1.Locking compression plate for treatment of unstable distal radius fractures

ObjectiveTo explore the application and outcome of locking compression plate in the treatment of unstable distal radius fracture.Methods From January 2006 to December 2008,eighty-two patients with consecutive unstable distal radius fractures were preformed with open reduction and locking compression plate fixation by volar, dorsal or bilateral approaches. There were 31 males (bilateral fractures in two patients) and 49 females with an average age of 51 years (range, 17-74 years).According to AO/OTA criterion, there were seven patientswith type A3 fractures, four with type B 1, 12with type B2, 10 with type B3, 16 with type C1, 21 with type C2 and 12 with type C3. The locking plate fixations through simple volar approach was performed in 61 patients, fixations through dorsal approach in 12, volar 1/3 radius cylindrical steel plate fixation through volar and dorsal approach in five, bilateral fixations through volar and dorsal approaches in four. Furthermore, the external fixator was used in 19 patients, fixation of the ulna fractures in seven, and bone graft in 39. ResultsThe patients were followed up for average 20.7 months ( range, 1-4 years), which showed postoperative complications including bubble appearing around the incision in three patients, infection in one, median nerve injury in three, screws penetrating into extensor compartment in four, threads penetrating into radiocarpal joint because of reduction loss in two, rejection in one and traumatic arthritis in seven. According to the Cooney criterion, the result was excellent in 56 pateints, good in 19, fair in six and poor in one, with excellence rate of 91%. Conclusions The locking compression plate can provide firm fixation and allow early functional exercise and hence is suitable for unstable distal radius fracture especially the osteoporosis patients with comminuted compression fracture.

Ischemic cerebral stroke is one of the diseases posing great threat to human health.It is of great value to have a correct evaluation and interpretation of cerebral angiography in interventional treatment of ischemic stroke together with efficacy.The authors reviewed the direct signs and collateral circulation of ischemic cerebral stroke and the evaluation of cerebral angiography in recent literature.
3.Surgical treatment of refractory ulcerative colitis, report of 60 cases

Objective To evaluate the clinical effect of restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) for refractory ulcerative colitis (UC).Methods In this study 60 refractory UC patients received IPAA operation during the period of 1990 to 2010.Data were collected regarding early and late postoperation complications,anal continence function,and characteristics of feces.The patients' quality of life was objectively accessed using the Clevend Global Quality of Life (CGQL)index.0ne-way analysis of variance was used.Results Mean follow-up period was 2 years.Early postoperative complication rate developed in 15% (9/60),including abdominal or pelvic infection,anastomotic leak,pouch bleeding,pouch-vaginal fistula,and intestinal obstruction.Late postoperative complication rate was 12% (7/60),including pouchitis,intestinal obstruction,and male sexual dysfunction.Stool frequency per 24 hours and that at night was 3.5 ± 1.3 and 1.4 ±0.6.93% (56/60)patients differentiated gas and feces well and 3% (2/60) needed daily pads.According to Kirwan Grading Scale,anal function outcomes were Grade Ⅰ:54 (90%),Grade Ⅱ:4 (7%),and Grade Ⅲ:2 (3%).According to Bristol Stool Form Scale,characteristics of feces were Grade Ⅳ:31 (52%),Grade Ⅴ:25(42%),and Grade Ⅵ:4 (6%).Postoperative CGQL result showed a much better quality of life than preoperative CGQL (F =12.368,P < 0.05).Conclusions Refractory UC is surgically indicated and IPAA is the operation of choice with safety,satisfactory long-term outcome and improved quality of life.
4.Influence of platelet-rich plasma (PRP) on autologous fat grafting breast augmentation surgery

