1.Expression of THANK in human PBMC activated with different stimulators

Aim To analyze THANK gene expression in peripheral blood mononuclear cells(PBMC) stimulated with different stimulators and to clone whole length human THANK gene. Methods PBMC were conventionally isolated and cultured in RPMI1640 containing 10% FCS. After stimulated with LPS,TNF α ,IL 2,IFN γ ,PHA or PMA,the THANK gene expression in PBMCs was analyzed by RT PCR and THANK cDNA was cloned. Result RT PCR detection showed that THNAK gene expressed in PBMCs after stimulated with interferon γ for 3 days, whereas THANK'expression could not be detected after stimulated with LPS,TNF α ,IL 2,IFN γ ,PHA or PMA respectively. Then THANK gene was cloned by cloning PCR product and sequenced. Conclusion Human THANK gene is cloned successfully, thus providing the possibility for further research of THANK'function.
2.Quantitative Study on Professor LV Renhe’s Prescriptions for Treating Chronic Nephritis Based on “CPIAS”

Objective To discuss the methods of specification and quantification of thinking process of syndrome differentiation and treatment of old and famous TCM practitioners; To inherit the clinical experience and ideas of TCM practitioners more effectively. Methods Based on one typical case of Professor Lv Renhe treating chronic nephritis, indexing the prescriptions with TCM Subject Headings thesaurus and analyzing them with “Chinese medicine prescription intelligence analysis system” (“CPIAS” for short), relative dose, comprehensive property, comprehensive taste, comprehensive channel tropism, and prescription efficacy were under quantitative analysis. Results The typical case included two prescriptions of initial diagnosis and six diagnosis. CPIAS quantitative analysis showed that relative dose, comprehensive property, comprehensive taste, comprehensive channel tropism, and prescription efficacy were in accordance with syndrome differentiation and treatment of Professor Lv Renhe for chronic nephritis. Conclusion This method has guiding significance for studying and inheriting the speculative regularities and academic thoughts of Professor Lv Renhe.
3.Effects of Ligustrazine(TMP) on Acute Drug-induced Free Radical Formation in Guinea Pig Cochlear

Objective To explore the effects of ligustrazine (TMP) on acute drug -induced free radical for-mation in guinea pig cochlear ,and to explore possible mechanisms of TMP on gentamycin(GM)-induced ototoxici-ty .Methods Sixty guinea pigs were randomly divided into four groups ,control group ,GM group ,TMP group and TMP+GM group .GM group were injected with GM 120 mg/kg per day .TMP group were injected with TMP 40 mg/kg per day .TMP+GM group were injected with GM and TMP at the same dosage .Control group were injected with normal saline .All groups were injected for consecutive 10 days .Before injection and one day after the last injec-tion ,ABR thresholds were measured .Biochemical assays of Superoxide Dismutase (SOD1 ,SOD2 ,T -SOD) activity and ma1ondia1dehyde(MDA) content in guinea pig cochlear were detected by TBA .Results After the injection , ABR thresholds in GM group were significantly increased compared with those of in control group (P<0 .01) ,with significant difference between GM +TMP and GM group(P<0 .01) .Before and after the experiment ,ABR thresholds in control group were almost unchanged(P>0 .05) .SOD activity was significantly decreased while MDA content was increased in cochlear tissues after GM injection (P<0 .05) .Co -treatment with TMP evidently enhanced SOD activity and decreased MDA content (P<0 .05) .Conclusion TMP may enhance SOD activity and prevent lipid per-oxidation ,thus alleviate GM ototoxicity ,and improve auditory function .
4.Jinchang Capsule for type 2 diabetes mellitus of blood-stasis syndrome: a clinical research

AIM: To study the clinical efficacy of Jinchang Capsule for the treatment of type 2 diabetes of blood-stasis syndrome. METHODS: From JAN.2001 to OCT.2004, 48 cases were divided randomly into two groups.32 cases in the treatment group were treated with oral Jinchang Capsule combined with Western medicine therapy, and 22 cases in the control group were simply treated with Western medicine therapy, with a course of 8 weeks. The changes of clinical symptoms, and fasting blood glucose (FBG), 2 h post prandial glucose(2hFBG), HbAlc, total cholesterol (Tc), triglycerides (TG), high-density lipoprotein (HDL-c), low-density lipoprotein (LDL-c) levels, and hemorheological parameters were analysed. The safety of the medicine was evaluated also. RESULTS: After treatment, the clinical symptomatic integrals were decreased remarkably in the treatment group (P0.05). There was no significant side-effect in the treatment group. CONCLUSION: The results obtained suggest that Jinchang Capsule can improve the clinical symptomatic integrals and remarkably enhance total clinical efficacy. Combining with the basic Western medicinal treatment of diabetes, it can control the obstinate hyperglycemia, and improve fat metabolism as well as bemorheological parameters. It has indicated that Jinchang Capsule has multipurpose therapeutic actions for diabetes mellitus.
5.Analysis of risk factors associated with protein-energy wasting in elderly hemodialysis and peritoneal dialysis patients

