1.Assessment of risk factors for postoperative complications in meningioma

Objective To investigate the risk factors that may affect postoperative complications in patients with meningiomas. Methods Of all 300 patients with diagnosed pathologically intracranial meningiomas undergoing resection were retrospectively ana-lyzed in terms of clinical datas and radiological features of preoperative MRI.These patients were divided into two groups according to whether postoperative complications occurred after tumor resection.Both univariate and multivariate regression models were used to analyze the effect of patient age and several MRI characteristics on postoperative complications.Results There were several signif-icant differences between the two groups in patient age,tumor location,tumor size,shape of tumor,wrapping around nerves and blood vessels,and tumor enhancement (P <0.05).According to the results of multivariate Logistic regression analysis,tumor loca-tion,tumor size,wrapping around nerves and blood vessels,and tumor enhancement were independently identified risk factors in the prediction of postoperative complications associated with meningioma resection.Conclusion The features of preoperative MRI may be useful for clinicians in predicting postoperative complications of intracranial meningiomas,and can provide imaging evdience for effective treatment and prognostic assessment in patients with meningiomas.
2.ISOLATION AND PHARMACOLOGICAL ACTIVITIES OF AN ANTITHROMBUS PROTEIN FROM FLAMMULINA VELUTIPES

A protein designated as Flammulina Velutipes (FV) protein was isolated from the cultured mycelium of FV and purified to homogeneity by CM cellulose CM 52 and gel filtration chromatography. The molecular weight of FV protein were found to be 1.75?10~4u with pI 4.3. Amino acid analysis of FV protein indicated the presence of 14 kinds of amino acids, especially rich in Glu, Asp and Thr.Potent inhibitory effect of FV protein on thrombus formation, in the blood was observed in vitro when rats were given intravenously. The length of thrombus was shortened and the dry weight of the thrombus decreased significantly at the dose of 7.5?10~(-7) mol/kg. Thrombus formation was completely inhibited at the dose of 10~()-8 mol/kg. The IC_(50) was found to be 4.4?10~(-7) mol/kg. FV protein was shown to inhibit collegen induced aggregation of rat platelets both in vitro and in vivo. Its concentration for complete inhibition in vitro was 7.5?10~(-5) mol/L. The IC_(50) was found to be 4?10~(-5)mol/ L. However, FV protein dose of 1.25?10~(-5) mol/kg was required for 60% inhibition in vivo

Objective To study the influence of madopar for 5-HT in platelet of Parkinson's disease patients.Methods To observe 5-HT in platelet of 35 PD cases in our hospital who take Madopar in one year.At the same time,to observe the mental symptoms change of every case before and after the treatment.Result Madopar can significantly lower the 5-HT of platelet in one year treatment,and HAMD score results show that Madopar significantly higher before treatment.Conclusion Madopar can damage serotonin neurons in the brain,which may enhance the patients with depressive symptoms.
4.Clinical application of video-assisted thoracoscopy plus minithoracotomy

Objective To discuss the value of video-assisted thoracoscopy plus minithoracotomy (VATM) in the management of thoracic diseases. Methods An 1.5 cm incision was made along the middle axillary line at the 7th costal interspace on the diseased side to introduce the thoracoscope. After the exploration of intrathoracic lesion, another incision 5~8 cm in length close to the lesion was made. The thoracic cavity was opened by way of the costal interspace. Surgical performance of exploration, dissection, hemostasis and suture was performed under thoracoscope and direct vision by using routine surgical instruments and thoracoscopic instruments. A total of 43 cases of VATM were carried out. Results The operation time was 40~150 min (mean, 67 min). The length of incision was 5~8 cm (mean, 6 cm). The chest drainage lasted 2~5 d. No postoperative complications occurred. The postoperative hospital stay was 5~8 d (mean, 6 d). Follow-up found no recurrence for 3~22 months in 18 cases of spontaneous pneumothorax and pleural effusion, and found no recurrence, distant metastasis or incision implantation for 5~20 months in 3 cases of lung cancer receiving either lobectomy or wedge resection. Simple biopsy of tumor was conducted in 6 cases of lung cancer, who obtained chemotherapy or gamma knife radiotherapy postoperatively and survived 5~21 months. Conclusions VATM is practical, minimally invasive and safe. By using routine surgical instruments it has an optimistic outlook.

6.Effects of different nutrition intervention on alimentary tract reconstruction in knowledge and practice nutrition and efficacies of patients with type 2 diabetes mellitus

A total of 206 patients of type 2 diabetes mellitus (T2DM) were assigned into the trial group (n =156) and the control group (n =50) according to whether real object-based diabetic diet was provided.Traditional nutritional education for both groups during hospitalization.The patients in the trial group received a real object-based diabetic diet guided by a nutritionist who adjusted feeding regimens and skills before and after operation.The control group had a free access to a normal diet.The patient understanding of knowledge,attitude and practice (KAP) was examined by nutrition surveys.And their postoperative nutrition states and complications were assessed by questionnaires during a 6-month follow-up.The total average score of cognitive divided was 93.4 ± 7.5,the average score of diet therapy knowledge 13.8 ± 1.6 and the average scores of attitude and practice 9.6 ± 1.9 and 32.2 ±3.9.Significant differences existed between two groups (P < 0.05).The incidence rates of hypoglycemia,hyperglycemia and gastrointestinal obstruction were lower than the control group (P < 0.05,P < 0.01).It suggested that real object-based diabetic diet guidance plus traditional nutrition education could improve the KAP level and may lower the incidence of postoperative complications in T2DM patients with gastric bypass.

