1.Injection of pingyangmycin and selective arterial embolization for juvenile nasopharyngeal angiofibroma

OBJECTIVE To explore the effect of pingyangmycin injection after selective arterial embolization on the early-stage and recurrent juvenile nasopharyngeal angiofibroma.METHODS Twelve cases of juvenile nasopharyngeal angiofibroma (JNA) were selected to receive digital substraction angiography (DSA) and successfully treated by selective permanent intrarterial embolization.Each case was injected with 12mg pingyanymycin after the embolization every 12 to 20 days,total 4 to 8 times.RESULTS All cases were followed up for at least one year.Six patients were cured or effective, 5 were improved,and one case was no effective. CONCLUSION Pingyangmycin injection combined with selective intrarterial embolization appears to be a safe and effective method in the treatment of early- stage and a part of recurrent juvenile nasopharyngeal angiofibroma.
2.Risk factors of acute kidney injury in 438 patients with severe pneumonia

Objective To investigate the incidence and risk factors of acute kidney injury (AKI). Methods The clinical data of 438 patients with severe pneumonia who were admitted in the intensive care unit in 2016 were retrospectively analyzed for analysis of AKI and assessment of risk factors. Results Among the 438 patients with severe pneumonia,the incidence of AKI was 53.42%. Within 72 hours after admission into the ICU, 86.75% of them developed AKI mainly from hypovolemic or septic shock(accounting for 80.34%). The detection rate of AKI by the standards of RIFLE and AKIN was lower than by the standards of KDIGO (45.89%,48.17%vs. 53.42%). Anemia ,shock /hypotension ,drugs ,RRT ,APPACHE II scores were the predictors of AKI. Conclusions The incidence of severe pneumonia complicated with AKI is high. Most of the patients develop it in 72 hours after admission into the ICU. Anemia,shock/hypotension,drugs,RRT,APPACHE II scores are its risk factors.
3.Impact of ribavirin cumulative dose on virological response rates in genotype 1 hepatitis C virus infected patients

Objective To study the impact of ribavirin cumulative dose on virological response rates in genotype 1 hepatitis C virus(HCV)infected patients.Methods The medical records of 225 genotype 1 chronic hepatitis C(CHC)patients treated with peginterferon α-2a plus ribavirin were retrospectively analyzed.These patients were divided into four groups according to ribavirin cumulative dose:>97%,80%-97%,60%-79%and<60%of standard cumulative dose.The relationship between ribavirin cumulative dose and virological response rates was studied.Data as analyzed by chisquare test or F test.Results The incidence of ribavirin cumulative dose<97%was 43.1%(97/225),which was higher than peginterferon alfa-2a(27.1%,61/225)(x2=12.641,P=0.001).The sustained virological response rate(SVR)was 27.8%(5/18)in group of ribavirin cumulative dose <60%,which was much lower than those in groups of ribavirin cumulative dose>97%(65.6%,84/128),80%-97%(60.5%,26/43),60%-79%(58.3%,21/36)(x2=9.538,P=0.023).The relapse rate was 61.5%(8/13)in group of ribavirin cumulative dose<60%,which was significantly higher than those in groups of ribavirin cumulative dose>97%(20.0%,21/105),80%-97%(23.5% ,8/34),60%-79%(27.6%,8/29)(x2=10.837,P-0.013).Among patients achieved rapid virological response(RVR),SVR in groups of ribavirin cumulative dose>97%,80%-97%,60%-79%and<60 % of standard dose were 92.0%(23/25),88.9%(8/9),85.7%(6/7)and 75.0%(3/4),respectively(x2=1.098,P=0.778).Conclusiom Mlid reduction of ribavirin dose not affect SVR of genotype 1 HCV infected patients.However,the relapse rate is high and SVR is low in patients treated with ribavirin cumulative dose<60% of standard dose.
4.A study of the relationship between neutropenia and clinical infection risk during treatment with peginterferon aifa-2a and ribavirin for chronic hepatitis C

