2.Percutaneous kyphoplasty with bone cement injection for treatment of spinal metastases: changes of spinal stability and pain

BACKGROUND:Patients with spinal metastases may appear different degrees of pain and abnormal spinal stability, and can be treated with percutaneous baloon kyphoplasty combined with bone cement implantation. OBJECTIVE:To explore the effect of percutaneous baloon kyphoplasty with bone cement implantation on spinal stability and pain in patients with spinal metastases. METHODS:Twenty-three patients with metastatic spinal tumor were treated with percutaneous baloon dilatation kyphoplasty with polymethyl methacrylate bone cement. There were 10 females and 13 males, and their age ranged from 23 to 71 years. The visual analogue scale score, anterior and posterior edge height of vertebral body, quality of life, and motor function score of patients were observed before and after treatment. RESULTS AND CONCLUSION: Compared with before treatment, the visual analogue scale score and motor function score were significantly decreased, while anterior and posterior edge height of vertebral body were significantly increased in 23 patients at 24 hours after treatment (P < 0.05). After 12 months of folow-up, no case appeared to have spinal nerve root damage, serious adverse reactions and bone cement embolism. The patient'squality of life was significantly improved, compared with before treatment (P < 0.05). Experimental findings indicate that percutaneous baloon dilatation kyphoplasty with polymethyl methacrylate bone cement can significantly improve the spinal stability, relieve the degree of pain, and exert exact effects in treatment of spinal metastases.
3.Improvement of surgical technique for adult-to-adult living donor liver transplantation

Objective To investigate the modifications of surgical technique in adult-to-adult living donor liver transplantation (A-A LDLT) using right lobe liver grafts.Methods From Jan.2002 to Aug.2005,16 patients underwent living donor liver transplantation using right lobe grafts.During operation,a modification,designed to improve the reconstruction of right hepatic vein,the reconstruction of the tributaries of the middle hepatic vein by interpositing a vein graft,and the anastomosis of the hepatic arteries and bile ducts,was done.Results In these series,there were no donor severe complication and death.The graft and recipient weight ratio(GRWR) was between(0.72%) and(1.24%),among these,9 cases
4.The effect of aprotinin on inflammatory response to intraoperative blood salvage using cell saver in patients undergoing spinal surgery

Objective To investigate the effect of aprotinin on the inflammation response to reinfusion of shed blood during operation after being aspirated, filtered and washed using cell saver.Methods Twenty-four ASA I - II patients undergoing elective spinal surgery under general anesthesia were randomly divided into 2 groups: aprotinin group ( n = 12) and control group ( n = 12) .The patients were premedicated with intramuscular pentobarbital 0.1 g and atropine 0.5 mg. Anesthesia was induced with propofol 2 mg ? kg -1 , fentanyl 2-4 ?g ? kg-1 and vecuronium 0.1-0.3 mg ? kg-1 and maintained with isoflurane, propofol infusion and intermittent i. v. boluses of fentanyl and vecuronium. The patients were mechanically ventilated after tracheal intubation. In aprotinin group aprotinin 10 ? 105 IU was given before skin incision and another dose of aprotinin 10 ? 105 IU was continuously infused during operation. The blood shed during operation was collected, anticoagulated with heparin, filtered, washed and reinfused using AutoLog cell saver. Blood samples were taken from CVP line before skin incision (T1 , baseline), 30 min after reinfusion of salvaged bloods (T2) and at the end of operation (T3 ) for WBC and neutrophil granulocyte counts and determination of expression of CD11b and CD18 on the surface of neutrophil granulocytes using flow cytometry. Results The two groups were similar with respect to the general condition of the patients, duration of operation and account of shed blood reinfused. (676? 353) ml was reinfused in control group. 5.Clinical Observation of Carboxymethylstarch Sodium Solution in Auxiliary Treatment of Pediatric Bronchi-al Asthma

OBJECTIVE:To observe clinical efficacy and safety of Carboxymethylstarch sodium solution in auxiliary treatment of pediatric bronchial asthma. METHODS:Totally 102 children with bronchial asthma selected from our hospital during Mar. 2015-Mar. 2016 were divided into observation group and control group according to admission order,with 51 cases in each group. Control group was given aerosol inhalation of Budesonide suspension for inhalation 0.5-1 mg,tid+ aerosol inhalation of Ipratropi-um bromide solution for inhalation 1-2 mL+Ambroxol hydrochloride and clenbuterol hydrochloride oral solution 2.5-15 mL,po, bid. Observation group was additionally given Carboxymethylstarch sodium solution 7 mL(1-<4 years old),10 mL(4-12 years old),po,tid. Both groups received treatment for 3 months. Clinical efficacy,the time of cough and wheezing relief,the time of lung wheezing disappearance,hospitalization time and the levels of immune indexes (IgG,IgM,IgA) were observed in 2 groups, and the occurrence of ADR was recorded. RESULTS:Total response rate of observation group(90.2%)was significantly higher than that of control group(70.6%),and the time of cough and wheezing relief,the time of lung wheezing disappearance and hospitaliza-tion time of observation group were significantly shorter than those of control group,with statistical significance (P<0.05). There was no statistical significance in the levels of IgG,IgM and IgA between 2 groups before treatment(P>0.05). After treatment,above immune indexes of observation group were significantly higher than before treatment and control group,with statistical significance (P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLUSIONS:Carboxymethylstarch sodium solution in aux-iliary treatment of pediatric bronchial asthma shows good therapeutic efficacy and improves immunity of children with good safety.
6.A cross-lagged regression analysis between resilience, mental health and psychological stress of recruits during training

