OBJECTIVETo analyze the causes of delayed union or nonunion of the ulna after intramedullary nailing in pediatric forearm fractures.

METHODSFrom February 2005 to February 2010,5 patients with forearm fractures who were treated with titanium elastic nailing (TEN) were identified to fulfill the criteria of having developed a delayed union or nonunion of the ulna. The causes of delayed union or nonunion were investigated according to mechanism of injury, fracture location, treatments methods and postoperative management. All patients were male and the age was 3 to 14 years old with an average of 9.4 years. All fractures were located on the mid-third part of forearm. Two cases had a re-fracture. Among them, 3 cases caused by high-energy injury and 2 cases by falling down. Open reduction were performed in 4 cases while the other one was treated with closed reduction. Four patients were immobilized in an above-elbow cast, postoperatively.

RESULTSAll patients were followed up from 7 to 19 months with an average of 11.4 months. There were 4 delayed union and 1 nonunion. Three patients healed after the removal of the nail and avoidance of weight-bearing. Two patients healed by replacing another fixation. No patients had soft-tissue irritation or nail-entry-site infections.. The clinical effect was evaluated according to Daruwalla and Price scores with 3 excellent and 2 good of the results.

CONCLUSIONSUsing titanium elastic nailing for the treatment of pediatric both-bone forearm fractures is a good method. However,strict indication selection should be followed to avoid delayed union or nonunion.

Objective To investigate the hemodynamics of increasing portal venous pressure(PVP) in hepatocellular carcinoma patients complicated with hepatic arterioportal vein fistulas (HAPVF)and the diagnosis and therapy of intractable upper gastrointestinal hemorrhage caused by HAPVF.Methods One hundred and fifteen cases of hepatocellular carcinoma with upper gastrointestinal hemorrhage were checked by hepatic arteriography and were treated through orifices embolization in cases with severe HAPCF by coils and/or ethanol. Results Twenty-six out of 31 patients suffering intractable upper gastrointestinal hemorrhage have severe HAPVF(the main stem of portal veins are visible).However,there are only 15 patients with light HAPVF among the 84 patients who have mild upper gastrointestinal hemorrhage (the main stem of portal veins are invisible).After the embolization,all of the 26 patients who have severe HAPVF stopped bleeding.Among them,the main stem of hepatic arteries are occluded in 2 patients. Conclusion The existence of severe HAPVF should be taken into consideration when intractable upper gastrointestinal hemorrhage occurs in hepatocellular carcinoma patients,and it can be diagnosed through hepatic artery DSA.Orifice embolization is the most effective method for such kind of hemorrhage.

Objective To determine the extent and distribution of error in knee joint proprioception and e- valuate the relationship between knee joint proprioception and pain and dysfunction.Methods Twenty-eight knee OA patients (19 female and 9 male) were compared with 27 age-matched healthy adults (20 female and 7 male). Objective To investigate the transport characteristics of levofloxacin.Method A model was established in vitro by gastric muco-sa combined with Ussing chamber for studying the gastric mucosa trans-port mechanisms of levofloxacin.The effects of concentration of donor so-lutions, direction, pH and P -glycoprotein ( P -gp ) inhibitor on the transport of levoflo-xacin were investigated.The concentrations of levo-floxacin were determined by high performance liquid chromatography ( HPLC) and the transport parameters such as apparent permeability co-efficient ( Papp) was calculated.Results The transport of levofloxacin obeyed a passive diffusion in a pH-dependent manner, and significant differences between the Papp of two directions.The P-gp inhibitor vera-pamil enhanced the transport amount of levofloxacin.Conclusion The reasults indicated that carrier-mediated transport manner might play a major role in the transport of levofloxacin in this model in vitro and P-gp probably participated in the transport of levofloxacin.

Objective To evaluate the effect of P-glycoprotein (P-gp) and organic cation transporter ( OCTs) on the transport of levofloxacin by using a model in vitro.Methods A model in vitro was established by rat gastric mucosa combined with Ussing chamber.The concentrations of levofloxacin collected from the transport assay were determined by HPLC and the transport parameters such as apparent permeability coefficient [Papp(M-S) and Papp(S-M)] and the ratio of Papp(S -M) versus Papp(M -S) were calculated and compared when the levofloxacin was used solely and co -used with P -gp inhibitor cyclosporin A or OCTs inhibitor cimetidine.Results When cyclosporin A was added , the Papp(S-M)/Papp(M-S) of levofloxacin decreased from 2.20 to 1.06 ( P<0.05).There was no significant difference compared with the blank group while cimetidine was added.Conclusion The reasults indicated that P-glycoprotein might be involved in the transport of levofloxacin in this model in vitro, while the organic cation transporter probably not participated in.

Objective To investigate the clinical effect of arthroscopic anterior cruciate ligament(ACL) reconstruction using autologous multi-strands of hamstring tendon.Method From May 2002 to Dec.2006,arthroscopic reconstruction of the anterior cruciate ligament using multi-strands of hamstring tendon was performed on 39 patients.Among them,28 cases were followed up for 7 to 48 months(average 13.5±2.3 months).Result The median Lysholm knee score was improved from 47.3±3.6 to 91.3±2.9 after operation.Conclusion Arthruscopic reconstruction of the anterior cruciate ligament using muhi-strands of hamstring tendons is a good alternative method with minimal invasion and convenient tendon harvesting and fewer complication and reliable clinical effect.

ObjectiveTo explore the clinical characteristics of pathological fracture in extremities caused by bone tumors or tumor-like lesions. MethodsFrom August 2002 to December 2010, 139 patients with pathological fractures were entered in the study, including 79 males and 60 females with an average age of 31.1 years. Fractures included tumor-like lesion in 55 cases, benign tumor in 13, giant cell tumor (GCT)in 7, primary malignant tumors in 28, and metastatic tumors in 36. Forces induced to fractures were classified into four grades: spontaneous fracture, functional activity, minor injury, severe injury. Age, fracture location, histological results, fractures forces, prodromes, and misdiagnosis were all observed. Chi-square test were use to compare forces and prodromes within different tumors. ResultsThe highest morbidity rate is 32.4％(45/139) which lies in 11-20 years old. The cites of fractures including femurs in 71 cases, humerus in 36, tibia in 15, fingers in 7, radiuses in 4, fibula in 3, ulnas in 2, and metatarsus in 1. Fracture forces include spontaneous fractures in 29 cases, functional activity in 42, minor injuries in 65, and traumatic injuries in 3. Sixty-seven patients(48.2％) had local prodromes. The prodromes of both malignant tumors and metastatic tumors were more than benign tumors. Twenty cases experienced misdiagnosis with average delay time of 12 weeks. ConclusionMinor injury forces and local prodromes are clinical key features of pathological fractures. Both of them are key points of avoiding misdiagnosis.
