Objective To investigate the relationship between the expression of VCAM-1 and oncogenesis, tumor angiogenesis and metastasis in breast carcinoma. Methods Using semi-RT-PCR and immunohistochemistry technique, specimens from 63 patients of breast cancer,20 of fibroadenoma were detected for expression of VCAM-1.Microvessel density(MVD) was counted by endothelial cells immunostained using monoclonal antibody CD34.Circulating sVCAM-1 concentrations were measured by enzyme linked immunosorbent assay. Results Of 63 breast cancer tumor tissues VCAM-1 mRNA was detected in 37 cases. The rate of VCAM-1 mRNA in the primary breast cancer tissues were significantly higher than that in the adjacent non-cancerous tissues and benign fibroadenoma tissues (?2=43,P

Objective To analysis the varying degrees of the M protein staining after immunofixation electrophoresis(IFE)and study its applications in clinical diagnosis.Methods 196 cases of clinical serum samples were tested by using IFE,we analyzed the positive electrophoretic bands of M protein and performed statistical analysis by using SPSS17.0.The M proteins were analyzed ret-rospectively.Results 103 patients were diagnosed with monoclonal gammopathy in 196 patients with positive M protein bands,in-cluding 96 cases of multiple myeloma(MM)and 7 cases of other monoclonal gammopathy;93 patients were non-monoclonal gam-mopathy.By analyzing the M band staining in different clinical groups,we found that M bands were mainly with dense and thick staining in monoclonal immunoglobulin group,the dense staining rate of MM was 90.6%,and the difference between MM and the other monoclonal gammopathy was not significant(P >0.05).In contrast,M bands were in light and narrow staining in non-mono-clonal immunoglobulin group,the rate of which was 25.8%,the difference between non-monoclonal immunoglobulin group and monoclonal immunoglobulin group was statistically significant(P <0.01).The proportion of allelic band in MM,other monoclonal gammopathy,non-monoclonal gammopathy were 39.6%,28.6% and 2.2% respectively,the differences were statistically significant (P <0.01).Conclusion The M band,accompanied by allelic band in IFE staining,is helpful in the diagnosis of monoclonal gam-mopathy,especially MM.The appearance of M protein provides early warning of monoclonal gammopathy.

Objective this research mainly analysis in the treatment of patients with ischemic cerebrovascular disease by comparison of clinical therapeutic effect of endovascular treatment and simple drug treatment. Methods 60 cases of patients with ischemic cerebrovascular disease admitted in our hospital from January 2016 to January 2017, and were randomly divided into control group and observation group, 30 cases in each group. The control group were treated by endovascular treatment, the observation group were treated with simple drug treatment, the two groups of patients treated for 6 months, 12 months to improve blood flow, the incidence of cerebrovascular events, mortality, prognosis Quality of life scores as a comparison basis.Results The patients in the observation group were treated for 6 months, 12 months after the blood flow improved superior to the patients in the control group (P<0.05); the patients in the observation group were treated for 6 months, 12 months of cerebrovascular disease incidence rate lower than that of the control group (P<0.05); the observation group after 6 months treatment and mortality of patients in the control group had no significant difference; the patients in the observation group after 12 months of treatment the mortality rate lower than that of the control group (P<0.05). The patients in the observation group survival quality scores were excellent in the control group (P<0.05). Conclusion The study proves that, in the treatment of patients with ischemic cerebrovascular disease in the treatment can effectively improve the treatment effect of endovascular intervention, improve the blood flow status of patients, reduce the events in patients with cerebrovascular disease incidence and mortality.

Objective To investigate and analyze the effect of pantoprazole and omeprazole in the treatment of gastric ulcer.Methods a total of 100 patients with gastric ulcer treated in Shizuishan First People's Hospital from January 2014 to June 2016 were selected and randomly divided into the control group and the experimental group, with 50 patients in each group.The control group was treated with omeprazole, and the experimental group was treated with pantoprazole.After taking one course of treatment, the clinical efficacy, treatment cost, eradication rate of Helicobacter pylori and recurrence rate of gastric ulcer were compared between the 2 groups.Results The total effective rate of the experimental group was 94%, which was significantly higher than that of the control group(72%), and the difference was statistically significant(P<0.05).After the corresponding treatment, the experimental group of 50 cases, Helicobacter pylori eradication rate was 72%, the recurrence rate of gastric ulcer was 12%.In the control group of 50 cases, the eradication rate of Helicobacter pylori was 52%, and the recurrence rate of gastric ulcer was 36%.As a result, the eradication rate of H.pylori in the experimental group was significantly higher than that in the control group.The cost of treatment in the control group was(499.78±74.81)yuan, and the treatment cost of the experimental group was(413.26±56.39)yuan.The treatment cost and gastric ulcer recurrence rate in the control group were significantly higher than those in the experimental group, with statistical difference(P<0.05).Conclusion Pantoprazole in the treatment of gastric ulcer is much better than omeprazole, high treatment efficiency, is the preferred drug for the treatment of gastric ulcer, after the treatment of gastric ulcer recurrence rate is low, the cost of treatment is low and high eradication rate, with the further promotion and application in clinic significance.

