1.CT and MRI findings of ovarian fibroma

Objective To explore the imaging characteristics of ovarian fibroma on CT and MRI.Methods The CT and MRI findings of 12 patients with ovarian fibroma proved by surgery and pathology,including 12 cases of CT scan and 5 cases of MRI scan,were retrospectively analyzed.Results All of 12 patients with ovarian fibroma showed solitary masses with well defined margin and roughly round or mildly lobulated shape.On CT plain imaging,3 ovarian fibromas were homogeneous or roughly homogeneous solid masses,8 mixed masses with cystic and solid parts,and 2 mainly cystic masses.5 masses with MRI showed isointensity or mildly hypointensity on T1WI and significantly inhomogeneous intensity on T2WI with stripy and flaky hypointensity in solid parts.The solid parts of mass were enhanced slightly and gradually after intravenous administration of contrast material,and 4 masses showed complete envelopes on enhanced MRI.10 cases showed little of ascites concomitantly.Conclusion The CT and MRI findings of ovarian fibroma are characteristic to some extent.Masses with well defined margin and roughly round or mildly lobulated shape,slight and gradual enhancement,hypointensity of solid part on T2WI and complete envelope showed on MRI,suggest the possible diagnosis of ovarian fibroma.
2.Study on changes of ROS, GSH content, and GSH - PX activity of serum in persons under different state of exercise

ObjectiveTo probe the alterated characteristic of reactive oxygen species, reduce glutathione and glutathione peroxidase (GSH - PX) in serum of persons. Method Under the controlled load by treadmill, correlative indexes were determined during resting state, sub - maximalload and exhaustive exercise. ResultROS level of serum increased significantly during sub- maximal load compaired with that of restingstate. The ROS level during exhaustive exercise was higher than that of sub - maximal load( P <0. 01 ) . GSH content and GSH - PX activity de-creased significantly( P <0. 001 ). GSH content and GSH - PX reactivity decreased significantly( P <0. 001 ) during sub - maxiaml load and in-creased after exhaustive exercise which was still lower that of resting state. Conclusion ROS increased with the enhancement of exercise intensityand the time. GSH and GSH - PX showed a significant decrease in content and activity respectively followed by apparent enhancement.

The stem cell hypothesis for cancer suggests that only a specific subset of cancer cells within each tumor is responsible for tumor initiation and propagation,termed cancer stem cells(CSCs),which have a character of self-renewal and multilineage differentiation,and responsible for the recurrence,metastasis and resistance to therapy.The CSC of pancreatic cancer were isolated and identified in 2007,and recent data has been reported to confirm the change of the related signal pathway,the relationship with metastasis,therapy resistance and the puzzle.This review article summarizes the latest advances with regard to the pancreatic cancer stem cells.
4.Reasoning and discussion of spiral CT localization formula

Using geometry principle of the spiral CT localization , functional relation was deduced between the bed coordinates and the diagram layers coordinates, i.e.when gantry angle is zero degree, the function relation type is Twm= S190/Cos? + M; Otherwise, it is Twm= S190/Cos? +M. Combined with the actual operation of CT localization, the formula of the CT localization is:Twm=S190/Cos?+M. Finally, the physical meaning and the simplified condition of the formula were discussed.
5.Arthroscopic posterior cruciate ligament reconsruction by double-tunnel of femur and inlaying of tibia

[Objective]To describe a new way of arthroscopic posterior cruciate ligament reconstruction and evaluate the early stage of follow up results.[Method]Twenty four consecutive cases of isolated and combined posterior cruciate ligament deficiency were studied.Quadriceps tendon patellar bone was used for preparation of "Y" shaped autograft.Double-tunnel of femur were drilled and tibia was inlayed at their anatomical footprint.[Result]All cases were followed up for 24 months to 30months,averaging 24.5 months.Lysholm rating scales were used to make clinical subjective evaluations.Lysholm scores raised from 49.20?1.44 before operation to 87.91?1.52 after operation.Finial Lysholm evaluation showed 7 patients were excellent,16 good,and 1 poor.The good to excellent rate was 95.8%.Anteroposterior translation as clinical objective evaluations was measured with KT-1000.KT1000 arthrometer measurement showed that the average posterior translation improved from 12.85?1.10mm preoperatively to 3.98?1.99mm postoperatively.One patient was graded as failure.[Conclusion]Arthrosopy assisted prodedure of double bundle posterior cruciate ligament reconsruction using quadriceps tendon patellar bone autograft by double-tunnel of femur and inlaying of tibia is effective and reproducible.The short follow up clinical results are encouraging.
6.Comparative Analysis of Local Anaesthesia and Epidural Anaesthesia in Tension-Free Herniorrhaphy

