1.Application of Pharmaceutical Care in the Treatment of Diabetes Mellitus at Home

OBJECTIVE:To explore the method of pharmaceutical care which influence the effect of the treatment for diabetes mellitus at home. METHODS:163 patients with diabetes mellitus in community who received treatment at home were selected, and then randomly divided into control group (58 cases),family visit group (51 cases) and group education group (54 cases);control group were given pharmaceutical intervention with traditional dispensing window;family visit group received door-to-door service or telephone follow-up service;group education group were given collective education and interactive way. The latter two groups were all given regular drug education,and the advisory desk was set up and provided consulting services about medication knowledge. RESULTS:After providing pharmaceutical care,the regular medication rate of 3 groups were 43.1%,64.8% and 78.4%;medication adherence were 51.7%,70.4% and 82.4%;the rates of blood glucose at target level were 29.3%,48.1% and 54.9%;the rates of glycosylated hemoglobin at target level were 13.7%,31.5% and 31.4%,ADR occurance rates were 31.0%, 13.0%,5.9%. These index of family visit group and group education group were all better than those of control group significantly (P<0.05). CONCLUSIONS:Humane pharmaceutical care can improve the quality and effect of the treatment for diabetes mellitus.
2.Impact of silver ions on features of BCG′s adhesion on the silicone tube surface

Objective To explore the impact of silver ions on the adhesion ability of Bacillus Calmette-Gueri vaccine on silicone tube surface. Methods Common and silver plated silicone tubes were cultured in BCG bacterial liquid for 30 days. A gas-bacilli was taken once every 3 days to measure OD value of microbial. The growth curve of BCG vaccine was made. The corresponding silicone tubes in divided different generations was removed and eluted based on the division cycle of BCG vaccine. The samples were inoculated on the solid medium for observing the colony growth amount, and adhesion curve was drawn. The silicone tube colony line was observed under the electron microscope at day 30. Results OD value and bacterial plaque number were less in silver silicone tube group than in common silicone tube group, with statistical significances (P < 0.05). Electron microscopy also showed that BCG adhesion on silver silicone tube group was relatively less than that on common silicone tube group. Conclusion Silver ions can reduce the adhesion of BCG vaccine on silicone tube surface and inhibit the growth of BCG vaccine.
3.Prevention and Treatment of Early Complications of Ureteroscopy and Pneumatic Lithotripsy

Objective To summary our experiences on the clinical management of early complications caused by ureteroscopy and pneumatic lithotripsy. Methods From May 2002 to December 2007,totally 660 patients received ureteroscopy and pneumatic lithotripsy in our hospital,25 of them had surgical complications. Their clinical data were analyzed retrospectively. Results Among the 25 cases,22 patients had ureteral perforation,2 had ureteral disruptions,and 1 showed pelvic hematoma owing to ureteral perforation and injury to the ovary vein. Of the 22 cases of ureteral perforation,13 patients received lithotripsy and placement of a double J stent into the renal pelvis via the perforation site;and 4 patients underwent repair of the ureter. The other 5 of the 22 cases refused open surgery,and showed symptoms of urinary extravasation,loin pain,fever,or hematuria postoperatively;PCN was successfully completed in 3 of them but failed in the other 2,who underwent open surgery afterwards. For the 2 patients with ureteral disruptions,ureteroureteral anastomosis was performed. And in the case of pelvic hematoma,we carried out open surgery to ligate the bleeding vessels. All of the 25 patients were cured and discharged from hospital prosperously. The double J stent was withdrawn in 2 months after the operation,and 1-year follow-up showed no ureteral stenosis or obstruction by intravenous pyelography. ConclusionsPrimary management of ureteral injury by indwelling double J stent or percutaneous nephrostomy results in significantly deceased rate of re-operation. Conversion to open surgery timely can stop the incidence of other severe complications.
4.Meta Analysis for the Relationship Between the Levels of Serum Uric Acid and Prevalence of Coronary Artery Disease

