To observe the effect and its potential mechanism of monoclonal antibody to tumor necrosis factor-alpha(TNF? MoAb)on systemic hemodynamics and survival rate after intestinal ischemia/reperfusion (Ⅱ/R). Method: SD rats were subjected to 75 rain of superior mesenteric artery occlusion followed by 6 hours of reperfusion. The animals were treated intravenously with either TNF? In order to investigate if there exists interaction between mitogen-activated protein kinase (MAPK) and signal transducer and activator of transcription 3 (STAT3) protein, and how the interaction regulates tumor necrosis factor-? (TNF-?) transcription activity, the human p38 and extracellular-signal regulated protein kinase 2 (ERK2) genes were amplified from human flag-p38 and flag-ERK2 by polymerase chain reaction (PCR) and cloned into pcDNA3-HA. Protein expression of the plasmids was examined by Western blotting. Co-immunoprecipitation was used to identify if there exists interaction between MAPK and STAT3 proteins. If the interaction was approved to be true, report gene system was applied to find how the interaction affect transcriptional expression of TNF-?. After STAT3 pathway was inhibited by RNA interfering, the action on TNF-? activity was determined. The results of DNA sequencing and enzyme digestion showed that the cloned p38 and ERK2 genes were correct, to be 1 080 bp or so. p38 and ERK2 proteins were expressed in 293T cell to be approximately 40 ku. Co-immunoprecipitation data showed that p38 and ERK2 proteins integrated with STAT3 protein in vivo. TNF-? reporter gene activity results found that protein complex of p38-STAT3 and ERK2-STAT3 coordinately increased TNF-? activity. After STAT3 was interfered, the TNF-? activity markedly decreased. These data indicated that there exists interaction between p38 and STAT3 protein, ERK2 and STAT3 protein. The complex of the proteins can coordinately regulate TNF-? expression. After interfereing STAT3 pathway, the coordinated action on TNF-? transcription activity might be obviously reduced.

Objective Castleman′s disease ( CD) is a rare lymphoproliferative disorder, which has a very high misdiagnosis rate according to its lack of clinical and imaging specificity.The study was to improve the recognition, diagnosis and management of this disease by analyzing the clinicopathological characteristics and concluding diagnosis and treatment as well as literature review. Methods Retrospective analysis was conducted on clinical data of 31 CD patients (13 males and 18 females) who were hospitalized in the Affiliated Drum Tower Hosipital to Medical college of Nanjing University from January 2006 to Feburary 2014.Their mean age was (47.4 ±13.3) years.There were 24 cases with unicentric Castleman′s disease (UCD) and 7 cases with multicentric Castleman′s disease ( MCD) .UCD tended to be present in the form of an enlarged and painless mass which generally remained asymptomatic by accidental touch or regular physical examination.MCD was usually associated with constitutional symptoms.All the 24 patients with UCD underwent complete surgical resection.7 patients with MCD received different treatment methods, such as surgical resection, ste-riod and rituximab in combination with chemotherapy ( CHOP) . Results Postoperative pathology confirmed 23 cases were hyaline vascular type while 8 were plasma cell type.25 cases had been followed up (46.3 ±32.3) months after operation, among which 1 UCD case died of pancreatic head carcinoma 13 months after operation, 1 case with MCD plus pemphigus died 2 months after surgical resection of retroperitoneal lymphadenectasis, another MCD case died in 1 month without any treatment.The remaining 22 cases were alive. Conclusion CD is lack of clinical specificity and its diagnosis should be made by pathology.Each subtype differ greatly in prognosis, therefore different treatments should be taken after definitive subtype diagnosis.

Purpose: To explore the therapeutic method for lameness caused by lumbar spinal stenosis. Methods: 28 cases of the patients with lameness were treated by puncturing Dachangshu (BL 25), Weizhong (BL 40), Chengshan (BL 57) and Kunlun (BL 60) and simultaneously another 26 cases were treated with routine method for control study. Results:After the treatments for four weeks, the scores of the symptoms in the two groups were remarkably elevated. In the acupuncture treatment, the results showed remarkable effect in 4 cases, effect in 21 cases. In the routine treatment, the results showed remarkable effect in 4 cases and effect in 8 cases. Conclusion: acupuncture treatment is better than the routine conventional treatment for lameness caused by lumbar spinal stenosis.

Objective To investigate the infection status and drug suscepetibility of mycoplasma from 6 573 patients with non-gonococcal urethritis ,and to provide the scientific bases for the clinical application of antibiotics .Methods Mycoplasma detection kit was used to detect ureaplasma urealyticum (Uu) and mycoplasma hominis(Mh) and the drug susceptibility .All the patients were divided into two groups :Chinese group and foreigner group .Results Among 5 675 Chinese patients ,2 985 patients were infected by mycoplasma(52 .6% ) .The infection rate of Uu was 2 312(40 .7% ) .35 .2% patients were male ,and 61 .4% patients were female .In 898 foreign patients ,440 patients were infected by mycoplasma(49 .0% ) .The infection rate of Uu was 327(36 .4% ) .32 .2% pa-tients were male ,and 59 .5% patients were female .In Chinese patients infected by Uu ,the susceptibility rates to MIN ,DOX ,JOS and CLA were 96 .7% ,96 .2% ,93 .7% ,89 .7% ,respectively .In foreign patients ,the susceptibility rates to MIN ,DOX ,JOS ,and CLA were 98 .9% ,98 .4% ,95 .8% ,92 .1% .Conclusion The mycoplasma infection rate of Chinese patients is higher than foreign patients .In both groups ,Uu infection is the main type .Female patients are more than male patients .The drug sensitivity rate in for-eign group is higher than that in Chinese group .mycoplasma are sensitivity to MIN ,DOX ,JOS .

