1.On patient-entered design of hospital environment

It elaborates the importance of designing a patient-centered hospital environment, and presents the concept of such a design. The authors hold that such a concept is mostly composed of systematic, people-oriented, informationized and variability designs, and describe the design trends in contemporary hospitals. It is also pointed out that patient-centered principle should be dominant in building a contemporary hospital, as the guideline and development trend, as a satisfactory hospital environment makes a hospital more competitive.

3.Detection of causative allergens of cosmetic allergic contact dermatitis

Objective To make a survey on common cosmetic allergens, and to provide epidemiological data and clinical evidence for cosmetic allergy. Methods Patch test was performed by using 49cosmetic allergens from a European cosmetic series and 5 Chinese standard screening allergens on 89patients with suspected cosmetic allergic contact dermatitis. Test results were determined according to the International Contact Dermatitis Research Group (ICDRG) recommendation. Results Of the 89 patients, 61(68.5%) showed positive reactions to one or more cosmetic allergens. The most common allergens were fragrances (33.7%), followed by preservatives (30.3%), para-phenylenediamine (25.8%) and amerchol L 101(10.1%). Conclusion Fragrances, preservatives, para-phenylenediamine and amerchol L 101 are dominant causative allergens in patients with cosmetic allergic contact dermatitis.
4.Effects on abnormal gait pattern in hemiparetic patients using ankle foot orthoses

Objective To observe the effect of ankle foot orthoses (AFO) on improving the abnormal gait pattern in hemiparetic patients Methods 106 hemiparetic patients were randomly divided into two groups. AFO was used in one group (52 cases) and not in the control group (54 cases). Both of them accepted conventional physical therapy. Motor function of affected ankle joint and lower extremities, spastic state of ankle joint, standing balance and walking ability were evaluated on each case before and after rehabilitation Results At the end of the trial, spastic state of ankle joint in both groups were decreased. 40 cases reached to degree of ⅠorⅠ + of spastic state and 12 cases to Ⅱ, Ⅲ or Ⅳ in the AFO used group,while the number of cases in the control group were respectively 27 and 27. The effects in AFO used group showed more significant than in the control group ( P 0.05) Conclusion Physical therapy combined with ankle foot orthoses was beneficial to improving the abnormal gait pattern and decreasing the spastic state of ankle joint in hemiparetic patients. However, it might not affect the motor function of ankle joint and lower extremities.
5.Investigation of androgen receptor gene expression in human peripheral leukocytes

Objective: To investigate androgen receptor (AR) mRNA expression in human peripheral leukocytes. Methods: Total RNA was isolated from peripheral leukocytes of healthy men and women, AR mRNA was determined by RT PCR and Northern blot. Results: An AR cDNA fragment of the expected size of 390 bp was determined by RT PCR. 9.4 kb AR mRNA was detected by Northern blot, which was consistent with AR mRNA in human prostate reported by Lubahn et al . Conclusion:AR mRNA expresses in human peripheral leukocytes, this provides basis for studying the effect of androgen on the function of the human peripheral leukocytes and the changes of AR in pathological stages. 6.Effect of resveratrol on proliferation of liver cancer SMMC-7721 cells andlowering levels of mTOR protein phosphorylation

Aim To study the effect of resveratrol on proliferation of liver cancer SMMC-7721 cells and the relevant mechanism.Methods SMMC-7721 cells were treated with different concentrations of resveratrol.Cell proliferation was tested by cell proliferation assay and colony-forming experiment.The effect of resveratrol on cell cycle of SMMC-7721 cells was detected by flow cytometry.The activation of mTOR/p-mTOR proteins was detected by Western blot.Results Compared with control group, resveratrol inhibited SMMC-7721 cell proliferation in a dose-dependent manner(P<0.01);cell cycle detection showed that SMMC-7721 cells were arrested at G0/G1 phase by resveratrol;Westren blot assay showed that resveratrol down-regulated p-mTOR expression.Conslusion Resveratrol inhibits human SMMC-7721 cell proliferation and arrests the cell cycle at G0/G1 phase, while the inhibition mechanism may be related to activation of mTOR signaling pathway by resveratrol.
7.Analysis of Clinical Manifestations of Dengue Fever in Guangzhou Higher Education Mega Center in 2014

Objective To analyze the clinical characteristics of dengue fever (DF) in Guangzhou Higher Education Mega Center (HEMC) in the year of 2014. Methods A retrospective analysis was carried out in the clinical data of 487 cases of DF patients. Results The incidence of DF accounted for 27.18% ( 527/1939) of the total emergeney fever cases. In 487 DF cases with detailed data, 261 were male, and 226 were female; 312 were young aged, 88 were middle aged, and 87 were old aged. For the systemic symptoms, fever accounted for 87.5%, headache 48.3%, and muscular soreness 47.6%. Cough ( 16.6%) , pharynx pain ( 16.8%) and running nose ( 10.9%) were common in respiratory symptoms; poor appetite ( 14.4%) and nausea ( 10.3%) were common in gastrointestinal symptoms. The abnormal laboratory parameters were mainly shown as WBC count ( 48.25%) and PLT count ( 41.68%) , following by creatine kinase ( CK, 39.49%) , aspartate aminotransferase ( AST, 34.12%) , and lactate dehydrogenase ( LDH, 31.96%) . Less cases had abnormal renal function. The distribution of Chinese medical syndrome types was shown as damp-heat blockage (65.7%), syndrome involving Weifen and Qifen simultaneously (23.6%), and Qi-yin deficiency (10.7%). Conclusion In the year of 2014, DF in Guangzhou HEMC occurred mainly among the youth people, the incidence of male DF was similar to that of the female DF, and DF cases usually have the primary symptoms of fever, aversion to cold, headache and muscular soreness. Bleeding is seldom seen in the DF patients, a few cases are complicated with the damage in the blood, liver and myocardium, and most of them have good prognosis.
8.Influences of routine-dose loratadine on the intensity of patch test reaction