Objective: To evaluate the relationship between the levels of serum uric acid (SUA) and prevalence of coronary artery disease (CAD) by Meta analysis. Methods: We searched the databases of Pub Med, Elsevier and Web of Science for internationally published cohort study for the relationship between SUA levels and CAD prevalence and conducted a general analysisby using Stata software. Results: A total of 11 cohort study including 463,918 subjects were enrolled in this study. For both male and female genders, increase SUA level was the risk factor for CAD occurrence (RR=1.11, 95% CI 1.00-1.24) and (RR=1.24, 95% CI 1.15-1.34). Dose-response Meta-analysis indicated that by 1 mg/dl SUA elevation, the risk of CAD occurrence would increase 4.8% in male and 12.4% in female, the risk in female gender was higher than male. Conclusion: SUA level has been closely related to CAD prevalence.
5.Relationship among A-type behavior,job burnout and psychosomatic health in grassroots officers and soldiers stationed in Xinjiang

Objective To investigate the relationship among A-type behavior,job burnout and psychosomatic health in grassroots officers and soldiers stationed in Xinjiang, which provides the theoretical basis to maintain their psychosomatic health.Methods Evaluation was carried out with A-type behavior questionnaire, military job burnout scale and simple soldiers psychosomatic health scale in 1 939 grassroots officers and soldiers chosen by random cluster sampling method from army stationed in Xinjiang.Results ①The total scores and each factor scores of the scale had significant positive correlation among the physical scale, the job buruout scale and time hurry (TH) ,competitive and hostility (CH) (r=0.170-0.716, P<0.01).②Job burnout entered into the physical and mental health regression equation and could explain 68.6% of the variance(β=0.195, t=32.211, P<0.01);TH, CH entered the regression equation of job burnout and could explain 29.5% of the variance(t=4.925,4.427).③ TH indirectly affected physical and mental health through job burnout, the mediating effect of the amount was 0.27 ×0.83=0.2241;CH indirectly affected physical and mental health by job bumout,the mediating effect of the amount was 0.30×0.83 =0.2490.Conclusion Job burnout has fully mediated effect between TH, CH and physical and mental health.
6.Application of autologous pleura patch in surgery for chronic tuberculous empyema

Objective To explore the efficacy of autologous pleural patch in surgical operation on the patient with chronic tuberculous empyema.Methods Autologous pleura patches were used to repair the pulmonary wound in the surgical operations for 7 patients with chronic tuberculous empyema,who received surgical management in Hebei Provincial Chest Hospital from August 2012 to November 2014.The leak time of the patient and the time of the chest tube were recorded,the pulmonary re-examination was observed by chest X-ray,and the follow-up results were recorded.Results The time of persistent pulmonary air leak of 6 patients was not more than 10 hours after operation,1 patient was not more than 24 hours.Review of X-ray chest showed that good for lung,4-7 d after operation removal of thoracic dosed drainage tube,there was no death and no chest infection in 7 patients and review of CT chest showed no significant residual cavity.After follow-up for 6-24 months 7 patients had satisfactory efficacy,there was no pneumothorax and thoracic infection,no residual cavily.Conclusion Autologous pleura can be used to patch the pulmonary wound in the surgical operation on the patient with chronic tuberculous empyema.
7.InVance bulbourethral sling for the treatment of stress urinary incontinence after radical prostatectomy

Objective To evaluate the efficacy and safety of InVance bulbourethral sling for stress urinary incontinence after radical prostatectomy.Methods Six male patients with stress urinary incontinence after radical prostatectomy were treated with perineal sling (InVance) from March 2005 to March 2011,and the clinical data were analyzed.Results The operation time was 80-120 min and intra-operative blood loss was 20-50 ml.The catheter was removed at the second day after surgery.In a follow-up of 12 to 29 months,4 patients were dry,1 patient was improved,and 1 patient suffered treatment failure.There were 2 patients transient acute urinary retention and 1 patient short-term perineal pain in the main post-operative complication.In six months after the operation,urodynamic examination showed (15.9 ± 7.7) ml/s at Qmax and 0-20 ml/s at postvoid residual.Conclusion InVance bulbourethral sling for the treatment of stress urinary incontinence after radical prostatectomy appears to be safe,simple in the procedure and good effect to mild and middle incontinence.
8.In vitro study on anti-HBV effects and mechanism of hypericin

