2.Detection of hepatitis C virus antigen in hemodialysis patients.

OBJECTIVETo investigate the utility of the HCV core antigen ELISA in the detection of HCV infection in hemodialysis patients and to compare with Anti-HCV antibodies 3rd generation ELISA.
METHODSTwo hundred fifty hemodialysis patients were included in the study. Anti-HCV antibodies and total HCVcAg was determined by ELISA kits. HCV RNA was determined using reverse transcriptase polymerase chain reaction (RT-PCR).
RESULTSForty-three out of 250 (17.2%) patients were positive for anti-HCV antibodies and 18% were positive for HCVcAg. 13/250 (5.2%) were positive for HCVcAg but anti-HCV negative, All 13 were confirmed viremic by in-house nested RT-PCR leading to specificity of 100%. Viral load of (49258 +/- 28682) copies/ml were detected in HCVcAg positive cases was higher in comparison to (23938 +/- 10780) copies/ml in the anti-HCV positive group (P < 0.05). The viral load of 4 negative cases for HCVcAg assay but anti-HCV positive was 306 +/- 161 copies/ml which was significantly lower in comparison to HCVcAg positive group (P < 0.001).
CONCLUSIONDetection of total HCVcAg together with anti-HCV will be useful for patients undergoing hemodialysis who have a longer window period due to immunosuppressed state. HCVcAg was compatible for the HCV RNA in serum and total HCVcAg ELISA is both a cost-effective and a less labor-intensive alternative to PCR, enhancing its clinical utility.

3.Monitoring of ticks parasitic on domestic animals in Yiwu City in 2021

ObjectiveTo determine the prevalence of ticks parasitic on domestic animals in Yiwu City of Zhejiang Province. MethodsWe monitored, identified and analyzed ticks parasitic on domestic animals in Yiwu in 2021. ResultsA total of 452 domestic animals were monitored, of which 306 were infected with ticks, suggesting a prevalence of 67.70%. A total of 2,603 ticks were captured, and the tick index was determined to be 5.76. The ticks were identified to belong to one family, four genera, and 10 species. Haemaphysalis longicornis was the dominant tick (59.16%), followed by Rhipicephalus haemaphysaloides (26.32%) and Rhipicephalus microplus (6.45%). Prevalence of ticks, tick index, and number of tick species were listed for domestic animals as follows: goats (74.37%, 7.45, 7 species), yellow cattle (62.50%, 4.14,6 species), and dogs (54.62%, 2.59, 8 species). Moreover, goat was the main domestic animal infected with more than two tick species (25.99%); H.longicornis (91.26%) was the species with most repated infections. The dominant species of ticks were H.longicornis (65.15%) and R.haemaphysaloides (29.42%) in goats, R.microplus (67.67%) and R.haemaphysaloides (20.69%) in yellow cattle, and H.longicornis (57.14%) and Haemaphysalis porcupines (21.10%) in dogs. However, seasonal fluctuation in tick index differed among goats, yellow cattle and dogs. The fluctuation in tick index in goats and dogs was consistent with that of H.longicornis, with a major peak in March and the minor peaks in September and October. The fluctuation in tick index in yellow cattle was consistent with that of R.microplus, with two peaks in April and August-September. In addition, larval ticks, nymphal ticks and adult ticks were caught in this study, among which nymphal ticks were most common. We found that single domestic animals were infected with ticks of the same species and different stages, which was more common in goats and those animals infected with H.longicornis. ConclusionTicks parasitic on domestic animals are common in Yiwu. Multiple species of ticks have been identified, with H.longicornis as the dominant species. It warrants further monitoring and countermeasures.
4.Helix hydro-jet lithotrity via choledochoscope in hepatolithiasis:an ex vivo experimental study and clinical application

