1.REASONS AND PREVENTIONS OF THE MEDICAL DISPUTES IN PRIMARY HOSPITAL

The medical disputes in the primary hospitals increase year by year , and the practical experience at the grass-roots level during 1999 and 2013 showed the medical disputes had various reasons .For primary hospi-tals, the reasons included incomplete reform and management , unsuitable technical level of medical staff , inappropri-ate physician -patient communication and medical service , disunity within the hospital , lack of self -protection awareness;For patients, the reasons included the high expectations of medical service , non-compliance with medi-cal workers, poor economic conditions , week legal awareness;and some reasons from the society , media, etc.To ef-fectively alleviate the tension of physician -patient relationship and build a harmonious medical environment , the pri-mary hospitals need to strengthen the internal management , improve the comprehensive quality of medical staff , en-hance the physician -patient communication , improve self-protection awareness , and establish a good image of pri-mary hospitals.
2.Short-and long-term effects of minimally invasive surgical treatment combined with drugs on localized small foci of nasopharyngeal carcinoma

Objective The objective of this study was to observe the short-term and long-term curative effects of minimally invasive surgical treatment combined with chemotherapy of localized nasopharyngeal carcinoma in patients,and to clarify the necessity of combination therapy in patients with nasopharyngeal carcinoma.Methods Sixty-two patients with nasopharyngeal carcinoma were enrolled in our hospital from March 2011 to September 2011.The patients were divided into the observation and control groups by random number table method.The patients in observation group were given the application of minimally invasive surgery with postoperative combined chemotherapy with cisplatin and 5-fluorouracil after nasal endoscopic resection.The patients in the control group were given single application of minimally invasive surgical treatment of nasal endoscopic resection.The patients in two groups were observed the efficiency,survival rate and postoperative quality of life after surgery for 1 year or 5 years.Results The effective rate was 83.87% and 100% for the survival rate in the observation group after surgery for 1 year;the effective rate was 58.06% and 87.10% for the survival rate in the control group.There was significant different between the observation and control groups (P < 0.05).The survival and survival rates were significantly higher than those in the control group after surgery for 5 years (P < 0.05).After surgery 1 year,the scores of the patients in the observation group were significantly higher than those in the control group (P < 0.05).Conclusion The combination of cisplatin and 5-fluorouracil in the treatment of minimally invasive endoscopic nasopharyngeal resection combined with postoperative chemotherapy can significantly improve the short-and long-term survival rates,long-term effective rate,and quality of life in tumor patients after surgery.This is suitable for clinical application.
3.Pulmonary Nodules:Diagnostic Value of Spiral CT Targeted Scan in Combination with Three-phase Enhanced Scan

0.05),while combined the targeted scan with three-phase enhanced CT scan,the sensitivity,the specificity and the corresponding rate in qualitative diagnosis of pulmonary nodules were 97.3%,83.3% and 80.6% respectively.Conclusion The targeted CT scan in combination with three-phase enhanced CT scan can improve the sensitivity and the accuracy in diagnosing pulmonary nodules.
4.Expression of SRPX2 in human endometrial carcinoma and its role in cell proliferation and apoptosis

Objective To detect the expression of Sushi Repeat Containing Protein X Linked 2 (SRPX2) in hunan endometrial carcinoma (EC) and its role in cell proliferation and apoptosis Methods Immunohistochemistry was applied to detect the expression of SRPX2 in EC and matched tumor-adjacent tissues (n =50).The correlation between SRPX2 expression and clinical features was analyzed by Pearson chi-square test.siRNA was used to down-regulate the expression of SRPX2 in HEC-1A cells.The effects of SRPX2 on cell proliferation and apoptosis were measured by CCK-8 assay and Flow cytometer,respectively.The FAK and Cyclin D1 expression were detected after silencing SRPX2 expression.Results The expression of SRPX2 was significantly up-regulated in EC tissues,and the high expression of SRPX2 was associated with the large tumor size (>3 cm) and advanced FIGO stage (Ⅲ+Ⅳ stage).Silencing SRPX2 could inhibit cell proliferation,induce cell apoptosis and repress FAK and Cyclin D1 expression in HEC-1A cells.Conclusion The expression of SRPX2 is significantly higher in EC tissues,which is closely related with cell proliferation.Scilencing SRPX2 could achieve an anti-cancer role through inhibiting cell proliferation and inducing cell apoptosis by the down-regulation of SRPX2.
5.Effect of mechanical ventilation on 43 patients with myocardial infarction

ObjectiveTo investigate the effect of mechanical ventilation (MV) on myocardial infarction (MI).Methods43 MI patients of the treating group were treated with mechanical ventilation when PaO2<55 mmHg, SaO2<85%.26 MI patients of the control group were treated with same treatment except for inhaling O2 through nose or face mask.ResultsThe survival rate of the treating group was 34.9%, but that of the control group was 7.8%, there was a significant difference between two groups (P<0.01).ConclusionMechanical ventilation can raise the survival rate of MI patients and improve functions of failing heart.
6.Treatment of bladder transitional cell carcinoma recurrence by transurethral resection followed by submucosal injection Epirubicin in renal transplantation recipients: Is it safe and effective?

