Objective To study the clinical value of VACORA vacuum-assisted rotation system in the treatment of benign breast lesions. Methods Under the screening of dynamic ultrasound,122 beast lesions in 93 patients diagnosed as benign lesions from ultrasound were treated with VACORA vacuum-assisted rotation system, and all the patients were examined for follow-up. Results All the lesions were totally rotated and 119 lesions demonstrated to be benign and 3 lesions were atypical hyperplasia from pathologic examination. In the benign lesions, 72 cases were demonstrated to be fibroadenoma, 39 cases were demonstrated to be fibrosis adenosis,4 cases were intraductal papilloma,2 cases were duct inflammation, 1 case was epithelial hyperplasia of the duct and 1 case was lymphode. In 8 cases, bleeding was seen along the needle tract, which was stopped after compression. Small hematoma was formed in 10 patients in the second day of follow-up, which was disappeared in the 3 to 6 months of the follow-up. And all patients had no remnant lesions in the local area and the incisions were not clear in the skin 3 to 6 months of the follow-up. Hypoechoic focuses were detected out at the original location in the 3 benign cases, which were possibly residual focuses. Conclusions Benign breast lesions could be clearly resected with VACORA rotation system under the screening of ultrasound, which is safe, minimal invasive, prosthetic, and has little complications.

Objective To investigate the correlation and significance of cyclooxygenase-2 (COX-2)and the breast cancer resistance protein (BCRP) expressions in non-Hodgkin lymphoma(NHL). Methods Ten patients with reactive lymph nodes (RLN) were considered as a control group. Compared with β-actin as internal control, the BCRP mRNA expressions of 61 NHL samples and 10 lymph tissues in control group were detected by semi-quantitative revere transcription polymerase chain reaction (RT-PCR) assay, while the expressions of COX-2 protein of the above specimens were detected by SP immunohistochemistry. Results The positive rates of COX-2 and BCRP in NHL were 50.8 %(31/61) and 45.8 % (28/61), respectively , and were higher than those in the control group (P <0.05). The expression of COX-2 was statistically positive correlated to that of BCRP (X2 =8.795, r=0.355, P<0.05). The expressions of COX-2 and BCRP were not correlated to clinical and pathological factors, such as age, sex, IPI, LDH, β2-microglobulin level and Ann Arbor stage, however, the expression of BCRP was statistically correlated to chemotherapy efficacy. Conclusion BCRP may be involved in multi-drug resistance (MDR) of NHL, so it may contribute to the assessment of chemotherapy and prediction of NHL. Since there is a strong correlation between COX-2 expression and MDR in NHL, the application of COX-2 inhibitors may enhance sensitivity of chemotherapy.

Objective To investigate the effects of different doses of propofol on expression of synaptophysin (SYP) mRNA and protein in the hippocampus of mentally depressed rats after electroconvulsive therapy (ECT) .Methods Fifty adult male SD rats weighing 200-250 g were randomly divided into 5 groups (n = 10 each): group Ⅰ control (group C); group D mental depression (group D) ; group Ⅲ , Ⅳ , Ⅴ propofol 90, 110, 130 mg/kg + ECT (group M_1, M_2, M_3). Mental depression was induced by isolation and chronic unpredictable mild stress (CUMS) in group Ⅱ-Ⅴ . Group M_1 , M_2 and M_3 received intraperitoneal (IP) propofol 90, 110 and 130 mg/kg respectively + ECT once a day ×7 consecutive days while group C and D received IP normal saline instead of propofol. The rats underwent open field test and sucrose liquid consumption test the day before and after mental depression was established and one day after treatment. The rats were killed after the last test for determination of expression of SYP mRNA (by RT-PCR) and protein (by immuno-histochemistry) in the hippocampus. Results The ambulation scores, rearing scores, grooming time and sucrose consumption percentage were significantly decreased while the center, grid detention time (CDT) was significantly increased in group Ⅱ-Ⅴ as compared with control group indicating mental depression was successfully induced. The ambulation scores, rearing scores, grooming time, sucrose consumption percentage, SYP mRNA and protein expression were significantly increased while CDT was significantly decreased after treatment in group M_1 and M_2 as compared with group D indicating that propofol 90 or 110 mg/kg combined with ECT was effective for the treatment of mental depression. Conclusion Propofol 90 or 110 mg/kg combined with ECT is effective in treating mental depression while proprofol 130 mg/kg combined with ECT is not. Excessive inhibition of SYP expression in hippocampus by large dose of propofol may explain the mechanism.`

Objective To explore the effect of MECT on learning memory in depressed rats and its synaptic plasticity mechanism. Methods Fifty SD rats were randomly divided into 5 groups( n =10): MECT (received ECT with intraperitoneal propofol), ECT (received ECT only), propofol (received intraperitoneal propofol), depression, and control. The treatments were given daily for 7 consecutive days. All rats underwent open field and Morris water maze test. The SYP protein and mRNA expressions in rat hippocampus were detected using immunochemistry and RT-PCR, respectively. Results After treatment, the open field scores were much higher in MECT and ECT groups than in propofol and depression groups ( P <0.05); the learning memory was worse in ECT group than in MECT, propofol and depression groups ( P <0.05); the expressions of SYP protein and mRNA was higher in MECT and ECT groups than in propofol and depression groups ( P <0.05), the expressions of SYP protein and mRNA were lower in MECT group than ECT group ( P <0.05). Conclusions Propofol can improve learning memory in ECT-treated depressed rats through attenuation of ECT-induced expression of SYP in rat hippocampus.

Objective To evaluate the effect of islet transplantation for patients with type 2diabetes mellitus (DM). Methods Since December 2007, 4 cases of islet transplantations were performed on 3 patients with type 2 DM and end-stage renal disease (ESRD). Two patients received simultaneous islet-kidney transplant from single-donor (SIK), and one received 2 consecutive islet transplants 5 months following kidney transplantion (IAK). All recipients given insulin with a dose of percutaneous transhepatic portal catheterization. Anti-CD25 monoclonal antibody was used as induction. For SIK, low-doses of Tacrolimus and sirolimus were used as maintenance immunosuppression protocol. For IAK, the maintenance protocol included cyclosporine and MMF.Insulin dose, the level of blood glucose, C-peptide and the value of HbA1 were observed. Results The first patient of SIKhad normal glucose level 3 days after surgery and became insulin independent within the first month, but insulin was administered gradually and the dose reduced to 1/3. The second patient of SIK died of bleeding and secondary infection of liver puncture site 5 days following operation, the blood glucose level recovered to normal 24 h after operation. The insulin dose of the patient of IAK was reduced to 1/2 after the first transplant. The patient became insulin free after the second operation. The level of fasting and postprandial C-peptide of the surviving recipients increased by 600 pmol/L. The value of HbA1 of the SIK was 6.7 %~7.3 %, while that of the IAK was 5. 5 %~ 5. 9 %. Conclusion Islet transplantation is an effective treatment for patients with type 2 DM.
