1.Combined Determination of Serum PG and CEA and its Clinical Application in Distinctive Diagnosis of Benign and Malignant Gastric Ulcer

Objective The aim of this study was to evaluate the clinical value and the possibilities of measuring the serum levels of pepsinogen I (PG I ) and careineembryonic antigen (CEA) in 402 patients with gastric ulcer.Methods The patients with gastric ulcer were all confirmed by either operation or gastrofiberscope,and divided into two groups,benign and malignant gastric ulcer.After comparing the relationship between the patients'clinical appearance and the Radioimmunoaasay(RIA) results,(CEA and PG I ).Results There were 73 patients were onfirmed malignant gastric ulee in 402 patients.The positive rate of PG was 52.05% (38/73).The positive rate of CEA was 64.38% (47/73).Both were 30.14% (22/73).Then there were 63(63/73) patients who was positive in the combined determination.Solo determination's positive rate of CEA and PG were significantly different from the combined determination.(P<0.05 and 0.01) Conclusions Feasibility and the necessity of combined determination were evaluated.Compared with solo determination,for patients with malignant gastric ulcer,it was more sensitive to determine and analysis the serum levels of PG I and CEA sYnthetically.So it is recommended to popularize the combined determination of serum PG I and CEA in clinical distinctive diagnosis of benign and malignant gastric ulcer.
2.Diagnostic and therapeutic value of intradiscal steroid injection in patients with discogenic low back pain combined with radial fissure of annulus fibrosis

Objective To retrospectively investigate diagnostic and therapeutic value of intradiscal steroid injection in patients with discogenic low back pain combined with radial fissure of annulus fibrosis.Methods Totally 120 patients who had undergone CT guided intervertebral discography and steroid injections,including 72 males and 48 females,aged from 25 to 60 years (average,48 years),were recruited to this study.According to Dallas discography description system,7 cases were rated as grade 0,36 grade 1,48grade 2,and 29 grade 3.The Visual Analogue Scale (VAS) and Roland Morris Disability Questionnaire (RMDQ) were used to evaluate clinical effect at 2 weeks,2 months,6 months,12 months and 24 months postoperatively.Results Most of patients who underwent steroid injection in single highly suspect diseased disc claimed significant symptom improvement.There were statistically significant differences between pre- and post-operative VAS and RMDQ.The clinical effect in grade 3 group was better than those in grade 1 and grade 2 groups,and the differences were significant.Patients with severe disc disruption (grade 3) had a lower recurrence rate of low back pain compared with other patients.Conclusion Intradiseal steroid injection not only can significantly relieve discogenic low back pain,but also can benefit diagnosis of discogenic low back pain,especially in patients who fail to reproduce concordant pain on discography.For patients with severe rupture of the annulus fibrosus,the relief of discogenic low back pain is more significant.

4.Effect of neoadjuvant chemotherapy on EC50 of propofol administered by TCI causing loss of consciousness in breast cancer patients

Objective To investigate the effect of neoadjuvant chemotherapy on EC50 of propofol given by TCI which produces loss of consciousness (LOC) in breast cancer patients.Methods Ninety ASA Ⅰ or Ⅱ breast cancer patients aged 30-60 yr scheduled for elective surgery were allocated into 3 groups according to the chemotherapy the patients received (n=30 each):group Ⅰ control (group C) received no chemotherapy;group Ⅱ taxol and group Ⅲ adriamycine+cyclophosphamide+5-Fu.The patients were unpremedicated.The EC50 of propofol given by TCI was determined by up-and-down technique.The initial effect-site concentration (Ce) of propofol was 2.07μg/ml and the ratio between the 2 successive Ce was 1.09.Loss of response to verbal command and eyelash stimulation was used as sighs of LOC.The EC50 of propofol was calculated.Results The EC50 of propofol causing LOC was significantly lower in group Ⅱ and Ⅲ than in control group.There was no significant difference in the EC50 of propofol which produces LOC between group Ⅱ and Ⅲ.Conclusion The EC50 of propofol causing LOC in breast cancerpatients is decreased by adjuvant chemotherapy.
5.Study on influence of Toxoplasma tachyzoites on human sperm motility parameters in vitro

