2.Comparison of Space Infections in Oral Maxillofacial Region between Patients with and without Diabetes

Objective To compare the clinical features of space infections in the oral maxillofacial region between patients with diabetes mellitus and patients without diabetes.Methods A retrospective study was conducted on 61 patients diagnosed with space infections in the oral maxillofacial region and receiving treatment at the department of oral and The 4nd Affiliated Hospital of Kunming Medical University between 2013 and 2016.The patients were divided into 2 groups:diabetic group (n =28) and non-diabetic group (n =33).The study variables were demography,etiology,clinical and laboratory examinations (bacteriology,admission blood glucose level,admission white blood cell count and percent of neutrophil on admission).The outcome was compared.The length of hospital stay and complications were also compared.Statistical analyses of the results between groups were performed using the t test and chi square test.Results For oral and maxillofacial space infection,there was no statistically significant difference in gender between the diabetic group and (28 cases) and non diabetes group (33 cases) (x 2 test,P=0.5929);while there were statistically significant differences between two groups in age (t test,P<0.001) and in the number of infection space (t test,P<0.001);there was no significant difference between the two groups in the sources of infection (x 2,P>0.05);The main source of infection was odontogenic,followed by adenogenous.There were statistically significant differences between two groups in blood glucose concentration at admission (t test,P<0.001) and in the hospitalization time and complications (t test and x 2 test P<0.05),while no statistically significant differences in the white blood cells count and neutrophil count (t test,P>0.05).Compared with the non diabetic group,for patients in the diabetic group,the age of onset was older,the involvd gaps were more,the blood glucose concentration was higher on admission,the length of stay was longer,complications were more likely to occur,and the consequences were more serious.Conclusion Compared with non diabetes group,space infection in oral and maxillofacial region is more serious and the hospital stay is longer and has more complications in diabetic patients which should be paid higher attention to.
3.Arthroscopic reduction and fixation of avulsion fracture of the posterior cruciate ligament from the tibia through an additional posterolateral portal

Objective To investigate surgical techniques and results of arthroscopic reduction and fixation for the treatment of avulsion fracture of the posterior cruciate ligament from the tibia through an additional posterolateral portal. Methods An additional posterolateral portal was established by using the inside-out technique under arthroscopic view.After a protective sleeve was placed through the portal and the displaced fracture was reduced,the fragment was fixed by the guide wire drilled through the sleeve temporarily.If the fluoroscopic control showed an anatomic reduction of the fragment and good placement of the wire,the self-attacked cannulated screw and washer was placed intra-articularly over the guide wire through the sleeve for the directive internal fixation of the avulsion fracture.Aggressive rehabilitation programs were recommended postoperatively.Results Except 1 case with 11 mm displacement and rotation of the fragment treated by the arthrotomy after the failure of arthroscopic reduction,the arthroscopic operation was accomplished in all the remaining 10 cases.The operative time was 63~98 min(mean,87.3 min).No injuries of popliteal vessels or nerves occurred.The postoperative X-ray firms showed an anatomic reduction of the fragment and good placement of the screw and washer.With the negative posterior drawer test confirmed by the physical examination,normal range of motion of the injured knee joint and gait were achieved in the 10 patients at 4~7 weeks after surgery.The bone union was confirmed by X-ray films at the 3 months postoperatively.The computer KT-2000 arthrometer measurements of posterior tibial displacement of both knee joints showed side-to-side difference not more than 1.2 mm in 6 patients at 5 months after surgery.Conclusions The additional posterolateral portal can be established by the arthroscopic inside-out technique safely.The displaced fragment of the avulsion fracture of the posterior cruciate ligament from the tibia can be reduced and fixed with the self-attacked cannulated screw and washer arthroscopicly.
4.Application of aortic root internal drainage in the operation of type A aortic dissection

Objective To review the experience of aortic root internal drainage in the operation of type A aortic dissection.Methods This was a prospective and observational study.Clinical data were available from 20 cases of type A aortic dissection patients (A group) from March 2003 to March 2008,and anothcr 36 cases of type A aortic dissection patients (B group) from March 2008 to May 2013.All of patients received the operation of replacement of ascending aortic aneurysm and aortic arch replacement and descending aortic stented elephant trunk implantation in our hospital.The additional inside drainage were made between the aortic root aneurysm sac and right atrial appendage in B group.Perioperative and postoperative data including the operation time,the amount of bleeding,cardiopulmonary bypass time,the amount of red cell transfusion,number of postoperative cases re-operated for hemostasis,total drainage volume in 24 hours,amount of postoperative red blood cell transfusion,number of cases of postoperative pulmonary complications,and postoperative mortality rate were analyzed retrospectively.The data was analyzed using SPSS version10.0 software.The chi-square test was used for constituent ratios,whilest was applied to analysis of differences in above variables betweens two groups.Results There were significant differences in the amount of bleeding during surgery,cardiopulmonary bypass time,the amount of red cell transfusion,rate of postoperative re-operation,total drainage volume in 24 hours,amount of postoperative red blood cell transfusion,number of cases of postoperative pulmonary complications and postoperative mortality between the two groups (x2/t =2.658,2.381,2.265,3.056,6.862,2.896,2.316,7.215,7.668,P =0.012,0.034,0.007,0.016,0.013,0.032,0.008,0.008,respectively).Conclusions The method of aortic root internal drainage in the operation of type A aortic dissection could reduce the amount of bleeding and improve the success rate of operation.
5.Prolongation of murine cardiac allograft survival by dendritic cells treated with NF-?B decoy

