1.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterised by diffuse symmetric deposits of adipose tissue, predominantly on the neck, shoulder, back and upper extremities. We report a case of benign symmetric lipomatosis in a 61-year-old-man, which was first noted one year previously. It began growing rapidly about 3 months ago and was associated with alcoholic liver disease. Histopathological findings showed that the reticular dermis had been replaced by normal uncapsulated mature fat cells that were slightly increased in number in the fibrous connective tissue. He was treated with theophylline 300mg/day for two months.

2.Diffuse Neonatal Hemangiomatosis Successfully Treated with High Dose Corticosteroid.

Diffuse neonatal hemangiomatosis is a fatal disorder characterired by multiple cutaneous and visceral hemangiomas. The organs most commonly affected are the gastrointestinal tract, brain, liver and lung. The complications are high output cardiac failure, gastrointestinal bleeding and hydrocephalus. We present a patient with diHuse neonatal hemangiomatosis associated with massive hepatic involvement and high output cardiac failure, which was successfully treated with high dose corticosteroid.

3.A Case of Maffucci's Syndrome.

Maffuccis syndrome was first reported by Maffucci, in 1881. This syndrome is manifested by multiple enchondromas and hemangiomas, characterized by non-hereditary mesenchymal neoplasia that has a propensity for malignant transformation. Involvement of long bones, particularly the bones of the hands and feet, is most common. The most important complication of this syndrome is the high frequency of chondr osarcomatous metaplasia. We report a case of Maffuccis syndrome in a 56-year-old woman with multiple hemangioma on both the hands and forearms, left foot and enchondromas of multiple bones. The disease started at the age of 8 and was associated with deformity, but malignant transformation was not found.

4.A Case of Furuncular Cutaneous Myiasis in a German Patient Who Has Traveled to Benin.

Myiasis is the infestation of any part of the body by Diptera larvae. It is the fourth most common travel-associated skin disease and cutaneous myiasis is the most frequently encountered clinical form. Furuncular lesions may result when the skin is affected. Cutaneous myiasis caused by Cordylobia anthropophaga has been endemic in the sub-Saharan African region for more than 135 years. There have been some reported cases from non-endemic regions, and yet C. anthropophaga infestations are rarely acquired outside of Africa. We report here a rare case of furuncular cutaneous myaisis by C. anthropophaga in a German patient who came to Korea after a three-week trip to Benin.

5.A Case of Apocrine Gland Carcinoma.

Apocrine gland carcinoma is a rare form of sweat gland neoplasm with a distinctive cytologic appearance. Their occurrence has been reported mainly in the axilla and breast area. The authors describe a 51-year-old man with an apocrine gland carcinoma in the right axilla area. Physical exam revealed a 2.0 X 2.5 cm sized strawberry-like mass and histologic sections showed multiple cribriform pattern tumor nodules throughout the dermis. The periodic acid-Schiffs(PAS) diastase stain revealed positively staining granules in the cytoplasm of the neoplastic cells and stains for lysozyme and GCDFP-15(Gross Cystic Disease Fluid Protein-15) were also positive.

6.A Case of Eosinoplilic Pustular Folliculitis.

Eosinophilic pustular folliculitis (EPF) or Ofuji's disease is a cutaneous inflammatory follicular disorder of unknown etiology. It usually affects adults and is clinically characterized by recurrent crops of sterile and pruritic papulopustules grouped in an annular or polycyclic pattern involving preferably the seborrheic areas. Histopathologic examination reveals eosinophilic follicular abscesses. We report a case of eosinophilic pustular folliculitis in a 21-year-old man who showed pruritic erythematous plaques with follicular papules and pustules on the face, back, upper arms, and thigh. Laboratory test showed blood eosinophilia and histopathologic study revealed numerous eosinophilic infiltration in and around the hair follicle. The patient relatively responded well to dapsone and topical steroid.

7.A Case of Periumbilical Perforating Pseudoxanthoma Elasticum.

Perforating pseudoxanthoma elasticum is a rare disorder. It is an hereditary connective tissue disease involving progressive fragmentation and dystrophic calcification of elastic fibers. The localized lesions usually occur in a periumbilical location in obese, middle-aged, multiparous black women. It is characterized clinically by yellowish, lax, well-circumscribed, reticulated or cobblestoned patches or plaques of the periumbilical region. Herein, we report a case of periumbilical perforating pseudoxanthoma elasticum which occurred in an obese, multiparous woman with no signs of hereditary systemic pseudoxanthoma elasticum.

8.An analysis of efficacy and related factors of itraconazole in the treatment of invasive fungal infection in hematological diseases

Objective To investigate the effects and related factors of itraconazole in the treatment of invasive fungal infection (IFI) in the patients with blood diseases ( BD). Methods A total of 156 BD patients with IFI treated with itraconazole in General Hospital, Tianjin Medical University from 2005 to 2008, were retrospectively analyzed. Results Of these patients, 92 were with underlying malignant BD, and 64 with non-malignant BD; 77 possible IFI, and others proven IFI. A total of 94 (63. 5% ) patients were responded to itraconazole successfully, while 54 (36. 5% ) failed. The underlying malignant BD, post-chemotherapy, neutrophil count less than 0. 5 x 109/L, positive fungus culture, and bacteria infection were related with the response to itraconazole significantly, while patient's age, application of other antibiotics,positive C test, IFI localization, haemoglobin level and platelet counts were not Five patients was changed other anti-IFI therapy because of side effects, including gastrointestinal ill (3 cases with nausea or vomiting) and tachycardia (2 cases). Conclusions Itraconazole was effective and safe in the treatment of IFI in the patients with BD. Underlying malignant BD, agranulocytosis, bacteria infection, and delayed anti-IFI therapy might reduce itraconazole therapeutic effects.

BACKGROUND: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that this is a refractory disease which may cause a deleterious effect on patients' quality of life (QOL). OBJECTIVE: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the changes of QOL after treatment. METHODS: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after systemic antifungal treatment. Responses to the questionnaire were scored by f-point scale (0~4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptom and functional impact, patients' views concerning treatment, and relationship with doctor. RESULTS: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients' QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS: from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients' view concerning treatment(AS: from 1.34 to 1.02) 4. The degree of patients' satisfaction at the therapeutic effect was very high - 62.4% (immediately after. treatment) and 65.8% (9 months after initiation of treatment) of patients answered excellent or good. CONCLUSION: This study confirms that toenail onychomycosis has significant Impact on the overall QOL of patients. Also the effect of antifungal therapy on patients' QOL were satisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis.