Objective To investigate the expression patterns and significance of aquaporin 5 (AQP‐5) and multidrug‐resistance associated genes in human colon cancer with different differentiation degree and their correlation .Methods The expression of aqua‐porin 5 and resistance genes P‐gp ,GST‐π,TopoⅡ ,and TS in human 45 cases colon cancer tissues with different differentiation de‐gree and 36 cases of adjacent mucosa tissues as well as 58 cases of normal colonic epithelium were detected by quantitative RT‐PCR ,Western blot and immunohistochemistry .Results Immunohistochemistry results showed that AQP‐5 distributed mainly in the cell membrane and the cytoplasm .Fluorescence quantitative RT‐PCR and Western blot showed that AQP‐5 expression could not be detected in adjacent mucosa tissues and normal colonic epithelium tissues .The AQP‐5 expression level was higher in colon cancer tissues compared with adjacent mucosa tissues and normal colonic epithelium tissues (P<0 .05) ,and the expression intensity was correlated with the differentiation degree of colon cancer tissues (P<0 .05) .The results of immunohistochemistry indicated that P‐gp distributed mainly in the cell membrane and the cytoplasm ,GST‐πmainly distributed in the nuclei and cytoplasm ,Topo Ⅱ main‐ly distributed in the nucleus ,and TS distributed mainly in the cytoplasm .Fluorescence quantitative RT‐PCR and Western blot re‐sults showed that the expression levels of all resistance genes detected were higher in colon cancer tissues compared with adjacent mucosa tissues and normal colonic epithelium tissues (P<0 .05) .Furthermore ,P‐gp ,GST‐π,and Topo Ⅱ expression were negative‐ly correlated with the differentiation degree of colon cancer tissues ,the more poor differentiation level of tissue ,the higher expres‐sion level of P‐gp ,GST‐π ,Topo Ⅱ .However ,the expression level of TS did not change significantly in different differentiation de‐gree colon cancer tissues (P>0 .05) .Positive correlation was found between the expression of AQP‐5 and P‐gp ,GST‐π,Topo Ⅱ(P<0 .05) .Negative correlation was found between the expression of AQP‐5 and TS (P>0 .05) .Conclusion The AQP‐5 and re‐sistance gene expression were increased in colon cancer tissues .The AQP‐5 expression level was higher in colon cancer compared with adjacent control or normal tissues ,which may promote the transfer and progress of colon cancer .

Objective To investigate the correlation of hepatitis B patients in Fuzhou between resistance patterns in HBV P region and genotype,HBeAg,the hepatitis B process.Methods This was a retrospective study.The serum and clinic data of 1 115 hepatitis B patients were collected from the inpatient and outpatient Center for Liver Diseases in First Affiliated Hospital of Fujian Medical University between October 2011 and January 2015.HBV DNA was extracted and sequenced using the Sanger method to detect HBV genotype and resistance mutations in P region,HBeAg and HBeAb concentration were detected by chemiluminescent assay.The relationship between P region resistance mutations pattern,HBV genotype,serum HBeAg and the hepatitis B process was analyzed.The x2-test was used to compare the resistance rate and positive rate.Results There were significant differences between 14 kinds of resistance loci and the genotype distribution(x2 =30.788,P =0.004),the C/B genotype ratio of three common resistance loci (rtM204V/I,rtL180M,rtA181T/V) were 85/82,49/25 and 27/9,respectively,which in genotype C was higher than genotype B.The resistance ratio of hepatocellular carcinoma,liver cirrhosis,chronic hepatitis B,hepatitis B carriers was 31.4％ (11/35),37.6％ (65/173),27.3％ (146/535) and 21.8％ (43/197),respectively,which showed significant difference between the four clinical diagnosis (x2 =11.858,P =0.008).The highest percentage of resistance was liver cirrhosis,followed by hepatocellular carcinoma and chronic hepatitis B.There was significant difference in the distribution of HBV genotype between HBeAg (+) group and HBeAg (-) group (x2 =11.093,P =0.001),the HBeAg positive rate in genotype C [37.53％ (295/786)] was higher than in genotype B[35.62％ (280/786)].However,the total resistance rate between HBeAg (+) group and HBeAg(-) group was not significantly different[23.7％ (136/573) and 24.6％ (52/211),respectively,x2 =0.07,P =0.791].Conclusions HBV genotype was related to the resistance rates,HBeAg levels and the progress of hepatitis B.The resistance rate and HBeAg positive rate of genotype C were higher than those of genotype B,and clinical outcomes were worse in genotype C.HBV resistance rates and HBeAg levels were related to the progress of hepatitis B,the higher the resistance rates,the worse clinical outcomes.

