1.Clinical application of selective salpingography and fallopian tube recanalization in diagnosis and treatment for obstructive infertility

Objective To evaluate the significance of selective salpingography(SSG)and fallopian tube recanalization(FTR)in diagnosis and treatment for obstructive infertility.Methods Totally, 268women with infertility caused by obstruction of the fallopian tube were divided randomly into two groups, one (Group A)with a non-operative hysterosalpingeal system by insertion of co-axial cannula and guiding filament made by COOK Corporation in the US and another(Group B)with a self-prepared hysterosalpingeal system modified based on the apparatus of COOK Corporation, for SSG and FTR.Then, efficacy of SSG and FTR for the infertile women was evaluated by one-year cumulative pregnancy rate and reeanalization rate of the fallopian tube.Resnits One hundred and seventy of 198 fallopian tubes were recanalized in Group A, with a recanalization rate of 85.8%, and 290 of 320 tubes in Group B were recanalized.with a recanalization rate of 90.6%.reaching statistically significant difference.Furthermore, recanalization rate varied with different sites of occlusion of the fallopian tube.Pregnancy rate was 35.3%(36/102)in Group A, significantly higher than that in Group B(47.6%, 79/166).As the same, pregnancy rate varied with different sites of occlusion of the fallopian tube.Conclusion Selective salpingography and fallopian tube recanalization with self-prepared hysterosalpingeal system have dual effects on diagnosis and treatment for tubal infertility of women, which are simpler, safer and more reliable, worth to be clinically applied.
2.Influence of katsutoxin and its extract on bone marrow colony-forming unit-granulocyte and monocyte following ~(60)Co gamma ray radiation

The experiment was performed at Basic Medical College and First Affiliated Hospital, Zhengzhou University from September 2004 to December 2005. Totally 60 Kunming mice were divided into 5 groups randomly: ①blank control group (n =15) and simple radiation group (n =15). The mice were given 0.2 mL sterile saline by intraperitoneal injection. ②antineoplastic polypeptide from Buthus Martensii Venom (APBMV) group (n =10) and APBMV plus radiation group (n =10) received 0.2 mL APBMV according to prepared concentration by intraperitoneal injection. ③Katsutoxin extract Ⅲ plus radiation group (n =10) received 0.2 mL katsutoxin extract Ⅲ by intraperitoneal injection every other 5.5 hours for 7 days. After 24 hours from the last injection, the mice were endured 60Co g ray radiation (80 cm, 7.5 Gy irradiation dose, 0.27 Gy/min dose rate). Then katsutoxin extract Ⅲ was given same as above for 7 days. Then bone marrow was extracted to be cultured to colony-forming unit-granulocyte and monocyte (CFU-GM). 3.CT Features of Solid Components in Pulmonary Mixtured Ground-glass Opacity

Purpose To evaluate the CT features and pathological manifestations of the solid components of mixture ground-glass opacity (GGO) in adenocarcinoma in situ (AIS), minimally invasive adenocarcinom (MIA) and invasive adenocarcinoma (IAC), to analyze the qualitative diagnosis value of solid components of mixture GGO in the diagnosis of AIS, MIA and IAC, to provide reference for the selection of clinical treatment.Materials and Methods Eighteen patients with AIS, 53 patients with MIA and 28 patients with IAC (the maximum diameter smaller than 2 cm) proved by surgery and pathology with CT features appearing as mixture GGO were retrospectively analyzed, CT features of the solid components in three groups were analyzed and compared with pathology.Results The solid components in AIS mainly appeared as punctiform or polygon, with extensive distribution, solid nodules were usually single (17 cases, 94.44%), located in the middle of the lesion (14 cases, 77.78%), with clear binderies (16 cases, 88.89%) and the same density with vessels in the same axis (13 cases, 72.22%); the majority of solid components in MIA appeared as circular or elliptical (33 cases, 62.26%), less than or equal to 5 mm (48 cases, 90.57%), with eccentric or multi-point distribution (45 cases, 84.90%), the boundaries were less sharp (40 cases, 75.47%), with slightly lower density than that of the vasculars in the same level (34 cases, 64.15%); the solid components in IAC mainly appeared as irregular lesions (21 cases, 75.00%), lager than 5 mm (24 cases, 85.71%), with eccentric growth (20 cases, 71.43%) and less sharp boundary (15 cases, 53.57%), the integration of multiple nodules could also be observed. There were statistically significant differences in the CT features of solid components within the lesions among the three groups (P<0.01).Conclusion It is possible to predict the pathological typing and the prognosis of pulmonary mixture GGO in a certain extent according to the different CT features of the solid components in it, and to guide clinical treatment principles.
4.Establishment of the Diagnostic Model in Solitary Pulmonary Nodule Appearing as Ground-glass Nodule