ObjectiveTo investigate the influences of routine-dose loratadine on the positive patch test reaction to nickel sulfate.MethodsA double-blind,controlled and randomized study was carried out.A total of 121 patients with a positive patch test reaction to nickel sulfate were divided into two groups to receive loratadine 10 mg (experimental group,n =61 ) or placebo (control group,n =60) once daily for 14 days.The patch testing of nickel sulfate was performed on day 11,and clinical evaluation of the test was carried out on day 14 after the start of treatment.The intention to treat population was used for data analysis.ResultsNo changes were observed in the intensity of patch test reaction to nickel sulfate in 55 patients in the experimental group or 53 patients in the control group,and there was no significant difference between the two groups in the percentage of patients showing changes in the intensity of patch test reaction(9.8% vs.11.7%,x2 =0.11,P > 0.05).ConclusionThe routine-dose loratadine has no inhibitory effect on the intensity of skin patch test reaction to nickel sulfate.
9.The differential expression and significance of regulatory T cell and T helper 17 related immunologic factors in peripheral blood of arsenic-exposed rats

Objective To investigate the differential expression levels and significance of regulatory T cell (Treg) and T helper 17 (Th17) related immunologic factors in peripheral blood of arsenic-exposed rats.Methods Thirty-two Wistar rats were numbered by weight,randomly divided into four groups [control,low (1.25 ml/kg),medium (2.50 ml/kg),and high (5.00 ml/kg)],8 rats per group.Rats in control group were given oral gavage of deionized water,and low,medium and high arsenic exposed groups were given oral gavage doses of 2.00 g/L sodium arsenite (NaAsO2) according to their body weight for 6 days every week.After 4 months,the urine and peripheral blood samples of rats were collected,urinary arsenic (uAs) was detected by inductively coupled plasma-mass spectrometry (ICP-MS),the results were shown in [median (minimum and maximum)],uAs was corrected by urinary creatinine (uCr),the unit was μg/g Cr;enzyme-linked immune-sorbent assay (ELISA) was applied to detect the levels of Treg,Th17,T lymphocytes activation related immunologic factors [interleukin-10 (IL-10),transforming growth factor beta1 (TGF-31),IL-17,IL-6,IL-2],the results were shown in (x) ± s.Results The uAs of the rats was compared between control,low,medium,and high arsenic exposed groups [7.50 (3.16-9.81),72.34 (62.34-106.63),209.15 (154.41-232.20),369.73 (289.50-516.55) μg/g Cr],the differences were statistically significant (F =337.55,P < 0.05).IL-10 [(85.03 ± 7.11),(93.96 ± 8.14),(97.48 ± 6.23),(93.47 ± 4.41) ng/L],TGF-β1 [(72.88 ± 2.96),(81.45 ± 8.15),(86.08 ± 7.55),(90.29 ± 5.35) ng/L],IL-17 [(18.15 ± 3.66),(25.54 ± 5.59),(31.48 ± 5.74),(37.25 ± 7.36) ng/L],IL-6 [(83.68 ± 8.48),(85.14 ± 7.11),(89.78 ± 5.36),(93.48 ± 5.77) ng/L],and IL-2 [(80.65 ± 6.90),(73.86 ± 8.00),(69.93 ± 7.77),(62.06 ± 9.82) ng/L] of the rats were compared between control,low,medium,and high arsenic exposed groups,the differences were statistically significant (F =5.094,11.054,16.249,3.474,5.119,all P < 0.05).There were positive correlations between uAs and TGF-β1,IL-17 concentration (r =0.723,0.605,all P < 0.01),while IL-2 showed a negative correlation (r =-0.484,P < 0.05).Concltsion Arsenic exposure could affect the secretion of Treg and Th17 related immunologic factors,so as to the imbalance of anti-inflammatory and pro-inflammatory,which may play a role in the formation and development of arsenic-related immune injury.
10. Curative effects of Ningmitai capsules on the residual fragments and postoperative complications following lithotripsy treatment of upper urinary calculi

Objective:
To observe the clinical efficacy of Ningmitai capsules, a traditional Chinese medicine using for clearing heat and dampness, in the treatment of residual fragments and postoperative complications following lithotripsy for upper urinary stones.
Methods:
During October 2016 and March 2018, patients from Wuhan 1st Hospital, Wuhan 2nd Hospital, Wuhan 3rd Hospital, and Wuhan Puai Hospital having upper urinary residual fragments following minimally-invasive stone treatment were randomly assigned to control group and Ningmitai group with a proportion of 1∶3. The patients in control group were treated with antibiotics or sodium diclofenac suppository on demand, while patients in Ningmitai group took additional Ningmitai capsule orally (4 capsules per time, 3 times per day). The observation was started when a patient was enrolled in this study and continued for a maximum of 12 weeks or until stone-free status. During the observation, the stone expulsion time, stone-free time, stone-free rate were observed, and the difference in curative effect between the two groups on postoperative complications such as pain and infection were compared.