ObjectiveTo investigate the protective effects of p38MAPK inhibitor SB203580 on acute necrotizing pancreatitis associated lung injuries in rats.Methods Fifty-four SD male rates were randomly divided into 3 groups,including control group,ANP group,SB203580 group with 18 rats in each group.ANP was induced by intraperitoneal injection of L-arginine solution. Rats in control group were intraperitoneally injected with same amount of saline.Before ANP induction,the rats in SB203580 group received 10 μmol/L SB203580 dissolved by dimethyl sulfoxide at a dose of 5mg/kg weight via intraperitoneal injection.The rats were sacrificed at 3,6,and 12 h after operation,the serum levels of amylase,TNF-α,IL-6 was determined.Pathological changes of pancreas and lung were observed.The wet/dry (W/D) weight ratio of lung and MPO were measured.CINC mRNA of lungs was determined by RT PCR. Expression of phosphated-p38MAPK (p-p38MAPK) protein was evaluated by Western blotting.ResultsThe serum levels of amylase,TNF-α,IL-6and wet/dry (W/D) weight ratio of lung,MPO activity,CINC mRNA and p-p38MAPK protein expression of lungs were (1035 ±73)U/L,(0.94 ±0.16)μg/L,(4.77 ±0.86) μg/L,3.92 ±0.29,(0.39 ±0.02)U/g,0.28 ±0.04,0.09 ±0.04 in control group at 6 h after operation,and the corresponding values were (5848 ±656) U/L,(3.84 ±0.32)μg/L,(103.54 ± 15.32)μg/L,4.97 ±0.47,(1.03 ±0.08) U/g,0.62 ±0.06,0.52 ±0.14 in ANP group,while they were (4259 ±286) U/L,( 1.64 ±0.21 ) μg/L,(76.56 ± 11.46) μg/L,4.32 ±0.34,(0.78 ±0.05)U/g,0.37 ±0.04,0.27 ±0.08 in SB203580 group.The values in ANP group were significantly higher than those in control group,and the values in SB203580 group were significantly lower than those in ANP group,but they were still significantly higher than those in control group ( P < 0.01 ).ConclusionsSB203580 may attenuate injury of lung and pancreas in ANP by blocking p38MAPK signal transduction pathway,and decreasing the production of inflammatory cytokines.

9.The effect of microRNA-27a on U251 glioma cells

Objective To discuss the effect of microRNA -27a on U251 glioma cells.Methods Over-expression or inhibition of miR -27a in U251 glioma cells were done by transient transfection of miR -27a mim-ics or AMO-27a in vitro.Cell viability was detected by MTT assay .Invasion ability of U251 was detected by tr-answell invasion assay.The level of miR-27a and PPARγwere detected by real -time PCR.Results Under in-hibition of miR-27a condition,the proliferation and invasiveness of U 251 glioma cells were decreased .The level of PPARγwas significantly increased ,whereas the level of miR-27 a was decreased .Overexpression of miR-27 a increased the proliferation and invasiveness of U 251 glioma cells and decreased the level of PPARγ.Conclusion Inhibition of miR-27a is benefit for inhibiting the proliferation and invasiveness of U 251 glioma cells.
10.Clinical Observation of Torasemide in the Treatment of Chronic Heart Failure

OBJECTIVE:To observe clinical efficacy and safety of torasemide in the treatment of chronic heart failure(CHF). METHODS:94 patients with CHF were selected and randomly divided into control group and observation group,with 47 cases in each group. Control group were treated with routine treatment,such as bed rest,oxygen inhalation,control of total body fluid in-take,routine anti-heart failure therapy and use of diuretic in 24 h. Based on the above treatment,observation group were treated with Torasemide injection with initial dose of 5-10 mg,qd,gradually increasing to 20 mg/d,maximal does of 40 mg/d. Both group re-ceived 7 days of continuous treatment. Clinical efficacies were observed in 2 groups as well as serum potassium and sodium,Scr,24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function before and after treatment. The occurrence of ADR was compared between 2 groups. RESULTS:The total effective rate of observation group was 61.70%,which was significantly higher than that of control group (46.81%),with statistical significance (P<0.05). There was no statistical significance in serum potassium and sodium and Scr between 2 groups before and after treatment(P>0.05). There was no statistical significance in 24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function between 2 groups before and after treatment(P>0.05);after treatment,24 h urine volume and the rate ofⅠ-Ⅱgrade cardiac function of 2 groups were increased significantly,the observation group was higher than the control group, with statistical significance (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups (P>0.05). CONCLUSIONS:Torasemide is effective for CHF,and can promote diuresis and the recovery of cardiac function with good safety.
