2.Analysis for the needs in general practice training

Objective To study the changes in needs of standard training for general practice during the period of 2000 to 2006.Methods Sixty-two trainees participated in a municipal training program for general practitioners(GPs)in Zhongshan Hospital,Shanghai were interviewed by questionnaire to understand their recognition of general practice and needs in training for general practice,and compared to other 53 trainees who entered an another training program in 2000.Results Nineteen of 53 trainees(36percent)trained in 2000 could understand all the six aspects of general practice,including medical and health care,preventive care,rehabilitative care,health education and family planning techniques service,51(94 percent)were willing to accept such training,and 30(57 percent)did not satisfy with current conditions in community health care service.As compared,their recognition for general practice,acceptance of training,and satisfaction with their working condition improved obviously in trainees trained in 2006,with proportions of 100 percent,100 percent and 98 percent,respectively.But,main reason for dissatisfaction with community health care service could still attribute to less attention paid to general practice by relevant leadership.Conclusions Nowadays.the needs in general practice training by health care workers in communities increased greatly,but which still should be supported by governmental policies.
3.Survey on employment intention of medical students in six Chinese universities

Objective To survey the employment intention of medical students in different universities in China.Methods A total of 1354 fourth-year or above medical students from 6 medical universities in China were selected for survey by random cluster sampling.A self-designed questionnaire was used to investigate the basic information of students and their employment intention.Results Overall 46.5% (620/1333) of all students choose cities in economically-developed region as their desired working sites.45.5 % (363/797) of students studying in 4 Shanghai medical universities and 34.5% (185/536) in other 2 non-Shanghai universities would first choose their original cities as working sites,respectively.81.3% (1078/1326) of students preferred working in large public hospital,especially for those studying for higher degrees (88.0%,581/660).51% of undergraduates expected 4000 RMB as monthly pay for graduation.Male students (54.6%,332/608) expected more income than female (47.9%,335/700),graduates (57.9%,377/651) expected higher payment than undergraduates (44.1%,290/657).Conclusion The survey shows that medical students in China mainly prefer cities and large public hospitals as working sites for their career development.
4.Expression and antibody preparation of stretching sensitive gene transcription factor 4

Objective:To study the function of transcription factor 4 (TCF4) and to prepare TCF4 polyclonal antibody.Methods:pET-41/TCF4 was transformed into E.coli BL21(DE3) and induced by IPTG.The purified GST-TCF4 fusion protein was applied to immunize rabbit to produce antiserum. The specificity of the affinity of purified anti-TCF4 antibody was examined by Western blotting analysis of the eukaryotic expressed products of TCF4. Dig-labeled probe and antibody against TCF4 were used to examine the expression of TCF4 in Saos-2 cells under mechanical stretch. Results:Western blotting showed that the antibody could bind to TCF4 specifically. The expression of TCF4 mRNA and protein were significantly increased in Saos-2 cells under mechanical stretch. Conclusion: TCF4 antibody has been prepared successfully.
5.The Application of MR Diffusion-weighted Imaging in Discriminating Benign and Malignant Vertebral Fractures

Objective To investigate the value in distinguishing benign and malignant vertebral fractures with MR diffusion-weighted imaging(DWI).Methods 31 patients with 58 vertebral fractures were underwent routine MRI scanning and echo-planar DWI(EPI/DWI).The materials were divided into benign group(n=32,20 cases),malignant group(n=18,8cases),tuberculosis group(n=8,3cases) and normal vertebral body group(n=31). Signal intensities on DWI were compared and apparent diffusion coeffient(ADC) values of normal and abnormal vertebral bodies were calculated. ADC values among the benign,malignant,tuberculosis vertebral bodies and normal ones were quantitatively analyzed and compared.Results The ADC values of benign,malingnant,tuberculosis and normal vertebral bodies were (1.44?0.41)?10 -3 mm2/s,(0.94?0.17)?10 -3 mm2/s,(0.98?0.13)?10 -3 mm2/s and(0.42?0.16)?10 -3 mm2/s ,respectively. The ADC values of the three abnormal groups were higher than normal group ( all P0.05). The sensitivity and specificity of ADC for the diagnosis of malignant lesion were 72.2% and 90.6% if the upper bound of 95% confidence interval was set as a differential level.Conclusion DWI provides an important supplement to the routine MRI in the differentiation of benign with malignant vertebral fractures.
6.Establishment and preliminary analysis of fluid dynamics model of the subjects with micrognathia

Objective:To establish a fluid dynamics model of upper airway before and after surgery and explore the changes of three-dimensional fluid dynamics in patients with micrognathia.Methods:A patient with micrognathia and severe obstructive sleep apnea-hypopnea syndrome(OSAHS)accepted CT scan before and six months after mandibular advancement operation.Computation-al fluid dynamics model was built on the base of CT scan by Mimics 1 0.01 and ANSYS ICEMCFD1 4.0.The internal flow of upper respiratory tract was simulated by ANSYS-FLUENT 1 4.0 and the results were analyzed by Tecplot.Results:Fluid dynamics model of upper airway was constructed before and after the surgery respectively.The volume of the upper airway of the patient increased from 37.284 cm3 to 44.498 cm3;the most narrow area of upper airway was located in the lower bound of pharyngopalatiae,and it was augmented from 1 .1 35 cm2 to 2.297 cm2;the minimum pressure was decreased from1 01 308 Pa to 1 01 272 Pa;the maximum air velocity increased from 3.476 m/s to 4.978 m/s.Conclusion:Mandibular advancement may correct the occlusal deformity,ex-panse the upper respiratory tract,decrease the negative pressure and maintain the patency of the airflow in the treatment of patients with micrognathia and OSAHS.
7.Study of ERPs and Topographic Distribution Mapping on Obse ssive-Compulsive Patients

