1.Effect and mechanism of ulinastatin on the efficacy of ventilator-associated pneumonia

Objective To investigate the effect of ulinastatin in adjuvant treatment of ventilator-associated pneumonia(VAP)and preliminary clinical efficacy of the therapy mechanism.Methods 76 patients with ventilator-associated pneumonia from January 2015 to February 2016 in Qingyuan People's Hospital of Zhejiang Province were selected and randomly divided into control group and observation group,38 cases in each group.Two groups were given mechanical ventilation,phlegm,anti infection,rehydration,nutritional support and other conventional treatment,the observation group on the basis of routine treatment for ulinastatin adjuvant therapy,comparison of two groups of treatment,the simultaneous determination of serum interleukin-6(IL-6),C-reactive protein(CRP)and procalcitonin(PCT)and tumor necrosis factor alpha(TNF-alpha)levels were measured before and after treatment.Results The total efficiency of the observation group was 94.74%,significantly higher than the control group 78.95%,the difference was statistically significant(P<0.05),the two groups after treatment of serum CRP and PCT levels were decreased significantly compared with before treatment,the observation group after treatment,serum CRP and PCT levels were(45.19+5.79)mg/L and(1.08+0.36)μg/L was significantly lower than that before treatment and control group after treatment,the difference was statistically significant(P<0.05),the two groups after treatment of serum IL-6 and TNF-α levels were decreased significantly compared with before treatment,the observation group after treatment,serum IL-6 and TNF-α levels were(165.29+19.23)pm/mL and(1.16+0.25)pm/mL,was significantly lower than that before treatment and control group after treatment,the difference was statistically significant(P<0.05).Conclusion The inflammatory reaction in patients with ventilator-associated pneumonia ulinastatin can effectively reduce auxiliary patients,reduce inflammatory factors on lung injury,can alleviate the disease progression and to improve its prognosis.
2.Clinical observation of multiple bone metastases with153Sm-EDTMP

Objective To evaluate the feasibility of 153 Sm-EDTMP for multiple bone metastasis.Methods 32 patients with multiple bone metastases were treated with 153 Sm-EDTMP injected into the veins,0.4~1.0 mci/kg,of which 7 cases received therapy for 2~3 times.Results 53.1%(17/32)was relieved completely,25.0%(8/32)was relieved partially and 9.4%(3/32)was relieved slightly.No effects were found in 12.5%(4/32).The overall effective rate was 78.1%.Side effects that were the decreasing of WBC and PLT were found in 15.6%(5/32),which rose to the preoperative levels within 6~8 weeks.Conclusion Internal radiotherapy with 153 Sm-EDTMP is safe and effective in the treatment of multiple bone metastases.

4.The effect of pulsed ultrasound on medial collateral ligament repair

Objective To investigate the effect of pulsed ultrasound therapy (US) on medial collateral liga-ment healing in rats and it's mechanism.MethodsEighteen 3-month-old male Sprague-Dawley rats with transected medial collateral ligaments (MCLs) were studied. They were randomly divided into a control group, a 0.5 W/cm~2 group and a 1. 0 W/cm~2 group.The control group was not given any treatment.The 0. 5 W/cm~2 group and 1.0 W/cm~2 group were given 10 minutes of pulsed US (duty cycle: on/off = 3 ms/1 ms) daily for 8 days at either 0.5 or 1.0 W/cm~2 intensity. All the rats were sacrificed on the 9th day. After macroscopic examination, their MCLs were harvested and studied using haematoxylin-eosin staining, Van Gieson's staining and immunohistochemical tech-niques in order to detect transforming growth factor beta-1(TGF-β1) and any histological or histochemical changes.ResultsMacroscopically, the lacerated MCLs had healed with scar tissue formation. Scarring appeared to be greater in the 0.5 W/cm~2 and 1.0 W/cm~2 groups than in the control group. Inflamed cells appeared to be more numerous in the treated groups than in the controls. There were significant differences in collagen fiber extent among all three groups. In the 1.0 W/cm~2 group, the average level of TGF-β1 was significantly up-regulated, and TGF-β1 expres-sion was higher than in the other two groups.ConclusionsPulsed US can improve ligament healing in the short term, however whether long-term treatment with US can yield further improvement is unknown. Pulsed US can in-crease the level of TGF-β1, which will be higher with higher US dosage. 5.Establishment of reference intervals of dry chemistry tests among healthy population in Chengdu

Objective To establish the reference intervals of 20 dry biochemical items in different age periods among healthy population in Chengdu to providing better support and service for clinical diagnosis and treatment .Methods The stratified random‐ized cluster method was used to collect healthy children and adults in 4 age periods(1 month-3 years old ,>3-7 years old ,>7-18 years old and >18 years old) .Totally 1 495 healthy people (740 males and 755 female) were screened out as the research sub‐jects by the questionnaire ,physical examination and laboratory screening .Fasting venous blood samples were collected from these cases ,then the VITROS 5600 dry biochemistry analyzer was used to detect 20 biochemical items .The obtained results were statisti‐cally analyzed .Results In different groups according to sex and age ,except the conjugated bilirubin (BC) was constant 0 μmol/L , the other items had statistical differences (P<0 .05) .After merging the different groups without statistically significant difference , the obtained reference intervals had significant differences compared with the reference intervals provided by the manufacturer .Con‐clusion Laboratory should establish the different reference intervals aiming at different age and gender populations according to the special character of hospital visiting populations in order to meet the clinical requirements .

