1.A PATHOLOGICAL STUDY ON SUDDEN DEATHS DUE TO ABNORMALITIES OF THE ATRIOVENTRICULAR CONDUCTING SYSTEM

Three young men died of developmental abnormalities of the AVN,HB and BB were reported.The victims collapesed suddenly and died silently after excitation. A variety of abnormalities were observed in the AVCS including fetal the pattern of the AVN, the malformation of the AVN and thepresence of the accessory routes. The histopathological feature, the mechanism and the precipitatingfactors of sudden death were discussed.

Objective To study the pathological changes related to cardiac sudden death. Methods The CCS from 179 cases with cardiac sudden death were examined histopathologically. Result There were 8 cases with hemorrhage, inflammation, or tumor infiltration in the cardiac nervous tissue or cardiac ganglion. Pathological changes such as edema, degeneration or necrosis could be found in the nervous tissue. However, no lethal pathological changes were observed out of cardiac nervous tissue. Conclusion Either primary or sencondary pathological changes in the cardiac nervous tissue might cause sudden death.
3.Analysis of prognostic factors of local advanced glottic laryngeal cancer patients with beyond five years survival

Objective To evaluate the clinical features of local advanced glottic laryngeal cancer patients with beyond five years survival and to provide help for the treatment of advanced glottic laryngeal cancer patients.Methods The data of 129 cases of patients with local advanced glottic laryngeal cancer treated in our hospital from January 1999 to August 2005 were analyzed retrospectively.They were divided into two groups based on their survival time.Forty six patients with beyond five years survival were included in the first group and the other eighty three patients were in the second group.The clinical features were compared and factors associated with survival were identified.Results Compared with the group with under five years survival,the lymph node metastatic rate before treatment was lower,patients treated concurrently with operation were more,and complete remission rate after treatment were higher in the group with beyond five years survival.The differences between two groups had statistical significant.Cox regression analysis identified only operation and complete remission rate were independent prognostic factors.Conclusion For local advanced glottic laryngeal cancer,rational comprehensive treatment and complete remission after treatment and complications prevention are important.But they need to be validated by the large-scale clinical trials.
4.Study of re-examination compliance of patients with breast cancer in the stage of recovery

Objective To investigate the influencing factors on re-examination compliance of patients with breast cancer Who were in the stage of recovery.Methods By clinic services and telephones,we investigated the reexamination compliance of 189 cases of breast cancer.Results 66.1% of the cases took periodic re-examinations and on the opposite,33.9% of the patients didn't do so.Statistical meaning could be found from the difference of ages,living sites and survival time limits(P<0.05).Conclusions The main factors to influence the re-examination compliance of patients with breast cancer are age,living site and survival time limit.And it is possible to improve the re-examination compliance of patients by enhancing health education,raising medicsl levels and service qualities,and creating comfortable environment for patients.
5.IMMUNOCYTOCHEMICAL STUDY WITH ANTI-MUSCLE ACTIN ANTIBODY(HHF_(35))ON MYOCARDIAL ISCHEMIA AND REPERFUSION INJURY IN RATS

Experimental studies on the myocardial ischemia and reperfusion injury in 16 anaethetized SDrats,of which,8 animals were pretreated with morphine(5 mg/kg,i.p.)for preventing of arrhyth-mias,were studied immunocytochemically with anti-muscle actin specific monoclonal antibody (HHF_(35)),8 shan-operated rats were used as control.With HHF_(35) ABC immunocytochemical method,the area of myocardial ischemia and reperfusion injury(without morphine)showed decrease or ab-sence of staining,large areas of staining loss were also seen.In the group with morphine,only smallfoci of staining absence were shown.The myocardium in control animals showed evenly positive stain-ing.No change were seen with HE staining in all groups.The results obtained with HHF_(35) stainingsupport its important value in studying on myocardic reperfusion injury,and indicated that the degreeof myocardic damage may be relative to the arrhythmias in myocardial reperfusion injury.
6.The Effect by of Inspiring High Concentration Oxygen on the Reactive Oxygen Species in the Rats Brain Homogenates

Objective To evaluate the security of inspiring high concentration oxygen by detecting the reactive oxygen species of the rats brain homogenates after inspiring high concentration oxygen for a certain time.Methods Sixty wister rats weighting 150~200g were randomly devided into five groups(the group of 4h、8h、12h、16h and the control group,12/group).The experimental rats were exposed to 85%~100% oxygen in a self-made chamber and the control animals were kept in normal room air.The Concentration of the MDA,SOD,GSH,NO,NOS,CAT and T-AOC of the brain homogenates was detected.Results Compared with the control group,the level of MDA,NO and NOS in the brain homogenates grew up markedly(P
7.Survey on the Current Status of Job Satisfaction of Clinical Medical Staff and Research on Coping Strategies

Objective To explore the current status of job satisfaction of clinical medical staff and factors that influence the job satisfaction,and to put forward coping strategies and suggestions for improving the job satisfaction of clinical medical staff,so as to maintain workforce stability.Methods The paper will investigate the job satisfaction of clinical medical staff by sending questionnaires to all doctors and nurses working in a hospital,and use descriptive analysis and T test statistic method to analyze the current status of job satisfaction of questionnaire receivers as well as different dimensions of job satisfaction among different population groups.Results More than 85% of questionnaire receivers think that the job they are doing is at higher risk.Over 80% feel stressed out by their jobs.Less than 35% are satisfied with their occupations,social status,working status,job security,and promotion opportunities.Less than 15% feel respected by their jobs.Conclusions Generally,questionnaire receivers' job satisfaction is low.Heavy workload and poor quality of life are leading factors that contribute to their low job satisfaction.In this sense,we should adopt various measures to give attention to these key population groups.
8.Protective effect of Xinyi capsule pretreatment on myocardial ischemia reperfusion injury in rabbits

