Objective To evaluate the postoperative shunt restenosis of transjugular intrahepatic portasystemic shunt (TIPS) using polytetrafluoroethyoethylene (PTFE) covered stent in patients with portal hypertension,and discuss its influencing factors.Methods The clinical data of 82 patients with portal hypertension were retrospectively analyzed.TIPS with PTFE covered stent was carried out in all patients,and the preoperative Child-Pugh score was (6.74 ± 1.05) scores.The patients were followed up for 12-55 (24.9 ± 10.4) months.Single and multiple factor analysis using Cox regression were performed to evaluate the influencing factors.Results Under the local anaesthesia,TIPS was successful performed in all 82 patients.The portosystemic pressure gradient decreased from preoperative (27.46 ±4.08) mmHg (1 mmHg =0.133 kPa) to postoperative (15.71 ±2.07) mmHg,and the difference was statistically significant (t =1.804,P ＜ 0.01).The Child-Pugh score 3 months after operation was (6.57 ± 1.43) scores,and there was no statistical difference compared with that before treatment (t =12.931,P＞ 0.05).During the follow-up period,shunt restenosis of covered stent occurred in 8 patients,and the rate of restenosis was 9.8％ (8/82).Cox single analysis showed that the covered stent length within portal vein 1.0 cm and its location in hepatic vein had statistical significance,and multiple factor regression analysis showed that the covered stent location in hepatic vein was the independent influencing factor of shunt restenosis (OR =26.647,95％ CI:8.257-20.061,P =0.005).Conclusions TIPS with PTFE covered stent can markedly increase the shunt patency rate.A proper location of the covered stent within hepatic vein carries lower restenosis incidence.Hyperextension of the stent into portal vein is not recommended.

In this article we review the relation between P2 purinoceptor subtypes and coronary heart disease (CHD).P2Y_1 ,P2Y_2 ,P2Y_(11) and P2X_7 receptors are related to inflammatory reaction in atheroselerosis plaque.P2X_3 receptor plays an important role in anginal nocuity response.P2X_4 receptor is involved in vascular remodeling and enhancement of myocardial contractility in non-infarct zone.P2Y2 and P2Y6 receptors are involved in resteno-sis.A polymorphism in P2Y_(11) receptors increase the risk of myocardial infarction.P2Y_1、P2Y_(12) and P2X_1 receptors play a role in thrombotic formation.The specific agonist and antagonist of P2 purinoceptor subtypes could be poten-tial targets for new drugs to treat CHD.

Objective To promote the level of morphological diagnosis and estimate prognosis combining the result of bone marrow histotomy and the swear of 82 cases of malignant hemology disease. Methods We collected 82 cases of malignant hemology disease, extracted marrow by one step and routinly made bone marrow swear and dyeing, made histotomy by embedding plastic. Diagnosis of malignant hemology disease refered to the standard of WHO 2001. Results Bone marrow histotomy was superior to bone marrow swear in diagnosing MF and hypocellular MDS. Bone marrow histotomy was easier to find cell of lymphoma than bone marrow swear. Bone marrow histotomy was a more sensitive index of tumor load of MM.It was also helpful for judging acute change of CML and relapse of acute leukemia. Conclusion Level of diagnosing malignant hemology disease can be promoted efficiently by combining marrow histotomy with bone marrow swear

OBJECTIVE: To establish a RP-HPLC method for concentration determination of m-nisoldipine (m-Nis) in the plasma of rhesus monkeys.METHODS: The analysis was performed on a C18 column at 30℃ with a mobile phase of methanol-water (63∶37) at a flow rate of 1.0ml/min.The detection wavelength was 236nm.The sensitivity was 0.05AUFS and the internal standard was nimoldipine.RESULTS: The detectable concentration of m-Nis showed good linearity in the range of 1.75～ 448?g/L.The average recovery of extraction was 84.1%(RSD=4.09%), and the average recovery of method was 98.7%(RSD=2.76%).The processed plasma samples remained stable in quality after having been stored for 7 days, and the plasma remained stable in quality through repeated freeze thawing or stored at low temperature.CONCLUSION: The present method is highly sensitive and reproducible, as well as convenient, which can be used for concentration determination of m-Nis in the plasma of rhesus monkeys.

Metabolic syndrome (MS)is a syndrome of multiple metabolic abnormality aggregation,including over-weight,hypertension,glucose and lipid metabolic abnormality and insulin resistance etc.Some researches demon-strated that over 80% EH patients may be complicated with MS,those patients with EH+MS possess higher risk for cardio-and cerebrovascular diseases than pure EH patients.This article made an overview on research about rela-tionship between EH and MS in recent years.

Emergency Medical Service (EMS) of Beijing Olympic Games needs to cover not only all the venues of the Olympic and Paralympic Games, but also emergency medical service for citizens at large and other emergency events. To this end, Beijing Emergency Medical Center made efficient preparations for the Olympic Games, namely upgraded dispatch system, first-aid stations, ambulances and equipment, and volunteer recruitment among others. Thanks to such efforts made, the EMS successfully served the Beijing Olympic Games and citizens at large. Furthermore, the Beijing Olympic Games promoted the development of the Beijing EMS system.

This paper introduces a new perspective of case choice in medical ethics teaching,which breaks through the limitations of the choice of teaching cases,chooses the hot-case analogy teaching methods out of medical field,and proves to have received good teaching results.

CD40 pathway plays an important role in occurrence and development of acute and chronic allograft rejection.Further research on immune characteristics of CD40 pathway will explore new field for investigation of early monitoring,prevention and control of rejection.sCD30 as the active component of CD30 activation and release into blood has good correlation with CD30.Expression of CD30 provides a high sensitive and specific non-traumatic examination method for early diagnosis of acute rejection and its identification with acute tubular necrosis in renal transplantation.It also provides the basis for immunosuppressive therapy after transplantation.Through the above research on factor associated with renal transplant rejection,the rejection can be effectively predicted and prevented,transplantation immunoreaction can be solved,successful rate of renal transplantation will be increased,and long-term survival of graft will be improved.
