Objective To improve the capability of diagnosing and treating renal chromophobe cell carcinoma (RCCC) and the recognition of this subtype of renal cell carcinoma (RCC). Methods The clinical data of 15 cases of RCCC (10 men and 5 women) were analyzed.Their age ranged from 47 to 74 years,with a mean of 57 years. Radical nephrectomy was performed in all these cases. Results Postoperative pathological findings confirmed the diagnosis of RCCC.The pathologic TNM stage of RCCC was as follows:pT 1N 0M 0 in 6 cases,pT 2N 0M 0 in 5,pT 3bN 0M 0 in 2,pT 1N 2M 0 in 1 and pT 2N 2M 0 in 1.The pathologic grade of RCCC was G 2 in 10 cases and G 3 in 5.Eleven cases were followed up.During the follow-up of 2 to 31 months (mean, 19 months),1 died of heart attack,1 had local recurrence and 9 were surviving tumor free. Conclusions RCCC is a morphologically distinctive uncommon subtype of RCC.Radical nephrectomy is the first choice for the treatment of RCCC.Compared with other types of RCC at the same stage and of the same grade,RCCC has a better prognosis.

Objective To summarize our experience in the diagnosis and management of popliteal artery entrapment syndrome (PAES). Methods This is a retrospective study on 10 patients (13 limbs)who were admitted for symptoms of claudication and the diagnosis of popliteal entrapment was established either with angiography,computed tomographic angiography,magnetic resonance angiogram or during the operation in recent 7 years (2002-2009).All patients were treated surgically. Results The mean age at the time of presentation was (25 ±7) years old (range,17-41 years).Claudication was the most frequent presenting symptom (12 limbs).The surgical procedures consisted of simple musculotendinous dissociation in 1 limb,thrombectomy with balloon angioplasty in 1 limb,musculotendinous dissociation plus thromboendarterectomy with autogenous saphenous vein (ASV)patch angioplasty in 2 limbs,ASV graft interposition or bypass in 6 limbs and graft interposition or bypass in 3 limbs.At a median follow-up of (35 ±27) months (range,2 months-7 years),there were no intraoperative or long-term postoperative complications and all the patients were cured. Conclusions PAES is an unusual but important cause of peripheral vascular insufficiency especially in young patients.A combined approach is necessary for diagnosis.Popliteal artery release alone or with vein bypass or reconstruction is the treatment of choice.

Objective To investigate the expression of phosphatidylinositol 3-kinase(PI-3K)in adipose tissue of polycystic ovary syndrome patients(PCOS),and explore molecular mechanisms of insulin resistance(IR)in PCOS.Methods Samples from patients with PCOS with IR(n=19),PCOS without IR (n=10)and controls(n=15)were collected.Serum fasting insulin(FIN)and fasting plasma glucose (FPG)were measure& Insulin resistance index was calculated using homeostasis model assessment (HOMA)to analyze the relationship between these markers and IR.Western blot technique was used to detect the PI-3K p85 subunit.Gene expression of PI-3K p85 subunit was detected by reverse transcription polymerase chain reaction(RT-PCR)method.Kinase activity was detected by immunoprecipitation,thin- layer chromatography and gamma scintillation counting.Results(1)The levels of FIN[(25.2?3.8) mU/L]and HOMA-IR(1.6?0.3)in PCOS with IR were significantly higher than those in PCOS without IR[(13.4 +3.8)mU/L,0.9?0.3]and controls[(9.5 +2.6)mU/L,0.5?0.3;all P＜0.05).(2) There was no significant difference in the protein(0.65?0.10)and gene expression(0.92?0.12)of PI-3 K p85 subunit in PCOS with IR compared with PCOS without IR(0.72?0.10,1.01?0.10)and control groups(0.73?0.14,1.00?0.12;P＞0.05).(3)PI-3K activity in PCOS with IR(81%)and PCOS without IR(89%)was significantly decreased(P＜0.01,P＜0.05)and negatively correlated with HOMA- IR(r=-0.69,P＜0.01;r=-0.62,P＜0.05).Conclusions No significant difference in the protein and gene expression of PI-3K p85 subunit in PCOS with IR is found.The decreased PI-3K activity may lead to IR of PCOS.