Objective To investigate the protective effect of Xinyi capsule pretreatment on myocardial ischemia reperfusion injury in rabbits and its possible mechanism. Methods Ninety-four rabbits were randomly divided into 6 groups: model group (n=16), tirofiban group (n=16), high-, medium- and low-dose Xinyi capsule groups (4.0, 2.0, 1.0 g/kg;n=16 in each group), and sham operation group (n=14). Five days after intragastric administration with drug, myocardial ischemia reperfusion was induced by ligation of the proximal left circumflex artery. The electrocardiogram (ECG) was continuously recorded. The serum levels of myeloperoxidase (MPO), lactate dehydrogenase (LDH) and creatine kinase-MB (CK-MB) were measured. Myocardial histopathological damage was evaluated. Results The changes of J-point amplitude on ECG in high-, medium-and low-dose Xinyi capsule groups (0.064 ± 0.049 mV, 0.069 ± 0.061 mV, 0.079 ± 0.060 mV) were significantly lower than that in the model group (0.158 ± 0.105 mV, P<0.01 or P<0.05), the serum levels of LDH (399.7 ± 202.3 U/L, 369.6 ± 229.0 U/L, 435.5 ± 152.4 U/L), CK-MB (900.8 ± 231.2 U/L, 1 268.3 ± 899.8 U/L, 1 386.7 ± 621.6 U/L), MPO (69.81 ± 5.51 U/L, 85.44 ± 10.31 U/L, 81.33 ± 16.87 U/L) were significantly lower than those in the model group (LDH:817.1 ± 401.9 U/L, CK-MB:2 071.3 ± 693.5 U/L, MPO:149.9 ± 20.11 U/L;P<0.01 or P<0.05). Histopathological examination showed that myocardial damage in high-, medium- and low-dose Xinyi capsule groups reduced compared with the model group. Conclusions Xinyi capsule pretreatment can protect against myocardial ischemia reperfusion injury in rabbits, and its mechanism may be related to inflammation inhibition.
9.THE NORMAL VARIATION OF HUMAN CARDIAC CONDUCTION SYSTEM

Objective To explore the delineation between normal variations and developmental anomalies (malformation) of human cardiac conduction system (CCS). Methods The CCS of 886 cases (737 cases were noncardiac death,149 cases died of sudden cardiac death) were examined histologically using the method designed by the authors that tissues containing the SAN and AVN were cut along the long axes of the nodes in 1-2 block and the HB perpendicular to its long axis 2-4 blocks.The morphology and causes of death in CCS of two groups were also studied. Results 1.There were congenital variations in human CCS with respect to the sizes,position and shape;2.There were postnatal variations in CCS accompanying the aging process;3.It was revealed that there were developmental anomalies in CCS with sudden cardiac death,which included fetal typed atrioventricular node (AVN) in adults, complete displacement of the AVN into the central fibrous body,complete separation of the HB into more than 3 bundles and the displacement of bifurcation of the HB into the root of tricuspid valve.These changes should not be grouped into normal variations,since they are related or potentially related to sudden cardiac deaths.Conclusion The following changes should be included in normal variation or CCS:displacement of the bifurcating portion of the HB into the pars membranacea of the ventricular septum;left sided deviation of downward and leftward displacement of that portion;displacement of less than 1/2 of the AVN in to the central fibrous body;displacement of the myocardium into the HB or LBB.;
10.Construction of type 2 diabetes rat models of lower limb ischemia by feeding with high-fat diet

BACKGROUND:The method of ligating or resecting rat lower limb femoral or iliac artery has been widely used to make rat models of lower limb ischemia, but there have been no stable and efective methods used to establish diabetic models of chronic atherosclerotic occlusive diseases and to evaluate the ischemic status of hindlimbs of models. OBJECTIVE: To establish type 2 diabetes rat models of lower limb ischemia by feeding with high-fat diet and to evaluate them. METHODS:Twenty rats were randomly and evenly divided into diabetes group (n=10) and control group (n=10). Rats in the diabetes group were fed with high-fat diet for 6 months, and were intraperitonealy injected with streptozotocin (50 mg/kg) to induce diabetes. Rats in the control group were fed with normal diet for 6 months. Rats in these two groups were subjected to ligation of femoral artery to establish right lower limb ischemia models. RESULTS AND CONCLUSION:At the first day of modeling, color Doppler flow imaging and CT angiography showed obviously decreased blood flow suggesting the success of establishing ischemia model. At 7 and14 days after modeling, the blood flow of rats in these two groups showed a gradual recovery as detected by color Doppler flow imaging. At 28 days after modeling, blood flow of rats in the diabetes group was significantly slower than that in the control group (P < 0.05). CT angiography showed that at 28 days after modeling, only a smal amount of compensatory increase in blood flow of the blood vessels at the ligation position of proximal right lower limb femoral artery was seen in the diabetes group, while no obvious blow was observed in the distal part. Histopathologic and immunohistochemical staining showed that at 28 days after modeling, destroyed tissue structure and inflammatory cel infiltration were observed in the ischemic region and capilary density on the affected side was lower than that on the healthy side. The protein expression of vascular endothelial growth factor in the ischemic muscle tissue of rats in the diabetes group was significantly increased compared with that in the control group (P < 0.05). These results show that diabetic rat models of lower limb ischemia can be successfuly established by long term high-fat diet feeding and femoral artery ligation and they can be validated by CT angiography.
