1.Research on Identification ofAstragalus membranaceus var.mongholicus in different growth years by Electronic Nose and Chemical analysis

In this article, the method of quick and accurate identification ofAstragalus membranaceus (Fisch.) Bge. var.mongholicus (Bge.) Hsiao in different growth years by electronic nose (e-nose) technique and chemical analysis was developed. An e-nose was used to detect the odors ofA. membranaceus. var.mongholicus samples in two-growth-year, seven-growth year and more than a decade growth years for establishing the classification model of response characteristics. Principal component analysis (PCA) and discriminant factor analysis (DFA) were performed to differentiate theA. membranaceus. var.mongholicus samples in different growth years. PCA was performed to investigate the difference in the chemical composition and quality of different growth years inA. membranaceus. var.mongholicus samples. The results of PCA and DFA analyses for e-nose demonstrated that the samples ofA. membranaceus. var.mongholicus in different growth years could be distinguished obviously. The contents of chemical composition were similar in same growth years inA. membranaceus. var.mongholicus and different from different growth years. The results of chemical composition analysis indicated that the identification forA. membranaceus. var.mongholicus in different growth years was significant for the quality control. E-nose technique could identify the samples ofA. membranaceus. var.mongholicus in different growth years rapidly, sensitively and intactly, and could be applied for the quality control of traditional Chinese medicine.

3.Correlation between characteristics of intracranial lesions and level of cognitive function in patients with an initial diagnosis of brain metastases

Objective To investigate the correlation between the characteristics of intracranial lesions and the level of cognitive function in patients with an initial diagnosis of brain metastases.Methods A retrospective analysis was performed in 51 patients with an initial diagnosis of brain metastases who were admitted to The Second Hospital Affiliated to Suzhou University from January 2015 to April 2016.CT and (or) MRI were used to determine the characteristics of intracranial lesions and the Montreal Cognitive Assessment was used to evaluate the cognitive function of patients.Comparison between groups was made by Mann-Whitney U test.The correlation between ranked data was analyzed by Spearman rank correlation test.Results Of the 51 patients with an initial diagnosis of brain metastases,47(92%) had cognitive impairment,including mild cognitive impairment in 31(61%) and dementia in 16(31%).There was no significant difference in level of cognitive function between the patients with involvement of the left hemisphere alone and those with involvement of the right hemisphere alone (P=0.425).The patients with involvement of both hemispheres had a significantly lower level of cognitive function than those with involvement of the left hemisphere alone (P=0.042).The patients with involvement of three or more brain lobes had a significantly lower level of cognitive function than those with involvement of one or two brain lobes (P=0.015,0.024).The intracranial lesion volume and edema volume had no significant effect on the overall cognitive function of patients (P=0.077,0.178).The patients with>3 intracranial lesions had a significantly lower level of cognitive function than those with 1-3 intracranial lesions (P=0.010).Conclusions More than 90% of patients with an initial diagnosis of brain metastases have cognitive impairment.Cognitive impairment is mainly associated with lesion site,involvement of brain lobes,and number of lesions,but not with lesion volume and edema volume.
4.Effect of ulinastatin on perioperative renal function in patients undergoing orthotopic liver transplantation

ObjectiveTo investigate the effect of ulinastatin on perioperative renal function in patients undergoing orthotopic liver transplantation.MethodsSixty ASA Ⅱ or Ⅲ patients of both sexes aged 35-64 yr weighing 50-75 kg with normal blood urea nitrogen (BUN) and creatinine (Cr) before operation undergoing orthotopic liver transplantation were randomly divided into 2 groups ( n = 30 each):control group (group C) and ulinastatin group ( group U).Anesthesia was induced with midazolam,fentanyl,etomidate and vecuronium and maintained with isoflurane inhalation,propofol TCI,continuous remifentanil infusion and intermittent iv boluses of fentanyl and vecuronium.The patients were tracheally intubated and mechanically ventilated.PET CO2 was maintained at 30-35 mm Hg.Ulinastatin 400 000 IU in normal saline 20 ml was infused iv after induction of anesthesia.Ulinastatin 200 000 IU was then infused every 4 h until 48 h after operation.Urine volume and the amount of furosemide administered were recorded before anhepatic phase,and during anhepatic and neohepatic phase.Venous blood samples and urine were collected before induction of anesthesia (T1),at 15 min of anhepatic phase ( T2 ),at 15min of neohepatic phase (T3),at the end of operation (T4) and 48 h after operation (T5) for determination of serum concentrations of BUN,Cr and creatinine clearance rate and urinary N-acetyl-beta-D-glucosaminidase (NAG)activity and microalbumin concentration.ResultsCompared with group C,ulinastatin significantly decreased the amount of furosemide administered and increased urine volume during anhepatic and neohepatic phase,decreased serum Cr concentration,increased creatinine clearance rate at T2.5,decreased urinary NAG activity and microalbumin concentration at T4.5 and serum BUN concentration at T3-s.ConclusionUlinastatin has protective effect on rehal function during perioperative period in patients undergoing orthotopic liver transplantation.
5.Diagnosis and treatment of 56 cases of splenic tumors

5 years, 1 for 3 years, 4 for less than 1 year. In addition, metastatic tumors to the spleen were found in 5 patients, splenectomy with or without combined resection of other organs was performed in all 5 patients. Conclusions Imaging examination is the main method of diagnosis for splenic tumors. Partial splenectomy is recommended for benign tumors. For malignant splenic tumors, surgery, as the main aspect of combined therapy, is required.