Objective To investigate the corelation between neutropenia (ANC) incidence and infection during treatment with peginterferon alfa and ribavirin for chronic hepatitis C.Methods A retrospective cohort study of 399 patients treated with peginterferon and ribavirin derived from database of Department of Infectious Diseases, the Second Affiliated Hospital, Harbin Medical University was conducted.The incidence of infections and their relation with ANC were investigated.Potential risk factors for infection were identified by multivariate analysis.Results During treatment,neutropenia (ANC < 1.50 ×109/L) occurred in 251 patients.Among which,mild neutropenia [ANC: ( > 0.75-< 1.50) x 109/L],moderate neutropenia [ANC: ( 0.50-0.75 ) × 109/L]and severe neutropenia ( ANC < 0.50 × 109/L)occurred in 132 patients,103 patients and 16 patients,respectively.A total of 80 infections (20.1% )occurred,among which,14 infections were defined as severe.There was no significant difference in infection rate between patients with and without neutropenia ( 19.9%,50/251 vs 20.3%,50/251 ; x2 =0.007,P =0.933).There was no significant difference in infection rate between patients with and without peginterferon dose reduction ( 21.5%,31/144 vs 19.2%,49/255 ; x2 =0.307,P =0.580 ).In multivariate logistic regression analysis,the independent factors associated with infection were age (P =0.021),diabetes (P =0.004) and cirrhosis (P =0.012).Conclusions Infections during treatment with peginterferon alfa and ribavirin for chronic hepatitis C are irrelevant to neutropenia.The independent factors associated with infection are age,diabetes and cirrhosis.
5.Compulsive drug-seeking motivation in rats provoked by escalating high-dose morphine pretreatment and the dopaminergic and nondopaminergic neuron activity in midbrain

Objective To probe the different expression of psychomotor in different model of pretreatment and the influence of pretreatment context on the expression of psychomotor.And to compare dopaminergic and non-dopaminergic neuron activity in midbrain after different pretreatment.Methods Rats were pretreated by escalated high-dose morphine( HD group) ,fixed low-dose morphine( LD group) and saline as control(S group).After withdrawal, rats were challenged by morphine in non-drug used environment.Locomotor behavior and stereotypy behavior were monitored as the index of compulsive drug-seeking motivation.By double labeling immunohistochemistry of tyrosine hydroxylase (TH) and Fos protein, the plasticity of neuronal activity in ventral tegmental area and substantia nigra of rats response to morphine challenge were disclose after pretreated with morphine or saline.Results After 10 mg/kg morphine challenge in non-drug used environment,the locomotor behavior and the stereotypy behavior of HD group were both significant higher than that of S group ( Locomotor behavior and Stereotypy behavior,HD group: 26907 ± 2003 and 129.6 ± 6.5; S group: 14962 ± 1888 and 89.9 ± 10.9, all P < 0.01 ).The results of immunohistochemistry showed that Fos protein expressed in the SNr and in lateral SNr was significantly higher in the HD group compared to S groups ( Fos expression in SNr and lateral SNr,HD group: 29.14 ±5.27 and 9.83 ± 2.84; S group: 17.29 ± 1.51 and 2.06 ±0.45; all P<0.05 ).Conclusion The sensitization behavior expression of the rat pretreated by escalated high-dose morphine is not modulated by drug using enviroment compared with fixed low-dose morphine pretreatment.The neuroplasticity of SNr,especially lateral SNr might be one of the mechanisms underlying the uncontroled sensitization behavior expression.
6.Significance of young females’ different vertical facial types on the aesthetic evaluation of facial profiles

