2.Observation of intracranial hemodynamics in brain death through transcranial doppler ultrasound

Objective To observe the evolution process of cerebral blood flow spectrum in brain death by transcranial doppler ultrasound.Methods To detect the blood flow of bilateral middle cerebral artery in brain dead patients by transcranial doppler ultrasound.Results Twenty brain dead patients were collected in the study.The blood flow velocity of middle cerebral artery ranged s from 6 to 37cm/s,and the mean velocity was (12.13 ± 3.15)cm/s.There were three characteristic spectrums which include oscillatory wave (reverse wave in diastolic),nails wave (tip small wave in systolic),no blood flow sign,and they appeared in turn accoerding to worsen of the disease.The heartbeat irreversibly stopped within 8 days after the appearing of oscillatory wave,and ninty percent of the cases had the regular blood flow spectrums of oscillatory wave-nails wave-no blood flow sign.Conclusion The three regularly characteristic transcranial doppler ultrasound blood flow spectrums,which include oscillatory wave,nails wave and no blood flow sign could facilitate the diagnosis of brain death.
3.Study on predictive value of cerebral arteriosclerosis to cerebral infarction evaluated by transcranial doppler ultrasound

Objective To assess the predictive value of cerebral arteriosclerosis to acute cerebral infarction evaluated by transcranial doppler ultrasound .Methods 170 cases suspected cerebral arteriosclerosis consecutively enrolled were given transcranial doppler ultrasound and were followed up for 6 months,then we assessed the correla-tion between the levels of cerebral arteriosclerosis and acute cerebral infarction within 6 months.Results By Spearman rank correlation analysis ,there was positive correlation between the severity of cerebral arteriosclerosis de-tected by transcranial doppler ultrasound and acute cerebral infarction within 6 months,the correlation coefficient was 0.575(P<0.05).Conclusion Through the detection of transcranial doppler ultrasound ,the higher levels of cere-bral arteriosclerosis,the greater probability of acute cerebral infarction within 6 months.
4.Observation of the effect of Tongsaimai Tablets on leptomeningeal collateral circulation established after acute cerebral infarction by TCD

Objective To observe the effect of Tongsaimai Tablets on the leptomeningeal collateral circulation established after acute cerebral infarction by TCD.Methods 50 patients with acute cerebral infarction were randomly divided into control group (received conventional treatment) and treatment group (received Tongsainai Tablets three times a day,1.75g each time,on the basis of conventional treatment).The leptomeningeal collateral circulation of the two groups were observed after 6 months.Results The factors which may affect the leptomeningeal collateral circulation,such as age,the prevalence of hypertension and hyperlipidemia,original compensatory status of cerebral artery between the two groups had no statistically significant differences.The open rate of leptomeningeal collateral circulation in the treatment group was 64.00%,which was significantly higher than that of the control group(32.00%)(P =0.024).Conclusion Tongsaimai Tablets can promote the formation of leptomeningeal collateral circulation in patients with acute cerebral infarction.
5.Application of improved quality evaluation method to Fufang Danshen products

AIM:To apply improved quality evaluation method for improving current quality criteria of Fufang Danshen products. METHODS:Considering both hydrophilic and lipophilic active components-danshensu,protocatechuic aldehyde,salvianolic acid B,cryptotanshinone and tanshinone ⅡA were selected as five markers. A HPLC method was developed to simultaneously determine their contents,which were used to compare the products from different companies,and different batches from the same company. Similarity factor method was used to evaluate the similarities of in-vitro dissolution profiles. Moreover,accelerated and long-term stability tests were performed to predict the shelf-life of Fufang Danshen products. RESULTS:All of the Fufang Danshen products passed the content requirements of Chinese Pharmacopeia. However,the content variations of the hydrophilic components were much higher than those of the lipophilic components. In dissolution tests,most of the hydrophilic components were dissolved completely while all of the lipophilic components could not be detected. The f2 values showed that there were no similar dissolution profiles among different brands of Fufang Danshen products. The contents of five markers degraded compatible with first-order kinetics processes in both the accelerated and long-term stability tests. The predicted shelf-life of Fufang Danshen products were shorter than those labelled on the products. CONCLUSION:It is suggested that more markers with defined content range instead of lower content limit are required for better control of Fufang Danshen products. Dissolution and stability tests need to be added into the quality control criteria in the Chinese Pharmacopeia.

