1.The role of Scleraxis and Tenomodulin in tendon development

Tendons are dense connective tissues that mediate the attachment of the muscle system to the skeletal system. While the methods for clinical tendon repair remain less satisfactory, tissue engineering may pro- vide promising future. Therefore, understanding the natural tendon development process is important for tendon en- gineering. Scleraxis and Tenomodulin are relatively specific molecule makers for tendon and ligment and play im- portant roles in the tendon development. This article gives review of the molecular structure, the expression regula- tion, and the roles of Scleraxis and Tenomodulin in tendon development, in order to better understand the process of tendon development.
2.Clinical analysis of 314 children with chest pain

Objective To understand the cause of chest pain (precordium indisposition) in children and explore the thread of thought in its analysis.Methods Clinical data of 314 children with chest pain who visited the cardiovascular department for children of our hospital were collected and analyzed to study its causes.Results The cause could be attributed to organic cardiac diseases in 43 of 314 children (13.7%) with chest pain,to non-organic cardiac diseases in 108 (34.4%),to psychiatric-psychological factors in 60 (19.1%) and to unknown cause in 41 (13.1%).Average age of children with organic cardiac diseases was younger than that of children with other causes (P<0.05).Symptoms accompanied simultaneously in children with organic cardiac diseases were much more frequent significantly than those in them with non-organic cardiac chest pain and unknown causes (P<0.05).Conclusions Chest pain in children could be caused by a few causes,which was non-specific symptom for organic cardiac diseases,so, more should be taken into account in analyzing the cause of chest pain in children.
3.Comparative analysis of two hybrid technique in treating multilevel cervical spondylotic myelopathy via anterior approach.

OBJECTIVE:
To compare the clinical effects and complications of two hybrid decompression techniques in treating multilevel (>=3) cervical spondylotic myelopathy(MCSM).
METHODS:
The clinical data of 124 patients with multilevel (>=3) cervical spondylotic myelopathy treated by surgery and follow-up from January 2008 to December 2016 were retrospectively analyzed. According to the different operative methods, the patients were divided into group A and group B. Anterior cervical discectomy and fusion (ACDF) combined with anterior cervical corpectomy and fusion with preserved posterior vertebral wall (PWCF) were performed in group A; ACDF combined with anterior cervical corpectomy and fusion(ACCF) were performed in group B. There were 44 males and 16 females in group A with an average age of (60.43±7.52) years old, 46 males and 18 females in group B with an average age of (61.61±6.39) years old. No significant differences were found in gender, age between two groups. The operative time, intraoperative blood loss, hospital stay, rate of complications, postoperative Japanese Orthopaedic Association (JOA) score and improvement of cervical curvature were compared between two groups.
RESULTS:
All the operations were successful. The follow-up time was 13 to 28(23.0±12.1) months in group A and 12 to 30(24.0±11.5) months in group B. The operative time, intraoperative blood loss, hospital stay in group B were higher than those in group A(<0.05). A total of 37 patients occurred complications, including 14 cases in group A and 23 cases in group B. The incidence of complications was 23.33%(14/60) in group A and 35.94%(23/64) in group B. The incidence of complications in group B was significantly higher than that in group A(<0.05). Postoperative JOA scores were significantly improved (<0.01), there was no significant difference between two groups (>0.05). The physiological curvature of cervical vertebra was improved significantly after operation, and there was no significant difference between two groups(>0.05).
CONCLUSIONS
Both surgical methods can obtain satisfactory clinical efficacy for multilevel(>=3) cervical spondylotic myelopathy, but compare with group B (ACDF combined with ACCF), group A (ACDF combined with PWCF) has shorter operation time, less trauma, less bleeding, and lower incidence of complications. ACDF combined with PWCF can be preferentially selected for the patients who corresponding to the indications.