Objective To investigate the feasibility and safety of helix hydro-jet lithotrity in treatment of calculus of intrahepatic duct and to establish its basic parameters.Methods The parameter of hydro-jet Was set at different levels of pressure(5000-9000 kPa)and different nozzle directions,and the effect of different parameter wag observed and analyzed in three groups.including 32 ex vivo intrahapatic gallstones.thirty-two ex vivo swine bile ducts and 31 ex vivo bile ducts obtained from patients with hepatolithiasis.Results Lithotrity was successful in all groups with a jet pressure of 5000-9000 kPa.The multiple regression analysis showed that the time of procedure depended linearly on the minimal stone diameter,stone weight andjet pressure.The time of procedure and the time needed to damage the bile duct decreased with the increase in jet pressure.At same jet pressure,the smaller the angle between nozzle and bile duct,the longer the time needed to cause damage in bile duct.The technique was successfully performed in 60 patients,with no severe complications,including bile duct injury.Conclusion It is feasible to treat hepatolithiasis with hydro-jet lithotrity.The most optimal operation condition was hydro-jet pressure of 6000-7500 kPa and frequency of 1 Hz.
5.Analysis of correlation between radiation dose parameters of pelvic bone and acute bone marrow suppression in cervical cancer treated with intensity-modulated radiation therapy

Objective:To investigate the association between pelvic bone dose-volume parameters and acute bone marrow suppression in cervical cancer treated with concurrent cisplatin and intensity-modulated radiation therapy, in order to provide the limited prescription for making radiotherapeutic plan.Methods:The clinical data of 40 cervical cancer patients receiving cisplatin with concurrent intensity-modulated radiation therapy in the Affiliated Hospital of Ningde Normal College from November 2017 to January 2020 were analyzed. The correlations of the irradiated volume of pelvic bone receiving doses of 5, 10, 15, 20, 25, 30, 35, 40, 45, 50 Gy as a percentage of the total volume of pelvic bone (V 5 Gy,V 10 Gy, V 15 Gy,V 20 Gy,V 25 Gy, V 30 Gy, V 35 Gy, V 40 Gy, V 45 Gy, V 50 Gy), the maximum dose (D max), the minimum dose (D min), and the mean dose (D mean) with the occurrence of ≥grade 3 acute bone marrow suppression were analyzed. The logistic multiple regression analysis was used to study the influencing factors of ≥grade 3 acute bone marrow suppression, and the receiver operating characteristic (ROC) curve was used to determine the diagnostic efficacy of influencing factors for ≥grade 3 acute bone marrow suppression. 6.Analysis of APC gene variants in a pedigree affected with familial adenomatous polyposis.

OBJECTIVE:
To explore the genetic basis for a pedigree affected with familial adenomatous polyposis (FAP).
METHODS:
The proband, with recurrence of blood in the stool, was diagnosed with FAP by endoscopy, pathological examination and a family history. She was subjected to next generation sequencing to detect genetic variant. Suspected variant was verified by Sanger sequencing of members from her pedigree.
RESULTS:
The proband, her mother and brother were found to carry a heterozygous c.532-1G>A variant of the APC gene, which may lead to aberrant splicing of mRNA resulting in a truncated protein, which may lose its normal function and promote the tumorigenesis. Based on the American College of Medical Genetics and Genomics standards and guidelines, c.532-1G>A variant of APC gene was predicted to be pathogenic(PVS1+PP1+PP4+PP5).
CONCLUSION
The c.532-1G>A variant of the APC gene probably underlay the pathogenesis of FAP in this pedigree.

7.Internal fixation for the treatment of anteromedial facet fracture of the coronoid process of ulna.

OBJECTIVETo investigate the effect of operative treatment for anteromedial facet fracture of the coronoid process of ulna,and to study its surgical exposures and fixation techniques.
METHODSFrom March 2005 to March 2010,18 patients with anteromedial facet fracture of the coronoid process of ulna were treated with open reduction and internal fixation. There were 12 males and 6 females with an average age of 37.8 years. A single midline posterior incision was used to expose the entire elbow joint. After elevating the full-thickness skin flaps, a lateral incision was made to expose and repair the lateral collateral ligament. Three intervals in the flexor-pronator musculature were used to gain access to the coronoid,depending on the size of the fracture fragment and the planned fixation technique. Fractures were fixed by using mini-plate or with screws. The therapeutic effects were evaluated by Mayo Elbow Performance Score (MEPS) and system of Broberg & Morrey.
RESULTSSeventeen patients were followed up, no patient complained pain and elbow unstable at a mean follow-up period of 38 months(1 to 6 years). The fractures were clinically healed at an average time of 11.6 weeks(ranged from 8 to 16 weeks). The average MEPS was 95.4+/-4.6 (ranged, 82 to 100). The average functional rating of system of Broberg & Morrey was 92.3+/-5.8 (ranged,75 to 100).
CONCLUSIONOpen reduction and internal fixation is effective to reach anatomical reduction and strong fixation for the treatment of anteromedial facet fracture of the coronoid process of ulna.

