1.Bacterial composition and resistance of wound infections

Objective To determine the bacterial composition and resistance of wound infections in China. Methods Disc diffusion test,MIC test and E-test were used to detect the antimicrohial resistance of bacterial isolates from wound secretions.WHONET 5.4 was applied for analysis of the bacterial sensitive data from 86 domestic tertiary hospitals from June 1,2006 to May 31,2007. Results (1)2125 bacterial strains were coileeted in the survey period,which included 994 strains (46.8%) of Gram positive and 1131 strains (53.2%) of Gram negative bacteria,Staphylococcus(780 strains,36.7%),E.coli(338 strains,15.9%) and Pseudomonas aeruginosa (231 strains,10.9%)were the most common isolates.(2)38.2% and 84.1% of Staphylococcus aureus and Staphylococcus epidermidis were methicillin-resistant.respectively.No Staphylococcus strain was resistant to vancomycin or teicoplanin.(3)ESBLs positive rate of E.coli was about 60%,while the resistant rate to quinolones Was about 70%.(4)The resistant rates of Staphylococci and E.coli isolated from adults to aminoglyeoside and quinolones were higher than those from children.(5)The resistant rates of Staphylococei to cephalosporin,aminoglyeoside and quinolones from inpatients were higher than those from out-patients. Conclusion Staphylococci,E.coli and Pseudomonas aeriginoso were among the most common organisms isolated from wound infections.The ESBLs positive rate of E.coli from wound infections was higher than that from all specimen bacteria found in other surveys performed during the same period and the methicillin resistant Staphylococci were less than that.
2.Relationship between Tei index and the myocardial enzymes in newborns after asphyxia LEI Xiao-yan,

Objective To study the relationship between Tei index and the myocardial enzymes in newborns with myocardial damage due to asphyxia. Methods Forty-four newborns with mild asphyxia,27 newborns with severe asphyxia and 20 healthy newborns hospitalized in our hospital from January 2005 to December 2008 were enrolled in this study.The peak E and peak A and E/A ratio at mitral valve,the ejection fraction,isovolumetric contraction time,isovolumetric relaxation time and Tei index were measured by conventional echocardiography,and aspartate aminotransferase,creatine kinase,lactate dehydrogenase,hydroxybutyrate dehydrogenase and creatine phosphate kinase isoenzyme,and cardiac troponin T were tested also.One-way ANOVA was used to analyze the difference between groups. Results Tei index,isovolumetric contraction time and isovolumetric relaxation time in the severe asphyxia newborns were 0.62±0.13,(47±7)ms and(52±8)ms,higher than those in the mild asphyxia newborns[0.51±0.14,(41±6)ms and(43±6)ms],those in the mild asphyxia newborns were higher than in the healthy newborns[0.39±0.12,(34±6)ms and (37±6)ms] (all P<0.01).Aspartate aminotransferase,lactate dehydrogenase,hydroxybutyrate dehydrogenase,creatine kinase and its isoenzymes and cardiac troponin T in the severe asphyxia newborns were higher than in the mild asphyxia newborns and healthy group(P<0.01).Tei index,isovolumetric contraction time and isovolumetric relaxation time were positively related to the myocardial enzymes.The positive relationships were more statistically significant in Tel index,creatine kinase,its isoenzymes and cardiac troponin T(r=0.762,0.821,0.778,P<0.01).Conclusions Combination of Tei index and myocardial enzymes can not only be applied to assess the cardiac function in newborns with myocardial damage due to asphyxia,but also to evaluate the degree of cardiac dysfunction accurately and dynamically.
3.Effects of baicalin on mitochondria apoptotic pathway of trophoblast cells in a preeclampsia rat model