Objective To explore the influence of Toxoplasma tachyzoites infection on motility of human spermatozoa in vitro, and explain its possible mechanism. Methods Semen samples obtained from 10 healthy volunteers by masturbation were prepared by the swim-up technique. The samples were then inoculated at 37 ℃ with different concentrations of live Toxoplasma tachyzoites varying among 1?103/ml (Group A), 1?104/ml (Group B), 1?105/ml (Group C), 1?106/ml (Group D), 1?107/ml (Group E) and Group F containing Ham’s F-10 only as the negative control. Motion parameters were analysed by Computer-aided sperm analyzer(CASA) in 0 hour, 1 hour, 2 hours, 4 hours respectively. Modalities of spermatozoa and possible adherence and/or agglutination were observed under the light microscope. Results Distinct adhesion of spermatozoa to Toxoplasma tachyzoites and agglutination were noticed. In all the motion parameters, progressive motility was affected most and dependent upon the incubation time and tachyzoites concentration. Objective To retrospectively investigate the clinical features of localized primary small bowel stromal tumors (SBSTs) and the impacting factors for prognosis.Methods The clinical and pathological data of 89 consecutive SBSTs patients,with pathologically confirmed,who underwent complete resection in Ruijin hospital between January 2003 and September 2007 were collected and analyzed.All patients were followed up for assessment of tumor recurrence and metastasis.The impacts of clinical and pathologic factors on rate of disease free survival (DFS) of the patients was evaluated.Results In total of 89 follow-up patients,15 patients were diagnosed with tumor recurrence and 9 of them died.The tumor size,mitotic index and pathological risk stratification were statistically related with DFS (P=0.000,P=0.006,P=0.000,respectively) by using Kaplan-Meier univaritate analysis.Tumor size and mitotic index were proved to be independent prognostic factors for tumor recurrence by multivariate analysis COX regression model.Conclusions Tumor size and mitotic index are related with tumor recurrence,and can be regarded as independent predictive factors of tumor recurrence.
7.The prognostic value of fragmented QRS in the recurrance of cardiac events in acute myocardial infarction patients underwent emergency percutaneous coronary intervention

Objective To study the prognostic value of fragmented QRS (fQRS) in the recurrence of cardiac events in acute STEMI patients underwent emergency percutaneous coronary intervention (PCI). Methods Ninety-two acute STEMI patients who underwent emergency PCI were enrolled. The presence of fQRS was evaluated by a 12-lead ECG in 72 h after PCI. Cardiac events (myocardial infarction, need for revascularization or cardiac death) and all-cause mortality were recorded in all patients during 12 months follow-up. Results Cardiac event rate[15 (31.3%) vs. 4 (9.1%)]were higher in the fQRS group (n=48) compared with the non-fQRS group (n=44) during a mean follow-up of 12 months. A Kaplan-Meier survival analysis revealed significantly lower event-free survival for cardiac events in the fQRS group (P < 0.004). The results of Cox regression model analysis revealed that signiifcant fQRS was an independent signiifcant predictor for cardiac events (HR 2.19, 95%CI 1.38-3.50, P=0.023). Conclusions The presence fQRS is an independent risk factor for poor prognosis in STEMI patients undergoing emergency PCI.
8.Observation of the clinical effect of individualized chemotherapy which was designed according to genetic characters in patients with advanced cancer

Objective To explore the effect of individualized chemotherapy plans which was designed depend on secific genetic characters in patients with advanced cancer.Methods The surgery or biopsy specimen samples from 25 patients with advanced recurrent tumors (study group) were analyzed.Different gene mRNA expressions were detected by PCR and sequencing.According to detection results,the most appropriate chemotherapy would be applied on 25 cases patients of study group.The chemotherapy from traditional experience and evidence-based medical evidence were applied for 20 cases patients of control group.The difference of RR and disease control rate (DCR) between two groups were compared.Results The DCR and RR were 84 % (21/25) and 44 % (11/25) in study group,35 % (7/20) and 15 % (3/20) in control group.The DCR and RR in study group were significantly higher than those in control group (P < 0.01).Conclusion Individualized chemotherapy could improve the efficient and prolong the survival period of the patients with advanced recurrent tumors.
9.A Preliminary Study of Comorbidities Associated with Tourette Syndrome

Objective:To explore the category,prevalence and related factors of comorbidities associated with Tourette syndrome.Methods:125 patients with TS according to CCMD-3(Chinese Classification of Mental Disorders,3rd edition)were assessed with a self-designed family circumstance questionnaire,YGTSS,CBCL,Leyton obsessive-compulsive scale,and Conner's Child Behavior Checklist.Results:Of 125 TS patients,the comorbidities included attention deficit and hyperactivity disorder(ADHD,41.6%),obsessive-compulsive disorder(OCD,25.6%), anxiety disorders(8.0%),depressive disorders(4.8%),conduct disorders(8.0%),self-injurious behavior(3.2%),and sleep disorder(2.4%).Conclusion:There are many kinds of comorbid disorders at high prevalence in TS patients. These comorbidities adversely influence the therapy and prognosis of TS and are taken as the possible reasons for social function deficit.
10.Assessment of DNA damage in radiation workers by using single cell gel electrophoresis

objective To assess the DNA damage of radiation workers in different grade hospitals,and to explore the correlation between the types of work or work time and the levels of DNA damage.Methods DNA single strand break were detected by using alkaline single cell gel electrophoresis(SCGE),and the comet was analyzed with CASP(Comet Assay Software Project).TDNA,TL,TM and OTM were calculated.Results The parameters of SCGE in the radiation greup were higher than those of control group(F=3.93,P<0.01).The significant difference was found not only among the different types of work or difierent work time,but also among the different grade hospitals(F=1.83,1.9 1,P<0.05).Conclusions Various levels of DNA damage could be detected in the radiation workers of the two hospitals.DNA damage of radiation workers is less serious in the higher-grade hospital than the lower grade one.Different types of work or work time might affect the DNA damage level.