Objective To evaluate the anti-HBV effect of hypericin from the cellular level and to preliminarily explore its po-tential drug target point.Methods Liver cell line HepG2.2.15 cells secreting HBV particles were selected as the experimental ob-jects.Hypericin served as the HY group,lamivudine was taken as 3TC group and deionized water as the blank control group.The cells were grouped and administrated.The HBV-DNA copy level was measured at72 h after medication by Southern blot and fluo-rescent quantitative PCR;the inhibition rate of HBsAg and HBeAg was detected by using ELISA assay;the pgRNA expression level was tested by using Northern blot and fluorescent quantitative PCR;Western blot and fluorescent quantitative PCR were adopted to detect the expression of regulatory factors including HNF3β,HNF4α,PPARαand RXRα.Results Compared to the blank control group,both hypericin and lamivudine had significant inhibiting effect on HBV DNA and expression level of HBsAg and HBeAg in HepG2.2.15 cells (P <0.05).Hypericin could significantly decrease the pgRNA expression compared with the blank control group (P <0.05),while lamivudine had no obvious change (P <0.05).Moreover,hypericin exhibited significant effects on the expression of HNF3βand regulatory factor HNF4αcompared with the blank control group and 3TC group(P <0.05).Conclusion Hypericin represents a strong anti-HBV effect,moreover could increase the negative regulatory factor HNF3βn expression and decreases the positive factor HNF4αexpression,prompting that its drug target point could be pgRNA.
9.Analysis of malaria situation and discussion of control strategy in Shandong Province,2013

Objective To understand the malaria situation of Shandong Province in 2013,so as to provide the evidence for formulating targeted prevention and control strategy and measures. Methods The data of malaria cases of Shandong Province in 2013 were collected from the Information Management System for Infectious Diseases Report and Information Management Sys-tem for Parasitic Diseases Control and Prevention. The data of epidemiological characteristics of malaria situation and the diagno-sis and treatment of malaria cases were analyzed by Microsoft Excell2007. Results There were 131 malaria cases reported in 2013,all of them were imported cases,and 127 cases(96.95%)were imported from African countries. A total of 116 cases (88.55%)were falciparum malaria cases. Totally 97.71%of the cases were male and the average age of malaria cases was 39 years. A total of 61.83%of the cases were peasants and 65.65%of the cases only received junior high school education. The dis-tribution of malaria cases was concentrated in Tai’an City(32 cases),Yantai City(19 cases)and Weihai City(17 cases),total-ly acounting for 53.13%. There was no significant seasonal variation in the reporting time of the cases. The median time from on-set to seeing doctor was four days and the median time from seeing doctor to being diagnosed was one day. Totally 35.88%of the cases were misdiagnosed when the first visit to a doctor. All of the cases were laboratory confirmed and 100%of them received the standard treatment after diagnosis. Conclusions All of the malaria cases were imported cases in Shandong Province in 2013. To control the imported malaria in Shandong Province,it is necessary to further strengthen the multi-sectoral cooperation, health education,malaria screening and professional training.
10.Therapeutic effect of 138 imported falciparum malaria patients

Objectives To summarize the clinical characteristics of imported falciparum malaria patients and the treatment so as to provide the evidences for improving the diagnosis and treatment of the disease. Methods A total of 138 imported falci?parum malaria patients who received the treatment in Shandong Institute of Parasitic Diseases from January 2007 to February 2013 were adopted as the observation subjects and their clinical data were collected and analyzed. Results All the 138 pa?tients were back from African countries. The main manifestations were fever headache asthenia and hepatosplenomegaly and most of them were with decreased RBC PLT levels and increased LDH levels and 36.96%of them were misdiagnosed as respiratory diseases nephritis hepatitis and so on. Through antimalarial treatment of artemether or artesunate or dihydroartemis?inin and primaquine or dihydroartemisinin and piperaquine and symptomatic treatment the short?term and long?term cure rates were 98.55%and 94.93%respectively with 1 case unrecovered and 1 died. Conclusions Artemisinins are still the most effective antimalarial drugs for falciparum malaria. However some patients recrudesce as the Plasmodium in their body is resis?tant or insensitive to these drugs. We should pay more attention to the antimalarial and symptomatic treatments in the early stage of severe malaria so as to improve the cure rate.
