1.Memantine and Vascular Dementia

Excitatory amino acid toxicity is one of the mechanisms of vascular dementia. Gluta-mate is the major excitatory neurotransmitter in brain. Memantine, a moderate affinity, uncom-petitive and voltage-dependent N-methyl-D-aspartate (NMDA) receptor blocker, can block NMDA receptor-induced intracellular calcium overload and prevent excitotoxicity. It can be used in the treatment of vascular dementia.
2.A study on neuroprotective effect of memantine in hippocampus of rats with vascular dementia

Objective To probe the neuroprotective effect of memantine in hippocampal CA1 region of rats with vascular dementia(VD). Methods 36 male Wistar rats were randomly divided into sham-operated group,VD group and memantine group,and each group have 12 rats. Rats of VD were treated with a permanent bilateral common carotid arteries occlusion(2-VO). Then memantine was given by intragastric administration.The behavioral abnormalities were investigated by Y-type maze test.The changes of cell morphology and the expressions of neuronal nitric oxide synthase(nNOS) and cysteine proteinase-3 (Caspase-3) of neuron in hippocampal CA1 region were detected by HE stain and immunohistochemistry respectively.The percentage of neuronal apoptosis by TUNEL. Results Compared with VD group and sham-operated group,the performance records of learning and memory(learning records:63.16±1.75,79.25±2.01,50.83±1.80,F =707.91,P <0.01;memory records:53.17±1.85,69.17±1.70,40.67±1.83,F =762.43,P <0.01),the expressions of nNOS (average gray scale:140.23±2.30,175.87±3.34,105.63±3.52,F =1539.67,P <0.01)and Caspase-3 (average gray scale:132.78±5.36,162.78±5.25,99.54±5.06,F =439.37,P <0.01)and the percentage of neuronal apoptosis (33.75±2.70,56.17±3.56,6.33±3.65,F =672.91,P <0.01)of memantine group were in the middle. Conclusion Memantine can improve spatial memory and attenuate neuronal apoptosis by down-regulation excessive expression of nNOS and Caspase-3 in rats with vascular dementia.
3.Effect of group psychological intervention and relaxation training on nurses' job burnout

Objective To evaluate the effect of group psychological intervention and relaxation training on nurses' job burnout.Methods A total of 236 female registered nurses [mean age (27±4) years,and mean employment (17±3) years] were randomly assigned to the intervention group (n=120) and the control group (n=116).The intervention group had 60-minute group psychological intervention,10-minute psychological relaxing exercises and 20-minute progressing muscle relaxing exercises per week for 8 weeks.The control group did not receive any instrument.The participants were assessed at the 1st and 8th week by using the Job Burnout Scale.Results There was no statistical significance in three dimensions of the Job Burnout Scale between the two groups at baseline (P>0.05).However,after the intervention,the intervention group had lower scores of EE (13.62±7.86 vs.19.83±4.89) and DP (5.25±3.27 vs.6.55±3.43) and higher scores of PA (38.28±6.34 vs.32.13±7.06) than the control group (all P<0.01),and had lower scores of EE (18.92±8.36 vs.13.62±7.86) and DP (6.44±3.56 vs.5.25±3.27) and higher scores of PA (28.39±7.78 vs.38.28 ± 6.34) than baseline (all P<0.01).The control group showed no significant difference in those parameters before and after the intervention (P>0.05).In the intervention group,1-9 years employment subgroup had lower scores of EE (13.21±7.32 vs.16.91±5.63) and DP (5.61±4.33 vs.7.80±4.59) and higher scores of PA (36.34±7.28 vs.31.39±6.88) than the control group; 10-19 years employment subgroup had lower scores of EE (11.68±5.95 vs.15.14±6.43) and DP (5.11±3.65 vs.7.82±5.21) and higher scores of PA (31.24±8.90 vs.27.33±5.96) than the control group (all P<0.01); 20-29 years employment subgroup had lower scores of EE (12.37±5.23 vs.15.36±4.98) than the control group (P<0.01).Conclusions Group psychological intervention and relaxation training could effectively relieve nurses' job burnout.
4.Risk factors of central lymph node metastasis and significance of prophylactic central lymph node dissection for cN0 papillary thyroid carcinoma

