3.Rat model of abdominal aortic banding and its clinical significance

Objective:To establish rat model of ventricular hypertrophy and heart failure by partially banding abdominal aorta for different time limits.Methods:60 Wistar rats were randomly divided into three groups:A group which were ligated for 4 weeks;B group being ligated for 8 weeks;and sham-operated group (each containing 20 Wistar rats).Left and right ventricular mass index,hemodynamic parameters,cardiac morphological changes and the time of achieving heart failure model were investigated in different groups.Results:There were 16.67% heart failure rats in A group,while 73.68% in B group.The death of models occurred in the first week after experimental procedure.MBP and LVSP were increased greatly in A group,and HR,MBP,LVSP and ?dp/dt were decreased while LVEDP increased in B group compared with A and sham-operated groups( P
4.Application of the two-paths double U-shaped fixer in the intravenous indwelling needle

Objective To study the application of the two-paths double U-shaped fixer in clinical intravenous indwelling needle fixation. Methods A total of 420 cases using of intravenous indwelling needle from January to June 2015 were involved and divided into experimental group(210 cases) and control group(210 cases) by random digits table method. The two-paths double U-shaped fixer was applied in experimental group and the control group were used tape by Y type to fix. The incidence of returning-blood, blocking, phlebitis, slippage and the comfort between two groups were observed and compared. Results The incidence of returning-blood, blocking, phlebitis, slippage in experimental group were 6.2%(13/210), 0.5%(1/210), 0 , which were lower than those in control group 48.6%(102/210), 5.7%(12/210), 1.4%(3/210) seperately, the differences were statistically significant (χ2=117.895, P<0.01). The incidence of phlebitis levelⅠ,Ⅱ,ⅢandⅣin experimental group were 3.3%(7/210), 1.4%(3/210), 0.9%(2/210), 0, which were lower than those in control group 11.9%(25/210), 9.0%(19/210), 3.8%(8/210), 2.4%(5/210), the differences were statistically significant(Z=-5.960, P<0.01). The incidence of the comfort level 1, 2, 3 and 4 in experimental group were 92.4%(194/210), 6.2%(13/210), 1.4%(3/210), 0 individually, which were higher than those in control group 36.7%(77/210), 46.7%(98/210), 12.4%(26/210), 4.3%(9/210), the differences were statistical significant (Z=-16.228, P < 0.01). Conclusions Two-paths double U-shaped fixationis is better than Y-shaped tape fixation. To compare the venous indwelling needle fixation, two-paths double U-shaped fixer could fix more effectively and safety. At the same time, it also has advantages in convenient, observation and appearance. It could be widely used in clinical practice.

ObjectiveTo study the risk factors of childhood functional constipation.MethodsA cross-sectional analysis of risk factors associated with 182 cases of functional constipation in children was conducted from March 2013 to February 2014. ResultsThe male patients were more than females, mainly in preschool children. The risk factors were did not like fruits and vegetables (52.2%), drank less water (44.0%), did not develop the habit of daily bowel movement (39.5%), defecation fear (36.3%), inadequate food intake (25.8%). There were different risk factors in different age stages.ConclusionsFunctional constipation is related to gender, age, diet habits and mental psychology.
8.Protective Effect of Compound Ginkgo Leaf Granules on Liver Function Damage of Acute Pancreatitis