4.Hermeneutic analysis on the problem of doctor-patient communication from the perspective of health education: taking the prognosis of hepatitis B patients as an example

The uncertainty of prognosis for chronic diseases is a persistent problem in the medical community.Mechanical attribution such as questioning disease (stubbornness), equipment (medical and nursing level), and patients (low compliance), it is believed that there is a bias in analyzing the deep causes of low compliance on prognosis after breakthrough progress in the treatment of this type of diseases.Taking hepatitis B patients from the perspective of hermeneutics as an example, this paper clarified the reasons for the non-identity of doctor-patient cognition on the problem of doctor-patient communication in health education.Meanwhile, it was found that there is a pre-structure of heterogeneous experience in the ontology of understanding between doctors and patients, which presents inherent limitations, openness, generativity, and external micro power penetration in their history of dialogue logic.Finally, epistemologically, this paper clarified the essence such as the history of identity effect was pointed by the reconstruction of heterogeneous “meaning ideals” of doctors and patients, the understanding of certainty and elimination of subjectivity was guided by meaning ideals under the fusion of horizons, as well as good personalized prognostic behavioral literacy was led by cultivations of embodied practice.
5.Prognostic value of arterial blood lactate level and early lactate clearance rate in the elderly patients with sepsis

Objective To explore the clinical prognostic significance of arterial blood lactate level and early lactate clearance rate in elderly septic patients. Methods Totally 82 elderly septic patients who were admitted into emergency department were enrolled.Their arterial blood lactate levels were tested at admission and 6 hours later. Early lactate clearance rate was calculated as following:(lactate level of admission minus lactate level of 6 hours later) /lactate level of admission ×100 %. Heart rate and blood pressure were recorded at admission,and the acute physiology and chronic health evaluationⅡ (APACHE Ⅱ ) score was evaluated in the first 24 hours.28 days after admission was the end of research.Patients were divided into survival group (n=59) and death group (n=23).Taking 10% as cut-off,patients were divided into high lactate clearance (≥ 10%) group (n=59) and low lactate clearance (<10%) group (n=23).Values of arterial blood lactate level,APACHE Ⅱ score and early lactate clearance to predict the prognosis of elderly septic patients were determined by ROC curves. Results Arterial blood lactate level was lower in survival group than death group[(2.4± 1.7)mmol/L vs.(3.9±3.2)mmol/L,P<0.01].Early lactate clearance was higher in survival group than death group [(339.0 ± 28.0) % and (300.0 ± 81.0) %,P < 0.01].Mortality of high lactate clearance group was decreased as compared with low lactate clearance group [18.6% (11/59) and 52.2% (12/23),P<0.01].The thresholds of early lactate clearance to predict mortality of elderly septic patients were 24.0% (sensitivity 72.9%,specificity 65.2%) which was more valuable than APACHE Ⅱ score ( sensitivity 65.2 %,specificity 57.6 %). Conclusions Arterial blood lactate level and early lactate clearance were meaningful predictors for prognosis of elderly septic patients.
6.Inference on Injury Mechanism of Ankle Fracture by Lauge-Hansen Classification.

OBJECTIVES:
To explore the injury mechanism of ankle fracture inferred by the Lauge-Hansen classification with the application of medical imageology and its application value in forensic medicine.
METHODS:
A total of 32 ankle fracture cases with known injury mechanism were collected from January 2013 to May 2018, which were identified in Yongkang Public Security Bureau, Zhejiang Province. The Lauge-Hansen classification of ankle fracture was performed by three forensic experts based on the data of X-ray and CT image. Fisher's exact test and Kappa consistency analysis were performed by SPSS 20.0 statistical software to compare the results of the Lauge-Hansen classification with the injury mechanism of ankle fracture obtained through the criminal evidence.
RESULTS:
In 32 cases, 84.4% (27/32) ankle fractures were classified according to the Lauge-Hansen classification based on medical imaging. The mechanism of ankle fracture identified by the Lauge-Hansen classification was consistent with that obtained through the criminal evidence.
CONCLUSIONS
Based on medical imaging, the Lauge-Hansen classification can be well applied to infer the injury mechanism of ankle fracture in part cases, and provide objective evidence for the crime scene reconstruction of criminal cases.

