1.Roles of TNF-?,Endotoxin in Upper Gastrointestinal Bleeding in Cirrhosis Patients and the Preventive and Treatment Mechanism of Rhubarb

Objective To explore the role of tumor necrosis factor (TNF-?) and endotoxin(LPS) in upper gastrointestinal bleeding in cirrhosis patients and the preventive and treatment mechanism of rhubarb. Methods 45 liver cirrhosis patients with upper gastrointestinal bleeding were treated with rhubarb, and its efficacy was compared with cemetidine's efficacy. Liver function, and serum levels of TNF-? and LPS were measured before and after the therapy. Results Patients’symptoms and liver function were improved, and serum levels of TNF-? and LPS significantly decreased after treated with rhubarb. Conclusions TNF-? and endotoxin might play an important role in upper gastrointestinal bleeding in cirrhosis patients. Rhubarb has protective and treatment effect on upper gastrointestinal bleeding in cirrhosis patients by reducing TNF-? and LPS release.
2.The Effect of Kurorinone on the Serum Level of TNF-? and IL-6 in patients with Chronic Hepatitis B

Objective To explore the effects of kurorinone on the serum level of TNF-? and IL-6 in the patients with chronic hepatitis B (CHB). Methods 87 patients with CHB were randomly divided into groups A and B. Patients in group A (n=45) and group B (n=42) received kurorinone and diammonium glycyrrhizin treatment, respectively, for 3 months. The liver histopathological changes were observed, and liver function and the serum level of TNF-? and IL-6 in the patients with CHB were detected after treatment. Objective To evaluate the effects of kurorinone combined with thymosin on the treatment of chronic hepatitis B. Methods 178 patients with chronic hepatitis B were randomly divided into group A, B and C. 64 patients in group A received kurorinone combined with thymosin for 3 months. 58 patients in group B received kurorinone only, and 56 patients in group C received thymosin only. 5.Motion Artifacts in Three-dimensional CT Images

Objective To investigate the influence of physiological motions on three-dimensional (3D) CT images and the rules of motion artifacts.Methods A mathematical model of motion artifact was set up based on the principle of spiral scanning, hereby the geometrical distortion of 3D image was analyzed.Experiments were carried out with home-made phantoms on a CT scanner.Results The details of 3D images with trivial distortion were legible under static condition.On the contrary, distinct distortion occured under moving condition, and its degree increased with the pitch and motion period. The 3D images were varied with difference of the motion initial phases.Conclusion The distortion of 3D-CT image depends on the motion characters of scanned objects and scanning parameters such as pitch and slice thickness.
6.Clinical study of the interferon ?-2b combined with kurorinone in the treatment of chronic hepatitis B

Objective To study the efficacy of interferon ? 2b (IFN? 2b) combined with kurorinone in the treatment of chronic hepatitis B. Methods 146 patients with chronic hepatitis B were randomly divided into four groups(A,B,C,D). Based on the similar general treatment(group D),37 patients in group A was treated with IFN ? 2b and kurorinone. However, 37 patients in group B and 36 patients in group C received IFN ? 2b and kurorinone,respectively.Dynamic changes and relationships among liver histopathology,expression of TGF ? in liver tissue and serum levels of TGF ?,HBeAg,HBV DNA were studied using immunohistochemistry,radioimmunoassay and ELISA before and after treatment of IFN ? 2b and kurorinone. OBJECTIVE:
To assess the expression of NGF, BDNF, NT-3 mRNA in the peripheral blood of patients with allergic rhinitis (AR). Meanwhile, to analysis whether the expression of NGF, BDNF, NT-3 mRNA correlate with the severity of rhinitis.
METHOD:
This study is a group controlled trial, which takes the healthy adults as control group. The total RNA have been extracted from the peripheral blood of AR patients. The expression of NGF, BDNF and NT-3 mRNA have been tested by real-time quantitative RT-PCR.
RESULT:
Comparing with the healthy adults, the expression of NGF, BDNF and NT-3 mRNA as 2(-deltadeltaCt) are 2.436 8, 4.4588 and 1.781 8 respectively. The increasing expression of NT-3 correlated positively with the scores of visual analog scale.
CONCLUSION
The expression of NGF, BDNF and NT-3 mRNA are as high as 2.4368, 4.4588 and 1.7818 times to healthy adults. We propose NGF, BDNF and NT-3 may contribute to the pathogenesis of AR. NT-3 could reflect the severity of rhinitis as a molecular biological index.

