1.Clinical Observation of Foot Bath Treatment by Tangkang Xifang in Patients with Type 0 Diabetes Foot

Objective:To observe the clinical treatment effects of Tangkang Xifang in the patients with type 0 diabetes foot ( DF) . Methods:Totally 73 patients with type 0 DF were randomly divided into the treatment group (n=37) and the control group (n=36). All the patients discontinued other medications for 7d before the treatment. Base on the blood glucose control with the traditional treat-ment by metformin hydrochloride (1. 5 g·d-1), the treatment group was additionally given warm water foot bath with Tangkang Xi-fang, while the control group was additionally given warm water foot bath, 30 min per time and twice a day for 60 days. The changes in the clinical symptoms, ankle brachial index ( ABI) , motor nerve conduction velocity ( MCV) and some other indices were measured before and after the treatment. Results:In comparison with the control group (58. 3%), the treatment group had higher total effective rate (86. 5%), and the usage of Tangkang Xifang also significantly improved the physiopathologic syndromes of peripheral artery dis-ease and neuropathy (P<0. 01). The ABI and MCV in the treatment group were significantly better than those in the control group (P<0. 05 or P<0. 01). Conclusion:Tangkang Xifang can effectively improve the clinical symptoms of type 0 DF, which is suitable for the clinical application.

3.Inhibitory effect of 6-hydroxy dopamine and MK-801 on spinal cord edema

BACKGROUND: After acute spinal cord injury (SCI), edema of spinal cord is an important factor for inducing and deteriorating pathological changes of spinal cord tissue. After injury, noradrenaline (NE) instantly causes microvascular contraction, endothelial injury, increase of arterial permeability and participation in edema. Recently, many researches suggest that excitatory amino acids (EAA) are related to cellular edema.OBJECTIVE: To study the effect and mechanism of selective phenol aminergic neuron, 6-hydroxy dopamine (6-OHNA)and aspartic acid (ASP) on edema after acute SCI.DESIGN: Randomized controlled study.SETTING: Department of Spine Surgery, the Third Hospital of Hebei Medical University.MATERIALS: The experiment was carried out at the Experimental Animal Center of the Third Hospital of Hebei Medical University from March to September 2003. A total of 160 Wistar rats weighing 300-350 g of both genders were randomly divided into three groups: 6-OHNA group (n =60), MK-801 group (n =50) and control group (n =50).METHODS: Acute SCI was induced at the level of T13 vertebral body with the static lcad technique. Rats in 6-OHNA group were injected with 6-OHNA into subarachnoid space; rats in MK-801 group were injected with MK-801 into caudal vein; rats in control group did not receive any treatment. The extent of edema was compared in the three groups by means of neurological scoring, water content measurement, light microscopy and electron microscopy.MAIN OUTCOME MEASURES: Neurological scores and water content.RESULTS: All 160 rats were involved in the final analysis. ① After SCl, content of NE in 6-OHNA group was decreased from (217.45±4.26) ng/g to (29.37±2.61) ng/g, and the difference was significant (P< 0.01). Edema in spinal cord tissue was effectively inhibited for 24 hours. At 12 hours after SCl, function recovered remarkably and vascular-derived edema was the mildest. ② In MK-801 group, there was no significant suppression of the edema until 24 hours after injury. Early recovery of neurological function was not significantly different from that in control group (P > 0.05), but functional recovery was obvious until 24 hours after injury (P<0.05). The degree of cytotoxic edema was the lightest.CONCLUSTON: NE can inhibit vascular-derived edema at early phase of SCI, and EAA can inhibit cytotoxic edemas,which develops at a relatively later stage.
4.Establishment of a rabbit model of oral soft tissue defects

BACKGROUND: In clinical, the research on the method and the material of the soft tissue defect of the operation area has been in depth. It wil have a positive impact on our exploration and research in this field through the establishment of reliable experimental animal oral soft tissue defect model. OBJECTIVE: To establish a rabbit model of oral soft tissue defects for oral treatment of soft tissue defects in-depth study. METHODS: Eighteen 3-month-old male New Zealand rabbits were enrol ed. A tissue ring cutting dril of 5 mm diameter was used to prepare round ful -thickness soft tissue defects in the front part of the hard palate and, respectively, from the back of the maxil ary incisors, about 2 mm from the hard palate mucosal edge. Morphology and histology were observed at 3, 7, 14, 21, 28 and 56 days after model establishment. RESULTS AND CONCLUSION: (1) Morphological observation: After 3 and 7 days, the wound’s inflammatory reaction was obvious. As the time goes, the inflammatory response subsided, the wound gradual y healed. Scar formation was observed at 21, 28 and 56 days after surgery. (2) Histological observation: 3 and 7 days after injury, many inflammatory cel s were infiltrating, and tissue necrosis area was larger. At 7 days after surgery, with the extensive formation of connective tissue proliferation and new blood capil aries, the wound gradual y shaping completely. (3) Results indicated that a rabbit model of oral soft tissue defects was successful y established, which was in line with the physical laws of wound healing and the characteristics of human oral soft tissue defects healing.
5.Scalp cluster acupuncture combined with constraint-induced movement therapy improves functional recovery after ischemic stroke