Objective To explore the effects of baicalin in the treatment of a preeclampsia (PE) rat model by detecting the expression of X-linked inhibitor of apoptosis protein (XIAP) and cysteine containing aspartate-9 (Caspase-9) and observing the ultrastructure of mitochondria in trophoblast cells.Methods Forty-eight pregnant Wistar rats were randomly divided into two groups:12 in the control group and 36 in the PE model group.The PE model was established with subcutaneous injection of l-nitro arginine methyl ester with 100 mg/kg per day from the 13th day of pregnancy.Beginning from the 16th day of pregnancy,the PE rats were injected with different doses of baicalin till cesarean section,and divided into three groups:non-intervention PE model group treated with saline (NIP group),low-dose baicalin intervention group (IDB group) at 50 mg/kg per day,and high-dose baicalin intervention group (HDB group) at 100 mg/kg per day.The rat tail artery blood pressure and 24-h urine protein level were measured at pregnant day 10,16 and 20.The levels of XIAP and Caspase-9 in placenta were measured by immunohistochemistry.The ultrastructure of mitochondria of trophoblast cells of the rat placenta was observed under electron microscope.T test,F test and LSD-t test were applied for statistical analysis.Results (1) On pregnant day 10,no significant differences were observed in rat tail artery systolic blood pressure,diastolic blood pressure and 24-h urine protein level between the control group and PE model group (all P>0.05).On pregnant day 16 and 20,the systolic blood pressure,diastolic blood pressure and 24-h urine protein level of NIP,IDB and HDB groups were significantly higher than those of control group [pregnant day 16:systolic blood pressure:(137.74±5.21),(136.15±4.86),(138.28±4.79) and (110.57±3.79) mmHg (1 mmHg=0.133 kPa),diastolic blood pressure:(89.58 ± 5.50),(88.45 ± 8.59),(89.42 ± 6.29) and (80.28 ± 7.36) mmHg,24-h urine protein:(7.78 ± 0.45),(7.53 ± 0.54),(7.86± 0.57) and (6.45 ± 0.56) mg;pregnant day 20:systolic blood pressure:(145.26 ± 4.67),(131.28 ± 4.34),(130.93 ± 5.33) and (110.40 ± 6.92) mmHg,diastolic blood pressure:(89.87±6.55),(85.34±7.33),(84.64±7.36) and (80.19±7.34) mmHg,24-h urine protein:(11.18±0.42),(9.65±0.54),(9.06±0.56) and (6.31 ±0.45) mg] (all P<0.01).On pregnant day 20,the systolic and diastolic blood pressure and 24-h urine protein level in IDB and HDB groups were significantly lower than in NIP group (all P<0.05),but showed no significant differences between IDB and HDB groups (allP>0.05).(2) Compared with NIP group,the expression of XIAP in control group,IDB and HDB groups were significantly increased(210.39±0.78,180.56±0.82,195.36±0.96 and 192.84± 1.06,all P<0.01).There was no significant difference in the expression of XIAP between IDB and HDB groups (P=0.66).The expression of Caspase-9 in control group,IDB and HDB groups were significantly decreased compared with NIP group (210.36±0.55,195.53±0.96,198.42± 1.01 and 185.25±0.64,all P<0.01).There was no significant difference in the expression of Caspase-9 between IDB and HDB groups (P=0.65).Ultrastructure of mitochondria in NIP group showed different degrees of damage,matrix swelling,and mitochondrial cristae bresk or disappearance.In IDB group,mitochondrial matrix swelling was not obvious,and mitochondrial cristae were visible.In HDB group,mitochondrial cristae were neat and clear.Conclusions Baicalin may play an important role in the treatment of preeclampsia by reversing the trophoblast apoptosis and improving the ultrastructure of mitochondria through its regulation of XIAP expression and downregulation of Caspase-9 expression.
4.Drift correction in fMRI signal based on mathematical morphology

Objective To establish a new approach based on mathematical morphology that can effectively reduce the drifts in functional magnetic resonance imaging (fMRI) signals. Methods Based on investigation of the characteristic of drifted fMRI signals, a mathematical morphology method for baseline drift correction was presented. Results With both simulated data and real fMRI data, the results of the experiment showed that the mathematical morphology method can effectively correct the baseline drifts. Conclusion Both linear and nonlinear drifts can be removed with the proposed method without any statistical model assumption.
5.Preliminary MRI study in patients with congenital complex strabismus

