RFA) is one of the effective treatment methods for primary liver cancer and metastatic liver cancer. The purpose of this study was to investigate the risk factors of residual tumor after RFA for hepatic malignancies. Methods:A total number of 302 hepatic malignancies cases with 691 tumors after ultrasound-guided RFA from Jan. 2010 to Mar. 2013 were retrospectively analyzed. Single factor and multi-factor Logistic regression model were used to analyze the risk factors of residual tumor after RFA. Results:Complete ablation cases were 90.07%(272/302) for patients and 91.46%(632/691) for tumors, and the ablation residual rate was 8.54%. Ablation residual rates for tumor ≤3 cm, 3-5 cm and >5 cm in diameter were 6.30%, 9.57% and 28.57%, for tumor close to the intrahepatic vascular and gallbladder were 17.14%and 18.52%, for with and without combination with other local treatments were 7.02%and 13.41%, respectively. Multivariate analysis showed that tumor size>5 cm (P=0.044), proximity to large vessel (P=0.039) and without combination with other local treatments (P=0.001) were independent risk factors for ablation residual. Multivariate analysis showed that tumor near the intrahepatic vascular (P=0.014), single needle RFA (P=0.047) and without combination with other local treatments (P=0.023) were independent ablation residua risk factors for tumors between 3-5 cm in maximum diameter. Conclusion:Ultrasound-guided RFA can achieve satisfactory ablation effect. Tumor close to the intrahepatic vascular, tumor diameter>5 cm and without combination with other local treatment act as the independent risk factors for ablation residual. For tumors between 3-5 cm in diameter, in addition to close to intrahepatic blood vessels and without combination with other local treatment, single needle RFA is also another independent risk factor for ablation residual, and double-needle or multi-needle treatment can improve the ablation efifciency and reduce residual rate.
9.Early outcome of proximal femoral nail antirotation and bipolar hemiarthroplasty in treatment of intertrochanteric fractures in elderly patients aged 90 years or more

Objective:To compare the early outcome of proximal femoral nail antirotation (PFNA) and bipolar hemiarthroplasty in treatment of intertrochanteric fractures in elderly patients aged 90 years or more.Methods:A retrospective case-control study was conducted to analyze the clinical data of 43 elderly patients aged 90 years or more with intertrochanteric fractures admitted to Henan Provincial People's Hospital from January 2017 to June 2018. There were 12 males and 31 females, aged 90-102 years [(92.3±2.5)years]. A total of 31 patients were treated by PFNA (PFNA group) and 12 patients by bipolar hemiarthroplasty (hemiarthroplasty group). Comparisons were made between the two groups in terms of operation time, intraoperative blood loss, time of weight bearing after operation, rate of blood transfusion, incidence of complications 2 weeks and 3 months after operation, Harris score 12 months after operation and mortality 12 months after operation.Results:All patients were followed up for 1-30 months [(19.1±9.8)months] in PFNA group and for 0.5-29 months [(18.6±10.6)months] in hemiarthroplasty group. Operative time was (95.8±31.0)minutes in PFNA group, shorter than (128.8±40.5)minutes in hemiarthroplasty group ( P<0.05). Intraoperative blood loss was (71.3±25.7)ml in PFNA group and (212.5±113.1)ml in hemiarthroplasty group ( P<0.05). Time of weight bearing after operation was (43.9±31.9)days in PFNA group, longer than (5.9±2.8)days in hemiarthroplasty group ( P<0.05). The perioperative blood transfusion rate in PFNA group (29%) was less than hemiarthroplasty group (75%) ( P<0.05). Incidence of complications 2 weeks after operation was similar between PFNA group (23%) and hemiarthroplasty group (42%) ( P>0.05). Incidence of complications 3 months after operation in PFNA group was 26%, similar with 42% in hemiarthroplasty group ( P>0.05). Harris score 12 months after operation in PFNA group was (56.3±32.3)points, comparable to (59.3±36.7)points in hemiarthroplasty group ( P>0.05). Mortality rate 12 months after operation was 19% in PFNA group and 25% in hemiarthroplasty group ( P>0.05). Conclusion:For intertrochanteric fractures in elderly patients aged 90 years or more, PFNA and bipolar hemiarthroplasty are both effective treatments, but PFNA may be a better choice with shorter operation time and less intraoperative blood loss.
10. Reconstruction of thumb defect by transplanting second toe combined with hallucis flap: 9 cases report

Objective:
To introduce the method for reconstruction of thumb defect by transplanting free flap of second toe combined with the hallucis flap.
Methods:
From June, 2012 to February, 2017, a total of 9 cases of thumb defect were treated. The average age of these 9 patients was 26 years. The plane of thumb defect was class II area A to class III area A. According to the condition of thumb defect, designed the incision on the hallux and the second toe of the contralateral foot, and cut the 2nd digit and big-toe nail flap with a common arterial trunk. The nail of hallucis flap was rotated 90 degree and connected to the distal end of the 2nd toe. The hallucis flap covered the narrow neck of the 2nd toe. Thus, the circumference of the reconstructed finger and the length of the toenail were increased. Regular followed-up was made after operation.
Results:
All 9 transplantation flaps survived, and donor sites healed primarily. Postoperative followed-up period ranged from 4 to 12 (averge,7) months. All the reconstructed thumbs survived and donor toes were in satisfactory appearances. According to the Functional Assessment Criteria of the Upper Limb Formulated by the Hand Surgery Society of Chinese Medical Association, the outcomes were graded as excellent in 6 cases and good in 3 cases. All the patients were able to manage their daily activities independently. Donor toe injury was small, and their shape and function was good.
Conclusion
Free flap of second toe combined with hallucis can effectively improve the appearance of the reconstructed thumb. The appearance of the reconstructed thumb is symmetrical, beautiful, and the function is good.
