Objective: To examine T-lymphocyte subsets of peripheral blood and splenic cells in rat with acute pancreatitis(AP),to investigate the changes of immune functional status in the development of acute pancreatitis.Methods: AP model was developed in adult male rats by retrograde injection of 4% sodium taurocholate in the pancreatic duct.T-lymphocyte subsets( CD3,CD4,CD8) of peripheral blood and splenic cells was measured by Flow Cytometer.Results: There was no singificant change in the stage of MAP.In the stage of SAP,the levels of CD3 and CD4 and CD4/CD8 of peripheral blood were decreased,the decrease of T-lymphocyte subsets in splenic cells was more obvious than those in peripheral blood.Conclusion:Spleen shows a negative immunologic function in the stage of SAP and promote the development of AP.

Internal hernia is an unusual cause of the intestinal obstruction. The advances of CT make the diagnosis more easier than in the past. We report one case of internal hernia with herniation of the ileum into the lesser omenturn. The diagnosis could be made when abdominal radiographs showed fixed clustering of the small bowel loops in upper abdomen medial to the stomach. CT and ultrasound showed characteristic interposition of the ileum between the left hepatic lobe and the stomach.

BACKGROUND: Conventional echocardiography provides fundamental information about mitral valve morphology and function but is often subjective and has a relatively low specificity in evaluating valve calcific deposit, which is critical information for the preoperative decision. We hypothesized that mitral valvular calcification could be detected in standard two-dimensional echocardiograms of mitral valve in vivo by evaluating regional gray level(echo amplitude) using computerized image analysis so that we could overcome the subjectivity and low specificity of conventional echocardiography. METHODS: We tested this hypothesis by performing standard 2.5MHz two-dimensional echoes on mitral valve and myocardium in 30 patients with mitral stenosis, scheduled to undergo mitral valve replacement. We compared gray level of each region of interest in mitral valve and myocardium in stop-frame images with the degree of calcifications identified by pathologic and radiographic examinations. RESULTS: Ratio of mean gray level of mitral valve to mean gray level of myocardium was the most reliable value in evaluating degree of calcification. Quantitatively, region of calcification displayed the ratio of significantly higher value than that of no calcification. In case of anterior mitral valve, the ratio of the evident calcified region was greater than 3.11, that of the region without calcification was less than 2.42 and that of microcalcification was betwwn 2.42 and 3.11. For posterior mitral valve, the ratio of the evident calcified region was greater than 3.50, that of the region without calcification was less than 2.19 and that of microcalcification was between 2.19 and 3.50. The sensitivity and specificity of this method for assessment of degree of calcification was 75% and 100% for anterior mitral valve and 9% and 87.5% for posterior mitral valve. CONCLUSION: Mitral valvular calcification could be detected quantitatively in standard two dimensional echocardiograms of mitral valve in vivo by evaluating regional gray level(echo amplitude) using computerized image analysis.

PURPOSE: The purpose of this study is to assess the sonographic findings of Osgood-Schlatter disease with correlation to radiographic findings and to determine value of sonography in the diagnosis of Osgood-Schlatter disease. MATERIALS AND METHODS: The population consist of 22 cases of Osgood-Schlatter disease and 15 healthy subjects of the same age range for comparison. The sonographic study was carried out with real time units equipped with linear probe of 7 MHz frequency. Radiography and sonography were done in all cases and follow-up sonography was performed in 10 cases. RESULTS: The sonographic features of the Osgood-Schlatter disease were swelling of cartilage over the anterior tibial tuberosity ossification center in 16 cases, fragmentation and/or irregularities of the ossification center of the anterior tibial tuberosity in 14 cases, localized thickening of the pateliar tendon in 14 cases, ossicles in the thickened pateliar tendon in 3 cases, and distension of the infrapatellar bursa in 2 cases. Fifteen of 22 cases of Osgood-Schlatter disease had abnormal findings on radiographs and sonograms. Out of 7 cases which had normal radiograph and abnormal findings on sonogram, 6 cases showed pateliar tendon thickening, cartilage swelling, and/or distended infrapatellar bursa and one case showed fragmentation of anterior tibial tuberosity on sonogram. CONCLUSION: Sonographic evaluation of Osgood-Schlatter disease provided a clear picture about pathologic changes in the superficial soft tissues and in the cartilage and also yielded the same diagnostic information on bony change as radiography Sonography is a reliable and useful method for the evaluation of young patients with clinically suspected Osgood-Schlatter disease.

Brunner's gland adenoma(or Brunneroma) is clinically relevant tumor-like lesion of the duodenum which is composed of submucosal Brunner's gland tissue. Since the first case by autopsy was described by Cruveihier in l935, there have been less than 190 reports in the world literature, Brunner's gland adenoma is an extremely rare entity that accounts for only 10.6% of benign duodenal tumors, which are themselves relatively rare representing 0.008% of all surgical and autopsy specimens. The clinical manifestation of this tumor are non-specific gastrointestinal complaints, such as bloating or epigastric pain, and tumor often gives rise to melena or obstruction, It is postulated that this tumor are hamartomas rather than true neoplasm. However, they undergo rarely malignant transformation. We report on a case of a large pedunculated Brunner's gland adenoma, which was successfully polypectomized.

Magnetic Resonance image(MRI) is used as the diagnostic modality for evaluation of suspected intramedullary tumors and differential diagnosis of these tumors at the spinal cord. We experienced intramedullary astrocytoma of cerviced cord with large syrinx and multiple peritumoral cysts consisted of subacute and chronic hemorrhage at the margin and within the syrinx and cysts on MRI.

A pathological study was performed on four cases of infantile hemangioendothelioma of the liver. All the patients were between the age of 1 -5 months and the tumors were typical hemangio-endotheliomas, type 1. The tumors were composed basically of two components; the endothelial cell proliferation and the myxoid matrix. The endothelial cells were cytologically innocuous and formed vascular channels of varying sizes and shapes from capillary to sinusoidal and cavernous vessels. Fibrosis of the matrix, albeit not a major component of the tumor, was found particularly near the center. Immunohistochemically, CD31 was expressed strongly in almost all endothelial cells, in contrast to the stain for von Willebrand factor which was only focally and weakly positive. Alpha-fetoprotein was expressed in hepatocytes within the tumor or in hepatocytes around the tumor. Intratumoral bile duct structures were located mainly at peripheral portion. The results indicated that the type I infantile hemangioendothelioma is a tumor of endothelial cells and myxoid stroma, and that the endothelial cells undergo gradual maturation to form sinusoidal and cavernous vessels in accordance with gradual fibrosis of the myxoid stroma.
