Objective Improving the hospital operation management , preparing for the establishment of the digital operation room and achieving the fine management goal .Methods Based on the actual medical and clinical demand, combined with the standard of the operation management , designed and analyzed the management rules and processes of the perioperative information management system .Results Straightened out the information management rules and procedures of the operation room , strengthened the communication , feedback and sharing of the periopera-tive medical information , and improved the work efficiency , the operation quality and the utilization rate of the opera-tion room.Conclusion The smooth management process and the efficient use of information are the keys to promote the hospital management by means of informationization .

3.Advance care planning acceptance and its influencing factors in heart failure patients

Objective:To investigate the acceptance of advance care planning and its influencing factors in heart failure patients.Methods:A total of 208 patients with heart failure were surveyed by general data questionnaires and advance care planning acceptance questionnaires.Results:The total score of advance care planning acceptance of heart failure patients was (44.26 ± 11.73), the score of feeling dimension was (13.67 ± 5.72), the score of attitude dimension was (30.59 ± 6.33). 53.4%(111/208) of patients were willing to accept the talking about advance care planning. Regression analysis results showed that education level, New York Heart Association (NYHA) classification, communication status with medical staff and whether they had received life-sustaining treatment were important factors influencing of the acceptance of advance care planning in patients with heart failure.Conclusion:Patients with heart failure had higher acceptance of advance care planning. In clinical work, it is necessary to strengthen the scientific popularization of advance care planning in patients with low education level, low NYHA grade and no exposure to life-sustaining treatment. And strengthen the daily communication with patients to prepare for the follow-up advance care planning related communication.
4.Adrenoleukodystrophy: a case report and pedigree study associated with genetic research

Objective To analyse the clinical manifestations of adrenoleukodystrophy (ALD) pedigree and the background of the associated genes. Methods The clinical data of an ALD pedigree were collected and PCR productsequencingwereperformedtoresearch into the change of ALD gene. Results Diagnosis of ALD was determined by the clinical manifestations and brain MRI. The homozygote mutation GGG(Gly)→AGG(Arg) at codon 266 in exon 1 was found in the ALD patient and the heterozygote mutation at the same loci was found in his mother,butwasnotfoundin other members of the family and 2 normal subjects. Conclusions The ALD patient′s mother is the first person taking this point mutation and the mutation causes severe clinical manifestations. The gene research could be regarded as the molecular base of the antenatal diagnosis.
5.Comparison of various assays in diagnosing Cushing′s syndrome

Objective To evaluate the sensitivity and specificity of various assays in diagnosing Cushing′s syndrome. Methods The plasma cortisol, urinary free cortisol (UFC), circadian rhythm in cortisol secretion and dexamethasone suppression test were assessed in 173 patients clinically diagnosed Cushing′s syndrome. The data were compared with the postoperative pathologic diagnosis. Results The normal diurnal rhythm of cortisol secretion was lost in 92.9% patients with Cushing′s syndrome. The loss of normal diurnal rhythm of cortisol secretion of 2 time points occurred in 85.1% (8:00, 16:00) and 91.8% (8:00, 24:00), and that of 3 time points (8:00, 16:00, 24:00) in 94.7% of the cases. The excretion of UFC was increased in 91.7% of patients with Cushing′s syndrome. Low-dosedexamethasonedidnotsuppressthe excessive secretion of glucocorticoid in 79.7% (1 mg) and 84.3% (2 mg) patients with Cushing′s syndrome. The basal level of plasma cortisol was raised in 75.6% patients. The sensitivity of 8 mg dexamethasone suppression test was 50%-70% as the standard was set at 50% suppression, and specificity was more than 95%. Conclusion The most sensitive tests for Cushing′s syndrome are the loss of normal circadian rhythm of cortisol secretion and increased UFC. The method of 3 time points is more sensitive than that of 2 time points in the assessment of circadian rhythm. The 8 mg dexamethasone suppression test is the most useful method in differentiating Cushing′s disease from adrenal adenoma.
6.Development of transcutaneous jaundice predictor for the neonates.

Neonatal jaundice is a common neonatal disease. Severe jaundices lead to kernicterus that affects intellectual development of infants or even causes death. Timely and early prediction is vital to the treatment and prevention. This paper presents a jaundice predictor, which uses C8051F020 as the core of single-chip microcomputer (SCM) system with prediction algorithms proven by a large number of clinical trials. The jaundice predictor can reduce the incidence rate of jaundice, alleviate the condition of infants with jaundice, improve the quality of perinatal, with predicting pathologic neonatal jaundice effectively and calling attention to the prophylactic treatment. In addition, compared with the existing transcutaneous jaundice meters, the new predictor has a smaller size, a lighter weight, more user-friendly, and easier to use by hand-holding.

7.Exploration and practice of improving the performance of hospital information systems with database integrated machine

Objective The rapid growth of data exchange and information volume in hospital information systems has brought significant challenges to the operation,maintenance,and service performance of the system.In order to overcome the performance bottleneck of the software and improve the response performance of the hospital information system,our hospital has tentatively introduced the Exadata database machine.Methods The machine was launched through hardware parameter selec-tion,high availability testing,and data migration.The benefits were evaluated by comparing the I/O operation time,SQL state-ment execution time,and business system module loading time before and after the launch.Results After using this machine,the overall waiting time of I/O operations decreased by 83.47% ,the execution time of the same statement decreased by over 22% ,and the average loadingtime of the business system decreased by43.74% .Conclusion The database machine can effec-tively reduce query time,accelerate the running speed of business modules,and help improve the performance of hospital infor-mation systems.
8.Practice of structured electronic medical record design and quality management

In order to assist in the standardization and maturity evaluation of national hospital information interconnection, and further standardize the application and management of hospital medical record data, a hospital carried out the practice of design of structured medical records and the corresponding quality management from April 2021. Based on the six sigma quality management method, the hospital had developed universal templates for electronic medical records and a list of candidate electronic medical record templates. The problems faced by medical record data had been analyzed, and improvement strategies had been proposed from three levels: template design, software functionality and management services. The clinical departments were guided to design and develop various structured electronic medical record templates for specialties and specialized diseases, and established a medical record template design and quality management method. The hospital had ultimately designed a total of 614 structured electronic medical record templates that met the actual needs of the hospital. This practice enhanced the scalability of structured templates and quality of the data, and achieved localization and specialization of medical record templates while meeting the requirements of information interconnection and sharing, providing reference for promoting the interconnection and sharing of electronic medical records of hospitals in China.
9.Design and implementation of blood transfusion management system based on electronic medical record application and grading

Objective To address the issues of decentralized blood transfusion services and the persistence of paper-based approval processes,as well as to meet the requirements of electronic medical record grading and interoperability assess-ments.Methods Combining the requirements of the assessments,we have restructured and designed the blood transfusion serv-ice processes,and developed a comprehensive closed-loop management system for clinical blood transfusion based on an integrat-ed platform.Results Six major closed-loop processes were established for blood transfusion services and blood management.This transformation led to more standardized transfusion documentation,centralized blood transfusion services,and enabled data sharing across the entire hospital.Conclusion The rationality,standardization,and safety of clinical blood transfusions have improved,leading to a reduction in the incidence of adverse transfusion reactions.
