1.Variance of Perinatal Asphyxia in Latest 10 Years and Analysis of Mortality and Risk Factors of Perinatal Asphyxia

Objective To analyze the incidence rate and mortality of perinatal asphyxia and effects of new resuscitation technique on asphyxia as well as the risk factors of asphyxia in latest 10 years.Methods A retrospective evaluation was done for all the newborns who were born in the provincial women and children′s health care hospital from 1995 to 2004.The morbidity,mortality and fatality rate were calculated for each observed year and different seasons.The influence of gender,body weight,gestational age as well as polyembryony and mode of delivery on the asphyxia was analyzed.Results The morbidity of mild birth asphyxia was decreased dramatically and maintained at about 1.5% after using new resuscitation technique,however,there were no obvious effects on the sever asphyxia.In the same time,no big influence on fatality rate of birth asphyxia was observed.The incidence rate was highest in April,but the mortality and fatality of asphyxia was highest in July.The incidence of asphyxia was also related with gender,polyembryony,birth weight,prematurity babies and aids to delivery from voginal.Conclusions The incidence of perinatal asphyxia is related with the gender,polyembryony,birth weight and gestation age as well as seasons.New resuscitation technique can reduce the morbidity of mild birth asphyxia,and no effect on the severe asphyxia as well as fatality rate.

Objective To find out the associated effect of intrauterine infection and interuterine asphyxia to fetal rat′s brain damage,cell apoptosis,and expression of glial fibrillary acidic protein(GFAP).Methods Pregnant rats of gestation 18 days were randomly divided into four groups:1.NS plus sham operation,2.intrauterine infection,3.intrauterine asphyxia,4.intrauterine infection plus intrauterine asphyxia.The fetal rats′ brains were taken out 72 h after different disposal and given HE coloration,immunohistochemistry of TUNEL and GFAP,respectively.Results The level of brain cell edema and tissue disorganization of group intrauterine infection plus intrauterine asphyxia were more serious than those of group intrauterine infection or group intrauterine asphyxia.TUNEL and GFAP had the same results:The number of positive cells in group intrauterine infection plus intrauterine asphyxia more than that in group intrauterine infection,and which in group intrauterine asphyxia more than that in group NS plus sham operation.There was significant difference between the first three groups and the group NS plus sham operation(P=0).There was also significant difference between group intrauterine infection plus intrauterine asphyxia and group intrauterine infection or group intrauterine asphyxia(P=0).Conclusions Both intrauterine infection and intrauterine asphyxia may induce premature rat brain damage,the association of intrauterine infection and intrauterine asphyxia may aggravate the degree of fetal rat brain damage,also increase the number of apoptosis cell and the expression of GFAP.
4.Three-dimensional finite element analysis of various tooth heights of ferrule effect in post crown

Objective To investigate the stress distribution in various tooth heights of ferrule effect in post crown,and to evaluate the biomechanical mechanism in post crown.Methods The three-dimensional finite element entity grid model was constructed,which was a maxillary central incisor restored with post and porcelain fused to metal crown (PFM crown).By imitating the various tooth heights under static loading,the dentin stress was analyzed.Results The stress distribution had an increasing tendency from tooth cervix to root,but it could be decreased by ferrule structures and the decrease degree accorded with the region and height of ferrule.The ferrule effect of the wrapping 1 cm coronal tooth could decrease the stress of the tooth cervix obviously but 2 cm decreased a little.Conclusion The crown wrapping 1 cm coronal can produce ferrule effect obviously and protect the tooth.We can choose to use crown wrapping 2 cm coronal to care the tooth condition.
5.Crossed aphasia:5 case report

ObjectiveTo observe the effect of language rehabilitation on crossed aphasia.MethodsFive right-handed patients with aphasia secondary to acute stroke in the right hemisphere were studied by means of oral fluent of Aphasia Battery of China, the Western Aphasia Battery, grade criterion of the Boston Diagnostic Aphasia and Frenchy Dysarthria Battery. After thirty days language rehabilitation, these cases were studied by the same ways again.ResultsAll five cases were meted with the diagnosis criterion of crossed aphasia, and got a well language function recovery after rehabilitation.ConclusionThere were two types of crossed aphasia:mirror image aphasia and atypical crossed aphasia.Language rehabilitation is effective on crossed aphasia.
7.Effects of huanglian jiedu decoction and its disassembled recipes containing serums on the proliferation and differentiation of preadipocytes.

OBJECTIVETo study the effects and mechanisms of Huanglian Jiedu Decoction (HJD) and its disassembled recipes containing serums on the proliferation and differentiation of preadipocytes.
METHODSHJD and its disassembled recipes containing serums were prepared. The 3T3-L1 preadipocytes were cultured. The proliferation of 3T3-L1 preadipocytes was detected by methyl thiazolyl tetrazolium (MTT) method. The accumulation of lipid droplets in the cytoplasm of differentiated preadipocytes was observed by oil red O staining and quantitatively analyzed by colorimetry. The mRNA expressions of peroxisome proliferation activated receptor y (PPAR gamma) and CAAT/enhancer binding protein (C/EBP alpha) were detected by reverse transcription polymerase chain reaction (RT-PCR).
RESULTSIntervention with serum containing HJD, Phellodendron amurense Rupr plus Gardenia jasminoides Ellis, or Gardenia jasminoides Ellis showed significantly stimulative effects on the proliferation of preadipocytes, as compared with that in the blank control group (P<0.05, P<0.01). The preadipocytes treated with serum containing HJD, Phellodendron amurense Rupr plus Gardenia jasminoides Ellis, Coptis chinensis Franch or Gardenia jasminoides Ellis showed that the lipid droplets in the cytoplasm were significantly lessened, so did the mRNA expressions of PPAR gamma and C/EBP alpha when compared with the blank control group (P<0.05, P<0.01).
CONCLUSIONSHJD promoted the proliferation of preadipocytes, decreased the accumulation of lipid droplets during the differentiation of adipocytes, and inhibited the differentiation of adipocytes, which might be associated with its effects on decreasing the mRNA expressions of PPAR gamma and C/EBP alpha. Phellodendron amurense Rupr and Gardenia jasminoides Ellis were the main components of HJD playing these roles.

