1.Malignant lymphoma of head and neck:an analysis of 139 cases

Objective:To study the clinical characteristics of malignant lymphomas of the head and neck.Methods:The clinical data of 139 patients with malignant lymphomas of the head and neck were retrogradely analyzed,and the long term follow up was carried out.Results:Among all the cases there were 128 non Hodgkin's lymphoma(NHL) and 11 Hodgkin's disease(HD). The average ages of patients with HD and NHL were 41.8 and 50.0 years old respectively.There were lymphadenopathy in all patients with HD and 72.7% of those with NHL. Extranodal involvement occurred in 27.3% HD patients and 75.8% NHL patients; general symptoms appeared in 45.5% HD patients and 37.5% NHL patients. Most patients received chemotherapy of CHOP and improved CHOP scheme.The average follow up time was 23.4 months. Twelve patients died and the 3 year survival rate was 55.6%.Conclusion:In our group, the patients present an older age and NHL is more common than HD.The cases with cervical lymphadenopathy are easy to be diagnosed,and the clinical stage classification is useful for the prognosis.
2.Simultaneous determination of puerarin and tanshinone Ⅱ A in Xinkeshu Capsule by HPLC

AIM: To determine puerarin and tanshinone ⅡA in Xinkeshu Capsule(Radix Puerariae Lobatae,Radix et Rhizoma Salviae Miltiorrhizae) by HPLC simultaneously. METHODS: HPLC C_(18) column was used with methanol-water(85∶15) as the mobile phase. The flow rate was 1.0 mL/min,and the wave length was at 250 nm. RESULTS: There were good liner relationships for puerarin and tanshinone ⅡA with the sample input quantity at the range of 21-63 ?g(r=0.999 8) and 20.02-60.06 ?g(r=0.999 7),respectively.The average recoveries of puerarin and tanshinone ⅡA were 97.7% and 97.0%,respectively. CONCLUSION: This method is simple,accurate,effective and suitable for the quality control of Xinkeshu Capsules.
4.Effect of dexmedetomidine on median effective target effect-site concentration of sufentanil inhibiting body movement evoked by skin incision in patients undergoing bilateral subtotal thyroidectomy

Objective To investigate the effect of dexmedetomidine on median effective target effect-site concentration ( EC50) of sufentanil inhibiting body movement evoked by skin incision in patients undergoing bilateral subtotal thyroidectomy. Methods Thirty-nine ASA I or II patients of both sexes aged 20-64 yr with a body mass index of 20-25 kg/m2 undergoing bilateral subtotal thyroidectomy were randomly divided into 2 groups: control group (group C) and dexmedetomidine group (group D). The patients were premedicated with intramuscular phenobarbital 0.1 g and scopolamine 0.3 mg. In group D dexmedetomidine 0.6 μg/kg was injected iv over 10 min at S min before induction of anesthesia. Anesthesia was induced with target-controlled infusion (TCI) of propofol and sufentanil. The target plasma concentration of propofol was set at 3.0 μg/ml which was maintained until the end of operation. TCI of sufentanil was started at 10 min after initiation of propofol TCI. The initial target effect-site concentration was set at 0.20 ng/ml and decreased/increased by 20% in the next patient according to whether the patient's body moved or not within 1 min after skin incision. Laryngeal mask airway was inserted at 3 min after initiation of sufentanil TCI. Spontaneous breathing was maintained. Skin incision was made at 10 min after initiation of sufentanil TCI. The EC50 and 95% confidence interval (CI) of sufentanil inhibiting skin incision-evoked body movement were calculated with sequential method. Results EC50, of sufentanil was 0.1148 ng/ml (95% CI 0.1055-0.1249 ng/ml) in group D and 0.1454 ng/ml (95% CI 0.1339-0.1580 ng/ml) in group C, and was significantly lower in group D than in group C. Conclusion Dexmedetomidine 0.6 μg/kg infused iv before operation can reduce the EC50 of sufentanil inhibiting body movement evoked by skin incision in patients undergoing bilateral subtotal thyroidectomy.
5.Effects of different concentrations of rhizoma drynariae decoction on proliferation and differentiation as well as ultrastructural change of human dental pulp cells cultured in vitro