Objective: To study the characteristics of P300 and topographic dist ribution mapping in obsessive-compulsive patients. Methods: The P300 and topog raphic distribution mapping were recorded in 36 patients, using a Bneuro Galileoinstrument. Results: Compared with normal subjects, the wa ve patterns of obsessi ve-compulsive patients were unstable; the frontal wave variation and dissymmetry between the two sides was 63.9%; the N2 and P3 latency was prolonged; the P2 an d P3 amplitude was decreased; the P3 topographic distribution mapping was uneven ly distributed, the normal high amplitude in the parietal region was absent. For the patient group, energy levels below grade 5 in the left brain area, the fron tal area, and for both were 42.3%, 30.3%, 15.15% respectively. Conclu sion: P300 and topographic distribution might be served as an objective index for reflectin g cognitive activity in obsessive-compulsive patients.
8.MR diffusion weighted imaging in the differential diagnosis of cystic lesions of the pancreas

Objective To evaluate the value of diffusion-weighted imaging (DWI) in the differential diagnosis of cystic pancreatic lesions.Methods Thirty-four cystic pancreatic lesions confirmed clinically or pathologically were collected,including 11 case of non-neoplastic pseudocyst,5 cases of simple cyst,6 cases of serous cystadenoma,10 cases of mucinous cystadenoma and 2 cases of mucinous cystadenocarcinoma.All the patients underwent routine serial MR and echo-planar DW imaging examination of the pancreas with b values of 0 and 600 s/mm2.The appearances of signal intensity of DWI with all cysts were recorded.ADC maps were reconstructed,and the ratio of the cysts and cyst-to-pancreas ADC (ADCR) were calculated.The receiver operating characteristic (ROC) curves were constructed to evaluate the diagnostic value of ADC and ADCR.Results Among all the 16 non-neoplastic cysts,2 cysts were slightly hyperintense in DWI,and 14 cysts were isointense.Among all the 18 neoplastic cysts,17 cysts appeared slightly hyperintense or hyperintense in DWI,and 1 cyst was isointense.The difference between the two groups was statistically significant (P < 0.001).The ADC values of non-neoplastic and neoplastic cysts were (3.30 ± 0.30) × 10-3,(2.74 ± 0.34) × 10-3 mm2/s ; and the ADCR values of non-neoplastic and neoplastic cysts were 1.85 ± 0.20,1.31 ± 0.21,the difference between the two groups was statistically significant (P < 0.001).The areas under the ROC curves of ADC and ADCR diagnosing neoplastic cysts were 0.94 ± 0.04,0.98 ± 0.02,respectively.An ADC of 3.105 × 10-3 mm2/s showed 81.3% sensitivity and 94.4% specificity for predicting neoplastic cysts.An ADCR of 1.525 showed 100% sensitivity and 88.9% specificity for predicting neoplastic cysts.Conclusions Diffusion-weighted imaging is of importance in the diagnosis and differential diagnosis of pancreatic cysts,and can be applied as a complementary tool for routine MRI.
9.Study on Effects of Fructus Mume on Myoelectric Activity of Uterine Smooth Muscle of Unpregnancy and Early pregnancy Rats and Its Action Mechanism

Objective:To investigate the effect of decoction of Fructus Mume (DFM) on the myoelectric activity of uterine smooth muscle of unpregnancy and early pregnancy rats and study its action mechanism.Methods: A pair of bipolar electrode was implanted on the serous surface of rat uterus. The DFM was injected into abdominal and the changes of uterine myoelectrical activity were observed.Results: The higher dose of DFM could enlarge the average amplitude of slow wave of uterine smooth muscle myoelectricity as well as increase the incidence and the maximal amplitude of the outbreak plexiform myoelectric wave. On the other hand, rats during early pregnancy were more sensitive to the DFM. Conclusions: The DFM enhances the myoelectricity activity of uterine smooth muscle of unpregnancy and early pregnancy rats which may result from enhacing the start cells electric activity and speeding up the action potential depolarization. Thus the DFM can serve as an effective clinical drug for anti early pregnancy.
10.Establishment of Murine Tumor Cell Line Expressing HPV types 6b and 11 E6/E7 Genes

Objective To construct four expression plasmids, pcDNA3.1-GFP/HPV6bE6, pcDNA3.1-GFP/HPV6bE7, pcDNA3.1-GFP/HPV11E6, pcDNA3.1-GFP/HPV11E7 and their transfected murine cell lines. Methods The Four recombinant expression plasmids comprising HPV6bE6,HPV6bE7,HPV11E6 and HPV11E7 linked with GFP, respectively, were constructed and transfected to B16 cells by lipofectamine kit. Positive clones were selected by G418 and observed by fluorescent microscopy and identified by RT-PCR. Results The four constructed recombinant plasmids were authenticated by restriction enzyme digestion and DNA sequencing. Under the fluorescent microscope, the green fluorescence could be observed in cytoplasm and nucleus of four transfected B16 cell lines. The RNA extracted from positively transfected clones resistant to G418 were analyzed by RT-PCR, which demonstrated the presence of four expected fragments. Conclusions The transfected murine cell lines B16 can express HPV6bE6,HPV6bE7,HPV11E6 and HPV11E7 gene. These transfected cell lines can be further transplanted to mice in order to investigate the biological properties and immunological mechanisms of these genes in vivo.