7.Development and evaluation of whole breast irradiation with volumetric modulated arc therapy-based hybrid intensity-modulated radiotherapy after breast conserving surgery for breast cancer

Objective To develop whole breast irradiation with volumetric-modulated arc therapy (VMAT)-based hybrid intensity-modulated radiotherapy (IMRT) after breast conserving surgery for breast cancer,and to evaluate its value in clinical application.Methods Ten patients with breast cancer undergoing breast conserving surgery were enrolled.Two radiotherapy plans were designed based on hybrid fixed-beam IMRT/IMRT technique and hybrid VMAT/IMRT technique,respectively.One group received whole breast irradiation with a prescribed dose of 50 Gy in 25 fractions.The other group received whole breast irradiation with a prescribed dose of 50 Gy in 25 fractions,as well as simultaneous integrated boost to the tumor bed with 60 Gy in 25 fractions.The dosimetric parameters and delivery efficiency were compared between the two plans using paired t test.Results For patients treated with whole breast irradiation alone,there were no significant differences in conformity index and homogeneity index of target volume between the two plans (P=0.866,0.056);however,compared with the hybrid fixed-beam IMRT/IMRT technique,the hybrid VMAT/IMRT technique significantly increased the doses to organs at risk (OARs) and the number of monitor units (MUs) in the intensity-modulated field (P=0.000-0.050,P=0.002).For patients treated with whole breast irradiation with a simultaneous integrated boost to the tumor bed;however,the hybrid VMAT/IMRT technique significantly reduced the doses to the lung and spinal cord,number of MUs in intensity-modulated field,and delivery time compared with the hybrid fixed-beam IMRT/IMRT technique (P=0.004,0.001,0.000,0.000).Conclusions For patients treated with whole breast irradiation with a simultaneous integrated boost to the tumor bed,the hybrid VMAT/IMRT technique is highly promising for clinical application due to better OAR sparing and higher delivery efficiency.
8.Epidemiology of hepatitis B, C, D and G viruses and cytokine levels among intravenous drug users.

To investigate the features of various hepatitis virus infection in intravenous drug users (IVDU), we conducted an epidemiological survey of hepatitis viruses including hepatitis B virus (HBV), hepatitis C virus (HCV), hepatitis D virus (HDV) and hepatitis G virus (HGV) in IVDU. The correlation of TH lymphocyte cytokine and hepatitis virus infection was examined. A study population of 406 IVDU consisted of 383 males and 23 females. HBV-DNA and HCV-RNA were detected by fluorescence quantitative polymerase chain reaction. HBsAg, HBeAg, anti-HBc, anti-HCV, HDV-Ag and anti-HGV were assayed by ELISA. The levels of cytokines of TH1 and TH2 were measured by ELISA. The similar indices taken from 102 healthy persons served as controls. The infection rate of each virus among IVDU was 36.45 % for HBV, 69.7 % for HCV, 2.22 % for HDV, and 1.97 % for HGV, respectively. The co-infection rate of HBV and HCV was detected in 113 of 406 (27.83 %). In contrast, among controls, the infection rate was 17.65 % for HBV and 0 % for the other hepatitis viruses. The levels of PHA-induced cytokines (IFN-gamma and IL-4) and the level of serum IL-2 were obviously decreased in IVDU. On the other hand, the level of serum IL-4 was increased. The IFN-gamma level was continuously decreased when the IVDU was infected with HBV/HCV. In conclusion, HBV and HCV infection were common in this population of IVDU and they had led to a high incidence of impaired TH1 cytokine levels.
9.SURVEILLANCE OF SCHISTOSOMIASIS BETWEEN 2000 AND 2002 IN JIANGSU PROVINCE

Objective To identify the trend of endemic situation among surveillance sites in Jiagsu Province from 2000 to 2002. Methods Twelve schistosomiasis surveillance sites were es-tablished ,and the longitudinal, surveillance was carried out. Results The related index of snail increased in most of surveillance sites, the rates of positive snails rose rapidly in marshlands. The infection rates of Schitosoma janponicum of cattle decreased and infection rates of human were relatively steady. However, there was still the danger of heavy endemic. Conclusion Current control strategies can not effectively adapt to the endemic situation of schistosomiasis, although which have some effects on control of morbidity. We need to study the new characteristics and rule of the endemic of schistosomiasis, and make out more effective control strategies which can suit with the current society, economies and nature environment.
10.Impact of project diverting Yangtze River water to Northern Jiangsu on transmission towards north of Oncomelania hupensis snails

Objective To evaluate the impact of the project diverting the Yangtze River water to the Northern Jiangsu on transmission towards the north of Oncomelania hupensis snails, in order to provide the scientific basis for the South-North Water Diversion Project (east route). Methods The running situation of the water diversion project, the data on hydrology of the waterway and the distribution of snails and schistosomiasis were investigated with the methods of epidemiology、hydrology and field test. Results There was no significant correlation between the region of waterhead and water supply on the change of snail areas. There was no significant correlation between the snail areas of the region of water supply and the volume of water diversion, either. The snail habitat was stable at the Gaoyou segment along the Grand Canal. The snail habitat in the natural place of the north of 33?15′ north latitude on the east route has not been discovered yet. Conclusion There is no sign of transmission towards north on the snail habitat at the Gaoyou segment along the Grand Canal since the project was constructed in 1961. There is no evidence that the project leads to the spread of snails and schistosomiasis, either.