BACKGROUND:It is confirmed that the characteristics of soft and hard tissue development vary largely in different vertical facial types, which reflect the development trend of the patients and the control ing degree on anchorage.
OBJECTIVE:To evaluate the influence of different vertical facial types on the aesthetic evaluation of facial profiles in young females, and analyze the perception of facial attractiveness from different people.
METHODS:Three beautiful females, with high, average and low mandibular angle, respectively were selected according to the measurement data of projection and the orthodontist’s aesthetic appreciation. They were yel ow race, bilateral facial appearance was basical y symmetrical, and no other maxil ofacial disease was found. They also received no orthodontic or orthognathic treatment. The profile digital photographs and cephalograms of three females were col ected. The positions of soft tissue lip and soft tissue chin were altered incremental y with Dolphin Imaging and Management software. Twenty-five new pictures were generated by the smal scale (2 mm per unit) modification in the sagittal direction. By using“acceptable facial appearance”as a filter, 15 orthodontists selected 11 from 25 pictures. Fifty parents of orthodontic patients, 50 orthodontic patients, and 50 amateurs were taken as the evaluators for subjective aesthetic ratings and for the assessment of the influence of vertical facial types on the facial profile attractiveness.
RESULTS AND CONCLUSION:With low and average angle, straight profile, slightly protrude chin was perceived to be beautiful. With high angle, straight profile, slightly protrude profile was perceived to be beautiful, and evaluation results are consistent between men and women. Evaluators are in agreement when it comes to evaluating the perception of facial attractiveness of three different vertical facial types profile in young females. They also have certain difference in aesthetic tendencies, but when it comes to straight profile they share the same preference. Although different vertical facial types profile has an impact on the profile of lip and chin, but it can’t represent the aesthetic preference of al evaluators.
8.Establishment of p53 trancated muteins group and its expression in HeLa cells

Objective: To establish p53 trancated muteins group and analyse their expression and effects on p53 related downstream genes in HeLa cells. Methods: Using PCR-based site-directed mutagenesis technique, a group of P53 muteins with the N- and/or C-terminals differently trancated were set up. The plasmids with p53 mutein genes were used to transfect HeLa cells, the p53 muteins and some p53 related downstream genes' expression were analyzed by RT-PCR. Results: The p53 muteins were expressed in HeLa cells and they had effects on some of p53 related downstream genes. Conclusion: The P53 muteins may be a good tool to analyze P53 function and has effects on p53 related downstream genes' expression model.
9.Aptamers as molecular probes in tumor molecular imaging

The ideal molecular imaging probe should have characteristics of specificity,sensitivity and safety.Aptamers are sing-strand DNA or RNA oligonucleotides which can bind their targets with high specificity and high affinity.Aptamers are new types of molecular imaging probe because they have many advantages,such as their large sum of possible targets,small molecule weights,easily production and modification,low immuno-genicity,high tissue penetration and so on.Now more and more aptamers research on tumor molecular imaging provides a new technology and tool for tumor diagnosis and therapy.
10.Detection and grading of endolymphatic hydrops in Ménière's disease using 3D FLAIR MRI.

OBJECTIVE:
To evaluate the extent of endolymphatic hydrops as shown by three-dimensional fluid-attenuated inversion recovery magnetic resonance imaging (3D FLAIR MRI) performed 24 hours after bilateral intratympanic gadolinium administration and discuss the positive rate of endolymphatic hydrops in vestibule and cochlea.
METHOD:
Twenty-four hours after bilateral intra-tympanic 8 times diluted gadolinium administration, three-dimensional fluid-attenuated inversion recovery MRI, using a three-Tesla unit, was performed in 48 patients, and then assessed the extent of endolymphatic hydrops in the MRI images.
RESULT:
Forty-eight patients showed different levels of enhancement of perilymth in the inner ear. In these patients, obvious signs of endolymphatic hydrops were visualized in vestibule,including 8 slight hydrops, 17 mild hydrops and 23 severe hydrops with a diagnostic rate of 83.3%. There were almost no complications in all 48 patients after bilateral intra-tympanic injection except short vertigo in some of them.
CONCLUSION
3D FLAIR MRI resonance imaging has a high positive diagnostic rate in assessing endolymphatic hydrops of Ménière's disease and provides direct imaging evidence for diagnosing Ménière's disease.