BACKGROUND: It is difficult to culture human dental pulp cells in vitro. Studies regarding effects of growth factors on proliferation and differentiation of dental pulp cells cultured in vitro have been reported. However, little is known about the Chinese herb rhizoma drynariae decoction on dental pulp cells cultured in vitro.OBJECTIVE: To observe the effects of different concentrations of rhizoma drynariae decoction on the proliferation and differentiation of human dental pulp cells cultured in vitro.DESIGN, TIME AND SETTING: A controlled observation was performed at the Scientific Resaarch Center, Fourth Hospital, Hebei Medical University between March 2006 and May 2007.MATERIALS: Human dental pulp cells were sourced from the patients who acquired orthotherapy through pulling out impacted wisdom tooth at the Department of Stomatology, Fourth Hospital, Hebei Medical University. Written informed content of sample collection was obtained from all patients. Rhizoma drynariae (place of production: Yunnan Province in China) was provided by the Dispensary of Traditional Chinese Medicine, Fourth Hospital, Hebei Medical University.METHODS: Human dental pulp cells were cultured in vitro using method of tissue piece. The effective ingredients of rhizoma drynariae were extracted by alcohol deposition. 1 mL of physic liquor contained 1 g crude drug and diluted into 10, 50, 100, 500, and 1000 mg/L culture medium utilizing fetal bovine serum. Subsequently, the prepared culture medium was used to culture human dental pulp cells in vitro. Cells that were cultured using culture medium without rhizoma drynariae decoction were used as controls.MAIN OUTCOME MEASURES: ①Primary culture and source identification of human dental pulp cells. ②Effects of different concentrations of rhizoma drynariae decoction on proliferation of human dental pulp cells by methyl thiazolyl tetrazolium (MTT) assay. ③ Effects of different concentrations of rhizoma drynariae decoction on fibronectin expression in human dental pulp cells by immunohistochemistry. ④ Effects of rhizoma drynariae decoction on ultrastructure of human dental pulp cells utilizing scanning electron microscope and transmission electron microscope.RESULTS: Primarily cultured human dental pulp cells displayed polygon- and shuttle-shaped appearance. Different concentrations of rhizoma drynariae decoctions, in particular 100 mg/L, exhibited proliferation-promoting effects on proliferation of human dental pulp cells, and could induce dental pulp cell synthesis and secrete fibronectin. Electron microscopy results revealed that following treatment of rhizoma drynariae decoctions, human dental pulp cells were found with abundant ridges on their surface, surround by extracellular matrix, cytoplasm full of abundant rough endoplasmic reticulum and dissociative ribosome, as well as evenly dispersed nuclear euchromatin, and occasionally seen heterochromatin.CONCLUSION: 100 mg/L rhizoma drynadae decoction apparently promotes the proliferation of human dental pulp cells cultured in vitro.
6.Construction of recombinant adenovirus harboring fusion gene NT4p53(C22)Ant and its killing effect on HepG2 tumor cells

ABSTRACT:Objective To construct a recombinant adenovirus vector harboring fusion gene NT4p53(C22)Ant and study its killing effect on HepG2 tumor cells.Methods Using molecular cloning technology,the rAVV-NT4p53(C22)Ant was produced by homologous recombination.Then we collected virus supernatant and measured its titer after it was amplified by PCR.The effect of this fusion gene on HepG2 tumor cells was evaluated by IHC, MTT assay,PI staining and flow cytometry.Results The recombinant adenovirus was successfully constructed. The p53 expression rate in rAAV-NT4p53(C22)Ant group was (44.88±2.45)%.MTT assay showed that rAAV-NT4p53(C22)Ant could strongly suppress the growth of HepG2 tumor cells.Flow cytometry showed that rAAV-NT4p53(C22)Ant could induce obvious apoptosis of HepG2 tumor cells.Conclusion The recombinant adenovirus vector encoding gene NT4p53(C22)Ant has been successfully constructed and expressed in this experiment,and it can inhibit proliferation and induce apoptosis of HepG2 tumor cells.
7.An ingenious method to suture laparoscopic puncture holes