Objective To evaluate the influence of platelet-rich plasma (PRP) on autologous fat grafting breast augmentation surgery.Methods A lot of 40 patients were divided randomly into the treatment group and control group, each group of 20 cases.In the treatment group cases PRP was used in addition to autologous fat grafting.Chest circumference and thickness of subcutaneous fat were measured before and 6 month after operation and 12 months later.The results were compared and the influence of PRP was evaluated after operation.Results Both groups of patients were satisfied with the results.Compared with the control group, patients in the treatment group showed the chest circumference and thickness of subcutaneous fat were significantly increased 12 months after operation.Conclusions The efficacy of PRP is significant in enhancing fat graft, reducing fat absorption in 12 mouths after operation.The fat tissue mixed with PRP is a reliable reconstruction option for the breast augmentation.
5.Changes of collagen fiber during reattaching of the musseter muscle following different curved osteotomies of prominent mandibular angle: a study in goats

Objective To investigate the changes of the constitution and its ratio of collagen fiber in the process of masseter reattachment following different osteotomies of the prominent mandibular angle so as to offer guidance for the resection of mandibular angle. Methods Sixteen adult goats were randomized into four groups. In group A we performed unilateral curved osteotomy of the mandibular angle. In group B unilateral curved ostectomy was performed with partial masseter resection. In group C unilateral angle splitting ostectomy, while in group D unilateral dissection of the masseter muscle was conducted. The constitution and its ratio of collagen fiber in the interface were observed at 1-month, 2-month, 3-month, and 6-month after operation. Results On the changes of collagen fiber in the process of muscular reattachment, at 1-month post-operation, the constitution of collagen fiber (types Ⅰ and Ⅲ) in groups A and B were significantly different from that of control group (P<0.05). However, both groups C and D had no statistic difference from control group (P>0.05). At 2-month, 3-month and 6-month post-operation, those of all experimental groups had no statistic difference from control group. And with time, the percentage of collagen fiber type Ⅰ increased and type Ⅲ decreased gradually. Conclusion The recovery sequences of masseter muscle reattachment in this study are firstly group C, secondly group A and finally group B. It suggests that the recoveries of mastication and other oral activities are different. Group B turns out to be with a slow muscle reattachment. Thus, we recommend treating different kinds of mandibular hypertrophy with different ostectomies.
6.Prevention and treatment of early hypertrophic scars with botulinum toxin type A

Objective To explore the effect of botulinum toxin type A(BTXA)in the treatment of early hypertrophic scars(HTS).Methods BTXA was injected into and around the eady HTS,and then the modal and histological changes of the scars as well as the clinical reaction were observed in the patient.BTXA was also injected into muscle around the incision and effect on the cicatrization observed.Results Injection of BTXA could obviously alleviate ache and pruritus of eady HTS and could impel the atrophy and inteneration of eady HTS.Changes were found in paraffin-embedded tissue section by the hemetoxylin and eosin(HE)staining.Injection of BTXA into muscle around the cut could can reduce occurrence of HTS.Conclusion BTXA can help prevent the early HTS to a certain extent.The mechanism underlying this effect may be related to the reducing the tension around scars and proliferative activity,interfering with the signal transduction of small nerves,affecting the proliferation and apoptosis of fibroblasts and subsequently decreasing the collagen synthesis.
7.Establishment of a model of rabbit auricular hematoma and its application

Objective To establish a rabbit auricular hematoma model,to observe the process of production and absorption,and to study the location of hematoma and method of its removal Methods Ten health New Zealand rabbits were divided randomly into control group and experimental group.A weight was dropped onto particular locality of auricles 3-6 times until a big hematoma appeared.The pathological processes of hematoma were observed,and the hematoma was formed 1 hour and the hematoma was almost absorbed after 4 days.The drainage experiment was conducted to observe the effect of needle aspiration or incision and drainage on the hematoma.Results All experimen tal auricles formed hematoma.The histopathological study showed that hematoma located between skin and cartilage,did not involve the cartilage.After 4 days,new-formed capillaries and fibroblasts were found,and then hematoma mostly absorbed; fibrous hyperplasia was been found.By cutting the hematoma,the blood clot could be thoroughly removed.Conclusions The auricular hematoma model can be established easily by striking with a heavy weight.Hematoma locates in subcutaneous connective tissue.A large hematoma should be early removed for prevention of auricular deformity.
8.Measurement of mandibular angle related angles by three dimensional reconstruction and their correlation analysis