Objective To construct,express,purify and identify the gene encodi ng major outer membrane protein of Neisseria gonorrhoeae (Porin I, or PI). Metho ds The gene encoding for PI of N.gonorrhoeae was amplified by PCR and cloned int o expression plasmid pGEX-4T-2 to form pGEX-4T-2/PI recombinants. A high lev el expression of GST-PI fusion protein was obtained in GST gene fusion system (GST:glutathione S transferase). The analysis indicated that the expressed pr otein was present predominantly in the insoluble form. Therefore, the induced pr otein was purified by SDS-PAGE, and bands corresponding to polypeptides of GST-PI fusion protein were excised and subjected to electroelution. A dot immunoch romatographic assay was employed to demonstrate whether the purified protein was gonococcal PI specific. Results The pGEX-4T-2/PI expression recombinants were constructed,expressed,purified and identified successfully. SDS-PAGE analysis and dot immunochromatographic assay suggested that the recombinant GST-PI fusio n protein was a 60 000 molecular weight protein andidentical in size to native PI and reacted with anti-PI monoclonal antibody. Conclusion Our results may lead to a potentiality for further study of diagnosti c kits and vaccine for Neisseria gonorrhoeae.

Objective To test the hypothesis that in a rat model of acute intestinal ischemia/reperfusion injury, endotoxemia from the gastrointestinal tract would play a role in mediating tumor necrosis factor (TNF) response and remote organ dysfunction. Methods Wistar rats underwent 45 minutes of superior mesenteric artery occlusion followed by 6 hours of reperfusion. The rats were treated intravenously with either TNF monoclonal antibody (MoAb) or saline solution 90 minutes prior to the onset of ischemia. Results Significant elevation of plasma TNF level in both portal and systemic circulation was detected immediately after the onset of reperfusion. The level peaked at two hours after reperfusion (P<0.01). Similarly, a remarkable TNF mRNA expression in the intestine in controls was detected at 0.5 hour post-reperfusion, and sustained a marked elevation throughout the observation period (P<0.05-0.01). TNF elevation was found associated with gut-origin endotoxemia, and the maximal TNF response occurred approximately 0.5-2 hours after the initial appearance of endotoxin in the portal vein. Concomitantly, multiple organ dysfunction in response to local ischemic insult was also observed in untreated controls upon reperfusion, but it was significantly attenuated by pretreatment with MoAb against TNF. Conclusions Intestinal injury can result in the gut becoming a cytokine-liberating organ. The escape of endotoxin and bacteria from the gut may be responsible for the TNF expression and release, which would be an important mechanism underlying pathophysiological alterations associated with intestinal ischemia/reperfusion injury.

OBJECTIVETo study active constituents of Macropanax rosthornii in treating rheumatoid arthritis.

METHODSilica gel column chromatography, preparative HPLC and modern spectrum techniques were applied for a systematic study on chemical constituents contained in M. rosthornii.

RESULTTwelve compounds were separated from M. rosthornii and identified as serratagenic acid (1), serjanic acid (2), betulinic acid (3), 6beta-hydroxy-3-oxo-olean-12-en-28-oic acid (4), 3-O-alpha-L-arabinopyranosyl serratagenic acid (5), 3-O-alpha-L-arabinopyranosyl serratagenic acid-29-methyl ester (6) , 3-O-alpha-L-arabinopyranosyl serratagenic acid-28-O-beta-D-glucopyranosyl ester (7), scopoletin (8), beta-sitosterol (9), daucosterol (10), 3,4-dihydroxybenzoic acid (11), and stearic acid (12).

CONCLUSIONAbove compounds are separated from M. rosthornii for the first time.

OBJECTIVETo investigate the diagnostic value of contrast-enhanced ultrasonography (CEUS) in preoperative Borrmann classification of gastric cancer.

METHODSAsulfur hexafluonde-filled microbubble ultrasound contrast agent and continuous real-time imaging technique of contrast pulse sequencing were used. Two hundred and eighty-five patients with gastric cancer confirmed by biopsies who received preoperative CEUS examination were involved in this study. CEUS results were compared with postoperative pathological findings.

RESULTSThe accuracy rate of CEUS in determining the Borrmann classification of gastric cancer was 92.3%(263/285). The accuracy rates of BorrmannI(, II(, III(, IIII(, and IIIII( were 100%(12/12), 90.6%(77/85), 92.6%(126/136), 95.7%(45/47), and 60.0%(3/5) respectively.

CONCLUSIONCEUS is a useful diagnostic method for preoperative Borrmann classification of gastric cancer.

The monitoring of pregnant women is very important. It plays an important role in reducing fetal mortality, ensuring the safety of perinatal mother and fetus, preventing premature delivery and pregnancy accidents. At present, regular examination is the mainstream method for pregnant women's monitoring, but the means of examination out of hospital is scarce, and the equipment of hospital monitoring is expensive and the operation is complex. Using intelligent information technology (such as machine learning algorithm) can analyze the physiological signals of pregnant women, so as to realize the early detection and accident warning for mother and fetus, and achieve the purpose of high-quality monitoring out of hospital. However, at present, there are not enough public research reports related to the intelligent processing methods of out-of-hospital monitoring for pregnant women, so this paper takes the out-of-hospital monitoring for pregnant women as the research background, summarizes the public research reports of intelligent processing methods, analyzes the advantages and disadvantages of the existing research methods, points out the possible problems, and expounds the future development trend, which could provide reference for future related researches.