Objective:To analyze the risk factors for protein-energy wasting(PEW)in elderly patients undergoing hemodialysis(HD)and peritoneal dialysis(PD), in order to provide evidence for the prevention of PEW and improve the prognosis in these patients.Methods:According to the diagnostic criteria for PEW proposed by the International Society for Renal Nutrition and Metabolism, 112 elderly patients who had undergone PD and HD without PEW from May 2016 to June 2020 in the renal medicine department of the Eighth People's Hospital of Shanghai, the geriatric medicine department of Zhongshan Hospital Affiliated to Fudan University and the renal medicine department of the Sixth People's Hospital Affiliated to Shanghai Jiaotong University were included as the non-PEW group.During the same period, 114 cases with PEW were recruited as the PEW group.Differences in general patient data, biochemical test values, anthropometric measurements, bioelectrical impedance and other index values were compared between the two groups.Independent risk factors for PEW in elderly HD and PD patients were analyzed by using binary Logistic regression analysis.Results:Compared with the non-PEW group, the PEW group had a higher mean age( t=0.951, P<0.001), a higher proportion of patients aged ≥65 years( χ2=17.161, P<0.001), a lower body mass index( t=6.740, P<0.001), a higher incidence of diabetic nephropathy( χ2=14.176, P<0.001), a higher hemodialysis rate( χ2=4.543, P=0.033), and a longer duration(months)of dialysis( t=2.306, P=0.023). Levels of serum transferrin, prealbumin, total protein, hemoglobin and total cholesterol and body cell mass index were lower( t=6.262, 13.405, 9.507, 8.341, 4.610, 5.599 and 2.499, all P<0.05), high-sensitivity C-reactive protein levels and the body water percentage were higher( t=6.380, 4.519, both P<0.001), and the upper arm muscle circumference was smaller( t=5.418, P=0.000)in the PEW group than in the non-PEW group.Binary Logistic regression analysis showed that age≥65 years, dialysis duration, serum albumin and high-sensitivity C-reactive protein levels were independent influencing factors for PEW in elderly HD patients( OR=2.762, 0.182, 2.694 and 2.980, P=0.023, 0.007, 0.009 and 0.027). Age ≥65 years, body mass index and the hemoglobin level were independent influencing factors for PEW in elderly PD patients( OR=2.452, 0.671 and 0.962, P=0.013, 0.000 and 0.000). Conclusions:The incidence of PEW is lower in elderly HD patients than in elderly PD patients.Clinical intervention should be carried out based on influencing factors, in order to prevent the occurrence of PEW.
6.High-risk endoscopic features and therapeutic efficacy of endoscopic treatment of sporadic non-ampullary descending duodenal adenoma

Objective:To investigate the safety and efficacy of endoscopic treatment for sporadic non-ampullary descending duodenal adenoma, and to analyze high-risk endoscopic features of malignant adenoma.Methods:Data of 54 patients diagnosed as having non-ampullary descending duodenal adenoma in Nanjing Drum Tower Hospital from November 2012 to September 2019 were retrospectively studied. The patients were divided into two groups, the high-grade intraepithelial neoplasia/adenocarcinoma (HGIN/AC) group and the low-grade intraepithelial neoplasia (LGIN) group according to pathological grade. Clinical features including gender, age, size and color of lesions, therapeutic methods, complications and postoperative follow-up results were analyzed.Results:A total of 54 patients were divided into the HGIN/AC group ( n=12) and the LGIN group ( n=42). There were significant differences in size or color of lesions between the two groups (both P<0.05). All 54 patients received endoscopic treatment. Biopsy, endoscopic mucosal resection and endoscopic submucosal dissection were performed on 8, 32 and 14 cases, respectively. A small perforation was found and clipped during operation without any complications. There were 2 cases of delayed hemorrhage, and the bleeding stopped under endoscopic treatment. The mean follow-up time was 2-58 months with no recurrence. Conclusion:Endoscopic treatment is safe and effective for non-ampullary descending duodenal adenoma. Lesions of size larger than 10 mm and those with a red surface have higher malignant tendency.
7.Heme oxygenase-1 induced autophagy in the alleviation of liver ischemia reperfusion injury

