1.Curative observation of ultrasound-guided microwave ablation therapy for hepatocellular carcinoma combined with arterio-venous fistula

Objective To observe the curative effect of the microwave ablation (MWA) therapy for hepatocellular carcinoma (HCC) with arterio-venous fistula (AVF) in patients with intervention and operative contraindication,and to observe the clinical effect.Methods MWA therapy was offered to 24 patients with HCC combined with AVF accompanied with intervention and operative contraindication.The complete ablation rates,volume changes of tumors before and after treatment were observed.Color Doppler ultrasonography was performed to evaluate the peak flow rate and resistance index(RI)of AVF,width and flow rate change of portal vein.The lifetime of patients were also observed.Results All patients were treated with double needle therapy.The first time complete ablation was 75.0% (18/24).Then they were followed up after 6 months.The volume of tumors was (72.9 ± 23.8)cm3 and (57.6 ± 19.8)cm3 (P <0.01) before and after treatment.The AVF disappeared in 20 cases,while flow rates of fistula orifice decreases obviously in the rest of them.The widths of portal veins decreased from (1.4 ± 0.9)cm to (1.3 ± 0.1)cm,while the flow rate of that increased from (7.5 ± 3.9)cm/s to (20.1 ± 2.9)cm/s.The survival rates of patients for one,two and three years were 66.6 % (16/24),37.5 % (9/24) and 20.8 % (5/24),respectively.Conclusions Most AVF disappeared after MWA therapy in HCC combined with AVF.The therapy can relieve portal hypertension symptom effectively and improve survival time and life quality of patients.So it is a good treatment for patients with HCC combined with AVF while with intervention and operative contraindication.
2.Effects of the multi-model analgesia in the old patients

Objective To study the effects of the multi-model analgesia for the old patients who received total hip replacement surgery.Methods Thiry-four patients who would received total hip replacement surgery were randomly divided into 2 group:Group E was the epidural analgesia group and Group M was the multi-model analgesia group.15min before the end of surgery,the load doses were given to the patients in both groups and the electric pumps for postoperative analgesia were connected.The multi-model analgesia was used for Group M:30min before the end of surgery,ropivacaine 7.5mg and morphine 1mg were given via epidural catheter,and at the end of surgery the catheter was pulled out.Intravenous analgesic pumb were used for Group M.Record the following data at different time point (T1 to T5):(1)the pain scores.(2)the scores of sedation and HR,MAP,RR and SpO2.(3)the numbers of the patients who suffered from adverse effects,such as nausea and vomiting and the numbers of patients who needed the rescue medicine were recorded in 48h after surgery.Results Compared to Group E,the scores of pain when the patients turned over in Group M,were lower significantly at T2.At T1 and T2 time point,the sedation scores of Group M were higher than those of Group E significantly.There was significant difference in the SpO2 change between groups at T1.Conclusion The multi-model analgesia of using long-acting local anesthetics and opioids for epidural analgesia once,then using sufentanil and tramadal for intravenous analgesia continuously,was safe and sufficient for the old patients.
3.Comprehensive Evaluation of Water Environment in the Eling Lake Area

Objective To know the situation of the water environment in the Eling Lake Area and provide the theoretical basis for protecting the biosystem of this area. Methods The water samples were collected from the sites 0.5 meters below the water surface of input and output water area, the water area near the east and west bank of the Eling Lake, the center of the lake, and also from the sites 0.5 meters above the bottom of the lake .The water sampling was carried out in August 2005, October 2005 and May 2006. Country standard method was used to determine the physical and chemical characteristics of water samples. The sampling, determination and evaluation in the investigation were according to related standard regulations. Results The water temperature was 5.6 ℃, pH value was 8.49, the total amount of Ca2+ and Mg2+, suspended matter, chlorides, sulfate, DO, COD, BOD5, Cu2+ and Zn2+ were 95.4 mg/L, 87.0 mg/L, 45.0 mg/L, 80.0 mg/L, 7.1mg/L, 11.0 mg/L, 2.0 mg/L, 0.002 mg/L and 0.009 mg/L respectively. Conclusion All of the physical and chemical characteristics of the water samples meet the water quality criterion of the class ruled by GB 3838-2002, and the water can be used as source water.
4.Effects of methylflavonolamine on arachidonic acid metabolism pathway of platelet

Effects of methylflavonolamine (MFA) on arachidonic acid(AA) metabolism pathway were studied. MFA (iv 40 mg ? kg-1) lightened the acute pulmonary thromboenblism signs in mice and reduced the mortality induced by AA. MFA(12. 5~200?mol ? L-1) in vitro dose-dependently inhibited rabbit platelet aggregation induced by AA. MFA(0.1~0. 4mmol ? L-1) in vitro dose - dependently inhibited rabbit platelet malondiadehyde(MDA) formation in-duced by AA. MFA(0. 4mmol?L-1) inhibited platelet MDA formation in rabbits induced by thrombin and AA. While propranolol inhibited MDA formation induced by thrombin but not by AA. MFA (0.4 mmol ? L-1) did not affect cAMP content in rabbit platelet. These results suggest that inhibitory effect of MFA on platelet AA metabolism may be one of the mechanism by which MFA inhibited platelet aggregation.
5.Effects of Jianwei Yuyang Granule on TGF-?_1 Expression during the Healing Process of Rat Gastric Ulcer Induced by Acetic Acid