Objective To compare the effect of clustered acupuncture of the scalp combined with constraint induced movement therapy (CIMT) with that of conventional acupuncture of the body points plus conventional rehabilitation intervention after cerebral infarction using blood oxygenation level dependent functional magnetic resonance imaging (BOLD-fMRI).Methods Thirty acute cerebral infarction patients with hemiplegia were equally divided into two groups at random.The test group received the clustered acupuncture of the scalp combined with CIMT,while the control group received the body point acupuncture plus conventional rehabilitation intervention.Before and after two weeks of treatment,blood oxygenation level-dependent functional magnetic resonance imaging at 3.0 T was performed during a passive digital opposition movement.The volume and intensity of the activated areas were compared.Fugl-Meyer assessment (FMA) was carried out as well.Results The most pronounced activation appeared in the contralateral somatosensory motor cortex for both groups.There was no significant difference in average FMA scores between the two groups before the treatment,but after the treatment the averages were significantly different.An increase in activated volume and intensity in the contralateral SMC was observed in both groups after the treatment,but the test group showed a much larger change in both activated volume and intensity.Conclusion The clustered acupuncture of the scalp combined with CIMT can achieve a more significant improvement in functional recovery after acute cerebral infarction than conventional acupuncture of the body points plus conventional rehabilitation intervention.Such combined therapy is worthy of application in clinical practice.
6.Expression and clinical significance of S100A11 in non-small cell lung cancer

Objective To investigate the expression of S100A11 protein in non-small cell lung cancer (NSCLC) and its association with clinical and pathological characteristics.Methods The expressions of S100A11 protein in 112 NSCLC tumor tissues (group A), tumor-adjacent tissues (group B) and 10 normal lung tissues (group C) were detected by immunohistochemical SP method.The association of S100A11 expression with clinical pathological characteristics was analyzed.Results The percentage of the cases with high expression cases of S100A11 protein was 78.6% (88/112) , and the low expression rate was 21.4 % (24/112) in group A.The low expression rate of S100A11 protein was 100.0% (112/112) in group B.The negative expression rate of S100A11 protein was 100.0% (10/10) in group C.The difference of S100A11 expression among the three groups was statistically significant (x2 =153.634, P <0.001).The S100A11 expression was associated with pathological type (x2 =6.807, P =0.009), differentiated degree (x2 =5.029, P =0.025), regional lymph node metastasis (x2 =11.721, P =0.001) in NSCLC, but it was not associated with gender (x2 =0.020, P =0.888) , age (x2 =0.816, P =0.366) and tumor size (x2 =0.406, P =0.524).Conclusion S100A11 is highly expressed in NSCLC, which is closely related with biological behavioral characteristics.S100A11 may participate in the occurrence and development of NSCLC, and it is expected to become the potential target of diagnosis and prognosis in patients with NSCLC.
7.Platelet-rich fibrinversus collagen membrane in the repair of gingival defects

BACKGROUND:Previous studies have found that platelet-rich fibrin has a good ability to induce gingival soft tissue repair and regeneration.
OBJECTIVE:To observe the effects of platelet-rich fibrinversus colagen membrane on gingival soft tissue healing, and to evaluate the ability of platelet-rich fibrin to repair gingival defects.
METHODS:Twenty-two patients (2 premolar teeth and 20 molars) scheduled for premolar or molar removal or ridge preservation due to various reasons were selected and randomized into two groups. Bio-Oss was implanted into the extraction socket folowed by covering with platelet-rich fibrin or colagen membrane. Healing time and healing rate of gingival defects were detected to evaluate the ability of platelet-rich fibrin to promote gingival tissue healing at 1-2 weeks after Bio-Oss implantation.
RESULTS AND CONCLUSION:The healing time was (12.17±2.25) days in the platelet-rich fibrin group and (17.30±2.58) days in the colagen group. The healing rate of the platelet-rich fibrin group was notably higher than that in the colagen membrane group at 1 and 2 weeks after Bio-Oss implantation. These findings indicate that platelet-rich fibrin is better than colagen membrane to improve the healing of gingival soft tissues with a shorter healing time.
8.Applications of global left ventricular myocardial strain in detection of anthracycline-induced cardiotoxicity in breast cancer survivors with postoperative chemotherapy