Purpose To explore the independent predictors of malignant solitary pulmonary nodule (SPN) manifesting as ground-glass nodule (GGN),and to establish a prediction model.Materials and Methods The clinical data and CT images of 362 patients (group A) with pathological-confirmed SPN appearing as GGN in Shanghai Chest Hospital Shanghai Jiaotong University from January 2014 to December 2015 were retrospectively analyzed.The independent predictors of malignant SPN were identified,and the clinical prediction model was established.Another 119 SPN patients in Affiliated Zhoushan Hospital of Wenzhou Medical University were selected as group B to verify the diagnostic efficiency of the prediction model.Results Using multivariate Logistic regression analysis,clear border (OR=6.274,P<0.01),smooth edge (OR=0.391,P<0.01),lobulation (OR=3.387,P<0.01),pleural retraction sign (OR=2.430,P<0.01),and vocule sign (OR=3.076,P<0.01)were identified as independent predictors of malignant SPN.The area of the model under the ROC curve was 0.859 with 95% CI (0.804-0.903).The diagnostic accuracy rate,sensitivity,specificity,positive predictive value and negative predictive value were 85.92%,91.03%,81.97%,92.03% and 73.53%,respectively.Conclusion In this study,the independent predictors of malignant SPN appearing as GGN were identified,and the prediction model was established.The model can accurately identify SPN and provide effective help for early diagnosis of SPN.
5.Expression and significance of TREM-1 gene in mice with severe acute pancreatitis

Objective Through detecting the expression of triggering receptor expressed on myeloid cells-1 (TREM-1) in pancreas and lung of rats with severe acute panereatitis,to approach the role of TREM-1 in severe actute pancreatitis pathogenesis,and provid a new molecular target for SAP.Methods Thirty healthy male Wistar rat were randomly divided into two groups:control group(group A),n =15 ; severe acute pancreatitis group (group B),n =15.The model of severe acute pancreatitis of rats was induced by retrograde injection of 5% sodium taurocholate into the pancreatic duct.Results TREM-1 in pancreatic tissues and lung tissues was significantly higher in group B than group A in three time points(6 h,12 h,24 h) and had significant changes(P < 0.05),and had a positive correlation with pancreatic pathology.Conclusion TREM-1 was significantly expressed in severe acute pancreatitis,and aggravated pancreas damage and systemic inflammatory response syndrome.
6.Efficacy and safety of multiple-dose 5-HT3 receptor antagonists in preventing multi-day-based and highly emetogenic chemotherapy-induced nausea and vomiting