Objective To investigate the most suitable anaesthesia method for the tension-free herniorrhaphy.Methods A total of 422 unilateral inguinal hernia cases from 2002 to 2005 were collected and randomly divided into the local anaesthesia group and epidural anaesthesia group. Observation indices and some relative data, such as operative duration, date of ambulation, date of foodintake, length of hospital stay, operation-correlated complications, anaesthesia complications, usage rate of ancillary drug, satisfactory rate for anesthesia, cost of hospitalization, were included and recorded in the questionnaire, and all the patients who took the tension-free herniorrhaphy were asked to answer it as the follow-up research. Results It was found that the occurrence of postoperative anaesthetic complications, the cost of hospitalization, length of stay of local anaesthesia group were significantly less than those of epidural anaesthesia group, and the date of moving and the date of foodintake were also significantly earlier than those of the other group (P0.05). Conclusion The local anaesthesia is suitable for most of the tension-free herniorrhaphy, and it may be used as the conventional anaesthetic method.
7.Comparison of postoperative recovery situation between laparoscopic radical resection and conventional open surgery for patients with colorectal cancer

Objective: To compare postoperative recovery situation between laparoscopic radical resection and conventional open surgery for patients with colorectal cancer and determine the optimal surgery scheme. Methods: 96 patients with colorectal cancer were divided into observation group (48 cases)and control group (48 cases) according to the random number table method. The patients of observation group were treated by using laparoscopic radical resection while the patients of control group were treated by conventional open surgery. A series of clinical manifestation, such as perioperative situation, abdominal pain, recovery situation, complication, pre-and post operative nutrition and quality of life, of these patients were compared between the two groups. Results: The operation time, average bleeding volume, postoperative exhaust time and hospitalization time of the observation group were significantly less than those of the control group (t=5.583, t=48.934, t=7.347, t=4.862; P<0.05). The incidence of abdominal complication of observation group (8.3%) was significantly lower than that of the control group (27.1%) (x2=5.79, P<0.05). The incidence of delayed healing of incision was 6.3% in observation group, which was significantly lower than that (20.8%) of control group (x2=4.36, P<0.05). For observation group or control group, the serum ALB, PA and LC of post-operation, respectively, was higher than those of pre-operation in 1 week after operation. And serum ALB, PA and LC of observation group, respectively, was significantly higher than that of control group (t=4.628, t=3.642, t=3.412; P<0.05). Compared with before operation, physical function, role function, emotional function, cognitive function and social functionthe quality of life of the two group were obviously improved, on the other hand, all of these aspects of the observation group were significantly higher than those of the control group (t=-12.845, t=-7.634, t=0.5001, t=-4.096, t=-2.647, P<0.05). Conclusion: Compared with the traditional open surgery for patients with colorectal cancer, the postoperative recovery of laparoscopic radical resection is better, more obvious and more effective for them.
8.The significance of serum N-terminal pro-brain natriuretic peptide levels for prognosis of patients in intensive care unit