Objective To investigate the function of donor-derived dendritic cells (DCs) treated with NF-?B decoy in prolonging murine cardiac allograft survival time.Methods Donor bone marrow- derived DCs were treated with NF-?B decoy in vitro.Heterotopic abdominal heart transplantation was performed from BALB/c to C57BL/6 mice.Recipients were grouped according to different pretreat ments as follows:(1) Control group,infusion of PBS (0.2 ml) alone intravenously via the portal vein 7 days before heart transplantation;(2) CsA group,treated with sub-therapeutic CsA only for 7 days (10 mg?kg~(-1)?day~(-1)) through intraperitoneal injection after transplantation,and the same as control group before transplantation;(3) Control DCs group,infusion of only cultured 5th-day recipient DCs untreated with NF-?B ODN decoy;(4) Treated DCs group,infusion of recipient DCs pretreated with NF-~cB ODN decoy;(5) Combined treatment group,infusion of recipient DCs treated with NF-~cB ODN decoy before transplantation and intraperitoneal injection of sub-therapeutic CsA for 7 days (10 mg?kg~(-1)?day~(-1)) after transplantation;(6) Third party donor group,C3H/HeJ mice used as donor, and recipient (C57BL/6) was treated the same as combined treatment group.Every group had 8 mice and graft survival time was observed.Cytokines (IL-2,INF-?,IL-4 and IL-10) in recipient serums were analyzed by ELISA at 7th day after transplantation.Results The graft mean survival time (MST) in control group,CsA group,Control DCs group,treated DCs group,combined treatment group and third party donor group was 7 days,10.3 days,7.6 days,21.4 days,53.6 days and 9 days,respectively.There was significant difference in MST between treated DCs group and control group or control DCs group (P

BACKGROUND: MyoD expression of skeletal muscle of rats increases distinctly in the earlier period of denervation, which can greatly postpone denervated skeletal muscle atrophy. The clinical test testifies that electrical stimulation is an effective method to cure denervated muscle atrophy. But the influence of electrical stimulation on MyoD expression during denervated muscle atrophy is still unproved. OBJECTIVE: To discuss the influences of electrical stimulation on the gene expression of MyoD of skeletal muscle of rats. DESIGN, TIME AND SETTING: Randomized control animal experiment was performed in the Animal Experimental Center of Shanxi Medical University between July and November in 2008. MATERIALS: A total of 36 healthy Sprague-Duwley rats of either gender were divided randomly into three groups, control group, denervation group and electrical stimulation group. Each group contained 12 rats. METHODS: Standard models of right sciatic nerve dissociation and gastrocnemius denervation were established in right limb of each rat in denervation group and electrical stimulation group. Thirty-minute electrical stimulation was given to the denervated gastrocnemius muscle of each rat of electrical stimulation group once a day. The rats were executed at 2, 7, 14, 28 days after denervation to dissect and dissociate their gastrocnemius muscles of the right lower limbs. MAIN OUTCOME MEASURES: MyoD mRNA expression was detected by reverse transcription polymerase chain reaction, and MyoD protein level by immunohistochemistry. BACKGROUND: Myogenic regulatory factors Myf-5 is an importance gene involved in muscle occurrence process to adjust and control, start and maintain the skeleton muscle cell growth. It has a possible relation with denervated skeletal muscle atrophy. OBJECTIVE: To detect the expression of Myf-5 gene in different skeletal muscle sitas and different periods following denervatad skeletal muscle atrophy.DESIGN, TIME AND SE'I-rlNG: Randomized control animal experiment was performed in the Shanxi Medical University between March and April in 2008. MATERIALS: A total of 24 Sprague-Dudley male rats was selected and divided randomly into four groups, with six in each, Le. sham operated group (innervations), denervated 2 d group, denarvated 7 d group and denervatad 28 d group. METHODS: Sham operations were made only for the rats in the sham operated group and sciatic nerve were not cut off. Sciatic nerves were cut off more than one centimeter at the mid-level of their right lower limb for the rats in denervated groups. The rats were executed by vertebrae dislocation method at 2, 7, 28 days after denervation. The skeletal muscles of the right lower limb (tibialis anterior, soleus, gastrocnemius and plantar) were dissected and dissociated. MAIN OUTCOME MEASURES: Myf-5 mRNA expression was detected by reverse transcription-polymerase chain reaction. Immunohistochemical stain (ABC method) with polyclonal antibody against Myf-5 protein was performed and the gray value was counted.RESULTS: Expressions of Myf-5 mRNA in denervatad skeletal muscle were up-regulated at 2, 7, 28 days in the eady stage of denervation (P < 0.05). The number of Myf-5 antibody positive-stained cell nuclei was the most in satellite cells at 28 days following denervation. CONCLUSION: At the early stage of denervated skeletal muscle atrophy in SD rats, expressions of Myf-5 in different skeletal muscles are all up-regulated. Denervation of rat skeletal muscle raises the Myf-5 expression in satellites cells.
8.A case report of primary hepatocellular carcinoma with gallbladder cancer

