BACKGROUND: Animal experiments demonstrated that adding ultraviolet blood irradiation and oxygenation (UBIO) in cardioplegic solutions has myocardial protection during cardiopulmonary bypass (CPB).OBJECTIVE: To investigate the myocardial protective effect of UBIO as cardiac arresting solution in the process of CPB in old patients undergoing valve replacement.DESIGN, TIME AND SETTING: The randomized controlled trial of biochemistry level was performed at Department of Cardiac Surgery, Guizhou Provincial People's Hospital, from October 2006 to April 2008.PARTICIPANTS: A total of 46 old patients scheduled for heart valve replacement were randomly divided into test group and control group, with 23 patients in each group.METHODS: The test group was infuoed with the UBIO blood as a cardiac arresting solution via the ascending aorta. The UBIO blood accumulated from the subclavian vein (10 mL/kg) was heparinized and dealt with the ultraviolet irradiation (wave length 240-300 nm) and oxygenation. And then it was infused for the first antegrade coronary perfusion when the ascending aorta was cross-clamped, when the UBIO blood was produced, the equivalent quantitative saline was infused from another vein. The 4:1 cold oxygenated blood hyperhalemia cardioplegia was perfused and then maintained for 30 minutes. The control group was the same as the test group except that the cardiac arresting solution presented without the 4:1 cold oxygenated blood.MAIN OUTCOME MEASURES: Blood samples were withdrawn from coronary venous sinus before cross-clamping and at minutes 5 and 10 after aorta declamping, to measure the levels of superoxide dismutase (SOD) and the content of malondialdehyde (MDA). Before the beginning of CPB, and at hours 4, 24, and 48 after the end of CPB, venous blood was drawn to test the serum cardiac troponin I(cTnI) and creatine kinase isozyme (CK-MB).RESULTS: MDA levels of plasma were less in the test group than that of the control group after aorta declamping (P < 0.05),while the SOD activity was obvious greater than the control group (P < 0.05). At hours 4-48 after CPB, CK-MB and cTnl levels were lower in the test group than the control group (P< 0.05).CONCLUSION: Antegrade coronary first perfusion with UBIO during CPB has evident cardiac protective in old patients undergoing valve replacement by elevating SOD activity, decreasing MDA level, relieving myocardial reperfusion injury, as well as decreasing myocardial damage markers levels.

Objective To construct a machine learning prediction model for postoperative liver injury in patients with non-liver surgery based on preoperative and intraoperative medication indicators. Methods A case-control study was conducted on 315 patients with liver injury after non-liver surgery selected from the databases developed by 3 large general hospitals from January 2014 to September 2022.With the positive/negative ratio of 1 ∶3,928 cases in corresponding period with non-liver surgery and without liver injury were randomly matched as negative control cases. These 1243 patients were randomly divided into the modeling group(n=869)and the validation group(n=374)in a ratio of 7∶3 using the R language setting code.Preoperative clinical indicators(basic information, medical history, relevant scale score, surgical information and results of laboratory tests)and intraoperative medication were used to construct the prediction model for liver injury after non-liver surgery based on 4 machine learning algorithms,k-nearest neighbor(KNN),support vector machine linear(SVM),logic regression(LR)and extreme gradient boosting(XGBoost). In the validation group, receiver operating characteristic(ROC)curve,precision-recall curve(P-R),decision curve analysis(DCA)curve,Kappa value, sensitivity, specificity,Brier score, and F1 score were applied to evaluate the efficacy of model. Results The model established by 4 machine learning algorithms to predict postoperative liver injury after non-liver surgery was optimal using the XGBoost algorithm. The area under the receiver operating characteristic curve(AUROC)was 0.916(95%CI:0.883~0.949),area under the precision-recall curve(AUPRC)was 0.841,Brier score was 0.097,and sensitivity and specificity was 78.95%and 87.10%,respectively.Conclusion The postoperative liver injury prediction model for non-liver surgery based on the XGBoost algorithm has effective prediction for the occurrence of postoperative liver injury.

OBJECTIVE To study the improvement effect and mechanism of Shuhou tongqi formula on intestinal injury in mice with postoperative ileus (POI). METHODS Mice were randomly divided into sham operation group, model group, positive control group (Mosapride citrate tablets, 1.95 mg/kg), and Shuhou tongqi formula group (1.88 g/kg), with 6 mice in each group. Except for the sham operation group, POI model was induced in other groups by typical small intestinal interference. Each group was given relevant drug liquid/water, once a day, for consecutive 2 days. After the last medication, the percentage of carbon powder propulsion in small intestine was detected, and pathomorphological changes in ileum tissue of mice were observed. The serum levels of interleukin-6 (IL-6), IL-10, tumor necrosis factor-α (TNF-α), motilin (MTL) and somatostatin (SS) were all detected; the expression levels of Toll-like receptor 4 (TLR4), nuclear factor κB (NF-κB p65) and p38 mitogen-activated protein kinase (p38 MAPK) were determined in ileal tissue of mice; the gut microbiota of colon contents was analyzed in each group of mice. RESULTS After the intervention of Shuhou tongqi formula, pathological damage such as intestinal wall atrophy and mucosal capillary congestion in ileum tissue were improved significantly; the percentage of carbon powder propulsion and the serum level of IL-10 and MTL were increased significantly (P<0.05); however, serum levels of TNF-α, IL-6 and SS, the expressions of TLR4, NF-κB p65 and p38 MAPK were decreased significantly (P<0.05). The analysis of gut microbiota showed that Shuhou tongqi formula could significantly increase ACE, Chao1, Shannon and PD indexes, and relative abundance of Akkermansia (P<0.05), but decreased relative abundance of Proteobacteria, Ligilactobacillus and Escherichia-Shigella significantly (P<0.05). CONCLUSIONS Shuhou tongqi formula can improve intestinal injury and inflammatory reaction of POI mice, the mechanism of which may be associated with inhibiting the activity of TLR4/ NF-κB/MAPK signaling pathway, regulating the levels of gastrointestinal hormones and improving the disturbance of intestinal flora in mice.
