Objective To compare the different dehydrations of microwave-vacuurn drying and high temperature drying on contents of the major effective in fructus momordicae.Methods Using two different drying methods for processing of fructus momordicae, the mogroside Ⅴ, vitamin C in dried products which was determined by high performance liquid chromatography (HPLC) was used to a main indicator for comparing different drying methods.Results The contents of mogroside Ⅴ in fructus momordicae which processed by microwave-vacuum drying, microwave-vacuum drying after ripening treatment and high temperature drying were 1.14％, 1.19％ and 0.60％;the contents of vitamin C were 0.21％, 0.30％ and 0.000 06％.Conclusion The processing method of microwave-vacuum drying to keep active components is obviously better than which of high temperature drying.

Objective To investigate the value of minimal invasion technique in treatment of breast benign disease.Methods 204 benign breast tumors of 74 women were completely excised by using Mammotome or VACORA.Results 108 masses were confirmed fibroadenoma,66 fibroadenosis,30 adenosis of breast.Postoperative followed up 3～12 months:there were 2 cases of local hematoma,5 cases of subcutaneously hematoma,1 case of recurrence,no infection.Conclusions There ale many advantages using uhrasound-guided minimal invasion technique to treat benign breast disease,including accurate localization,minimal invasion,less bleeding,easy performation,less complications and good cosmetology.

Objective To study Barrett's esophageal consistency with its histological findings,which were found with magnifying chromoendo6copy,and to advance the endoscopic diagnosis of Barrett's esophagus. Methods The patients with BE were diagnosed by endoscopy and histology, and their histological findings and endoscopic appearances of a total of 67 patients with BE were observed. Results Endoscopic appearances type of BE showed island (28 cases) ,tongue(7 cases) ,and circum ference(32 cases). Three types of BE under magnifying chromoendoscopy,and in magnifying endoscope group,type Ⅲ which were found with magnifying endoscope were verified intestinalisation epithelium pat ho-type, compared with type I and type Ⅱ,the discrepancy of them had statistical significance(P＜0.01). Conclusion Magnifying chromoendoscopy had a relatively better consistency in the diagnosis of Barrett's esophagus with histological findings.

Objective To investigate the impact of cigarette smoking on inflammatary factors in sputum including cell component,IL-8 and eotaxin,and the effect on responses to treatment with inhaled corticosteroids (ICS)in patients with asthma.Methods Thirty-eight outpatients with chronic stable asthma who visited from January 2009 to February 2010 were enrolled in the study.Twenty-three cases were nonsmokers and 15 cases were smokers.All of them were treated by daily inhaled budesonide,and[32 agonist when necessary.Induced sputum eosinophil and neutrophil proportion were measured,and the levels of interleukin(IL-8)and eotaxin in induced-sputum by enzymatic immunoassay(ELISA)were compared between non-smoking and smoking asthmatic patients.Results The eosinophil proportion was(2.0 ± 0.4)％ in smokers and(4.6 ± 2.5)％ in nonsmokers before therapy,and(1.1 ± 0.5)％ in smokers and(1.8 ± 0.6)％ in nonsmokers after therapy.The difference was statistical significant(F =15.271,P ＜ 0.05).The eotaxin was(3.5 ± 2.1)× 10-3 mg/L insmokers and(8.6 + 2.3)x 10-3 mg/L in nonsmokers before therapy,and(3.1 + 1.5)x 10-3 mg/L insmokers)and(3.6 ± 1.3)x 10-3 mg/L in nonsmokers after therapy.There was statistical significant(F =24.172,P ＜ 0.05).There were no significant difference on neutrophil proportion and IL-8 before and after treatment(F =1.563 and 1.793,respectively,Ps ＞ 0.05).Neutrophil proportion(F =9.632,P ＜ 0.05)and IL-8(F =5.720,P ＜ 0.05)in smokers were significantly higher than non-smokers,whereas eosinophil proportion (F =15.879,P ＜ 0.05)and eotaxin(F =12.365,P ＜ 0.05)in smokers were significantly lower than nonsmokers.Conclusion There was a significantly increase in inflammatary cells andfactors in induced-sputum in two groups after ICS treatment,but the effect in smokers was lower than in non-smokers.Neutrophil proportion and IL-8 were higher in induced sputum in smokers,whereas eosinophil proportion and eotaxin were higher in non-smokers.