ABSTRACT:Objective To a rare case of double primary cancer (hepatocellular carcinoma with gallbladder)to guide clinical application.Methods and Results We reported one case of primary hepatocellular carcinoma and gallbladder.A male patient,54 years old,had main complaints of intermittent right upper quadrant pain for 4 days. The abdominal CT of the local county hospital showed gallstones,gallbladder with liver infiltration.And then he went to the First Affiliated Hospital of Xi’an Jiaotong University for further treatment.Laboratory examination revealed:HBsAg(+),HBcAb(+),alpha-fetoprotein (AFP)> 60 500 ng/mL,carcinoembryonic antigen (CEA) 5.25 ng/mL.Abdominal CT showed hilar slightly stronger light echo groups:liver cancer or gallbladder cancer? Hepatic artery and portal vein CT imaging (CTA+CTV)examination showed the malignant tumor shadow in the inside of the left hepatic lobe huge,uneven thickening of the gallbladder wall,suspected liver disease with gallbladder infringement or gallbladder disease with liver infringement. With the preoperative preliminary consideration of primary liver cancer with infiltration of the gallbladder,we chose the operation as the resection of segment Ⅳ b and Ⅴ of the liver,cholecystectomy and T tube drainage.Pathological examination postoperative showed the bulky liver carcinoma grade Ⅲ and the poorly differentiated adenocarcinoma of gallbladder.A month later,abdominal CT showed the tumor spread intrahepatic,prompting the poor prognosis.Conclusion The two which are not continuous,which is the standard of double primary cancer,are not suitable for all double primary cancers.This case provides useful experience for future similar diagnosis and treatment of disease,and also helps us with timely and accurate identification of “metastatic”or “primary”,which is the key point for clinicians to give patients an effective treatment.
9.Influence of the timing of microsurgical clipping on the prognosis of patients with poor-grade aneurysmal subarachnoid hemorrhage

Objective To investigate the influence of different timing of surgery on patients with poor-grade intracranial aneurysm. Methods The clinical data of 86 patients with aneurysmal subarachnoid hemorrhage (Hunt-Hess gradeⅣ toⅤ)were analyzed retrospectively. The patients were divided into an ultra-early treatment group (the operative time within 24 h after onset;n=40),an early treatment group, (24 to 72 h after onset;n=27),and middle and late treatment group (>72 h after onset;n=19)according to the different timing of surgery. The prognosis of patients at 6 months after procedure was evaluated with the modified Rankin Scale (mRS)scores. The differences of the rate of good prognosis (mRS 0 to 2)and mortality in patients of the 3 groups were compared. Results (1 )The ratios of good prognosis in the ultra-early treatment group,early treatment group,and middle and late treatment group were 55. 0%(n=22),33. 3%(n=9),and 21. 1%(n=4),respectively. There were significant differences (P<0. 05). The mortality rate of the ultra-early treatment group (7. 5%)was lower than that of the early treatment group (25. 9%) or middle and late treatment group (42. 1%). There were significant differences (P<0. 05). (2)There were significant differences in the rate of good prognosis in different timing of surgery in patients with gradeⅣ(P<0. 05);there was no significant difference in the rate of good prognosis in patients with grade Ⅴ(P>0. 05). The mortality ratios in patients of the 3 groups were 2/18,4/11,and 6/11,respectively. There were significant differences (P<0. 05). Conclusion Ultra-early operation may benefit part of the patients with poor-grade aneurysm,particularly in patients with gradeⅣ. As for the patients with grade Ⅴ, ultra-early operation may help to reduce short-term mortality rate,however,the rate of vegetative state is high.