Objective To investigate the risk factors of central lymph node metastasis and significance of prophylactic central lymph node dissection for clinical N0 (cN0) patients with papillary thyroid carcinoma (PTC).Methods The clinical data of 315 patients with cN0 PTC in Department of General surgery,the Second Affiliated Hospital of Dalian Medical University from Jan.2012 to Jan.2014 were analyzed retrospectively.Results (Iumor size,infiltration of thyroid capsule,and tumor number were associated with central lymph node metastasis in patients with cN0 PTC(P<0.05),and the high risk factors of central lymph node metastasis were infiltration of thyroid capsule and multiple lesions (P<0.05);()The overall complication rate was 3.17% (10/315),the rate of transient recurrent laryngeal nerve paralysis was 0.63% (2/315),and the rate of transient hypoparathyroidism was 2.54% (8/315).All patients with complications recovered after treatment.No patient developed permanent recurrent laryngeal nerve paralysis or hypoparathyroidism;()The follow-up time was 6 to 30 months,and 2 cases were lost.No patient developed local tumor recurrence,distant metastasis,or death.Conclusions The high risk factors of central lymph node metastasis in patients with cN0 PTC were infiltration of thyroid capsule and multiple lesions.No patient developed local tumor recurrence,distant metastasis,or death.It is preferable and necessary to perform prophylactic central lymph node dissection in patients with cN0 PTC.
5.Imaging study of non-bronchial systemic collateral supply in hemoptysis

Objective To investigate the significance of non-bronchial systemic collaterals (NBSCs) in supplying bronchial hemoptysic lesions,and to study the morphological features of bronchial artery (BA) when NBSCs become the predominant supplying vessels. Methods Multi-slice helical CT angiographic findings in 124 patients with bronchial hemoptysis were retrospectively analyzed. 3D reconstruction of thoracic systemic arteries,including BAs and NBSCs,was performed at the console work station with the help of real-time thin-slice enhanced helical CT scanning. The number of NBSCs and BAs was calculated,and the internal diameter of the arteries and the thickness of pleura in the vicinity of the pulmonary lesion were measured. According to the presence or absence of NBSCs,the patients were divided into NBSCs group and non-NBSCs group. The relevant data was statistically analyzed. Results NBSCs group included 36 cases,the mean internal diameter of BA was (1.850 ? 0.631)mm and the pleura adjacent to the pulmonary lesion was obviously thickened in 22 cases (61%) with a thickness of 2.7-16.0 mm [mean(7.71 ? 4.12) mm]. In the non-NBSCs group (n = 88),the mean internal diameter of BA was (2.200 ? 0.528) mm and the pleura adjacent to the pulmonary lesion was obviously thickened in 7 cases (8%) with a thickness of 1.1-2.4 mm [mean(1.7 ?0.53) mm]. 6.A Model Study: Simulation of Marker Gene Deletion and Target Gene Replacement Mediated by Cre/lox System in Cell Line

The incorporation of site-specific recombination systems can help to overcome bottlenecks in livestock transgenic technology. For evaluating the efficiency of Cre/lox mediated DNA recombination in embryos and somatic cells, a working model was established using rat mammary carcinoma cell line SHZ-88, aimed at creation of and use repeatedly of selected "friendly loci" in transgenic livestock. An integration vector pTE-lox2272-DsRed-loxP-GFP-loxP, which red fluorescence gene DsRed served as the first target gene and green fluorescence gene GFP as marker gene, was constructed for introduction of acceptor loci in genome. At the same time a replacement vector pT-lox2272-neo-loxP in which Neo coding sequence served as the second target gene was also constructed for replacing DsRed gene. Transgenic cell clones were produced by electroporating SHZ-88 cell with the integration vector. Cells from three transgenic clones selected randomly were further amplified and were then co-electroporated with the replacement vector as well as cre gene. Analysis of the expression patterns of DsRed and GFP indicated that among the 1 070 cell colonies the efficiency on marker GFP deletion was 91.1% and the efficiency on gene replacement was 29.3%. Molecular analysis by PCR and Southern blotting confirmed that the color patterns as expressed by cell colonies could represent the actual molecular events. This working model mediated by Cre/lox system should be useful for the improvement of the present animal transgenic technology.
7.Anatomical study on self-made intramedullary nails for fixation of clavicular fractures