Objective Although the ocular motility examination has been traditionally used in the diagnosis of complex strabismus resulting from cranial nerve (CN) and extraocular muscles (EOM) abnormalities, magnetic resonance imaging (MRI) now permits the direct imaging of lesions in CNs and EOMs. Methods Twenty-six patients with complex strabismus underwent MRI examination on 1.5 T MR unit (Twinspeed, GE). Nerves to EOMs were imaged with T1 weighted in orbits in all patients using phase array surface colls. Results Patients with Duane syndrome ( 15 cases, 19 eyes) all exhibited absence or hypoplasia of abducens nerve (CN6), always with mild hypoplasia and apparent misdirection of oculomotor nerve ( CN3 ) to the lateral rectus muscle in the orbit, and there were no hypoplasia of EOMs. Patients with congenital fibrosis of EOMs (9 cases, 16 eyes) exhibited severe hypoplasia of CN3 and CN6, and EOMs appeared hypoplasia to a different degree, particularly severe for the superior rectus and levated palpted muscles. Multiple nerves displayed aplasia in patients with Mobius syndrome (1 case, 2 eyes) and there was abnormal branch from CN3 to lateral rectus. The tendons of bilateral superior oblique muscles were thin in the patients with Brown syndrome ( 1 case, 2 eyes). Conclusion MRI can directly demonstrate absence or hypoplasia of CNs and corresponding EOM abnormalities in congenital complex strabismus, which suggests that the mechanism of congenital complex strabismus is perhaps abnormal innervation or displasia of the ocular motor nerves.
6.Discussion and practice of the training mode of talents majoring in Chinese medicine based on apprentice and college education

In recent years, a lot of universities has accumulated abundant experiences to set up summer school in China. This article focuses on the practice of summer school in Beijing University of Chinese Medicine, in order to provide some promoting strategies for summer school in domestic universities of Traditional Chinese Medicine in future.
7.Analysis of influencing factors and training strategies of young talents′ scientific research performance in the third level public hospital

Objective:To investigate the influencing factors of the scientific research performance and analyze the differences between subjective and objective ones among the different characteristics of staffs in work and then propose tailored management strategies based on the research findings.Methods:A total number of 963 young researchers under the age of 45 (including doctors, medical technicians, scientific researchers) were surveyed and the impact on the scientific research performance of four subjective and objective factors including demographic characteristics and research motivation, self-efficacy, team support and the external environment were analyzed through self-developed questionnaire.Results:The academic degree, age, time spent overseas, administrative position, postgraduate mentor, professional title and levels, category of departments on the scores of scientific research performance had reached statistical significance ( P <0.05) by using Single factor analysis. The results showed that the impact on the scientific research performance followed a descending order of the category of postgraduate mentors, academic degree, professional title levels, the category of departments, self-efficacy, time spent overseas, and professional title series. Conclusions:According to the survey, this research focuses on the predictive factors which affecting scientific research performance as the important methods of scientific research promotion and talent training, and puts forward to the ideas of talent training, scheme of optimizing talent selection, carrying out accurate policy implementation and scientific assessment.
8.Practice and effect analysis of scientific papers delicacy management in "double track parallel"

Objective To provide authors with more quality services,so as to improve management efficiency,reduce management time and manpower cost,ultimately achieve the expected delicacy management of scientific papers.Methods Indicators of traditional management pattern and "double track parallel" management mode of the scientific papers management process were compared through retrospective study.Such indicators include authors' operation,data error rate,average working cycle,management cost,user experience,data barriers,self-management consciousness and others.Resuits Significant improvements were shown in the data accuracy,management cost savings,user satisfaction,and data sharing in the "double track parallel" management model.Conclusions 5 years of practice demonstrated that the "double track parallel" management model not only effectively connects hospital policy and technical method,but also solves the key problems of data barriers,disjointed management and low efficiency.Scientific paper management is expected to be more refined and effective.
9.Process reengineering and effectiveness analysis of scientific research achievements reward based on scientific research management system

Objective To make good use of the incentive function of the reward policy of scientific research achievements in hospitals ,shorten the reward cycle of scientific and technological achievements ,reduce the management cost ,achieve the ul-timate goal of details management of scientific and technological achievements .Methods Through retrospective analysis of the incentive process to identify possible barriers ,reconstruct the rewarding procedures of scientific and technological achievements by bringing in informatization and publicity strategy ,compare the bonus distribution cycle with the traditional model .Results The results show that the reward model based on the scientific research management system reduces the bonus arrival waiting period by about 62% ,saves management costs and improves staff compliance .Conclusions According to the 4-year practice experiences ,the new management model has further released the positive impact of the hospital award policy ,solved the key problem of restricting the rewarding effect by technological means in the process of rewarding scientific research achievements .