Systematic pharmacological evaluation of material basis for the efficacy of Chinese herbal formula is one of the key issues in Chinese materia medica(CMM),which is in urgent need of solution.However,its scientific connotation is still controversial in academic circles.The material basis for the efficacy of Chinese herbal formula is the total non-endogenous bioactive compound(derived either directly or indirectly from this formula) exerting the therapeutic effects,which is generated by the interaction of the formula with human body and reaches certain concentration in the target organs or tissues.Systematic evaluation of material basis for the efficacy of Chinese herbal formula is the cornerstone of modernization and internationalization of CMM,while the quality control of CMM is its prerequisite.
7.Advances in Proliferation Model of Vascular Smooth Muscle Cells and Inhibitory Effects and Mechanisms of Chinese Herbal Medicine

The proliferation of vascular smooth muscle cells is closely related to the pathogenesis of a variety of diseases, such as atherosclerosis, hypertension, diabetes, vascular restenosis, and etc. A proper model for smooth muscle cell proliferation can be served as an important tool for exploring the molecular mechanisms of smooth muscle cell proliferation and the screening of potential inhibitory agents. Various factors can induce the proliferation of smooth muscle cells with different mechanisms and pathological significance. The ox-LDL, AGEs, angiotensin II (AngⅡ), high glucose, tumor necrosis factor (TNF) and etc. have been successfully applied in the establishment of smooth muscle cell proliferation model. This article summarized the most commonly used models for smooth muscle cell proliferation. The inhibitory effects of Chinese herbal medicine, especially the isolated pure compounds were discussed. It was aimed to provide references for the screening of smooth muscle cells based medications as well as studies on related research and development of Chinese herbal medicine.
8.The influence of Gastrodia elata capsule on cerebral blood flow in the treatment of patients with hypertension

Objective To observe the influence of Gastrodia data capsule on cerebral blood flow in the treatment of patients with hypertension.Methods A total of 69 patients with hypertension were divided into the treatment group(36 cases) and the control group(33 cases).The treatment group received treatment of Gastrodia clara capsule combined with antihypertensive drug,the control group received treatment of antihypertensive drug only.Two groups received TCD test before and 20 days after treatment,and the changes of blood flow of intracranial vessels before and after treatment were assessed.Results The total effective rate in treatment group was 97.2%,12 cases cured,15 cases of tangible effectiveness,8 cases of effectiveness,1 case of invalidity.The total effective rate in control group was 75.8%,7 cases cured,12 cases of tangible effectiveness,6 cases of effectiveness,8 case of invalidity,The difference of clinical efficacy between the two groups was statistically significant ( Z =2.240,P < 0.05 ).The mean velocity of MCA,ACA,PCA,BA,VA of control group 20 days after treatment was not statistically different compared to those before treatment.The mean velocity of MCA,ACA,PCA,BA,VA of treatment group 20 days after treatment was lower than those before treatment(t =-2.65,-2.44,-3.92,-3.31,-4.68,all P<0.05).Conclusion The treatment of Gastrodia elata capsule conbined with antihypertensive drug can significantly improve curative effect of patients with hypertension and improve the condition of cerebral blood flow.

OBJECTIVE: To discuss the advantages and disadvantages of the non-price competition in Chinese hospital market and suggest some solutions. METHODS: Using the competition theory in health economics to analyze the situation in China and the advantages and disadvantages of the price competition as well as the causes accountable for the disadvantages. RESULTS & CONCLUSIONS: The government is suggested to understand the comparative (dis)advantages brought about by non-price competition and take possible solutions such as to clarify the role of market mechanism in regulating hospital performance, establish effective supervision system and referral system, and rationalize the income of physicians etc.