Objective To study the effects of Jianwei Yuyang granule on TGF-? 1 expression in the healing process of rat gastric ulcer induced by acetic acid. Methods Animal model of gastric ulcer in rat was set up with the modified Okabe's method, and the TGF-? 1 expression in the gastric ulcerated tissues was detected by immunohistochemical technique. Results TGF-? 1 expression in gastric ulcerated tissues during the healing process in Jianwei Yuyang group significantly increased compared with the normal, model and famotidine treatment groups. Conclusion TGF-? 1 expression in gastric ulcerated tissues induced by acetic acid during the healing process significantly increased,especially in Jianwei Yuyang group. Jianwei Yuyang granule may accelerate gastric ulcer healing by increasing TGF-? 1 expression in gastric ulcerated tissues.
6.Minimally Invasive Radiofrequency Ablation Therapy for Hepatic Cavernous Hemangioma

Objective To evaluate the efficacy, safety and complications of minimally invasive percutaneous radiofrequency ablation(PRFA) therapy for hepatic cavernous hemangioma(HCH). Methods Twelve patients with total 15 HCHs (2.5cm to 9.5cm) were treated by using RF-2000 generator and 10-needle LeVeen electrode percutaneously guided by B-ultrasound. Tumors larger than 3cm were treated by multiple overlapping ablations that encompassed the entire lesions as well as an approximately 0.5cm rim of normal liver tissue. Results The procedures were successful and the patients recovered well without the complications of bleeding or bile leakage after mean 48-month follow up period. Spiral CT scanning showed that the contrast enhancement was disappeared within the lesions or at their periphery in all cases. The mean diameter of the ablated HCHs reduced 68%. Conclusion The results suggested that PRFA therapy was a minimally invasive, simple, safe, and effective approach for treating HCHs in selected patients.
7.Prediction of Promoter Motifs in Virophages.

Virophages have crucial roles in ecosystems and are the transport vectors of genetic materials. To shed light on regulation and control mechanisms in virophage--host systems as well as evolution between virophages and their hosts, the promoter motifs of virophages were predicted on the upstream regions of start codons using an analytical tool for prediction of promoter motifs: Multiple EM for Motif Elicitation. Seventeen potential promoter motifs were identified based on the E-value, location, number and length of promoters in genomes. Sputnik and zamilon motif 2 with AT-rich regions were distributed widely on genomes, suggesting that these motifs may be associated with regulation of the expression of various genes. Motifs containing the TCTA box were predicted to be late promoter motif in mavirus; motifs containing the ATCT box were the potential late promoter motif in the Ace Lake mavirus . AT-rich regions were identified on motif 2 in the Organic Lake virophage, motif 3 in Yellowstone Lake virophage (YSLV)1 and 2, motif 1 in YSLV3, and motif 1 and 2 in YSLV4, respectively. AT-rich regions were distributed widely on the genomes of virophages. All of these motifs may be promoter motifs of virophages. Our results provide insights into further exploration of temporal expression of genes in virophages as well as associations between virophages and giant viruses.

9.Mechanics effect of lumbar physiological curvature adjustment by three-dimensional swirl angle program control:A randomized controlled analysis

AIM:To explore the regulation of lumbar physiological curvature by angulation traction and longitudinal rotation and the clinical significance of repairing spinal balance on lumbar disc herniation(LDH). METHODS:①Totally 71 patients with LDH diagnosed by CT at the 451 Hospital of Chinese PLA were enrolled from October 2005 to January 2006. Three-dimensional swirl angle program control restitution bed was applied. This bed was composed of fixing chest swirl section,magnetic heat lumbar flexure massage section and traction haunch angulation section with the pull strength of 20-90 kg. ②The patients were divided into experimental group(n =38) and control group(n =33). ③All patients of the two groups received supine genuflex traction for 20 minutes and combined with longitudinal rotation for 10 minutes. Patients in the experimental group received 20? traction from below lumbar to bed,while patients in the control group received plane traction combined with longitudinal rotation. ④Lumbar function was determined by criterion of the lumbar disease made by Japanese Shaping Association to observe improved indexes and times of treatment. 10.The role of liver function test in diagnosing concomitant asymptomatic common bile duct stone in patients with cholecystolithiasis

Objective To determine the role of liver function test in the preoperative diagnosis of concomitant asymptomatic common bile duct (CBD)stone in patients with cholecystolithiasis.Methods A retrospective study was conducted from January 2012 to October 2013 at the Department of Hepatobiliary Surgery,the Affiliated Hospital of Zunyi Medical College on 426 patients who were operated for cholecystolithiasis.According to whether they had abnormal liver function,these patients were divided into the CBD stone group (n =44) and the cholecystolithiasis group (n =382).The values of the different components of liver functions test such as ALT,AST,AKP,GGT,TBIL and DBIL in diagnosing CBD stone were statistically analyzed.Results This study involved 426 patients,159 men and 231 women,with a mean age of (50.96 ± 12.93) years.44 patients with both CBD stone and cholecystolithiasis.The rates of abnormal liver function were 77.27% (34/44) vs 4.45% (17/382) in CBD stone group vs cholecystolithiasis group.The difference was significant (x2 =198.54 ; P =0.000).In logistic regression analysis,only elevated serum level of GGT (OR =10.067 ; P =0.007) remained as an independent predictor of CBD stone.On ROC (receiver operating characteristic) curves analysis,the area under the curve was 0.881.With a cut-off point for GGT at 84.5 U/L,there was a sensitivity of 72.2 per cent,specificity of 96.1 per cent,and positive and negative predictive values of 76.3 per cent and 96.2 per cent respectively.Conclusion Our study showed that liver function,especially GGT,was a predictor of CBD stone.