Objective To assess the clinical significance of three-dimensional speckle tracking imaging (3D-STI) in the evaluation of left ventricular myocardial function in breast cancer survivors with postoperative anthracycline-based chemotherapy.Methods A total of 51 breast cancer survivors with postoperative anthracycline-based chemotherapy were recruited and 31 female healthy volunteers as the controls.Conventional echocardiography and 3D-STI derived parameters [including global longitudinal strain (GLS) and global area strain (GAS)] were measured before and at 3 and 6 anthracycline-based chemotherapeutic cycles.The receiver operating characteristics (ROC) curve was constructed to determine optimal sensitivity and specificity of 3ID-STI derived parameters for the prediction of future cardiotoxicity.Results In comparison with the controls and baseline cases,GLS and GAS deteriorated significantly (P < 0.05 for both).No statistical difference in GCS and conventional parameters showed before and after the initiation of chemotherapy (P <0.05,respectively).The ROC curves showed that GAS as the best 3D-STI predictor of patients who develop cardiotoxicity during the follow-up.The area under ROC(AUC) of GAS was 0.894,and its optimal cut-off value was-28.4%,with a specificity of 88.0% and a sensitivity of 82.9%.AUC of GLS was 0.802,and its optimal cut-off value was-14.3%,with a specificity of 62.0% and a sensitivity of 85.4%.Conclusions Early decreases in GAS and GLS based on 3D-STI may allow the prediction of subsequent cardiotoxic development accurately in breast cancer survivors with postoperative anthracycline-based chemotherapy.

10.Effects of Salvianolate on Myosin Heavy Chain in Cardiomyocytes of Congestive Heart Failure Rats.

OBJECTIVETo explore the effect of Salvianolate on myosin heavy chain (MHC) in cardiomyocytes of congestive heart failure (CHF) rats.
METHODSSixty male SD rats were divided into 6 groups according to random digit table, i.e., the normal control group (NCG), the model group, the Captopril group (CAG), the low dose Salvianolate group (LSG), the high dose Salvianolate group (HSG), the Captopril and high dose Salvianolate group (CSG), 10 in each group. CHF rat model was established with peritoneal injection of adriamycin in all rats except those in the NCG. Equal volume of normal saline was peritoneally injected to rats in the NCG, once per week for 6 successive weeks. Corresponding medication was started from the 5th week of injecting adriamycin. Rats in the CAG were administered with Captopril solution at the daily dose of 10 mg/kg by gastrogavage. Rats in the LSG and the HSG were administered with Salvianolate solution at the daily dose of 24.219 mg/kg and 48.438 mg/kg respectively by gastrogavage. Salvianolate was dissolved in 2 mL 5% glucose solution and administered by peritoneal injection. Rats in the CSG were peritoneally injected with high dose Salvianolate solution and administered with Captopril solution by gastrogavage. Two mL normal saline was peritoneally injected to rats in the model group, once per day for 8 successive weeks. Eight weeks later, the cardiac function and myocardial hypertrophy indices were detected by biological signal collecting and processing system. mRNA expression levels of alpha-MHC and beta-MHC in cardiac muscle were detected by fluorescence quantitative PCR. Expressions of protein kinase C (PKC) in cardiac muscle were detected by Western blot.
RESULTSCompared with the normal control group, heart mass index (HMI) and left ventricular mass index (LVMI) obviously increased in the model group (P < 0.01). Compared with the model group, HMI and LVMI decreased in HSG, CAG, and CSG groups (P < 0.05, P < 0.01). It was more obviously lowered in the CSG group than in the CAG group (P < 0.05). Compared with the NCG, the mRNA expression level of alpha-MHC in cardiac muscle decreased, the mRNA expression level of p-MHC and the expression of PKC in cardiac muscle increased in the model group (P < 0.01). Compared with the model group, the mRNA expression level of alpha-MHC in cardiac muscle was increased, and the mRNA expression level of beta-MHC and the expression of PKC in cardiac muscle were decreased in HSG, CAG, and CSG groups (P < 0.05, P < 0.01). There was statistical difference between the CSG group and the CAG group (P < 0.05).
CONCLUSIONSSalvianolate could up-regulate the mRNA expression level of alpha-MHC, and down-regulate the mRNA expression level of beta-MHC in cardiac muscle. Its mechanism might be related to decreasing the expression of PKC.
