Anti-phospholipid antibody and lupus anticoagulant appear in the blood of patients with human immunodeficiency virus (HIV)infection,which result in the increased levels of von Willebrand factor and D-dimer,and decreased the levels of protein S,protein C,antithrombin and heparin cofactor,and thus rgsult in hypercoagulation status.In addition,the HIV infection itself and secondary opportunistic infections may also cause endothelial injury.Both hypercoag ulation status and endothelial injury may promote the occurrence of thrombotic diseases.

As a rational economic man and agent, in the primary care Physicians not only pursue target in-come, but also pursue the utility of the agent. Different health care payment has different effect on physician, due to imperfect physician agent of the primary health care, the health care payment is difficult to achieve economic neutral. Based on the theory of target income and physician-induced demand, the paper proves that in the primary health care with the third party payment, there should be a prospective component of payment and fees should be lower than mar-ginal cost, which can not only improve the role of agent, but also constraint the physician-induced demand, so as to achieve health care for physician’s behavior constraints.
3.Effect of α-keto acid combined with low protein diet on kidney function, calcium-phosphorus metabolism and adenosine deaminase activity in malnourished patients with long term peritoneal dialysis

Objective To investigate the effect of α-keto acid combined with low protein diet on kidney function, calcium-phosphorus metabolism and adenosine deaminase activity in malnourished patients with long term peritoneal dialysis.Methods Forty-six cases malnourished patients with long term peritoneal dialysis were selected from May 2013 to August 2014 in the hospital and divided into two groups.The control group (n=23) were treated by conventional therapy, acceptable daily intake of protein 1.2 g/(kg? d).The experiment group (n=23) were treated by α-keto acid combined with low protein diet, acceptable daily intake of protein 0.8 g/(kg? d).Two groups were treated for three months.Serum calcium, phosphorus, nutritional status assessment, adenosine deaminase activity and renal function were observed and compared after treatment.ResuIts After twelve months ’ treatment, compared with control group, serum creatinine, blood urea nitrogen, serum uric acid of experiment group were lower ( P<0.05); serum calcium level was higher (P<0.05), serum phosphorus level was lower (P<0.05);ADA activity and white blood cell count were lower (P<0.05). The incidence of malnutrition rate had no significant difference between two groups(χ2 =3.136,P>0.05).ConcIusion α-keto acid combined with low protein diet could effectively increase serum calcium level, reduce serum phosphorus level, ADA activity and WBC count, and improve nutritional status in long-term peritoneal dialysis patients, which has important significance.

Objective To detect the expressions of vascular endothelial growth factor(VEGF) and Periostin in patients with diabetic retinopathy (DR),and to explore its chnical significance.Methods A total of 52 patients with DR (DR group),36 non-DR diabetic patients (non-DR group) and 30 healthy person (normal control group) were enrolled.The 52 cases of DR patients were divided into non-proliferative DR (PDR) group (non-PDR group,24 cases) and PDR group (28 cases).The expressions of serum VEGF and Periostin were detected with enzyme-linked immunosorbent assay analysis.Results The serum expressions of VEGF and Periostin were significantly higher in non-DR group and DR group than those in normal control group [(122.63 ±28.74),(163.58 ±42.37) mg/L vs.(91.53 ± 19.58) mg/L,(110.15 ±32.62),(146.51 ± 41.74) mg/L vs.(82.26 ± 21.17) mg/L,P < 0.05 or < 0.01].The serum expressions of VEGF and Periostin were significantly higher in DR group than those in non-DR group (P <0.05).The serum expressions of VEGF and Periostin were significantly higher in PDR group than those in non-PDR group [(174.15 ±47.31) mg/L vs.(147.66 ±38.25) mag/L,(160.31 ±46.43) mg/L vs.(132.14 ±35.62)mg/L,P < 0.05].The serum expression of VEGF was positively related with Periostin (r =0.415,P < 0.01).Conclusion The high-expressions of VEGF and Periostin are found in DR patients,and which maybe play a key role in the early diagnosis and assessment the progression of DR patients.
10.Exploration for pathogenic factors and diagnosis of cerebral palsy in children

To explore the pathogenic factors and diagnosi s of cerebral palsy (CP) in children, perinatal history, clinical evaluation, im aging and electrophysiological data and parents' attitude for rehabilitation of CP were analyzed and investigated in 50 children with CP retrospectively. The re sults showed that prem ature birth, low birth weight and perinatal asphyxia were the most pathogenic fa ctors induced CP. The common CP types was spastic paralysis, in particular quadr iparesis. Abnormal CT and MRI findings were detected in 68% of CP children. It w as noticed that 44% of CP children were with speech and language disorder, 32% w i th mental retardation, 18% with hearing impairment, 16% with visual impairment a nd 16% with epilepsy. There were 86% parents to accept with physiotherapy on the ir CP children, 68% with acupuncture therapy, 46% with occupational therapy and 100% with speech therapy, respectively. It is suggested that multi-subjec t cooperation will be necessary for the follow-up study of the high- risk infants. The realization of the related-speciality knowledge a nd the enhancement of rehabilitational conception will be helpful for the early diagnosis and intervention of CP patients. The popularization of rehabilitationa l conception will be also important in the parents with CP children.