8.Neuroprotective Effect of Recombinant Human Erythyropoietin Theraphy for Neonatal Hypoxic-Ischemic Encephalopathy

Objective To evaluate the neuroprotective effect and safety of neonatal hypoxic-ischemic encephalopathy(HIE)treated with recombinant human erythropoietin(rhEPO).Methods Fifty-three neonates with HIE were randomly divided into rhEPO treated group(n=29) with the dosage of 300 U/(kg?time),three times a week for 2 weeks and control group(n=24)without rhEPO.All supportive measures were same between 2 groups.Neurological scoring was evaluated at d3,d5 and d7 Neonatal behavioral neurological assessment(NBNA) was evaluated at d7,d14 and d28.The neurodevelopment quote was evaluated at age of 3 and 6 months.Blood pressure,liver and renal function,blood electrolytes and blood hemoglobin,platelet and reticular red blood cell count were monitored before and after treatment in all infants.Results The neurological scoring between two groups had no difference at d3.The significant difference was found at d7(P0.05).Conclusions Teraphy with rhEPO on neonatal HIE infants can promote neurological recovery,and there is no serious side effect with rhEPO treatment.
9.Experiences and the special medical conditions for a Chinese secondary hospital on the early stage of arterial switch operation

Objective This retrospective study is to analysis the special medical conditions that most Chinese secondary hospitals are facing with,and to review the safeguards and pitfalls for arterial switch operations,in order to probe intothe feasibility of this procedure for Chinese secondary hospitals and provide our experiences to help other surgeons to avoid pitfalls on complex procedures.Methods Between January 2006 and December 2011,totally 21 newborns and infants with TGA/VSD and TGA/IVS underwent arterial switch operation.There were 15 males and 6 females.In the TGA/VSD group,there were 16 cases,ranging from 30 days to 1 year at surgeries,and weight from 3.4-8.5 kg with average of (5.33 ± 1.42) kg.In the TGA/IVS group,there were 5 cases,ranging from 13 days to 1 month at surgeries,and weight from 3.1-5.5 kg with average of (3.75 ± 1.17)kg.All patients underwent one stage of arterial switch operation.Routine follow-up checking points are set at discharging,3 months,half year and every year after operation.Results The early death rate is 9.5% (2/21),and the reexploration rate is 9.5% (2/21).In the TGA/VSD group,average cardiopulmonary bypass time is (151 ± 33) minuntes with the aortic crossclamp time is (1 19 ± 26) minutes.Ventilator support time is 24-159 hours,and the length of ICU stay is 3-17 days.1 case has residual VSD with the diameter less than 2 mm.The pulmonary flow velocity in 2 cases increase mildly to 2.0 m/s and 2.2 m/s,and another 2 cases increase severely to 3.1 m/s and 3.7 m/s.The aortic flow velocity in 3 cases increase to 2.0m/s.ECG instructs no case has myoinfarction signs.In the TGA/IVS group,average cardiopulmonary bypass time is (170 ± 52) minuntes with the aortic crossclamp time is (137 ± 48) minutes.Ventilator support time is 51-144 hours,and the length of ICU stay is 4-14 days;The pulmonary flow velocity in 2 cases increase mildly to 2.0 m/s.The aortic flow velocity in 1 cases increase to 2.0 m/s.ECG instructs no case has myoinfarction signs.Conclusion Unbalanced medical resources distribution causes significant differences between heart centers and the secondary hospitals in China,especially on the complex congenital heart diseases procedures.However,with relatively solid background on correction of the simple congenital heart diseases,the Chinese secondary hospitals can still perform arterial switch operation with satisfactory mortality and morbidity,and provide more prompt medical services for more population.
10.Effect of Subhypothermia and Erythropoietin Combined Treatment on Neonatal Hypoxic-Ischemic Brain Damage

Objective To explore the effect of systemic subhypothermia and erythropoietin combined treatment on neonatal hypoxic-ischemic brain damage(HIBD).Methods Sixty-five asphyxiated newborns enrolled in this study were divided randomly into combination group(n=16),hypothermia group(n=24)and conventional group(n=25).Subhypothermia was adepted in hypotheria group as well as conventional therapy.Subhypotheria and erythropoietin were both applied in combination group excepy for conventional therapy.The short-term effect was evaluated by neonatal neurological score as well as neonatal behavioral neurological assessment(NBNA)on postnatal days of 7,14 and 28,and the long-term effect was evaluated by using intellectual development table made by Children's Development Center of China(CDCC)at 3 and 6 month of age.Results The neonatal neurological score in combination group at 24,48,72 and 80 h were lower than that of 0 and 12 h.NBNA evaluation was much higher on 14 d and 28 d in combination group and hypothermia group than that in conventional group(Pa0.05).Conclusions Combination therapy with systemic subhypothermia and erythropoietin exerts obvious short and longer-time neuroprotective effect on HIBD,but there are no differences compared with hypothermia alone.