7.The prevalence and associated factors with depression and anxiety among the patients attending the community-based methadone maintenance treatment program in China

Objective To understand the prevalence and the factors associated with depression and anxiety among patients attending the community-based methadone maintenance treatment(MMT) program.Methods 9 MMT clinics in 3 provinces were randomly selected,in which 1 301 MMT patients who met the criteria of recruitment were all interviewed.A cross-sectional study by administrative questionnaire was employed to investigate the demographics,drug abuse and MMT treatment status of the respondents.The 1301 respondents were also evaluated for depression and anxiety by SDS(Self-Rating Depression Scale) and SAS(Self-Rating Anxiety Scale) respectively.ANOVA and multivariate linear regressions analysis was used to verify the factors associated with depression and anxiety respectively.Results The prevalence of depression and anxiety among the MMT patients were 38.3%(498/1 301) and 18.4%(239/1 301).The multivariates linear regression analysis indicated that at the level of ?=0.05,gender,employment status were associated with depression,while gender,employnment status,educational level,drug abuse and methadone daily dosage were associated with anxiety.Conclusions There are certain proportions of MMT clients who suffer from depression and anxiety.The MMT clinic staff should put more emphasis on the efforts to reduce psychological disorders and its potential risks by enhancing psychological councelling,intervention and social support towards the MMT patients.
8.A new definition of fatty liver disease: from nonalcoholic fatty liver disease to metabolic asso-ciated fatty liver disease

In recent years, there has been increasing evidence that nonalcoholic fatty liver disease (NAFLD) is a manifestation of a systemic metabolic disease in liver. However, limitations in the definition and diagnostic criteria of NAFLD could not fully and accurately describe the metabolic disorder. A consensus statement proposed renaming NAFLD to metabolic associated fatty liver disease (MAFLD). Compared with NAFLD, MAFLD links fatty liver disease with metabolic factors such as overweight or obesity and diabetes, and the diagnosis of the disease no longer excludes the presence of metabolic abnormalities in patients with alcohol consumption or other chronic liver diseases. The authors focus on definition changes of fatty liver disease, discuss the epidemiological changes from NAFLD to MAFLD and the relationship of MAFLD with liver fibrosis and extrahepatic diseases, and also the significance of MAFLD for public health.
9.Preparation and identification of rabbit anti-marmoset IgG-HRP conjugate antibody

Objective To purify marmoset serum IgG, prepare and identify the antiserum and the rabbit anti-marmoset antibody IgG-HRP (horseradish peroxidase). Methods Using SDS-PAGE analysis to identify the serum IgG from HiTrapTM Protein G. The antiserum titer was determined by double immunodiffusion assay. The rabbit anti-marmoset IgG was labeled with HRP by improved sodium periodate method. ELISA and western blotting were used to evaluate the concentration and specificity of rabbit anti-marmoset IgG-HRP. Results The purity of purified marmoset serum IgG determined by SDS-PAGE was higher than 95% , and the anti-serum titer of the anti-marmoset IgG polyclonal antibody was 1∶64. The concentration of rabbit anti-marmoset IgG-HRP identified by direct ELISA was 1∶256 000, and that by western-blotting was 1∶15 000, with a strong specificity. Conclusions The IgG-HRP marker antibody is prepared and the specificity and concentration are identified by ELISA and western blotting. It reserves the resources for the detection system of marmoset pathogens and the molecular immunological testing system.

ObjectiveTo investigate the Character of Symptom Checklist 90 (SCL-90) in detoxification addicts in reeducation center.Methods100 detoxification addicts in reeducation center were evaluated.ResultsThe results showed that the mean scores of all factors in detoxification addicts were higher than those of normal population, and there was difference between different drug dependence addicts.ConclusionThe detoxification addicts shows serious psychological disorders.