OBJECTIVE:To investigate the protective effect of Compound ginkgo leaf granules on liver function damage of acute pancreatitis. METHODS:Totally 82 patients with severe acute pancreatitis(SAP)complicated with liver function damage se-lected from our hospital during Sept. 2013-Sept. 2015 were randomized into control group and observation group according to ran-dom number table,with 41 cases in each group. Control group was given SAP related routine therapy. On the basis of the control group,observation group was additionally given Compound ginkgo leaf granules 5 g added into warm boiled water 80 mL via naso-gastric tube,tid,one week as a treatment course,for 2 courses. The remission time of abdominal pain,serum amylase recovery time,length of hospital stay and adverse drug reactions were observed in 2 groups. The levels of serum amylase,ALT,TBIL, AST,γ-GT,ALB,IL-6,TNF-α,creotoxin concentration,MDA,SOD,GSH-Px,total antioxidant capacity(TAC)and peripher-al blood PMN NF-κB levels were determined in 2 groups before and after treatment. RESULTS:In control group,3 patients with-drew from the study and 38 patients completed the study;in observation group,6 patients withdrew from the study and 35 patients completed the study. The remission time of abdominal pain,recovery time of serum amylase and the length of hospital stay in obser-vation group were significantly shorter than control group,with statistical significance (P<0.05). Before treatment,there was no statistical significance in the levels of serum amylase,ALT,TBIL,AST,γ-GT,ALB,IL-6,TNF-α,creotoxin concentration, MDA,SOD,GSH-Px,TAC and NF-κB between 2 groups(P>0.05). After treatment,the levels of serum amylase,ALT,TBIL, AST,γ-GT,IL-6,TNF-α,creotoxin concentration,NF-κB and MDA in 2 groups were decreased significantly,while the levels of ALB,SOD,GSH-Px and TAC were increased significantly;the observation group was significantly lower or higher than the con-trol group,with statistically significant (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Compound ginkgo leaf granules can significantly decrease serum amylase,protect liver function, with good safety. The mechanismmay be inhibiting the release of inflammatory factors,improving oxidative stress reactions,reduce the concentration of creotoxin and inhibiting the activity of NF-κB.
9.Genotype Distribution in Extended-spectrum ?-lactamases-producing Escherichia coli

OBJECTIVE To investigate the genotype distribution in extended-spectrum ?-lactamases-producing Escherichia coli in our hospital.METHODS Clinical strains of E.coli confirmed to produce ESBLs were collected from our hospital.ESBLs genotype was analyzed by plasmid conjugation,PCR and nucleotide sequencing analysis.RESULTS The susceptiblility rate of ESBLs-producing strains were 100% to imipenem,75.4% to piperacillin/tazobactam.A total of 4 genotypes were identified in the 28 ESBLs-producing strains.CTX-M type was identified in 92.9% of the strains,including CTX-M-14,CTX-M-24 and CTX-M-3.SHV Type was identified in 7.1% of the strains,including SHV-12.CONCLUSIONS ESBLs producers are common in E.coli isolated from our hospital.Most of them are multidrug resistant and CTX-M is the main genotype of ESBLs.Other genotypes of ESBLs are not found.
10.Effects of rational-emotive therapy on adherence to fluid restrictions of patients maintained on hemodialysis prior to and after kidney transplantation

BACKGROUND: Non-adherence to fluid restrictions is common in patients maintained on hemodialysis prior to and after kidney transplantation,which has a profound influence on the development and transfer of illness of the patients.Mental factors have great influences on adherence to fluid restrictions.OBJECTIVE: To investigate the effects of rational-emotive therapy on adherence to fluid restrictions of patients maintained on hemodialysis prior to and after kidney transplantation.METHODS: In total 100 patients maintained on hemodialysis were randomly assigned to an intervention group(n = 50)and a control group(n = 50).The control group received the conventional therapy.At the same time,the intervention group received the conventional therapy combined with rational-emotive therapy.All cases were assessed with the Symptom Checklist 90(SCL-90)and Medical Coping Modes Questionnaire(MCMQ)prior to and after intervention.Clinical indices related to adherence to fluid restrictions were measured three successive times and the average score of each index was calculated.RESULTS AND CONCLUSION: Forty-eight patients from the intervention group and 44 patients from the control group accomplished the study.After intervention,the scores of the two subscales,confronce and avoidance,were significantly higher than those in the control group(P < 0.05).However,some scores were significantly lower than those in the control group,including the score of acceptance/resignation subscale of MCMQ,total average score of SCL-90,the score of somatization,obsessive-compulsive,interpersonal sensitivity,depression,anxiety,hostility,photic anxiety and additional items,and the score of four clinical indices related to adherence to fluid restrictions(the ratio of interdialysis weight gain to dry weight,mean systolic blood pressure,mean diastolic blood pressure,and mean ultrafiltration volume prior to hemodialysis)(P < 0.05).All these findings suggest that rational-emotive therapy can effectively amend the coping modes and the mental states of the patients,and then improve their adherence to fluid restrictions.