AIM: To investigate the effect of granulocyte colony stimulating factor (G-CSF) on arabinosyl cytosine (Ara-C)-induced apoptosis in HL-60 leukemia cells. METHODS: The proliferation of HL-60 leukemia cell was observed by hemopoietic cell culture. Apoptosis was measured by the morphology of apoptosis cell , the quantitation of DNA fragmentation with the diphenylamine reaction. The change in drug sensitivity was measured by “MTT”. 7.Diagnostic value of morphological characteristics of mesenteric vessel whirl sign and its three-dimensional images for volvulus

Objective To study the sensitivity and specificity of the Chinese eating assessment tool (EAT-10) in screening acute stroke patients for oropharyngeal dysphagia (OD).Methods A total of 130 inpatients with acute stroke were screened using the Chinese EAT-10.On the same day they were also screened using the gold standard technique for diagnosing dysphasia-videofluoroscopy.A receiver operating characteristics (ROC) curve was developed to study EAT-10's sensitivity and specificity.A Youden index,positive predictive value (PPV),negative predictive value (NPV),and positive and negative likelihood ratios (LHR+ and LHR) were quantified.Results According to the ROC curve,a cut-off point of 1 (EAT-10 score≥ 1) gave the best sensitivity (77.9%),the highest NPV (73.2%),with 66.1% specificity,71.6% PPV,2.30 LHR+ and 0.33 LHR in screening for OD.The test-retest reliability was above 0.7.An investigator consistency reliability test showed good repeatability,and the consistency between each item and the mean total score was high.Conclusion The Chinese EAT-10 has good test-retest reliability and investigator consistency.The optimal cut-off point is 1,with good sensitivity and NPV at scores ≥ 1.The test can be recommended as a screening tool for OD in acute stroke patients.

10.The role of dual source CT in the evaluation of aortic mechanical valves

Objective To study the value of dual-source CT(DSCT)in assessing aortic mechanical valve with 3D cardiovascular imaging technique.Methods Thirty-four patients with mechanical aortic valve transplantation were enrolled.Nineteen cases transplanted with On-x valve and 15 cases with St jude valre.CT scan was performed in all 34 patients and MRI scan in 30 patients.The CT imaging quality of On-x valve with St jude valve was compared by two independent samples Wilcoxon rank test.Two independent sampies test was used to compare tlle valve angle in the two kinds of valve and also in the same group.The relative functional index of left ventricle in CT and MR analysis by paired-samples t Test. Results The image quality was dirided into three grades.Six hundred and eighty phases in all 34 patients.which included 391 phases with grade 1,171 phases with grade 2 and 118 phases with grade 3.There were no significant different between two kinds of mechanical valve in image quality(image quality of grade 1:Z=-1.084,P=0.286;grade 2:Z=-0.088,P=0.945;grad 3:Z=-1.045,P=0.319).The mean bileaflet angle of 34 cases was 82.0°±4.4°in completely opening and 34.5°±5.7°in completely closing.The mean radian gradient over openess and closure was 47.5°±7.5°.The radian gradient of St iude group(55.0°±1.8°)was obviously larger than On-x group(41.5°±4.1°)(t=-11.732,P=0.000).But,there were no significant difference between dual-source CT and 1.5 T MR when evaluate the left ventrical fuction,including EF[MR.EF(57.2-1-2.8)%,CT-EF(57.9±2.8)%,f:-0.496,P=0.623],EDV[MR-EDV (121.6±9.1)ml,CT-EDV(132.9±11.2)ml,t=-1.198,P:0.240],ESV[MR-ESV(55.1±6.9)ml,CT-ESV(59.5±7.6)ml,t=-1.094,P=0.283],myocardium mass at ED[MR-Myo.mass (155.5 ±12.6)g,CT-Myo.mass(147.9±11.6)g,t=1.823,P=0.079]and CO[MR-CO(5.7±0.4)ml/min,CT-CO(5.9±0.5)ml/min,t=-3.211,P=0.775].Conclusions Dual source CT with 3D cardiovascular imaging technique is valuable and effective for evaluating aortic mechanical valve.The shape of the whole valve and movement of bileaflet reappears well.With the postprocessing software,it allows evaluating aortic mechanical valve function,reliable measurements of opening and closing leaflet angles and left ventrieal function.