Objective To review reoperation on blood vessel prosthesis occlusion after arterial bypass graft in lower limbs. Method The treatment effect of 21 patients with reoperation on blood vessel prothesis occlusion after arterial bypass graft in lower limbs was analyzed retrospectively. Results All the cases were followed up 6-36 (12 ±3) months. The limb salvage rate was 71.4%(15/21) ,the amputation rate was 28.6% (6/21). All 9 limbs that underwent revascularization from deep femoral artery reserved. Conclusions Endomembrane hyperplasy, occlusion of the inflow and outflow tracts are the major reasons for the occlusion of blood vessel prosthesis after arterial bypass graft in lower limbs. Appropriate procedures should be based on careful consideration of the occlusion reasons. Profundaplasty is an effective therapy for those who are treated by reoperation on blood vessel prosthesis occlusion in lower limbs.

Objective To summarize experiences in treatment of traumatic aortic rupture. Methods Between July 2001 and December 2008, 17 patients with acute traumatic aortic rupture were treated in our department. One patient died of hemorrhagic shock one hour after admission before opera-tion. Nine patients underwent thoracotomy under general anesthesia with double lumen endotracheal tube and normothermic femoral-femoral partial cardiopulmonary bypass, with bypass time for 35-139 minutes and aortic clamping time for 25-87 minutes. Successful operation was performed in seven patients inclu-ding one treated with simple repair and the other six with partial replacement of thoracic aorta with artifi-cial vascular graft. The other seven patients underwent endovascular repair and received stent grafts at the site of thoracic injury via right lilac-femoral artery under general or local anesthesia. Results One pa-tient free from operation was died of hemorrhagic shock. Of nine patients treated with thoracotomy, two patients died of hemorrhagic shock during operation and the other seven survived, with operation time ran-ging from 100 to 180 minutes. Seven survivors were followed-up for 2-6 years, with no death during fol-low-up period. Seven patients in endovascular repair group recovered, wiht operation time ranging from 50 to 70 minutes. All these seven patients were followed up for 3-14 months, which showed no death. Reex-amined CT in six patients showed no mediastinal hematoma or leakage of contrast medium from the aorta isthmus at 2-5 months after operation. Conclusions Endovascular repair is simple, safe and effective for traumatic aortic rupture. The selection of thoracotomy and endovascular repair is based on following conditions: the combined injuries of patients, the equipments of hospital and the skills of operators.

OBJECTIVE To observe xerostomia and its effect on quality of life of nasopharyngeal carcinoma patients after radiotherapy and discuss related factors that may improve the patients' quality of life. METHODS Seventy-nine nasopharyngeal carcinoma patients were given a questionnaire in clinic regarding xerostomia 1 year following radiotherapy. Its severity and effect on the quality of life were studied. RESULTS Xerostomia was found in all 79 patients. Moderate to severe xerostomia occurred in 74.7 % of the patients. Of the 79 patients, 13.9 % often drank water at night, 74.7 % needed to drink water while eating, 59.5 % felt difficulties speaking and 91.1 % thought that their daily life was effected significantly. CONCLUSION The occurrence rate of xerostomia was very high in nasopharyngeal carcinoma patients after radiotherapy. 74.7 % of the patients suffered from moderate to severe xerostomia and the daily life of the 91.1 % of patients was effected significantly.