Objective To design an intramedullary nail that had the same ends and could be connected to hairline pointer and employ it to fixate mid-shaft clavicular fractures. Methods Sixty adult clavicles and five fresh adult cadaveric clavicles were employed to measure full length, angle between lateral and middle segments, angle between middle and medial segments as well as the narrowest outer and inner diameters of the clavicle. The interlocked intramedullary nail with the same structure of two ends was designed and used to fixate mid-shaft clavicular fractures. The nail was inserted in retrograde manner and locking screws could be inserted at beth ends. Results The full length, angle between lateral and middle segments, angle between middle and media] segments as well as the narrowest outer and inner diameters of the clavicle were (14.82 ± 1.15) cm, 30°, 30°, (1.00±0.16) cm and (0.50± 0. 18) cm respectively. After conversely placing guide pin from the fracture end, expanding medullary cavity and adjusting the nail, the designed nail was inserted into the medullary cavity from the distal or proximal ends of the clavicle, which was locked by interlocked screws. Conclusions Self-made nail takes advantages of less surgical invasion, visible fracture reduction and stable fracture fixation for treatment of mid-shaft clavicular fractures. While the biomechanieal properties of the nail need further investigation.
8.Effect of terlipressin on hepatic and renal function in cirrhotic patients undergoing hepatectomy

Objective To investigate the effect of terlipressin on hepatic and renal function in cirrhotic patients undergoing hepatectomy.Methods Aanlyze the clinical data of 57 patients following irregular hepatectomy for hepatocellular carcinoma with cirrhosis,according to whether use terlipressin or not after operation,which were divided into terlipressin group (A group,n =27) and control group (B group,n =30).Liver function parameters (ALT,AST,TB),ascites,urine volume and renal function parameters (Cr,BUN) preoperatively and on postoperative day(POD) 1,3,5 and 7 were compared between the two gruops.Results Compared with those of POD 1,the levels of ALT,AST and ascites on POD 3,5,7 were significantly lower in two groups (P < 0.05),urine volume was significantly increased (P < 0.05),Cr of POD 7 was significantly lower (P <0.05),but it is more remarkable in group A than group B.The levels of ALT in terlipressin group on POD 5,7 were (144.9 ±76.3) U/L,(100.5 ±61.5) U/L,which were lower than those of (267.2±91.2) U/L,(199.3 ±70.5) U/L in control group.On POD 3,5,7,the levels of AST,BUN,Cr and peritoneal fluid in terlipressin group,which were respectively(211.1 ±99.8) U/L,(80.4 ±54.6) U/L,(50.6 ±46.5) U/L,(6.6 ± 1.9) mmol/L,(6.5 ± 1.7) mmol/L,(6.3 ± 2.1) mmol/L,(74.3 ± 10.9) μmol/L,(71.5 ± 8.9) μmol/L,(58.7 ±4.1) μmol/L,(247.6±60.3) ml,(58.8±54.3) ml,(40.2±31.8) ml,were significantly lower than those in control group which were (298.7 ±131.2) U/L,(201.1 ±93.4) U/L,(114.7 ±70.3) U/L,(7.3 ± 1.9) mmol/L,(7.2±1.8) mmol/L,(7.1±1.7) mmol/L,(79.5±15.1) μmol/L,(76.9±16.2) μmol/L,(69.4±11.4) μmol/L,(275.2±88.1) ml,(191.7±71.6) ml,(93.2±50.2) ml.while urine volume of (2232.3±409.8) ml,(2270.5 ±395.8) ml,(2179.0±301.4) ml was much more than that of (1921 ± 510.4) ml,(2019.1 ±411.2) ml,(1978.7±323.7) ml in the control group,the differences in the two groups were statistically significant (P < 0.05).There were 11 (36.7%) patients with hepatic and renal dysfunction and hepatorenal syndrome after operation in group B,while only 2 (7.4%) patients in group A.Conclusions The use of terlipressin after partial liver resection has a protective effect on hepatic and renal function in patients with cirrhosis,and can reduce postoperative ascites and prevent hepatorenal syndrome.
9.The research and development of new diagnostic gene chips for SARS coronavirus.

The loci of cDNA sequences for valid diagnosis have been identified through the selection of the genome of SARS coronaries. The gene chips for diagnosing such virus have been developed, based on our own-developed technology for manufacturing and application of gene chips. The diagnoses given by such gene chips were consistent well with the reports of clinical laboratories (94.29%) and the sensitivity reached 10(-2)/ml virus molecules. This method is well suited for the clinical use in SARS coronaries diagnosis.