Objective To investigate the relationship betweenthe stability of carotid artery plaque and cystatin C,homocysteine. Methods A total of 1 50 cerebral infarction patients with carotid artery plaque treated in Hospital of the First Electricity Authority in Guangdong were selected as the research subjects.According to Color Doppler ultrasound scanning,the patients were divided into the observation group (86 unstable plaque patients)and control group(64 stable plaque patients).Peripheral venous blood from the patients of both groups were collected for the detection of cystatin C,homocysteine,serum total cholesterol and fibrinogen levels and the results of detection were compared between the two groups.Results The levels of cystatin C,homocysteine,serum total choles-terol and fibrinogen in the observation group were significant higher than those of the control group(t =48.760,20.530,29.860, 33.720,P <0.05).The level of cystatin C was weak positive correlated with homocysteine(r =0.21,P <0.003).Multivariate lo-gistic regression analysis showed cystatin C,homocysteine,serum total cholesterol and fibrinogen were independent risk factors for carotid artery plaque instability in patients with cerebral infarction.Conclusion Cystatin C,homocysteine,serum total cholesterol and fibrinogen are important risk factors for carotid plaque instability,the cerebral infarction is closely related to the carotid artery plaque stability,so controlling the risk factors is the focus of prevention and treatment of cerebral infarction.

Objective To compare the clinical efficacy of minimally invasive removal of intracranial hematomas and medical con-servative treatment for hypertensive cerebral hemorrhage .Methods A total of 75 patients with hypertensive cerebral hemorrhage were collected and randomly divided into two groups according to the treatments :38 patients treated with conservative medicine and 37 patients treated with minimally invasive removal of intracranial hematomas based on standard drug therapy .Neurological impair-ment and recovery of self-care ability of patients in both groups before and after three weeks and six weeks of treatment were scored by National Institute of Health Stroke Scale (NIHSS) .Changes of diffusion tensor imaging before treatment and after two weeks of treatment were evaluated by fractional anisotropy value (FA) .Results There was no significant difference between neurological im-pairments of the patients in two groups before treatment .But the recovery degree of functional impairment and muscle nerve in min-imally traumatic treatment group was more significant than those in the conservative medical treatment group after treatments . There was significant difference between two groups (P< 0 .05) .Conclusion Minimally traumatic of hematomas is an effective method of treating hypertensive intracerebral hemorrhage .

Objective To evaluate the efficacy of mobile endospine system for sequested lumbar disc herniation.Methods From May 2007 to December 2009,31 patients of sequested lumbar disc herniation were treated with mobile endospine system,including 17 men and 14 women with a mean age of 46 years (32-59 years).Patients complained severe leg and low back pain with disability.According to MRI,the sequested nucleus herniated from the disc of L2-3 in 1 case,L3-4 in 1,L4-5 in 16 and L5S1 in 13;and the direction of herniation was caudal in 24 cases,cephalic in 6 and indistinguishable in 1.The patients were followed up for 12 months (range,6-24 months) ,and the results were evaluated according to Macnab scale.Results The procedure was technically successful in all the patients:the sequested nucleus was completely extracted.The protruded disc was treated with discectomy in 30 cases,and the nearly intact disc was left untouched in one patient.Among the 24 cases with caudal herniation,sequested nucleus situated ventrally beneath the dural theca and the transitional nerve root in 15 cases,between the nerve root and dural theca in 5,and dorsally on the nerve root and dural theca in 4.All the 6 cephalic herniation situated beneath the dural theca,1 of them reached the level of pedicle,and 2 herniated into the intervertebral canal.The sequested disc presented as indistinguishable signal on MRI included nucleus,annulus and cartilage endplate,and situated dorsally around the dural theca.The mean operative time was 50 min (range,40-70 min) with a mean blood loss of 80 ml (range,30-200 ml).There was no complication of nerve injury.Only too much facet was resected in 2 patients without clinical symptom.The results were excellent in 21 cases and good in 10 cases,and all of them were satisfied with this procedure.Conclusion The working canal and visual field of mobile endospine system is movable,so the sequested nucleus can be extracted completely with good results.