Objective:To evaluate efficacy and safety of multiple-dose tropisetron plus dexamethasone (DXM) versus palonosetron plus DXM for chemotherapy-induced nausea and vomiting. (CINV) in patients received multiple day-based highly emetogenic chemotherapy. Methods:Cancer patients who were receiving multiday-based highly emetogenic chemotherapy were randomly assigned to AB or BA groups. A randomized, cross self-control ed method was applied. Patients in AB group received palonosetron (0.25 mg) 30 min before chemotherapy on day 1 and 3 or additional day 5 in the first cycle;and with tropisetron (5 mg) 30 min before chemotherapy on day 1, 2, and 3, or sup-plementary days (day 4 and 5) in the second cycle. Patients in BA group were treated with tropisetron in the first cycle and with palonosetron in the second cycle. Tropisetron and palonosetron were administered with DXM (10 mg) on day 1, followed by additional doses (5 mg) on days 2 to 5. Palonosetron group comprised patients in the AB group in the first cycle and BA group in the second cycle, whereas tropisetron group included patients in the AB group in the second cycle and BA group in the first cycle. Efficacy and safety of tropisetron versus palonosetron in preventing CINV were evaluated. Results:Ninety-one patients were included in analyses. At day 3, 4, and 5, incidence rates of nausea in the palonosetron group reached 28.6%, 30.8%, and 24.2%, respectively, and those of the tropisetron group totaled 42.8%, 47.3%, and 39.6%, respectively (P<0.05). At day 4, 5, and 6, incidence rates of vomiting in the palonosetron group measured 28.6%, 18.7%, and 5.5%, respectively, and those of the tropisetron group reached 42.9%, 34.1%, and 14.3%, respectively (P<0.05). From day 4 to day 5, day 6 to day 7, and day 1 to day 7, the palonosetron group yielded significantly lower incidence rates of nausea and vomiting than tropisetron group (P<0.05). Rate of rescue treatment in the palonosetron group was lower than that in tropisetron group (13.2%vs. 24.2%, P=0.057). No statistical difference in toxicities was observed between the two groups. Conclusion:Palonosetron plus DXM features better efficacy than that of tropisetron plus DXM against delayed CINV induced by multiple day-based highly emetogenic chemotherapy, which was well tolerated in the two treatments.
7.Association of iodine nutrition and thyroid health conditions among residents in Zhoushan archipelago

Objective To investigate iodine nutrition and thyroid health status among residents in Zhoushan archipelago, and to analyse their relationship.Methods A total of 3 284 residents in Zhoushan archipelago were surveyed by questionnaire and their thyroids were examined by B-mode ultrasound.The levels of urinary iodine and thyroid function were detected.Results The median level of urinary iodine in 3 284 residents was 226.0 μg/L, being 320.7 μg/L in citizens, 188.9 μg/L in farmers, 122.2 μg/L in salt-makers, 193.6 μg/L in fishers, and 271.7 μg/L in buddhist.The prevalence of diffuse goiter, nodular goiter, colloid goiter, thyroid adenoma, thyroid carcinoma, hyperthyroidism, subclinical hyperthyroidism, hypothyroidism, subclinical hypothyroidism, and positve rate of TPOAb were 1.7% ,25.3% ,8.7% ,0.2% ,0.4% ,0.5% ,0.8% ,0.03%,1.0% ,and 9.5% repectively.The prevalence of thyroid diseases was increasing with aging, and higher in women than in men (P<0.05).There was no significant relationship of the thyroid diseases with seafood, smoking,drinking, and tea (P>0.05).Conclusions The citizens of Zhoushan archipelago have adequate iodine intake.It is pertinent to discuss Universal Salt Iodization.Excessive iodine intake may contribute to the high prevalence rate of thyroid diseases in Zhoushan.
8.Study on relationship between blood supply from pulmonary artery and pathological characteristics of patients with primary bronchogenic carcinoma.