8.Construction and application of an intelligent injury monitoring system in Yinzhou District

Injury monitoring is an important approach for injury control and is an important part of comprehensive disease monitoring. With the development of medical digitalization, an intelligent injury monitoring system has been created in Yinzhou District, Ningbo City, Zhejiang Province using artificial intelligence techniques based on Ningbo Municipal Health Information Platform and has been applied across the district since 2019. The manual card-reporting mode has been transformed to intelligent card-reporting mode in this system, which achieves functions of epidemiological analyses of the monitoring data, early warning of high-incidence injuries, classified management of injury and quality control of report cards. Nearly 300 thousand cards have been automatically reported since the system was online available since November 2022, and the epidemiological characteristics of injury were preliminarily understood, which provide data supports to early earning and interventions of injury. The intelligent injury monitoring system greatly improves the injury monitoring efficiency and card-reporting quality, and saves a large number of manpower and material resources, which provides a powerful technical support to widespread injury monitoring.
9.Clinicopathologic characteristics and survival analysis of 183 cases of triple negative breast cancer

Objective To investigate the clinicopathologic characteristics and prognosis of triple negative breast cancer patients. Methods A retrospective analysis was performed for 1042 primary breast cancer patients admitted in the First Affiliated Hospital of Nanjing Medical University from January 2003 to December 2009.All breast cancer patients were categorized into three subgroups by immunohistochemistry:ERBB2 +,HR +/ERBB2 - and triple negative. Results Of 1042 breast cancer patients recruited,183 patients were in triple negative group.The rate of larger tumors ( greater than 2 cm in diameter) and grade Ⅲ in triple negative patients was higher than that in ERBB2 + and HR +/ERBB2 - patients (P <0.01 ).The positive rate of p53 status in ERBB2 + patients was higher than that in triple negative and HR +/ERBB2- patients (P < 0.01 ).No significant differences were observed in other clinical variables.In survival analysis,more bone metastases were observed in HR +/ERBB2 - patients than in ERBB2 + and triple negative patients (P =0.006).However,no significant difference was observed in visceral metastases among the subgroups.There were significantly different recurrence-free survivals (RFS) among the three subgroups throughout the follow-up period ( P =0.029),the 5-year RFS of ERBB2 + was 80.3%,which was the worst in three groups. Conclusions Triple negative patients had higher rate of larger tumors (greater than 2 cm in diameter) and grade Ⅲ than that in ERBB2 + and HR +/ERBB2 - patients,while its 5-year RFS was higher than ERBB2 + patients.
10.Minimally invasive percutaneous mono-segment pedicle instrumentation for thoracolumbar burst fractures

Objective To investigate the feasibility,safety and therapeutic effects of minimally invasive percutaneous mono-segment pedicle instrumentation in treating thoracolumbar burst fractures ( AO classification:A 3.1 and A 3.2 ).Methods Twenty-four inpatients with thoracolumbar burst fractures (AO classification:A 3.1 and A 3.2) treated with percutaneous mono-segment pedicle instrumentation from March 2010 to December 2010 were retrospectively studied.The operation time,blood loss,pre-and post-operative visual analogue scale ( VAS),ratio of anterior height between compressed vertebral body and normal vertebral body and vertebral kyphotic Cobb' s angle were evaluated.Results The operation lasted for (90 ± 25) minutes,with intraoperative blood loss of (20 ± 10) ml.The rate of anterior body height rose from pre-operative (56.5 ± 10.1 ) to (92.3 ± 12.2) one week post-operatively and to (90.2 ± 11.l)at the follow-up one year later.The vertebral kyphotic Cobb' s angle was pre-operative ( 16.5 ± 5.2) °,which was reduced to ( 7.3 ± 2.4 )° at oneweek after surgery and ( 7.9 ± 3.5 )° at the follow-up one year later respectively.The VAS scored ( 7.0 ± 1.2) points before surgery,( 1.2 ±0.7) points at one week after surgery and ( 1.1 ± 6..5) points at the follow-up one year later.The ratio of anterior body height at one week after surgery and at the follow-up one year later were both obviously higher than that before surgery (P < 0.05 ),but the ratio one week postoperatively showed no significant difference in comparison with that one year postoperatively (P >0.05).The kyphotic Cobb' s angle had significant decrease at one week after surgery and at the follow-up one year later,as compared with that before operation (P <0.0 5).Also,the VAS score showed marked improvement at one week after surgery and at the follow-up one year later.Conclusions Minimally invasive percutaneous mono-segmental pedicle instrumentation is effective and safe for thoracolumbar burst fractures (AO classification:A 3.1and A 3.2),but it is not suitable for thoracolumbar burst fracture with severely compressed vertebra.