ObjectiveTo explore the resilience and mental health of recruits during training,and to investigate mutual prediction of resilience between mental health and psychological stress to provide the scientific evidence for the mental health training and psychological selection of recruits.MethodsConnor-Davidson resilience questionnaire ( CD-RISC),symptom checklist ( SCL-90 ),psychological stress self-evaluation test ( PSET),satisfaction with life scale (SWLS),positive and negative affect scale (PANAS) were applied to 650 recruits in 2011 before starting training and after two months of training.Results ① Chinese recruits had good resilience,and the CD scores of pre-and post-test were (61.60 ± 14.36) and (61.41 ± 14.55 ) respectively.②Mental health (SCL-90) of two tests were good in recruits.③ Correlation analysis showed that the same premise of phase correlation,resilience (pre-test) could significantly predict mental health (post-test) ( β =0.070,P < 0.05 ),mental health ( pretest) could not predict resilience (post-test) (β =0.015,P > 0.05 ).Resilience (pre-test) could significantly predict psychological stress (post-test) ( β =0.075,P < 0.05 ),psychological stress (pre-test) could not predict resilience (post-test) (β =0.017,P > 0.05 ).④ Compared with high resilience group,low-resilience group of recruits had higher total score of SCL-90 and psychological stress and less positive emotion,more negative emotion and lower life satisfaction.ConclusionThe recruits training have good resilience.Resilience of recruits have predictive ability to mental health,psychological stress during training.

8.The correlation analysis of the prognosis of hand-foot-and-mouth disease and prealbumin

Objective To explore the curative effect of prealbumin (PA) in the hand-foot-and-mouth dis-ease and analysis its value in monitoring and prognosing this disease. Method Detect levels of serum PA of 390 ca-ses of children before, after and during the treatment and compare the changes during the treatment. Then observe the prognosis. Results There are significant difference of PV levels between cases of different prognosis (P < 0.05). Conclusion The PV levels of children of hand-foot-and-mouth disease will change significantly after effective treat- ment. There are clinical significance in observing and prognosing the hand-foot-and-mouth disease by detecting the se-rum PV levels.
9.Clinical Observation of Salbutamol Sulfate by Atomized Inhalation in the Treatment of Pediatric Asthmatic Disease

OBJECTIVE:To observe the efficacy and safety of salbutamol sulfate by atomized inhalation in the treatment of pe-diatric asthmatic disease. METHODS:156 children with asthmatic disease were randomly divided into control group (78 cases) and observation group (78 cases). Control group was given oxygen inhalation, anti-infection, relieving cough and reducing phlegm,aminophylline and other conventional treatment;observation group was additionally given 0.03 ml/kg salbutamol sulfate, adding into 2 ml 0.9% Sodium chloride injection,putting into mask atomizer for nebulization,5-10 min every time,twice a day. They were treated for 5 d. Clinical efficacy,asthma remission time,pulmonary rales disappearing time,average hospitalization days and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in observation group was sig-nificantly higher than control group,asthma remission time,pulmonary rales disappearing time and average hospitalization days were significantly shorter than control group,the differences were statistically significant(P<0.05). There were obvious adverse re-actions in 2 groups during treatment. CONCLUSIONS:Based on conventional treatment,salbutamol sulfate by atomized inhalation has obvious efficacy in the treatment of pediatric asthmatic disease,with good safety.
10.The clinical effect of specific immune therapy for children with allergic rhinitis and its influence on the level of serum IL -17 and IL -35

Objective To study the clinical effect of specific immune therapy for children with allergic rhini-tis(AR)and its influence on the level of serum IL -17 and IL -35.Methods 174 children with AR were chosen as the research group,who were used specific immune therapy for 24 months.In same period,110 cases of healthy chil-dren were selected as the control group.Curative effect was evaluated by rhinitis symptoms total score(TRSS)points rate.quality of life was evaluated by nasal conjunctivitis related quality of life questionnaire(RQLQ)score.Pulmonary function before and after treatment,serum Eos counting,IL -17,IL -35 were detected.Results (1 )In research group,the total effective rate after treatment of 2 years was 89.66%,which was significantly higher than 71.26% after treatment of 1 year,there was statistically significant difference(χ2 =18.716,P <0.05).(2)In the research group, TRSS score and RQLQ score after treatment of 1 year and 2 years were lower than that before treatment(t =28.360, 42.850,7.749,42.850,all P <0.05 ),and the data after treatment of 2 years were less than that after treatment 1 year(t =19.207,10.558,all P <0.05).(3)In the research group,FEV1 /predictive value after treatment of 1 year and 2 years elevated compared to that before the treatment,the airway resistance value /forecast and Eos count were lower than that before the treatment(t =15.972,27.811,48.780,62.211,10.930,62.211,all P <0.05).FEV1 /pre-dicted value after treatment of two years was higher than that after treatment of 1 year,airway resistance value /forecast and Eos counts were less than that after treatment of 1 year(t =8.728,14.707,16.488,all P <0.05 ).(4)In research group,serum IL -17 after treatment of 1 year and 2 years reduced,while IL -35 rose (t =9.162,14.522, 10.235,14.522,all P <0.05).And IL -17 after treatment of 2 years was lower than that after treatment of 1 year,IL-35 was higher than after treatment of 1 year(t =5.795,7.731,all P <0.05).(5)Correlation analysis showed that the serum IL -35 and the level of IL -17 showed a negative correlation(r =-0.36,P <0.05).Conclusion Effect of specific immune treatment on children with allergic rhinitis is better,specific immune treatment can improve the clinical symptoms,inhibit IL -17,promote IL35 and improve lung function and quality of life of patients.