Objective To investigate the correlation of heme oxygenase-1 (HO-1) expression and autophagy in mice with liver ischemia reperfusion (IR) injury,and to study the influence of HO-1 on the hepatic function.Methods Control group,IR group,Hemin + IR group and Znpp + IR group were constructed.Mice in the Hemin + IR group and the Znpp + IR group were pretreated by Hemin (HO-1 inducer) and Znpp (HO-1 inhibitor) before IR.According to different reperfusion time,the IR group was divided into the 0,2,6,12,24,48 and 72 hours groups,and the Hemin + IR group and the Znpp + IR group were divided into the 0,2,6,12,24 hours groups.The levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were detected to evaluate the hepatic function.The autophagosome and pathological changes of liver were observed under electron microscope and hematoxylin and eosin (HE) staining,respectively.The expressions of HO-1 and autophagyassociated protein (LC3Ⅱ) in the protein level were detected by Western blot.All data were analyzed using the analysis of variance,t test and q test,respectively.Results The levels of ALT and AST of the IR group,Hemin + IR group and Znpp + IR group were significantly higher than those of the control group (F =96.17,85.53,P <0.05).The levels of ALT and AST of the IR 12 hours group and 24 hours group were significantly higher than those of the Hemin + IR 12 hours group and 24 hours group (qALT=--14.46,-7.85 ; qAST=-12.98,-5.26,P <0.05),but significantly lower than those of the Znpp + IR 12 hours group and 24 hours group (qALT=4.25,4.94;qAST=4.98,3.53,P < 0.05).The results of HE staining showed that hepatic IR injury was significalty alleviated in the Hemin + IR group when compared with the IR group and the Znpp + IR group.The Suzuki degrees of hepatic IR injury in the control group,IR 12 hours group,Hemin + IR 12 hours group and the Znpp + IR 12 hours group were 0.5 ± 0.3,2.6 ± 0.5,1.1 ± 0.3,3.0 ± 0.4,respectively,with significant differences among the 4 groups (F =53.62,P <0.05).There were significant difference in the Suzuki degrees of hepatic IR injury between the IR 12 hours group and the Hemin + IR 12 hours group,and between the IR 12 hours group and the Znpp + IR 12 hours group (q =10.67,14.02,P < 0.05).The results of electron microscopy showed that the number of autophagosome in the IR group was greater than the control group and Znpp + IR group,but lesser than the Hemin + IR group.The numbers of autophagosome in every 10 high power fields of the control group,IR 12 hours group,Hemin + IR 12 hours group and the Znpp + IR 12 hours group were 0.4 ±0.2,1.8 ±0.6,4.0 ± 1.8,0.7 ±0.5,respectively,with significant difference among the 4 groups (F =21.35,P < 0.05).There were significant differences in the number of autophagosome between the IR 12 hours group and the Hemin + IR 12 hours group,and between the IR 12 hours group and the Znpp + IR 12 hours group (q =6.05,-3.03,P < 0.05).The expressions of HO-1 and LC3Ⅱ in the IR group and the Hemin + IR group were significantly increased.The relative expressions of HO-1 protein in the Hemin + IR 6 hours group and the 12 hours group were 0.64 ±0.17 and 0.51 ±0.12,which were significantly higher than 0.45 ± 0.08 and 0.17 ± 0.03 of the IR 6 hours group and 12 hours group (t =4.03,4.69,P < 0.05).The relative expressions of LC3Ⅱ protein in the Hemin + IR 6 hours group and the 12 hours group were 1.04 ± 0.20 and 1.20 ± 0.23,which were significantly higher than 0.58 ± 0.04 and 0.95 ±0.14 of the IR 6 hours group and 12 hours group (t =4.29,6.69,P<0.05).The relative expressions of HO-1 protein in the Znpp + IR 6 hours group and 12 hours group were 0.23 ± 0.03,0.14 ± 0.02,respectively,and the relative expressions of LC3Ⅱ protein were 0.35 ± 0.04 and 0.49 ± 0.14,which were significantly lower than the HO-1 and LC3Ⅱ protein expressions in the IR 6 hours group and 12 hours group (tHO-1 =13.82,7.04; tLC3Ⅱ =7.21,4.03,P <0.05).Conclusion HO-1 is positively related to the amelioration of hepatic function,and it may be achieved by induction of autophagy.
8.The clone and expression of human THANK gene

Objective: To clone THANK gene and express its extracelluar fragment. Methods: Using RNA isolated from HL 60 cell lines, THANK cDNA was amplified by RT PCR. The fragment was linked to pMD18 T vector and sequenced, and then the extracellular fragment of THANK was subcloned into pET vector and THANK protein expression was induced.Results: A 858 bp DNA fragment was amplified and the cDNA sequence was identical with the published sequence encoding THANK gene. Western blot showed that THANK protein with a relative molecular weight of 2.6?10 4 was expressed. Conclusion: Human THANK gene was cloned and expressed successfully, which provides a base of further research of THANK gene. [

10.Value of pancreatic stent placement for endoscopic resection of duodenal papilla adenoma

Objective To assess the application value of pancreatic stent placement for endoscopic resection of duodenal major papilla adenoma.Methods A total of 67 cases of duodenal major papilla adenoma that were confirmed by biopsy and underwent endoscopic papillectomy from August 2007 to July 2016 in endoscopy center of Drum Tower Hospital of Nanjing were analyzed retrospectively.There were 50 patients treated with pancreatic stent placement and 17 patients without as the control group.The general information, efficacy and complications of two groups were collected at the same time.Results There were no significant differences in gender(P=0.070), age(P=0.151) or tumor size(P=0.136) between pancreatic stent placement group and the control group.There were no statistical differences in en bloc resection rates or complete resection rates between the two groups.And there were no significant differences in short-term complications of bleeding(P=0.428), pancreatitis(P=0.982), cholangitis(P=1.000), perforation(P=1.000)or long-term complications of distal common pancreatic duct stricture between the two groups.Conclusion Pancreatic stent should not be routinely placed in endoscopic papillectomy, and should be considered for specific cases.