Objective To study the ingenious method to suture laparoscopic puncture holes.Methods A curved needle and a hook-like needle were used in 650 patients during biliary tract surgery.Results The curved needle was used in 200 patients and the hook-like needle was used in 450 patients.All patients had good wound healing except for one patient who developed puncture hole seroma under the xiphoid.The patient was treated with drainage and improved rapidly.No other complications such as puncture hole hemorrhage,infection and puncture hole hernia occurred.Conclusion Suturing using a curved needle and a hook-like needle was easy and effective,and avoided complications.
8.Determination of ferulic acid and gastrodine in Dachuanxiong Hydrochloride for Injection by HPLC

AIM: To set up a method for determining ferulic acid and gastrodine in Dachuanxiong Hydrochloride for Injection(Rhizoma Chuanxiong, Rhizoma Gastrodiae). METHODS: HPLC conditions consisted of ODS column, methanol-water-acetic acid (30 ∶68 ∶2) and water-acetic acid (100 ∶1) as mobile phases, detection wavelengths were at 320 nm and 270 nm. RESULTS: For ferulic acid, the linear range was within 0.031 6 - 0.505 6 ?g and the average recovery was 98.43% with RSD= 1.52% . For gastrodine, the linear range was within 0.442 - 3.536 ?g and the average recovery was 98.15% with RSD= 1.68% . CONCLUSION: The method proves to be simple, precise and reproduciable and is suitable for the use of quantitative control of Dachuanxiong Hydrochloride for Injection.
9.The teaching practice of propagating healthy lifestyle by combining physiology with “health cornerstones”

Teaching healthy lifestyle in combination with physiology is hardly practiced in schools of medicine. Individual lifestyle factor accounts for 60% among the factors that lead to dis-eases. Physiology contains the basic theories for health knowledge. Therefore, during physiology teach-ing , healthy lifestyle was introduced by combining such contents of physiology as digestion and absorption , energy metabolism balance and emotional physiological reaction with four “health corner-stones” such as health diet, regular physical activity, psychological balance, quitting smoking and drinking less alcohol. This teaching mode has widened the teaching ideas, which has not only in-creased the students' interest and significance in physiology learning, but also promoted them to de-velop good healthy lifestyle.
10.Diagnosis and treatment of unexpected gallbladder carcinoma in laparoscopic cholecystectomy

Objective To explore the feasibility of radical cholecystectomy for early gallbladder car cinoma found during or after laparoscopic cholecystectomy.Methods A retrospective study was conducted on patients who received laparoscopic cholecystectomy between January 2007 to August 2013 and were diagnosed to have gallbladder cancer during or after the operation.There were 34 patients.In 29 patients intraoperative frozen section diagnosed gallbladder carcinoma.In 5 patients postoperative histopathological study diagnosed stage Ⅰ or Ⅱ gallbladder carcinoma.Results Surgery was conducted successfully on these 34 patients.In 20 patients with stage Ⅰ,Ⅱ and Ⅲ,the tumor had invaded the serosa,or into the liver with a depth of less than 2 cm,laparoscopic cholecystectomy alone or radical/extended radical cholecystectomy were carried out.In 9 patients,the laparoscopic surgery was converted to open surgery and these patients underwent cholecystectomy with resection of the adjacent liver segments/sections.In 5 patients who were diagnosed to have gallbladder carcinoma after laparoscopic cholecystectomy,they were re-operated with laparoscopic radical cholecystectomy.Conclusions Stage Ⅰ,Ⅱ and Ⅲ gallbladder carcinoma with tumor invasion into serosa,or patients with tumor invasion into the liver with a depth of less than 2 cm should undergo radical or extended radical cholecystectomy.Laparoscopic assisted radical or extended radical cholecystectomy could achieve the same operation as with open surgery but with better short-term results.There were less pain,smaller incisions,better scars and shorter hospitalization stay.