Objective To estimate the excess relative risk coefficients of stomach cancer for Chinese population attributable to ionizing radiation.Methods The excess relative risk and excess absolute risk coefficients of stomach cancer were estimated based on Life Span Study by using risk models developed by BEIR Ⅶ committee (Biological Effect of Ionizing Radiation).Guided by transportation methods from Life Span Study to Americans,we determined that transportation method for Chinese population includes both multiplicative and additive models with a weight of 0.7 and 0.3 respectively,on an arithmetic scale.Besides,curve fitting was used to obtain sex-age-specific stomach cancer baseline incidence based on Chinese cancer annual report.Then,Chinese excess relative risk coefficients of stomach cancer were obtained by substituting excess relative risk,excess absolute risk of Life Span Study and Chinese baseline incidence rate into risk transportation model.Results Excess relative risk coefficients of stomach cancer for Chinese population are 0.26/Sv for male and 0.64/Sv for female,whose exposure age is 30 years old and cancer age is 60 years old.Coefficients increase with decreased exposure age and cancer age.Conclusions Excess relative risk coefficients of stomach cancer for Chinese population are by larger higher than that of Life Span Study,and their sex-age tendency are similar.

Objective To estimate the averaged excess relative risk(ERR) in Chinese population based on the radiogenic cancer risk of leukemia in Japanese atomic bomb survivor cohort,and to discuss proper method suitable for risk transfer between populations.Methods Based on BEIR Ⅶ radiogenic cancer model and population transfer model,and the 2009 Chinese leukemia baseline rates given in 2012 Chinese Cancer Registry Annual Report,comparison was made of population incidences in seveal countries to adjust the weighting factors.Results The ERR of three subtypes of leukemia as a whole was obtained,and the weighting factors for risk transfer model was assumed.The additive factor for male was 0.2,and the multiplicative factor was 0.8,while the additive factor for female was 0.15,and the multiplicative factor was 0.85.Conclusions For the risk transfer between populations,weighting factor was adjusted as a whole to obtain the ERR value for estimating the risk to Chinese population.The risk transfer method suitable for Chinese population was obtained by using the incidence rate available for Chinese population to directly transfer radiation-induced leukemia risk to Chinese from Japanese.

This study was aimed to improve the transfer rate of sinomenine in the intermediate product of Caulis Sinomenii in order to optimize the purification process of intermediate product ofCaulis Sinomenii fromTong-An (TA) injection. The transfer rate of sinomenine and the stability of fingerprints in the intermediate product of Caulis Sinomenii were used as indexes for the investigation on the impact from different pH ofCaulis Sinomenii extract before extraction. The transfer rate of sinomenine was used as an index for the investigation on the impact by different pH of hydrochloric acid created to dry extract solution. The transfer rate of sinomenine was used as an index for the investigation of four separation ways, which included the vacuum filtration, plate and frame filters, high-speed tube separator, and flat direct centrifuge, on the liquid separation of sinomenium acutum acid. The results showed that the pH ofCaulis Sinomenii extract before extraction was 10-11; the transfer rate of sinomenine was the highest in the extraction process and the fingerprints of TA injection was stable. The pH of hydrochloric acid was 2.0-2.5; and the highest transfer rate of sinomenine in acid dissolution process was 92.94%. The high-speed tube separator had the best separation to sinomenium acutum acid-dissolving liquid. The highest transfer rate of sinomenine was 93.34%. It was concluded that the optimized process can effectively improve the transfer rate of sinomenine in the intermediate product ofCaulis Sinomenii. Meanwhile, fingerprints of the product were stable. The process was simple with good repeatability.