Objective To explore the correlation of data obtained through three dimensional reconstruction and measurement of mandibular angle related angles and bigonial breadth.Methods Sixty cases of adult female head spiral CT data were involved in this study.By the three dimensional reconstruction,mandibular angle related angles and bigonial breadth were measured.Through SPSS13.0 software their correlation was analyzed,including among mandibular angle related angles and these with bigonial breadth.Results The related data of the mandibular angle were measured on the image of three dimensional reconstruction; the mandibular angle was (124.28-±-4.12)°,the mandibular elevated angle was (25.52±2.22)°,the valgus angle of mandibular angle was (9.35±7.85)°,the mandiblular included angle was (77.32 ± 2.34)°,and the tangent angle of mandible was (105.53 ± 1.79)°.The correlation analysis of the related angles of mandibular angle and bigonial breadth showed that there was a significant correlation between the angles.Conclusions Three dimensional reconstruction and measurement of mandibular angle related angles and their correlation analysis can provide theoretical foundation and basis for the preoperative design and effect evaluation of mandibular angle plastic surgery.
9.Influence of hemoglobin level(Hb) on survival and prognosis of elderly hip fracture at admission.

OBJECTIVE:
To analyze the relationship between hemoglobin(Hb) level on admission and survival prognosis of patients with hip fracture.
METHODS:
From February 2016 to October 2018, 249 elderly patients with hip fracture were surgically treated including 62 males and 187 females;the age ranged from 60 to 91(73.67±10.52) years;the time from injury to operation was (6.79±2.27) d. The clinical and laboratory examination results were collected. The Hb level at admission and the mortality at 30, 90, 180 and 360 days after operation were observed. According to the Hb level at admission, the patients were divided into Hb<120 g/L and Hb≥120 g/L groups. The survival conditions of the two groups at 30, 90, 180 and 360 days after operation were compared and analyzed. Logistic regression was used to analyze the effect of Hb level on death 30, 90, 180 and 360 days after operation.
RESULTS:
The mortality rates at 30, 90, 180 and 360 days after operation were 5.22%, 9.24%, 16.87% and 20.48% respectively. The level of Hb at admission was a risk factor for prognosis and death 30, 90, 180 and 360 days after operation(P<0.05). The OR(95% CI) were 2.431(1.475-4.006), 2.625(1.468-4.695), 2.276(1.320-3.925) and 2.082(1.221-3.551) respectively.
CONCLUSION
The level of Hb at admission can affect the survival and prognosis of elderly patients with hip fracture. We should further study how to manage the level of Hb before operation.

10.Repair of excessive width of fold line and conglutination of upper eyelid after double eyelid operation

Objective To investigate the methods of rectifying the excessive width of fold line and conglutination of the upper eyelid following blepharoplasty. Methods The width of double eyelid was redesigned, ranging from 6 mm to 8 mm. Then the scar of upper eyelid was excised about 1 mm to 2 mm, and the orbital septum fat flap, orbicularis oculi flap, infraorbicularis oculi fat flap and injection of lipochondria were utilized to fill the depression of upper eyelid. Results In our series, there were 20 eyelids of 16 cases receiving rectification. Orbital septum fat flap was used to correct 2 eye-lids, orbicularis oculi flap 4 eyelids, infraorbicularis oculi fat flap 10 eyelids and injection of lipochon-dria 4 cases. Among 12 eases of following-up, 10 of them were satisfied with the postoperative effects. The results were acceptable in other 2 cases. Conclusion It is a good approach to correct the excessive width of fold line and conglutination of the upper eyelid with local flaps, including the orbital septum fat flap, orbicularis oculi flap, infraorbicularis oculi fat flap and injection of lipochondria.