Objective To assess the significance of elevated serum N-terminal pro-brain natriuretic peptide (NT-proBNP) levels in predicting 30-day mortality rate for patients in intensive care unit (ICU).Methods A retrospective study was conducted.The clinical data of 480 patients admitted to ICU in Tianjin First Center Hospital from June 2012 to October 2013 were studied.The characteristics of patients were recorded,the acute physiology and chronic health evaluation Ⅱ (APACHE I) score was calculated,and the routine blood tests,blood biochemistry,and serum NT-proBNP level within 24 hours after admission were determined.Patients were divided into non-survival and survival groups according to 30-day outcome,and the characteristics and clinical parameters were compared between two groups,and single factor analysis affecting prognosis was analyzed.All patients were divided into four groups according to four percentile method as NT-proBNP<280 ng/L (group Q1),280 ng/L≤NT-proBNP< 1 200 ng/L (group Q2),1 200 ng/L≤NT-proBNP<1 990 ng/L (group Q3),1 990 ng/L≤NT-proBNP<4 700 ng/L (group Q4).A Cox regression analysis was used to test the independence of the predictors.A receiver operator characteristic curve (ROC curve) was plotted to compare the power of APACHE Ⅱ score and NT-proBNP in predicting the outcome.Kaplan-Meier analysis was used to assess survival rate among different NT-proBNP patients.Results By single factor analysis it was shown that age,APACHE Ⅱ score,creatinine,NT-proBNP and severe infection were those influencing the outcome.NT-proBNP 1 200-4 700 ng/L (groups Q3 and Q4) and APACHE Ⅱ score >20 were independent risk factors for predicting outcome [relative risk (RR) for group Q3 was 3.624,95% credibility interval (95%CI) was 1.258-10.138; RR for group Q4 was 5.059,95% CI was 3.202-9.993; RR for APACHE Ⅱ score >20 was 2.987,95% CI was 1.345-5.823].The area under the ROC curve (AUC) for APACHE Ⅱ score and serum NT-proBNP was similar,and it was 0.831 (95%CI 0.778-0.884) and 0.876 (95%CI 0.827-0.925),respectively.The cut-off value of NT-proBNP for predicting mortality was 1 250 ng/L.Kaplan-Meier analysis showed that groups of Q2,Q3 and Q4 had lower cumulative survival rate compared with group Q1 (26.5%,25.4%,16.2% vs.29.6%),and group Q4 showed most significant extent of lowering of survival rate (x2=122.920,P< 0.05).Conclusion Elevated NT-proBNP level in ICU patients signify severity of the disease,and it is an independent predictor of death of ICU patients.
9.Construction of calcium sulfate/bone marrow mesenchymal stem cells tissue-engineered bone for spinal fusion

BACKGROUND:Calcium sulfate has good biocompatibility and biodegradability, which is a safe and effective bone graft substitute.
OBJECTIVE:To investigate the osteogenesis ability of calcium sulfate combined with bone marrow mesenchymal stem cells.
METHODS:After L4/5 posterior lumbar discectomy, 36 rabbits were randomized into three groups:rabbits in autologous bone group were implanted with autologous iliac bone via the intervertebral space;animals in al ogenic bone group were implanted with decalcified bovine bone;rabbits in tissue-engineered bone group were implanted with calcium sulfate combined with bone marrow mesenchymal stem cells. Bone formation and molding were observed by gross observation, anteroposterior and lateral X-ray, histology and biomechanics at 4, 8 and 16 weeks. Cal us specimens were employed for histological observation of interbody fusion. Biomechanical analysis of spinal fusion site was conducted at 16 weeks.
RESULTS AND CONCLUSION:Sixteen weeks later, interbody fusion was complete in the autologous bone group, the trabecular bone bridged continuously and a large amount of woven bone was merged into pieces;in the al ogenic bone group, incomplete bony fusion was found between the intervertebral space, most of cartilage tissues differentiated into bone, but fibrous tissue was also ful of the central part;in the tissue-engineered bone group, interbody fusion was complete, and a large amount of woven bone was fused into pieces, while the artificial bone was absorbed and ossified with smal residual. Failure strength and stiffness in the autologous bone and tissue-engineered bone groups were superior to those in the al ogenic bone group. These findings indicate that the calcium sulfate/bone marrow mesenchymal stem cells tissue-engineered bone has excellent osteogenic and osteoinductive capacity that can exert a good function of promoting spinal interbody fusion.