8.Relation between eardrum flap area and healing outcome of traumatic eardrum perforation.

OBJECTIVETo retrospectively study the clinical effects of eardrum flap area on the healing outcome following traumatic perforation.
METHODSTotally 291 traumatic eardrum perforations with in-/everted edges were included in this study. They were randomly divided into three groups and received conservative treatment, epidermal growth factor (EGF) via Gelfoam patching, or edge-approximation plus Gelfoam patching respectively. Patients in each group were further divided into two subgroups according to the eardrum flap area less than or equal to 1/2 or >1/2 of the perforation size. The healing rate and mean closure time after tympanic membrane perforation were evaluated at three months.
RESULTSOf the total 291 participants, 281 were included in the final statistical analysis. The area of curled edge did not affect the healing outcome significantly in any groups (P>0.05). The healing rate varied slightly: 90.7% vs 92.3% in spontaneous healing group, 98.2% vs 97.4% in EGF via Gelfoam patching group, and 96.5% vs 100% in edge-approximation plus Gelfoam patching group. In addition, in all groups the area of curled edge did not affect the mean closure time significantly (P>0.05). The closure time was (32.3+/-2.4) d vs (30.6+/-3.1) d in sponaneous healing group, (13.4+/-2.5) d vs (13.1+/-1.9) d in EGF via Gelfoam patching group, and (11.9+/-3.1) d vs (12.2+/-2.1) d in edge-approximation plus Gelfoam patching group.
CONCLUSIONThe eardrum flap area of traumatic eardrum perforation does not significantly affect the clinical outcomes.

9.Recent progress in the treatment of posterior tibial plateau fractures.

There is a lack of ideal clinical classification systems for posterior tibial plateau fractures, and they need to be perfected in clinical practices. There are controversies over surgical approaches due to special characters of posterior tibial plateau fractures. It is difficult for the conventional techniques and approaches to achieve satisfactory reduction and fixation. A modified posterior approach is an ideal approach for the treatment of posterior tibial plateau fractures. 10.Clinical application of the combination of postural reduction and kyphoplasty for traumatic thoracolumbar spine fractures.

OBJECTIVETo determine the efficacy and feasibility of the combination of postural reduction and percutaneous kyphoplasty for traumatic thoracolumbar spine fractures.
METHODSThirty-seven patients with single level traumatic thoracolumbar spine fractures were included in this study. There are 28 males and 9 females, with an average age of 48 years (range 24 to 79 years). Patients were treated with postural reduction and then percutaneous vertebroplasty with Calcium Phosphate Cement (CPC) were performed on the fractured vertebra. The results were quantitatively evaluated, according to the concept of estimated vertebral height loss and kyphotic angle of the vertebral fractures by preoperative and postoperative plain standing lateral radiographs. Visual analog scale (VAS) and the fracture vertebra shape changes were recorded.
RESULTSPatients were followed up for 9 to 24 months (average 13 months), pain was significantly relieved compared with the preoperative, VAS was reduced averagely from 7.6 +/- 2.5 to 1.8 +/- 1.5, the anterior and middle vertebral height was restored and kyphotic angle was corrected. During the period of follow up, outcomes were satisfactory, without notable correction loss.
CONCLUSIONIf the indications are correctly handled, the combination of postural reduction and percutaneous kyphoplasty for the treatment of traumatic thoracolumbar spine fractures can provide significant pain relieve and restore the vertebral height and kyphotic angle.