BACKGROUNDAt present, it has been known that the bronchogenic artery participates in the blood supply of primary bronchogenic carcinoma, but there is controversy about the blood supply from pulmonary artery in primary bronchogenic carcinoma. The aim of this study is to assess the relationship between the blood supply from pulmonary artery and pathological characteristis of patients with primary bronchogenic carcinoma.
METHODSThe pulmonary arteries in 43 surgical samples of bronchogenic carcinoma were marked, then the iopromide was used to selective pulmonary arteriography in digital subtraction angiography (DSA). The relationship between tumor with blood supply from pulmonary artery and the pathologic characteristics was observed.
RESULTSThere were 34 samples with blood supply from pulmonary artery ( 79.07%) , and 9 samples without blood supply from pulmonary artery (20.93%). The development rate of peripheral lung cancer (100.00%) was significantly higher than that of central lung cancer (64.00%) (P < 0.01) . The development rate of squamous cell carcinoma (91.30%) was remarkably higher than that of adenocarcinoma (61.11%) (P < 0.05). The development rate of poorly differentiated lung cancer (95.00%) was remarkably higher than that of well and moderately differentiated lung cancer (65.22%) (P < 0.05). There was a positive relationship between the tumor size and the development rate (P < 0.05).
CONCLUSIONSIn primary bronchogenic carcinoma, the pulmonary artery blood supply exists in most of tumors. There is relationship between the blood supply from pulmonary artery and general type, histopathology, cell differentiation and tumor size of lung cancer. The blood supply from pulmonary artery doesn't relate to tumor stage.
9.MicroRNA-548 down-regulates host antiviral response via direct targeting of IFN-λ1.

Interferon (IFN)-mediated pathways are a crucial part of the cellular response against viral infection. Type III IFNs, which include IFN-λ1, 2 and 3, mediate antiviral responses similar to Type I IFNs via a distinct receptor complex. IFN-λ1 is more effective than the other two members. Transcription of IFN-λ1 requires activation of IRF3/7 and nuclear factor-kappa B (NF-κB), similar to the transcriptional mechanism of Type I IFNs. Using reporter assays, we discovered that viral infection induced both IFN-λ1 promoter activity and that of the 3'-untranslated region (UTR), indicating that IFN-λ1 expression is also regulated at the post-transcriptional level. After analysis with microRNA (miRNA) prediction programs and 3'UTR targeting site assays, the miRNA-548 family, including miR-548b-5p, miR-548c-5p, miR-548i, miR-548j, and miR-548n, was identified to target the 3'UTR of IFN-λ1. Further study demonstrated that miRNA-548 mimics down-regulated the expression of IFN-λ1. In contrast, their inhibitors, the complementary RNAs, enhanced the expression of IFN-λ1 and IFN-stimulated genes. Furthermore, miRNA-548 mimics promoted infection by enterovirus-71 (EV71) and vesicular stomatitis virus (VSV), whereas their inhibitors significantly suppressed the replication of EV71 and VSV. Endogenous miRNA-548 levels were suppressed during viral infection. In conclusion, our results suggest that miRNA-548 regulates host antiviral response via direct targeting of IFN-λ1, which may offer a potential candidate for antiviral therapy.

10.Studies on the effect of the p.Arg352Gln mutation on Bw07 transferase

Objective:To investigate the molecular biological mechanism of Bw07 allele and its transferase alteration carried by a proband of ABw07 subtype.Methods:A 2-year-old male child was selected as the research object. The peripheral blood of the proband and his parents was identified for ABO blood type by the test tube method, and the ABO subgroup PCR-SSP detection and ABO gene sequencing were performed on the three individuals to determine their blood type genotypes. Finally, the effect of the p.Arg352Gln mutation on Bw07 transferase was verified by virtual mutation, DUET structure prediction, molecular dynamics analysis, and in vitro cellular experiments.Results:The serological phenotypes of the proband and his mother were ABw and Bw, respectively, while his father was normal A. The ABO subgroup PCR-SSP assay identified the three genotypes as Bw07/A, Bw07/O, and A/A, respectively.Sanger sequencing further verified that the proband and his mother carried the Bw07 gene, and virtual mutation showed that the intermolecular forces were weakened by the R352Q mutation. DUET predicted that this p.Arg352Gln mutation could affect the thermodynamic stability of Bw07 transferase. Molecular dynamics analysis confirmed that the alteration of thermodynamic stability was mainly related to the appearance of large fluctuations in the amino acid backbone atoms in the 125-133, 193-198 and 336-354 regions, and in vitro cellular experiments further verified the weakened antigen synthesis of Bw07 transferase.Conclusion:The formation of the ABw07 phenotype is associated with the mutation of the highly conserved Arg352 to Gln in Bw07 transferase.