BACKGROUND: The renal transplanted recipients were in poor immunosuppressive state. Compared to common person, the bladder transitional carcinoma in recipients was aggressive and easy to recurrence. Looking for a more effective therapy method to decrease the recurrence of recipients' bladder transitional carcinoma is the hot and difficult problem in clinical study.OBJECTIVE: To analyze the efficacy and safety of submucosal injection epirubicin following transurethral resection of bladder tumor (TUR-Bt) to treat the recurrence of bladder transitional cell carcinoma in renal transplantation recipients.METHODS: Totally 9 renal transplantation recipients with transitional cell carcinoma of bladder were retrospectively studied. The patients' periods without cancer, the frequency of recurrence within one year, the rates of side effect, the changes of tumor grading following recurrence and allograff function were recorded when the routine method and submucosal injection epirubicin following TUR-Bt were used in different period respectively.RESULTS AND CONCLUSION: Submucosal injection epirubicin following transurethral resection of bladder tumor was safe and effective to treat bladder transitional cell carcinoma recurrence in renal transplantation recipients. Compared to the routine perfusion, periods without cancer and the frequency of recurrence within 1 year were significantly decreased, which can elevate recipients life quality and long-term survival rates.
7.Preliminary application of antibody-capture ELISA detection the antibody of Japanese encephalitis virus

Objective To set up an antibody-capture ELISA method to detect the Japanese encephalitis virus(JEV)antibody.Methods ELISA plate was coated with the monoclonal antibody which was specific to the envelope protein epitope E39 of JEV,JEV SA14-14-2 strain as the source of antigen was used to absorb the monoclonal antibody,the absorbed virus used to capture the JEV'S antibody.The antibody that captured ELISA was established.The indirect ELISA method using the virus particles from cell culture was compared with coating ELISA plate,105 clinical serum were checked.Results The background in indirect ELISA assay could not be abscised,positive and negative serum diluted in a ratio of 1:10,1:100,1:1000,the relative value of A posative/A negative were 1.02,0.99,1.13,all<2.1.But the antibody-captured ELISA method when the serum dilution was 1:10,1:100,the A posative/A negative were 3.57,2.94,all>2.1;when the dilution was 1:1000,the A posative/A negative was 1.42,<2.1,it meant the method could distinguish the positive and negative serum efficiently when the dilution Was 1:100,the background problem in indirect ELISA assay could be solved.Antibody-capture method was used to check 105 serum samples,the A posative/A negative over a range of 0.257~0.321(0.262±0.050),all<2.1,no positive sample found.Conclusion The antibody-capture ELISA method has been preliminary set up with a high specificity,capable of quickly identifying JEV from other virus.
8.Expression and relationship between PTEN and VEGF in bladder transitional cell carcinoma

ObjectiveTo investigate the expression and relationship between phosphatase and tensin homology deleted on chromosome ten(PTEN) and vascular endothelial growth factor(VEGF) in bladder transitional cell carcinoma(BTCC) and their clinical significance.MethodsExpression of PTEN and VEGF were detected in 60 specimens by immunohistochemistry test(SP method).ResultsThe positive rate of PTEN was 47%(28/60) in BTCC;VEGF was 68%(41/60).With the pathological grade and the clinical stage of tumors being higher,the lower expression level of PTEN showed(P<0.05);while the expression of VEGF increased with no statistical significance(P>0.05),but having a significant difference between specimens with lymph nodes metastasis and without lymph nodes metastasis(P<0.05).The expression of PTEN was negatively correlated with that of VEGF(r=-0.439,P<0.01).ConclusionThe expression of PTEN and VEGF in BTCC plays an important role during the progress of carcinoma and is helpful to evaluate the prognosis of patients.
9.Effect of breviscapine on delayed outward potassium current in single ventricular cell of guinea pig

AIM To Investigate the effect of breviscapine on delayed outward potassium current in single ventricular cell of guinea pig. METHOD Using whole cell patch clamp technique. RESULTS Breviscapine enhanced I k dose dependently. Breviscapine with the concentration of 0\^01 and 0\^02 mg?L -1 increased I k by 44.6% and 78.3%,respectively. The effect could be reversed by washing. CONCLUSION Breviscapine can open the K + channel and incrase I k .
10.Comparison between ultrasound and nerve stimulator guidance for interscalene brachial plexus block

Objective To compare the clinical effect of ultrasound and nerve stimulator guidance for interscalene brachial plexus block. Methods Sixty patients scheduled for upper extremity surgery were randomly allocated into two groups to receive either nerve stimulator guidance (group N,30 cases) or ultrasound guidance (group U, 30 cases) for interscalene brachial plexus block. The time for completing the procedure, the onset time of sensory block and motor block, as well as success rate of the procedure were recorded. The requirement for fentanyl during surgery, pain related with procedure and incidence of other complications were also recorded. Results The success rate of the procedure was 100% in both groups.Longer time was needed for completing procedure in group U [(16±7)min] than that in group N [(11±5)min](P<0.05). The onset time of sensory block was significantly shorter in group U [(13±9)min] than that in group N [(18±7)min] (P < 0.05 ), while the onset time of motor block was similar between the two groups. Twelve patients in group N and 5 patients in group U complained pain related with procedure, which were significantly different (P<0.01 ). Conclusion Interscalene brachial plexus block with ultrasound guidance has similar clinical effect and success rate compare with nerve stimulator guidance.
